ABSTRACT

La insuficiencia renal crónica es una enfermedad no trasmisible que se caracteriza por una disfunción renal que causa complicaciones y alteraciones metabólicas, las cuales disminuyen la capacidad funcional y la percepción de la calidad de vida relacionada con la salud de los pacientes. La atención de los pacientes con esta enfermedad demanda de cuidados integrales; entre los cuales los cuidados de enfermería ocupan un papel fundamental. El proceso de atención de enfermería es una herramienta necesaria para ofrecer el máximo nivel de calidad en los cuidados que recibe el paciente con enfermedad renal crónica en tratamiento de hemodiálisis. El objetivo de la presente investigación es exponer las particularidades del programa de atención de enfermería incluidas en el proceso de atención de enfermería de pacientes con insuficiencia renal crónica sometidos a tratamiento de hemodiálisis(AU)


Chronic kidney failure is a non-communicable disease in which there is kidney dysfunction that generates a significant number of complications and metabolic disorders, these decrease the functional capacity and the perception of quality of life related to the health of patients. The care of patients with this disease demands comprehensive care; within which nursing care plays a fundamental role. The nursing care process is a necessary tool to offer the highest level of quality in the care received by patients with chronic kidney disease undergoing hemodialysis. The objective of the present investigation is to contribute the elements from the pedagogical teaching point of view that guarantee an adequate knowledge of the actions to be carry out during the nursing care of patients with chronic kidney disease(AU)

ABSTRACT

Objetivo: avaliar a cultura de segurança organizacional de um hospital universitário português a partir da percepção dos enfermeiros. Método: estudo transversal, tipo survey, com 567 enfermeiros de um hospital universitário do Porto, Portugal, realizado de abril a dezembro de 2014. Utilizou-se o instrumento do Hospital Survey on Patient Safety Culture. Para análise, realizou-se a recodificação da Escala de Likert e aplicou-se a estatística descritiva. Resultados: as dimensões que apresentaram percentual elevado de respostas positivas foram: Trabalho em equipe dentro das unidades (78,69%) e Expectativa e ações de promoção de segurança do paciente dos supervisores (65,94%). Evidenciou-se que 248 (44,4%) profissionais não notificaram nenhum evento e apenas 13 (2,3%) notificaram mais de seis eventos no último ano. Conclusão: a avalição permitiu identificar uma dimensão forte e algumas fragilidades da organização. Os resultados são relevantes e servem de embasamento para os gestores e líderes de enfermagem, permitindo avançar na cultura de segurança.


Objective: to evaluate the organizational safety culture of a Portuguese university hospital on the basis of nurses' perceptions. Method: this cross-sectional, survey-type study of 567 nurses at a university hospital in Porto, Portugal, was conducted from April to December 2014, using the Survey on Patient Safety Culture instrument. For the analysis, the Likert Scale was recoded, and descriptive statistics were used. Results: the dimensions that returned high percentage positive responses were: Teamwork within units (78.69%) and Supervisor Expectations and Actions Promoting Patient Safety (65.94%). It was found that 248 (44.4%) of personnel did not report any event, and only 13 (2.3%) had reported more than six events in the prior year. Conclusion: the assessment identified one strong dimension in the organization, and some weaknesses. The results are significant and offer a foundation for nursing managers and leaders to improve safety culture.

Utiliza el instrumento de Hospital Survey on Patient Safety Culture. Para el análisis, se realizó recodificación de la Escala de Likert y se utilizó la estadística descriptiva. Resultados: Las dimensiones que presentaron un alto porcentaje de respuestas positivas fueron: Trabajo en equipo dentro de las unidades (78,69%) y Expectativa y acciones de promoción de seguridad del paciente de los supervisores (65,94%). Se evidenció que 248 (44,4%) de los profesionales no notificaron ningún evento y sólo 13 (2,3%) notificaron más de seis eventos el último año. Conclusión: la evaluación permitió identificar una dimensión fuerte y algunas fragilidades de la organización. Los resultados son relevantes y sirven de base para los gestores y líderes de enfermería, permitiendo avanzar en la cultura de seguridad.

ABSTRACT

Objective To investigate the effects of precautionary rescue nursing intervention on the nursing quality score and efficacy of rescue for neonates with hypoxic ischemic encephalopathy. Methods One hundred children with neonatal hypoxic ischemic encephalopathy admitted to the First Hospital of Hebei Medical University from January 2017 to December 2018 were enrolled. Fifty children from January to December 2017 were assigned in the routine rescue nursing group; and 50 children from January to December 2018 were in the precautionary rescue nursing group. The routine rescue nursing group received routine rescue nursing; and the precautionary rescue nursing group was given pre-planned rescue nursing intervention. The rescue success rate, rescue time, Apgar score within rescue for 5 minutes, poor prognosis, etc. were recorded in both groups; the infant intelligence development scale was used to evaluate the situation of children's development of IQ in 6 months; a self-made questionnaire was used to investigate the rate of satisfaction in 100 children family guardians; the hospital's self-made Nursing Quality Assessment Form was applied to assess the quality of care of nursing staff. Results The rescue successful rate and the 5-minute Apgar score of the precautionary rescue nursing group were significantly higher than those of the routine rescue nursing group [rescue success rate: 98.0% (49/50) vs. 72.0% (36/50), Apgar score: 8.43±1.28 vs. 5.74±1.39], and the rescue time and poor prognosis rate were significantly lower than those in the routine rescue nursing group [rescue time (minutes):3.18±1.20 vs. 6.78±2.23, poor prognosis rate: 2.0% (1/50) vs. 14.0% (7/50), both P < 0.05]. The 6-month intelligence index and psychomotor index in the precautionary rescue nursing group were significantly higher than those in the routine rescue nursing group (intelligence index scores: 96.43±5.85 vs. 85.40±8.90, psychomotor index scores:97.38±4.03 vs. 78.43±10.22, both P < 0.05). The satisfaction rate in precautionary rescue nursing group was obviously higher than that in the routine rescue nursing group [92.0% (46/50) vs. 76.0% (38/50), P < 0.05]; the rescue technology, rescue efficiency, rescue effect, humanistic care score in precautionary rescue nursing group were significantly higher than those in the routine rescue nursing group (8.12±0.89 vs. 2.74±1.02, 8.23±1.24 vs. 3.01±0.83, 7.80±1.33 vs. 3.44±1.25, 8.03±1.32 vs. 3.29±1.38, respectively all P < 0.05). Conclusion The precautionary rescue nursing intervention used for neonates with hypoxic ischemic encephalopathy can significantly improve the rescue effect, degree of satisfaction to nursing and the quality of nursing care, so that it is worthy to be applied clinically.

ABSTRACT

Objective To investigate the effect of Montelukast Combined with Symbicort in severe asthma treatment and intervention. Methods The treatment according to different interventions will be January 2015 to January 2017 in our hospital 80 cases of severe asthma groups:control group with routine nursing intervention for a letter form treatment, the observation group with high quality nursing the intervention of montelukast Symbicort treatment; detailed records of patients in the two groups of experimental data, and the data for comparative analysis, to explore the combination of montelukast and Symbicort in severe asthma treatment and intervention effect. Results The high quality nursing intervention meng montelukast Symbicort (observation group) for the treatment of severe clinical the effect of nursing intervention in patients with asthma is better than conventional Symbicort (control group) clinical efficacy, clinical symptoms improved better than the control group, nursing satisfaction was higher than the control group, the difference was statistically significant (P< 0.05). Conclusion The high quality nursing intervention of patients with severe asthma montelukast Symbicort treatment effect significantly, can effectively improve the clinical symptoms of patients, nursing satisfaction is high, it is widely used in clinical treatment of patients with severe asthma intervention.

ABSTRACT

Objective To describe the prevalence and change of rationing of nursing care (RNC) in medical and surgical units of level 2 and level 3 general hospitals across Guangdong province. Methods RNC were measured by asking nurses about the completion status of 12 necessary nursing care activities, and the prevalence of RNC in 2014 was compared with the study results from the China Hospital Nursing Workforce Research(CNWR) in 2009 (exclusively using the data of Guangdong province) to describe the change of RNC. Results Six activities were rationed averagely in 2014, and the most frequent activities rationed wereComfort/talk with patients accounted for 69.8% (975/1 396) andEducating patients and familiesaccounted for 67.3%(940/1 396). According to the CNWR in 2009, the average number of nursing care activities rationed was 3 which was statistically lower than the number in 2014 (P < 0.01). Conclusions RNC is prevalent in level 2 and level 3 general hospitals across Guangdong province, especially psychosocial care to the patients; the prevalence of RNC has not decreased since 2009.

ABSTRACT

Objective To translate the English version of the Oncology Patients' Perceptions of the Quality of Nursing Care Scale (OPPQNCS) into Chinese and to test the reliability and validity of the Chinese version of OPPQNCS in oncology patients.Methods The English version of OPPQNCS was translated by Brislin translation mode.Totally 320 oncology patients were recruited and were investigated by the Chinese version of OPPQNCS.Results The internal consistency coefficient of the Chinese version of OPPQNCS was more than 0.70,ranged from 0.804 to 0.964;the test-retest was 0.826.The content validity index(CVI) was 0.845-1.000,the average CVI was 0.925.Factor analysis got four factors,which explained 78.548% of the total variance.And every objective factor loading were greater than 0.4.Conclusions The Chinese version of OPPQNCS has been proved to be reliable and valid.It can be used as a valid tool for the oncology patients to measure their perceptions of the quality of nursing care they got.

ABSTRACT

Objective To know the effects of using hierarchical management for psychiatric nursing in psychiatric ward,and then to offer some rationales and theoretical support for hospital management.Methods The fine hierarchical management was implemented from Janurary 2012 in Xi'an Mental Health Center and the metal hygiene clinics of Xi'an Jiaotong University.Before and after the hierarchical management carried out,the nursing care quality was assessed among the family members of hospitalized patients by using satisfaction survey questionnaires,flight rate of patients was evaluated,and at the same time,nursing care quality and the nurse comprehensive ability was inspected.And then,the EPIDATE 3.0 and SPSS16.0 software was used to analyze and compare the satisfaction scores.The nurse comprehensive ability,and nursing care quality scores before and after implementation.Results The nurse comprehensive ability were improved.The satisfaction scores of family members of hospitalized patients was higher than that before implementation after the fine management implementation,which had statistical difference,P< 0.05.The satisfaction rate of patients' family member was 96.77%(2 543/2 628) after the fine management implementation,which was significant higher than that of before implementation,91.19%(2 382/2 612),x2=107.66,P<0.05.Flight rate of patients was reduced after the hierarchical management,which was significantly lower than that of in before,P<0.05.Nursing care quality scores were higher than those of in before,which has a statistical difference respectively,P<0.05.Conclusions The hierarchical management could improve the satisfaction of family numbers,reduce the flight rate of patients,enhance nursing care quality and improve the nurse comprehensive ability.It would reduce the incidence of medical malpractice.The fine hierarchical management is worth to be applied and generalized in nursing care of psychiatric nursing in psychiatric ward.

ABSTRACT

O nível de complexidade da assistência de enfermagem pode ser categorizado através de um Sistema de Classificação de Pacientes (SPC). O conhecimento do perfil assistencial dos pacientes pode subsidiar o planejamento e a implementação de programas assistenciais que melhor atendam às necessidades desses indivíduos, propondo a capacitação da equipe de enfermagem e redistribuição das responsabilidades. A enfermagem é considerada uma das profissões de maior relevância deste século e representa a maioria da força de trabalho das instituições hospitalares. No entanto, a prática da enfermagem sofre influência do modelo organizacional, a luz da abordagem clássica da administração. Para a enfermagem, a organização do trabalho no ambiente hospitalar apresenta aspectos diversificados tanto quanto ao grau da assistência prestada como ao nível de satisfação pessoal, dependo do contexto em que se encontra inserida. As atividades de enfermagem são focalizadas no mercado de trabalho, perfil dos profissionais e condições de trabalho, é de extremamente importância aplicar a Análise Ergonômica do Trabalho (AET) como recurso para verificação do grau de motivação e satisfação dos trabalhadores no serviço de saúde. A avaliação de desempenho é um processo que aprecia os méritos e julga o valor da capacidade de atuação dos profissionais no seu ambiente de trabalho, com a finalidade de redirecionar a melhoria da qualidade da assistência, e a organização e funcionabilidade dos serviços de enfermagem. Baseado na temática discutida, propomos neste estudo apresentar uma proposta que adeque um instrumento, para avaliação de desempenho da equipe de enfermagem, formulada nas particularidades do ambiente hospitalar em estudo, pontuando os níveis de complexidade da assistência de enfermagem paralelamente aos níveis de motivação e satisfação do trabalho executado...

ABSTRACT

This study aims at the developing of a computer program for the evaluation of nursing care quality. Since the professional nursing care requires a consistent evaluation, the computer program for the measurement of quality of nursing care is necessary. It provides the nursing care with an effective and efficient management of nursing quality. In this study, a computer program is developed as a module. The evaluation criteria are structured in a hierarchical manner. Each evaluation area includes several items, which again have their own indicators. The system consists of 7 evaluation areas, 32 evaluation items, and 71 indicators. Scoring is possible with the evaluation items. The scoring types of the program are of two types, that is, the norm-referenced type(option 1) and the criterion-referenced type(option 2). With this program, an accurate and consistent evaluation of nursing care with the rapid feedback to nursing care practice is expected.
