ABSTRACT

@#Among pregnant women, 1-2% are anti-Ro positive and while half of them have symptoms of connective tissue disease, the rest are asymptomatic. The presence of anti-Ro is of concern because of the risk of congenital heart block in the child. We report the case of an asymptomatic 27-year-old G2P1(1001) woman, who presented with persistent fetal bradycardia in her 21st week of gestation (AOG) and was found to have elevated titers for anti-Ro (>320 U/ml). Hydroxychloroquine 200 mg/day and prednisone 10 mg/day were given from the 33rd week of gestation up until the delivery. At 37 weeks AOG, she delivered a live male neonate with a complete heart block. On the 6th day of life, the infant remained bradycardic, hence a pacemaker was inserted and heart rate maintained at 100-120 bpm. On subsequent follow-ups, the mother and child did not develop any systemic manifestations and the infant was thriving well. While a diseased condition may not be apparent in a pregnant anti-Ro positive woman, the risk of neonatal lupus (NL) is demonstrated in this patient’s case. This report illustrates how prenatal care of an asymptomatic woman led to the discovery of a fetal abnormality and served to prepare the family and the medical team to ably handle the birth and subsequent care of a neonate with NL.

ABSTRACT

Introduction: Reverse osmosis (RO) is a water purification process that uses a partial permeable membrane to remove ions, unwanted molecules and larger particles from drinking water. In reverse osmosis, an applied pressure is used to overcome osmotic pressure, a colligative property that is driven by chemical potential differences of the solvent, a thermodynamic parameter. In the process of reverse osmosis the amount of water that is drained is a concern area for the people using the R.O. filtration device in their household because it wasted about 70% of the water to purify just one liter of water. This R.O. waste water can be utilized for various purposes such as washing vehicle like car bike etc, cleaning toilet this study is aimed to assess the knowledge reverse osmosis waste water utilization among general public at Indore. Objectives: 1. To assess the pretest knowledge regarding reverse osmosis (R.O) waste water utilization among general public. 2. To assess the posttest knowledge regarding reverse osmosis waste water utilization among general public. 3. To evaluate the effectiveness of structured teaching program on reverse osmosis (R.O) waste water utilization among general public. Hypothesis: H1- there will be significant difference between pretest and posttest knowledge who received structured teaching program regarding the utilization of waste R.O water. Materials and Methods: Quantitative, pre-experimental, one group pretest posttest design was adopted for the study. Total of 60 general public selected by using simple randomized sampling technique was used. Description of Tools: Structured knowledge questionnaire. Result: Data was analyzes using descriptive and inferential statistics. Major Finding: In the pre-test majority of the sample (44 out of 60, 73.3%) had inadequate knowledge and in the post-test, majority (54 out of 60, 90%) had adequate knowledge regarding reverse osmosis. A paired‘t’ test was done and it showed a‘t’ value of 22.34 at 0.05 level of significance, this indicates the effectiveness of structured teaching programme in enhancing the knowledge of the general public. There was no association found between the mean pre-test knowledge of the general public. There was no association found between the mean pre-test knowledge scorer with the selected socio-demographic variable such as age (?2 = 8.643), gender (?2 = 4.455), education qualification (?2 = 4.706), Occupation (?2 = 2.531), number of family member (?2 = 5.653) and previous knowledge about reverse osmosis filter water (?2 =0.393). Conclusion: There is a significant difference between the mean pre-test and post-test knowledge score among general public regarding reverse osmosis waste water utilization.

Resumen Introducción: Los anticuerpos anti-Ro52/TRIM21 son marcadores de varias enfermedades reumáticas autoinmunes sistémicas (ERAS). Objetivo: Evaluar si los anticuerpos anti-Ro52/TRIM21 están relacionados con anomalías en los circuitos inflamatorios. Métodos: Estudio transversal de pacientes consecutivos y ambulatorios con ERAS. Los anticuerpos anti-Ro52/TRIM21 y la proteína amiloide sérica se midieron mediante ELISA; los paneles para 18 citocinas y nueve quimiocinas se analizaron en una plataforma de lectura Luminex; la proteína C reactiva (hs-CRP) y el complemento se midieron mediante nefelometría. Resultados: Se incluyeron 167 pacientes, 143 con lupus eritematoso sistémico (LES), 16 con síndrome de Sjögren primario y ocho con esclerosis sistémica; 41 fueron positivos para anticuerpos anti-Ro52/TRIM21 (24 %). Los pacientes con anticuerpos anti-Ro52/TRIM21 tuvieron niveles séricos más altos de IL-2, IL-4, IL-6, GM-CSF, IL-21, IL-22, hs-CRP y quimiocinas CCL4, CXCL8, CXCL10 y CXCL12; y más bajos de complemento C4. Los títulos de anticuerpos anti-Ro52/TRIM21 correlacionaron positivamente con IL-2, IL-4, IL-6, IL-10, IL-21, IL-22, CXCL10 y hs-CRP; y negativamente con complemento C3 y C4. Al incluir solo LES, no se identificó asociación entre los anticuerpos anti-Ro52/TRIM21 y la actividad de la enfermedad o la afectación específica de órganos. Conclusiones: Los anticuerpos anti-Ro52/TRIM21 se asocian a circuitos aberrantes de citocinas y niveles elevados de moléculas angiogénicas y quimioatrayentes de neutrófilos y monocitos, lo que sugiere un papel activo de esos anticuerpos en las ERAS.


Abstract Introduction: Anti-Ro52/TRIM21 antibodies are markers for several systemic autoimmune rheumatic diseases (SARD). Objective: To assess whether anti-Ro52/TRIM21 antibodies are related to abnormalities in inflammatory circuits. Methods: Cross-sectional study of consecutive outpatients with SARD. Anti-Ro52/TRIM21 antibodies and serum amyloid A protein were measured by ELISA; panels for 18 cytokines and nine chemokines were analyzed on a Luminex reading platform, while high-sensitivity C-reactive protein (hs-CRP) and complement were measured by nephelometry. Results: Among 167 included patients, 143 had systemic lupus erythematosus (SLE), 16 had primary Sjögren's syndrome and eight had systemic sclerosis; 41 (24%) were positive for anti-Ro52/TRIM21 antibodies. Patients with anti-Ro52/TRIM21 antibodies had higher serum levels of IL-2, IL-4, IL-6, GM-CSF, IL-21, IL-22, hs-CRP and chemokines CCL4, CXCL8, CXCL10 and CXCL12, but lower levels of complement C4. Anti-Ro52/TRIM21 antibody titers were positively correlated with IL-2, IL-4, IL-6, IL-10, IL-21, IL-22, CXCL10, and hs-CRP, and negatively with complement C3 and C4. When only SLE patients were included, no association was identified between anti-Ro52/TRIM21 antibodies and disease activity or organ-specific involvement. Conclusions: Anti-Ro52/TRIM21 antibodies are associated with aberrant cytokine circuits and elevated levels of angiogenic molecules and neutrophil and monocyte chemoattractants, which suggests an active role for these antibodies in SARD.

Introducción: La interpretación de los diversos anticuerpos vinculados a las Enfermedades Autoinmunes Sistémicas (EAS) es un reto clínico. Entre los anticuerpos específicos, los anti Ro son los de mayor frecuencia. Se asocian a un fenotipo clínico, relacionados tanto a manifestaciones clínicas diversas como a EAS establecidas. Metodología : Estudio descriptivo, observacional, transversal, realizado en Hospital Maciel (Montevideo, Uruguay). Resultados: Se enrolaron 70 pacientes, la mayoría de sexo femenino. 60 pacientes (85%) presentaron Ro52, 40 (57%) Ro60 y 31 (44%) ambos anticuerpos. Del total de los pacientes que tuvieron Ro positivo en 59 (93%) se evidenciaron manifestaciones clínicas. Las más frecuentes fueron: fotosensibilidad 26 (42%); Raynaud 18 (30%); xerostomía 26 (42%), xeroftalmia 25 (41%), enfermedad pulmonar intersticial 10 (15%), nefropatía 20 (32%), y manifestaciones hematológicas 16 (23%). En 59 casos (86%) se diagnosticó una enfermedad autoinmune. Las más frecuentes fueron Síndrome de Sjögren 26 (41%), Lupus eritematoso sistémico 26 (41%), Artritis reumatoidea 14 (22%), Esclerosis sistémica 10 (16%), Hepatopatías autoinmunes 10 (14%). Se evidenciaron otros autoanticuerpos positivos en el 68 (97%) de los casos. El ANA fue positivo en 67 pacientes (95%). La proporción de pacientes con Ro60 positivo fue significativamente mayor en pacientes que presentaron fotosensibilidad (p 0.006), Lupus cutáneo agudo (p 0.003), manifestaciones articulares (p 0.031) y nefropatía (p 0.016). La positividad de ambos Ro52/60 fue significativamente mayor en pacientes con manifestaciones musculares (p 0.028) y nefropatía (p 0.047). En cuanto al tipo de enfermedad autoinmune se evidenció una proporción significativa de pacientes con Ro60 y LES (p 0.004), así como de ambos anticuerpos y ES (p 0.04). Hubo mayor proporción de pacientes con Ro60 y anti SCL 70 (p 0.008) que aquellos que no lo presentaban, así mismo sucede con Ro60 y el FR (p 0.018); y Ro 52/60 con anti La (p 0.049) y anti SCL 70 (p 0.027). Conclusiones: Los anticuerpos específicos Ro52/60 son frecuentes, predominando el Ro52. La mayoría de los pacientes que presentan positividad tienen una enfermedad autoinmune de base, observándose también en pacientes con patología oncológica. Conocer sus asociaciones es de suma importancia dado que presentan un rol diagnóstico, pronostico, y ayudan a predecir el fenotipo clínico.

They are associated with a clinical phenotype, related to both diverse clinical manifestations and established SAD. Methodology: descriptive, observational, cross-sectional study carried out at the Maciel Hospital, Montevideo, Uruguay. Results: 70 patients were enrolled, most of them female. 60 patients (85%) presented Ro52, 40 (57%) Ro60 and 31 (44%) both antibodies. Of the total number of patients who had positive Ro in 59 (93%) clinical manifestations were evidenced. The most frequent were: photosensitivity 26 (42%); Raynaud 18 (30%); Xerostomia 26 (42%), Xerophthalmia 25 (41%), interstitial lung disease 10 (15%), nephropathy 20 (32%), and hematological manifestations 16 (23%). In 59 cases (86%) an autoimmune disease was diagnosed. The most frequent were: Sjögrens syndrome 26 (41%), systemic lupus erythematosus 26 (41%), rheumatoid arthritis 14 (22%), systemic sclerosis 10 (16%), autoimmune liver diseases 10 (14%). Other positive autoantibodies were found in 68 (97%) of the cases. ANA was positive in 67 patients (95%). The proportion of patients with positive Ro60 was significantly higher in patients with photosensitivity (p: 0.006), acute cutaneous lupus (p: 0.003), joint manifestations (p: 0.031), and nephropathy (p: 0.016). The positivity of both Ro52/60 was significantly higher in patients with muscular manifestations (p 0.028) and nephropathy (p 0.047). Regarding the type of autoimmune disease, there was a significant proportion of patients with Ro60 and SLE (p 0.004), as well as both antibodies and SE (p 0.04). There was a higher proportion of patients with Ro60 and anti SCL 70 (p 0.008) than those who did not, the same happens with Ro60 and RF (p 0.018); and Ro 52/60 with anti La (p 0.049) and anti SCL 70 (p 0.027). Conclusions: Ro52/60 specific antibodies are frequent, predominating Ro52. Most of the patients who present positivity have an underlying autoimmune disease, which is also observed in patients with oncological pathology. Knowing their associations is of the utmost importance given that they present a diagnostic and prognostic role, and help to predict the clinical phenotype.

Abstract Objective: The prognostic importance of Tumor-Infiltrating Lymphocytes (TILs) in the tumor microenvironment of various cancers is increasingly recognized. In the present study, we aimed to investigate the prognostic value of CD3+, CD4+, CD8+, and CD45RO + TILs and their relation to histopathological features in larynx squamous cell carcinoma. Methods: Formalin-Fixed and Paraffin-Embedded (FFPE) samples from 63 primary larynx squamous cell carcinoma patients were immunostained for CD3, CD4, CD8, and CD45RO expression. Positive cells in micrographs from Invasive Margin (IM) and Tumor Center (CT) of tissue specimens counted by ImageJ software and their correlation with disease outcome were analyzed. Results: The expression level of TILs subpopulations was associated with clinicopathological markers as well as Overall Survival (OS) and Disease-Free Survival (DFS). In multivariate analysis, high frequency of CD45RO + cells in IM were confirmed as an independent prognostic marker for DFS (p = 0.007, HR = 4.968) and OS (p = 0.007, HR = 4.957). Similar findings were observed in the multivariate analysis of the combined frequency of CD45RO+cells in IM and CT. Conclusion: TILs are associated with patients clinicopathological features. Also, our findings indicate that CD45RO + TILs are a valuable marker for risk prediction in larynx SCC and could predict patients' outcomes.

ABSTRACT

To analyze the clinical characteristics of patients with antisynthetase syndrome (ASS) and positive anti-Ro52 antibody. The clinical data of 203 ASS patients admitted to the First Affiliated Hospital of Zhengzhou University from 2017 to 2020 were analyzed retrospectively. Demographics, clinical manifestations, laboratory results, treatment and outcome were collected including data of 18 patients with rapidly progressive interstitial lung disease (RP-ILD). In total, the majority were women (148,72.9%). The average onset age was (51.9±13.3) years. There were 163 (80.3%) patients with positive anti-Ro52 antibody. The positivity in women (77.3% vs. 55.0%, P=0.004) was higher, and the median time from disease onset to diagnosis [4.5 (2.0, 24.0) months vs. 2.0 (1.0, 12.0) months, P=0.024] was longer in patients with positive anti-Ro52 antibody than those negative. Compared with negative patients, patients with positive anti-Ro52 antibody had a higher incidence of interstitial lung disease (ILD) (96.9% vs. 65.0%, P<0.001), arthritis (33.7% vs. 17.5%, P=0.046), and arthralgia (39.3% vs. 20.0%, P=0.022). Higher rate of positve antinuclear antibody (ANA) (85.3% vs. 55.0%, P<0.001), lower rate of positive anti-Jo-1 antibody (32.5% vs. 50.0%, P=0.039), lower albumin level [(34.6±5.2) g/L vs. (37.3±4.7) g/L, P=0.004] and lower lymphocyte counts [(1.4±0.8) ×10 9/L vs. (1.8±0.8) ×10 9/L, P=0.014] were more common in patients with positive anti-Ro52 antibody. The presence of anti-Ro52 antibody is associated with a particular phenotype of ASS, leading to common ILD, involvement of joints, high ANA positivity, low albumin and low lymphocyte counts.

Introducción: Las miopatías inflamatorias idiopáticas constituyen un grupo de enfermedades musculares caracterizadas por debilidad muscular crónica e inflamación muscular de etiología desconocida. Objetivo: Identificar las características clínicas e inmunológicas y su relación con el daño de órganos en los pacientes con miopatías inflamatorias idiopáticas. Métodos: Se realizó estudio observacional, descriptivo, transversal, en 52 pacientes con diagnóstico de miopatía inflamatoria idiopática, seguidos en la consulta protocolizada de Reumatología del Hospital Clínico Quirúrgico Hermanos Ameijeiras entre enero 2016 y enero 2017. Para las variables cualitativas se calcularon los porcentajes de cada grupo. Se utilizó Chi-cuadrado de Pearson (estadístico exacto de Fisher). Nivel de significación del 95 por ciento (α = 0,05) para relacionar la presencia de anticuerpos y el tipo de miopatía así como la presencia de manifestaciones clínicas de MII. Resultados: El 80,8 por ciento fueron mujeres y 86,5 por ciento de procedencia urbana. La edad media al comienzo fue 42,8 ± 13,2 años, tiempo de demora al diagnóstico de 8,8 ± 7,0 meses, tiempo medio de evolución de la enfermedad de 7,5 ± 7,1 años. El 80,8 por ciento estaba en remisión, 50 por ciento tenía anticuerpos específicos. La hipertensión arterial se encontró en 28,8 por ciento de los pacientes y 23,1 por ciento presentó neumonía intersticial. La artritis estuvo presente en 96,2 por ciento. El 26,9 por ciento presentaron anticuerpos específicos Jo-1 y 21,2 por ciento Ro 52. Conclusiones: Predominaron los pacientes del sexo femenino en la cuarta década de la vida de procedencia urbana, los anticuerpos específicos encontrados más frecuentes fue el anti Jo-1, asociado a la presencia de neumopatía intersticial(AU)


Introduction: Idiopathic inflammatory myopathies constitute a group of muscle diseases characterized by chronic muscle weakness and muscle inflammation of unknown etiology. Objective: To identify the clinical and immunological characteristics and their relationship with organ damage in patients with idiopathic inflammatory myopathies. Methods: An observational, descriptive, cross-sectional study was carried out in 52 patients with diagnosis of idiopathic inflammatory myopathy, followed in the protocolized consultation of Rheumatology at Hermanos Ameijeiras Clinical and Surgical Hospital from January 2016 to January 2017. For the qualitative variables, the percentages of each group were calculated. Pearson's Chi-square (Fisher's exact statistic) was used. 95percent significance level (α = 0.05) was used to relate the presence of antibodies and the type of myopathy as well as the presence of clinical manifestations of MII. Results: 80.8percent were women and 86.5percent of urban origin. The mean age at the beginning was 42.8 ± 13.2 years, time delay to diagnosis was 8.8 ± 7.0 months, mean time of evolution of the disease of 7.5 ± 7.1 years. 80.8percent were in remission, 50percent had specific antibodies. Hypertension was found in 28.8percent of the patients and 23.1percent had interstitial pneumonia. Arthritis was present in 96.2percent. 26.9percent had specific Jo1 antibodies and 21.2percent had Ro 52. Conclusions: Urban female patients in the fourth decade of life predominated, the most frequent specific antibodies found was anti-Jo-1, associated with the presence of interstitial lung disease(AU)

Results: 80.8% were women of the total patients studied, 61.5% non-white skin color, 86.5% of urban origin. The mean age at the beginning was 42.8 ± 13.2 years, time delay to diagnosis was 8.8 ± 7.0 months, mean time of evolution of the disease of 7.5 ± 7.1 years. 80.8% were in remission, 50% had specific antibodies. Hypertension was found in 28.8% of the patients and 23.1% had interstitial pneumonia. Arthritis was present in 96.2%. We found 26.9% had specific Jo1 antibodies and 21.2% Ro 52. Conclusions: Urban origin female patients predominated, in their fourth decade of life, the more frequent specific antibodies found was anti Jo-1, which was associated with the presence of interstitial lung disease.

ABSTRACT

BACKGROUND@#Lung cancer is the most common malignancy world-wide. There are a variety of immune infiltrating cells in tumor microenvironment, which is an important component of tumor immunity and has clinical significance for the prognosis of patients. CD45RO is a surface marker of memory T cells. The expression of CD45RO⁺ tumor infiltrating lymphocytes (TILs) is associated with the prognosis of many tumors. The purpose of this study was to evaluate the relationship between the density of CD45RO⁺ TILs in tumor and stromal area and the clinical characteristics of patients with non-small cell lung cancer (NSCLC) and its impact on the prognosis of patients. We aimed to explore the clinical value of CD45RO⁺ TILs and programmed cell death ligand 1 (PD-L1) as prognostic markers.@*METHODS@#Multiple fluorescent immunohistochemical staining was used to stain the tissue microarray chips of 167 patients with NSCLC, marking CD45RO, cytokeratin (CK) and PD-L1. Using artificial intelligence image recognition technology and tumor cell-specific CK staining, divide the tumor and stromal area in the tissue, evaluate the density of CD45RO⁺ TILs in the tumor and stromal area, and the expression level of PD-L1 in tumor cells. The non-parametric test was used to analyze the relationship between CD45RO⁺ TILs and the clinical characteristics of patients, and the Kaplan-Meier method and Cox risk ratio model were used to analyze the relationship between CD45RO⁺ TILs independently or in combination with PD-L1 and tumor prognosis.@*RESULTS@#The density of CD45RO⁺ TILs was significantly associated with patient age, smoking, tumor stage, and pathological type. Single-factor survival analysis showed that NSCLC (P=0.007) stromal region and lung adenocarcinoma (LUAD) (P<0.001) with CD45RO⁺ TILs high density had better OS. Multivariate survival analysis showed that the high density of CD45RO⁺ TILs in the stromal region of NSCLC (HR=0.559, 95%CI: 0.377-0.829, P=0.004) and lung adenocarcinoma (HR=0.352, 95%CI: 0.193-0.641, P=0.001) were independent prognostic factors for overall survival time (OS). Combined with PD-L1 score of tumor cells in tumor tissues and infiltration score of CD45RO⁺ TILs in all tumor tissues, the patients were divided into 4 groups: patients with PD-L1⁺/CD45RO⁺ had the longest disease-free survival (DFS) time, and patients with PD-L1⁺/CD45RO- had the shortest DFS time. Multivariate Cox regression analysis showed that PD-L1⁺/CD45RO- was an independent prognostic factor for DFS and had a higher risk of poor prognosis compared to the other three groups (HR=2.221, 95%CI: 1.258-3.919, P=0.006).@*CONCLUSIONS@#In tumor tissues, the density of CD45RO⁺ TILs, as well as the combination of CD45RO⁺ TILs and PD-L1 in tumor areas, significantly correlated with clinicopathological features and prognosis of NSCLC, which can be used as a new prognosis marker.

Objective:In view of the complexity of the "Prescription-Formula-Dosage-Property" relationship of Tibetan medicine and the outstanding common relationship,it is difficult to reveal the hidden and specific rules of clinical medication of Tibetan medicine. Method:Based on the attribute partial order structure and the vector structure model of "Ro-Nus-Zhu-Rjes"(Taste,Post-Digestive Tastes and Potency),clustering analysis and other methods and software,this study analyzed the "Prescription-Formula-Dosage-Property" relationship of 184 commonly used prescriptions in the 1995 edition of the standards issued by the Ministry of Tibetan medicine(SIMTM). Result:Among them,the analysis of the relationship between prescription and formula found that 11 prescriptions with the largest common attribute,such as Chebulae Fructus and Carthami Flos,were the key components of classification and compatibility,which could effectively classify the 8 kinds of prescriptions for the treatment of lung disease,tripa disease and blood fever. Among them,the san Yin and auxiliary viscera function prescriptions,such as Sanguotang powder and Liuwei Liangyao powder,had the strongest commonality. According to the analysis of relationship between formula and dosage ,the dosage of Chebulae Fructus,Carthami Flos,pomegranate seed,Phyllanthi Fructus was the highest,which suggested that these drugs were often used as primary drugs,while the Liuwei Liangyao powder,such as Amomi Fructus Rotundus and Tsaoko Fructus,had a higher frequency but a lower dose,which mainly played a role in regulating the overall drug property of the prescription and protecting the viscera. The Tibetan medicine-specific drugs including Moschus,Bovis Calculus,and Zhaxun,which were used in a high frequency but very low dose,had the effect of enhancing the drug property and guiding the affected part. According to the analysis of the relationship between dosage and property,there were many prescriptions belonging to cool nature,accounting for 75.6%. It was found that 67 prescriptions did not conform to the efficacy due to their different dosage. Conclusion:There are many common components and common usages in Tibetan medicine prescriptions. If these common associations are not treated,it will lead to the result that all diseases take these common associations as the core,but the hidden key factors cannot be solved. Therefore,it is necessary to pay attention to sensitivity and specificity at the multi-dimensional level of "prescription-formula-dosage-property",so as to reveal the clinical medication thought of Tibetan medicine more effectively.

Objective:To construct the database of Tibetan medicine prescriptions for "Gnyan-rims" disease, and to explore the invisible medication law of Tibetan medicine in the treatment of "Gnyan-rims" disease, such as prescription compatibility and combination of drug properties. Method:The prescriptions for treating "Gnyan-rims" were retrieved from four Tibetan medical literatures such as <italic>The Four Medical Tantras</italic>,<italic> Kong-sprul-zin-tig,</italic> <italic>Phyag-rdor-gso-rig-phyogs-bsgrigs</italic> and <italic>Sman-sbyor-lag-len-phyogs-bsgrigs</italic>, and the database was constructed under Python code, and the Apriori algorithm and the vector structure model of taste property flavor transformation were used for analysis. Result:According to the characteristics of Tibetan medicine prescription data, with six fields of prescription name, formula, dosage, efficacy, source and original text as the core, a Tibetan medicine treatment "Gnyan-rims" prescription database with functions of cleaning, searching and exporting was established. A total of 7 602 prescriptions were included in the database, among which 598 prescriptions had therapeutic effects of "Gnyan" and "Rims". The results of compatibility analysis showed that Shexiang, Hezi, Honghua, Mukuer Moyao, Tiebangchui, Tianzhuhuang and Bangga were the most frequently used drugs, while the correlation degrees of Shexiang-Mukuer Moyao, Honghua-Tianzhuhuang, Shexiang-Hezi and Shexiang-Tiebangchui were the strongest, and all the drug composition of Wuwei Shexiang pills appeared in the top ten correlations. According to the property analysis of 40 prescriptions containing high-frequency drugs, 19 prescriptions were found to have excessive bitter taste, followed by 9 prescriptions such as Sanchen powders with excessive sweetish taste, and the ratios of sweetish and bitter tastes in six tastes were >35%. The total of sweetish and bitter prescriptions accounted for 70% of the total prescriptions. Among the three flavors, the bitter flavor was the most abundant. The cool effect, dull effect and heavy effect were prominent among the seventeen effects. Conclusion:The prescription database of Tibetan medicine for "Gnyan-rims" can promote the high-quality development of research on prevention and treatment of plague with ethnic medicine. Tibetan medicine treatment of "Gnyan-rims" focuses on the composition of Wuwei Shexiang pills, with the property combination of "cool-bitter and sweet-bitter flavor-cool, dull and heavy", which mainly treats diseases such as "heat sharp light-mkhris pa-heat". These studies can provide data basis and theoretical reference for the selection of Tibetan medicine prescription and its composition for treating plague.

Objective:To investigate the expression of IL-2/IL-15 receptor β subunit (IL-2/IL-15Rβ) on memory CD3 + CD8 + CD45RO + T cells in patients with chronic hepatitis B (CHB) receiving antiviral treatment and its significance. Methods:Sixty-eight patients with chronic active hepatitis B (CAHB) and 47 asymptomatic hepatitis B virus (HBV) carriers attending in the Department of Infectious Diseases, the First Affiliated Hospital of Wannan Medical College from March 2019 to December 2020 were enrolled in the study; and 30 health subjects were also enrolled as healthy control group. Among 60 CAHB patients there were 30 cases with positive HBeAg and 30 cases with negative HBeAg. All CAHB patients received nucleos(t)ide analogue therapy, the HBV-related markers, Alanine aminotransferase (ALT) and the expression of IL-2/IL-15Rβ on CD3 + CD8 + CD45RO + T cells were determined and compared between HBeAg-positive and negative patients, before and after treatment. Normal distribution measurement data among 3 groups were compared with One-way ANOVA; normal distribution measurement data between 2 groups were compared with paired samples t test; non-normal distribution measurement data between the two groups were compared with Mann-Whitney U test; Pearson’s correlation coefficient was performed for correlation analysis. P<0.05 was considered statistically significant. Results:The proportion of CD8 + CD45RO + T cells on PBMC CD3 + T cells in CAHB group [(8.6±3.7)%] was higher than that of asymptomatic HBV carriers group [(5.7±2.5)%] and healthy control group [(5.5±1.5)%] (all P<0.05). The expression percentage of IL-2/IL-15Rβ on PBMC CD3 + CD8 + CD45RO + T cells in CAHB group [(6.8±4.7)%] was higher than that of asymptomatic HBV carriers group [(4.7±2.8)%] and healthy control group [(4.3±2.2)%] (all P<0.05). The MFI of IL-2/IL-15Rβ on PBMC CD3 + CD8 + CD45RO + T cells in CAHB group (243±168) was higher than those of asymptomatic HBV carriers group (160±91) and healthy control group [160±63] (all P<0.05). The expression percentage and MFI of IL-2/IL-15Rβ on PBMC CD3 + CD8 + CD45RO + T cells were positively correlated with the percentage of CD3 + CD8 + CD45RO + T cells in CAHB patients ( r=0.33 and 0.28, all P<0.05). The proliferation percentage of PBMC CD3 + CD8 + CD45RO + T cells in CAHB group[ (43.7±16.0)%] was higher than that of asymptomatic HBV carriers group [(29.1±9.4)%] and healthy control group [(26.8±9.6)%] after stimulation with Anti-CD3+ super-2 (all P<0.05). After the expression of IL-2/IL-15Rβ was blocked, the proliferation percentage of CD3 + CD8 + CD45RO + T cells was decreased [(11.2±6.3)%] compared with the untreated CAHB group ( P<0.05). The percentages of PBMC CD3 + CD8 + CD45RO + T cells secreting IFN-γ, IL-2 and TNF-α in CAHB group were (13.8±5.4)%, (14.0±4.3)% and (12.3±4.6)% respectively, which were higher than those of asymptomatic HBV carriers [(8.4±2.6)%, (9.4±3.2)% and (6.8±3.3)%] and healthy control group [(6.9±2.7)%, (9.9±3.0)% and (7.7±3.8)%] after stimulation with Anti-CD3+ super-2 (all P<0.05). After the expression of IL-2/IL-15Rβ was blocked, the percentages of PBMC CD3 + CD8 + CD45RO + T cells secreting IFN-γ [(2.4±1.6)%], IL-2 [(4.1±1.9)%] and TNF-α [(4.1±1.8)%] were decreased compared with the untreated CAHB group (all P<0.05). HBeAg, ALT, the expression percentage and MFI of IL-2/IL-15Rβ on CD3 + CD8 + CD45RO + T cells were 521.4 (68.9, 1 339.0) COI, 292 (160, 528) U/L, (6.4±3.2)% and (239±136) in 30 HBeAg-positive CAHB patients before treatment, which were higher than those after treatment [3.5(1.5, 17.5)COI、20(14, 31) U/L, (4.1±2.4)% and (134±58)] ( Z=5.337 and 6.403, t=3.229 and 3.892, all P<0.05). HBsAg, ALT, the expression percentage and MFI of IL-2/IL-15Rβ on CD3 + CD8 + CD45RO + T cells were (5 310±2 851) COI, (328±207) U/L, (7.1±5.8)% and (252±110) in 30 HBeAg-negative CAHB patients before treatment, which were higher than those after 48 weeks of treatment [(3 811±2 495) COI, (33±14) U/L, (4.6±2.9)% and (154±73)] ( t=2.167, 5.595, 2.116 and 2.383, all P<0.05). Conclusion:The study suggests that up-regulated expression of IL-2/IL-15Rβ is associated with elevated frequency, proliferation and secretion function of memory CD3 + CD8 + CD45RO + T cells in CAHB patients.

ABSTRACT

Objective:To analyze the potential role and prognostic value of platelet-to-lymphocyte ratio (PLR) at an early stage in arterial-venous extracorporeal membrane oxygenation (VA -ECMO).Methods:Totally 83 adult patients with VA-ECMO from June 2018 to June 2020 treated at Emergency Department of Jiangsu Provincial Hospital were retrospectively analyzed. Baseline characteristics between survivors ( n=46) and non-survivors ( n=37) were compared. Logistic regression analysis was used to predict the risk factors associated with 28-day mortality in VA-ECMO patients. The cut-off value was calculated by the receiver operating characteristic (ROC) curve. Results:PLR48-h ( OR=1.018,95% CI: 1.001-1.036, P=0.039) and continuous renal replacement therapy (CRRT) ( OR=7.095,95% CI: 1.099-45.799, P=0.039) were relevant risk factors of 28-day mortality in VA-ECMO patients. The cut-off value of PLR48-h was 156.3 [sensitivity: 57.8%, specificity: 86.1%, and area under the curve (AUC): 0.756]. Compared with the high PLR group (>156.3), the incidences of acute kidney injury (AKI) ( P<0.001) and bleeding events ( P=0.013) were significantly higher in the low PLR group (<156.3). Conclusions:The early PLR reduction and CRRT application during VA-ECMO support are related to poor prognosis.

@#Neonatal lupus is a passively acquired autoimmune disease that occurs in children of mothers with anti-Ro/SSA and/or anti-La/SSB antibodies. [1-4] The most serious complication in the neonate is complete heart block. [3-8] This is a case report of a newborn female presenting with persistent bradycardia detected in utero. The diagnosis was confirmed by maternal anti-Ro/SSA and/or anti-La/SSB antibodies and in utero detection of fetal heart block on echocardiogram. Therapeutic management involved placement of a permanent pacemaker.

Las Miopatías Inflamatorias Idiopáticas (MII) son un grupo heterogéneo de enfermedades que se caracterizan por debilidad muscular e inflamación subyacente en la biopsia muscular. Los principales órganos afectados son el músculo, la piel y también puede afectarse el pulmón. Se distinguen dentro de los subtipos clínicos como Polimiositis (PM), Dermatomiositis (DM), DM con la variante Dermatomiositis Clínicamente Amiopática (DMCA), el Síndrome Antisintetasa (SAS), la Miositis Necrotizante Inmunomediada, la Miositis por Cuerpos de Inclusión (MCI) y la Miositis Asociada a Neoplasia. La presencia de ciertos anticuerpos específicos y asociados predispone al desarrollo de manifestaciones clínicas, determinando el pronóstico de la enfermedad. Se presentan 4 pacientes del Registro de MII de la Sociedad Argentina de Reumatología (SAR) con estas características: un paciente con PM y anti Jo-1 positivo y tres pacientes con DM (uno con DMCA y anti-RO 52 y dos pacientes con anti-PL7 y anti-TIF1γ respectivamente).


Idiopathic Inflammatory Myopathies (MII) are a heterogeneous group of diseases characterized by muscle weakness and inflammation underlying muscle biopsy. The main organs affected are muscle, skin and the lung can also be affected. They are distinguished within clinical subtypes such as Polymyositis (PM), Dermatomyositis (DM), DM with the variant Clinically Amiopathic Dermatomyositis (DMCA), the Syndrome Antisynthetase (SAS), Immune-mediated Necrotizing Myositis, Body Myositis Inclusion (MCI) and Neoplasia-Associated Myositis. The presence of certain specific and associated antibodies predisposes to the development of clinical manifestations, determining the disease prognosis. 4 patients from the Registry of MII of the Argentine Society of Rheumatology (SAR) are presented with these characteristics: one patient with PM and anti Jo-1 positive and three patients with DM (one with DMCA and anti-RO 52 and two patients with anti-PL7 and anti-TIF1γ respectively).

ABSTRACT

OBJECTIVE:To investigate t he p otential mechanism of Tibetan medicine Wuwei shexiang pills in the treatment of “Grum bu ”disease(i.e. rheumatoid arthritis ). METHODS :Referring to Sibu Yidian ,Drug Standard of the Ministry of Public Health(Tibetan Medicine ),Jingzhubencao,the constituent ,dose and flavor of Wuwei shexiang pills were collected ;vector structure model was built. From the aspects of six tastes ,three tastes after digestion and seventeen effects ,the properties of the formulation were analyzed ;the“formulation-drug property-disease ”network for the treatment of “Grum bu ”disease was constructed by using Gephi 0.9.2 complex network software. The effective components of Wuwei shexiang pills were screened by using the pharmacology analysis platform of TCM system and the database of organic small molecule biological activity. The target proteins corresponding to the active components were predicted by using BATMAN-TCM network pharmacology research platform ; the target proteins of “Grum bu ”disease were searched by ETCM database. On the basis of screening the common targets of the two,David 6.8 bioinformatics resource database was adopted to conduct gene ontology (GO) analysis and KEGG pathway enrichment analysis ;the network of Wuwei shexiang pills- “Grum bu ”- target-pathway was constructed by using Cytoscape 3.7.0 software,and the network topology analysis was carried out to screen the core targets. Using Glide score as evaluation index , Maestro Version 11.1.011 software was used for molecular docking of above core targets with effective components of Wuwei shexiang pills. RESULTS & CONCLUSIONS :Wuwei shexiang pills contains myrobalan ,Aconitum pendulum ,Aucklandia lappa , Acorus calamus and artificial moschus ,and the highest dosage was myrobalan. The six tastes of this formulation were mainly bitter and sweet ;the three tastes after digestion were mainly bitter ;the seventeen effects were mainly blunt ,cool and heavy ,which were mainly used to treat twenty characteristics such as acute ,hot and light. In “formulation-property-disease”network,higher values of edge weight between seventeen effects and twenty characteristics were cooling effect-heat , blunt effect-sharpness , heavy effect-light,dilute effect-odor (edge weight values ≥430),etc. A total of 2 306 potential targets of effective components of Wuwei shexiang pills ,211 targets related to “Grum bu ”disease,32 common targets and 29 corresponding effective components were obtained. The results of GO analysis showed that 52 relative results were predicted ,the common targets mainly located in the extracellular area ,nucleus and other parts ,mainly including biological processes and molecular functions such as immune response,inflammatory response ,cytokine activity. The results of KEGG enrichment were significant in 31 pathways(P<0.05), involving TNF signaling pathway ,cancer pathway and other signaling pathways. There were 94 active components ,targets and pathway nodes in the network of Wuwei shexiang pills- “Grum bu ”-target-pathway,and 460 edges;TNF,FAS,IL6,IL10, FASLG,PTGS2 and IL 1B were the core targets of the network , connected with effective components such as quinic acid , thymol,dehydroepiandrosterone,norcaxone by Van Der Waals force ,hydrogen bond and hydrophobic interaction ,Pi-cation bond and other bonds ,so as to play a role in the treatment of “Grum bu ”disease.

ABSTRACT

Ginsenoside Ro decreased measures of inflammation, aging, oxidants and thrombus formation in a previous study. To measure ginsenoside Ro content in red ginseng from different years, an optimized extraction method was developed to determine ginsenoside Rg1, Re, Rb1 and Ro content by HPLC in 43 batches of red ginseng from different origins, growing years and manufacturers. The results indicate that the best extraction method was to ultrasonify a 1 g sample in 70% methanol for 50 min. The total running time of the optimized gradient was 50 min using a C18 core-shell column and was half the time described in the Chinese Pharmacopoeia, 2015 edition. The separation resolution of all of targeted compounds was greater than 1.6. The peak shape of ginsenoside Ro was optimal when the mobile phase consisted of acetonitrile and water with 0.1% phosphoric acid. The content of ginsenoside Ro was in the range of 0.11% to 0.43%, and the average content was 0.26%, which was higher than that of ginsenoside Rg1 and Re. The ratio of ginsenoside Ro and Re as a threshold could be used to discriminate red ginseng from different growing years; in addition, 100%, 94.4% and 46.6% of red ginseng from six, five and four years exceeded the threshold of 1.3. Our optimized analytical method for characterization of red ginseng is convenient and shortens the assay time.

OBJECTIVE:To invest igate the medication regulation of Tibetan medicine prescription of Punica granatum seeds, and to analyze the pharmaceutical property of it in the treatment of “Sanweihuo”(Me drod gusm) dysfunction induced-related diseases. METHODS :Using Clinical Notes of Tibetan Medicine ,Medicine Standard of the Ministry of Public Health :Tibetan Medicine as standard ,Tibetan medicine prescriptions of P. granatum seeds recorded in Sibu Yidian ,Yuewang Yaozhen and other medical records were collected ,and TCMISS V 2.5 software was used to analyze main indication and medicine frequency statistically. The core medicine combinations of the prescriptions were analyzed with Apriori algorithm (support degree ≥50%, confidence degree ≥90%),and medication regulations were analyzed. Tibetan medicine “Ro Nus Zhu rJes ”vector model was used to analyze the properties of Tibetan medicine prescription of P. granatum seeds in the treatment of high frequency diseases ,and K-means cluster analysis was carried out for seventeen effectiveness. RESULTS :A total of 300 Tibetan medicine prescriptions of P. granatum seeds were collected ,involving 104 kinds of main indications ,among which frequency of 18 kinds of indications was more than or equal to 10 times,including“non-indigestion syndrome ”(Ma Zhu ba) (76 times),“wood cloth disease ”(Smug po) (46 times),“edema”(Chu nad) (34 times),etc. Totally 322 kinds of Tibetan medicines were involved. Among them ,21 flavors were more than or equal to 50 in frequency ,including Piper longum (192 times),Amomum kravanh (164 times) and Carthamus tinctorius(134 times),etc. Fifty-six core combinations were obtained ,mainly including P. granatum seeds-P. longum (192 times), P. granatum seeds-A. kravanh (164 times) and P. granatum seeds-C. tinctorius (134 times). The prescriptions (13 pieces) with six flavors as pungent ,sweet and sour ,and with prominent medicinal properties such as roughness ,heat and dry effects could be used to treat “non-indigestion syndrome ”,and then included into type Ⅰ. The prescriptions (13 pieces) with six flavors as bitter ,sweet and pungent ,and with prominent medicinal properties such as cooling ,dull and heavy effects could be used to treat “wood cloth disease”,and then included into type Ⅱ. The prescriptions (11 pieces) with six flavors as sweet ,sour and pungent ,and with prominent medicinal properties such as dull ,heavy and moistening effects could be used to treat “edema”,and then included into type Ⅲ. CONCLUSIONS :Tibetan medicine P. granatum seeds is often used in combination with the isothermal medicines such as P. longum ,A. kravanh . P. granatum seeds are also commonly found in cool prescriptions ,revealing that Tibetan medicine pays attention to “Sanweihuo”while treating diseases ,which embodies the academic thought of Tibetan medicine that “there is cooling in temperature ,and there is temperature in cooling ”.

ABSTRACT

Objective To systematically investigate the chemical constituents of the roots of Panaxginseng. Methods Mild cold-soaked extraction by 70% aqueous ethanol, successive solvent extraction by ethyl acetate and n-butanol, column chromatography by D101 macroporous absorption resin and reversed-phase silica gel, and semi-preparative HPLC, were used for compounds isolation and purification, while high-resolution mass spectrometry, 1D and 2D NMR data were analyzed for compounds identification. Results A new oleanolic acid tetraglycoside (1) and 19 known ginsenosides (2-20) were isolated and identified. Compound 1 was identified as oleanolic acid 3-O-[β-D-glucopyranosyl-(1→2)-β-D-glucuronopyranosyl]-28-O-β-D-glucopyranosyl-(1→6)-β-D-glucopyranoside, named ginsenoside Ro1 (1). The 19 known ginsenosides were notoginsenoside FP1 (2), ginsenoside Re3 (3), notoginsenoside Rt (4), 20-O-glucosyl ginsenoside Rf (5), ginsenoside Re2 (6), ginsenoside Rg2 (7), ginsenoside Ra2 (8), ginsenoside Rb1 (9), ginsenoside Rc (10), ginsenoside Ra1 (11), malonylginsenoside Rb1 (12), malonylfloralginsenoside Rd5 (13), malonylginsenoside Rc (14), ginsenoside Ro (15), ginsenoside Rd (16), ginsenoside F2 (17), 20(R)-ginsenoside Rh2 (18), ginsenoside F3 (19) and ginsenoside F1 (20). Conclusion Compound1is a new compound. Compound 2, notoginsenoside FP1, is isolated from this plant for the first time.

ABSTRACT

Las Miopatías Inflamatorias Idiopáticas (MII) son un grupo heterogéneo de enfermedades que se caracterizan por debilidad muscular e inflamación subyacente en la biopsia muscular. Los principales órganos afectados son el músculo, la piel y también puede afectarse el pulmón. Se distinguen dentro de los subtipos clínicos como Polimiositis (PM), Dermatomiositis (DM), DM con la variante Dermatomiositis Clínicamente Amiopática (DMCA), el Síndrome Antisintetasa (SAS), la Miositis Necrotizante Inmunomediada, la Miositis por Cuerpos de Inclusión (MCI) y la Miositis Asociada a Neoplasia. La presencia de ciertos anticuerpos específicos y asociados predispone al desarrollo de manifestaciones clínicas, determinando el pronóstico de la enfermedad. Se presentan 4 pacientes del Registro de MII de la Sociedad Argentina de Reumatología (SAR) con estas características: un paciente con PM y anti Jo-1 positivo y tres pacientes con DM (uno con DMCA y anti- RO 52 y dos pacientes con anti-PL7 y anti-TIF1γ respectivamente).


Idiopathic Inflammatory Myopathies (MII) are a heterogeneous group of diseases characterized by muscle weakness and inflammation underlying muscle biopsy. The main organs affected are muscle, skin and the lung can also be affected. They are distinguished within clinical subtypes such as Polymyositis (PM), Dermatomyositis (DM), DM with the variant Clinically Amiopathic Dermatomyositis (DMCA), the Syndrome Antisynthetase (SAS), Immune-mediated Necrotizing Myositis, Body Myositis Inclusion (MCI) and Neoplasia-Associated Myositis. The presence of certain specific and associated antibodies predisposes to the development of clinical manifestations, determining the disease prognosis. 4 patients from the Registry of MII of the Argentine Society of Rheumatology (SAR) are presented with these characteristics: one patient with PM and anti Jo-1 positive and three patients with DM (one with DMCA and anti-RO 52 and two patients with anti-PL7 and anti-TIF1γ respectively).
