ABSTRACT

Polycystic liver disease is a rare inherited disorder which affects 1 in 1,00,000 population. There are two forms of polycystic liver disease (PLD) i.e. isolated PLD and autosomal dominant PLD which is associated with renal cysts. PLD is usually asymptomatic and incidentally detected. Some may present with complaints of abdominal distension, bloating, early satiety, weight loss. Computed tomography (CT) or magnetic resonance imaging is the investigation of choice to diagnose polycystic liver disease. Below we report you a case of 55-year-old female who came with the complaints of pain abdomen since 8 days and was diagnosed as polycystic liver disease on CT, who received treatment by laparoscopic fenestration surgery of cysts and showed symptomatic improvement.

Introducción: en Colombia, el último estudio de salud bucal evidenció que cerca del 70 por ciento de la población presenta edentulismo parcial mientras que el 5,2 por ciento habrá perdido todos los dientes entre los 65 y 79 años. La rehabilitación con implantes es una opción cada vez más utilizada, que requiere seguimiento clínico y radiográfico, siendo la radiografía panorámica una opción de bajo costo, en la que es posible observar zonas de pérdida ósea, angulación mesodistal del implante, relación con estructuras anatómicas y lesiones sugerentes de periimplantitis. Se requieren reportes y análisis de datos relevantes sobre los hallazgos en radiografías asociados a implantes dentales para determinar los factores de riesgo para su éxito, en los pacientes que los usan. Objetivo: determinar la prevalencia y caracterizar los hallazgos asociados a los implantes de oseointegración en radiografías panorámicas. Métodos: se realizó un estudio observacional descriptivo de corte transversal, con 10 000 radiografías panorámicas digitales seleccionadas por conveniencia de centros radiológicos en la ciudad de Bogotá, Colombia, de las cuales 543 correspondieron a la muestra analizada por la presencia de implantes. Se evaluó para cada implante: localización, posición, angulación y distancias con las estructuras adyacentes, utilizando el programa Clínicalview® (Orthopantomograph OP200D, Instrumentarium, USA). Resultados: La frecuencia de radiografías con implantes fue del 5,43 por ciento con un total de 1 791 implantes, con un promedio 3,2 por radiografía. Se encontraron en mayor proporción en el maxilar superior con una localización supracrestal y una angulación de 10,3 grados. El 32 por ciento presentaba distancias implante/diente o implante/implante inferiores a las óptimas. El 40,9 por ciento estaban restaurados y 1,2 por ciento mostraban lesiones compatibles con periimplantitis. Conclusiones: un alto porcentaje de los implantes revisados tiene un factor de riesgo que afecta su viabilidad a largo plazo ya sea por angulación, posición supracrestal o crestal, vecindad con dientes u otros implantes o bien por no ser restaurable(AU)


Introduction: the most recent survey about oral health conducted in Colombia revealed that nearly 70 percent of the population is partially edentulous, whereas 5.2 percent will have lost all their teeth by the time they are 65-79 years old. Implant rehabilitation is an ever more common alternative which requires clinical and radiographic follow-up, panoramic radiography being a low-cost option to observe areas of bone loss, mesiodistal angulation of the implant, relationship to anatomical structures and lesions suggesting peri-implantitis. Relevant data analyses and reports are needed about radiographic findings associated with dental implants to determine the risk factors for their success in patients wearing them. Objective: determine the prevalence of and characterize the findings associated to osseointegration implants in panoramic radiographs. Methods: a cross-sectional observational descriptive study was conducted of 10 000 digital panoramic radiographs chosen by convenience sampling at radiology centers from the city of Bogotá, Colombia, of which 543 made up the study sample, due to the presence of implants. Each implant was evaluated for location, position, angulation and distance from adjacent structures, using the software Clínicalview® (Orthopantomograph OP200D, Instrumentarium, USA). Results: the frequency of radiographs with implants was 5.43 percent with a total 1 791 implants and an average 3.2 per radiograph. Implants were more common on the upper maxilla with a supracrestal location and an angulation of 10.3 degrees. In 32 percent implant-tooth or implant-implant distances were below optimum values. 40.9 percent had been restored and 1.2 percent showed lesions compatible with peri-implantitis. Conclusions: a large proportion of the implants examined exhibit a risk factor affecting their long-term viability, namely angulation, supracrestal or crestal position, proximity to teeth or other implants, or not being restorable(AU)

ABSTRACT

Introdução: a osteonecrose dos maxilares associada ao uso de bisfosfonatos (OMMBF) é uma complicação bucal importante. Com o relato de casos clínicos atípicos, os quais apresentaram a OMMBF, o rompimento da mucosa bucal adjacente se encontrou ausente, levantou-se discussão sobre essa variante clínica. Objetivo: avaliar casos de osteonecrose medicamentosa dos maxilares (OM), sem exposição óssea clínica, diante da escassez de investigações sobre essa variante clínica. Metodologia: Após o parecer favorável do Comitê de Ética, foram avaliados, retrospectivamente, 35 prontuários e exa mes radiográficos de pacientes com o diagnóstico de OM. Foram incluídos no estudo somente os casos de OM sem exposição óssea clínica e excluídos os pacientes que foram tratados através de radioterapia de cabeça e pescoço, além dos que apresentaram exposição óssea clínica. Através dos prontuários selecionados, foram coletadas as seguintes informações: idade e gênero do paciente, tipo de doença sistêmica, tipo de bisfosfonato, tempo de uso do bisfosfonato, forma de administração do medicamento. Análise radiográfica foi realizada utilizando radiografia panorâmica. Os maxilares foram divididos em sextantes para avaliação da presença de: osteólise, sequestro ósseo, esclerose óssea, reação periosteal, anormalidades na lâmina dura, presença de fratura patológica. Resultados: apenas cinco pacientes foram incluídos neste estudo, sendo todos oncológicos e do gênero feminino. A idade média foi de 57.6 meses, o tipo de bisfosfonato foi o Zometa, administrado de forma intravenosa, com o tempo médio de 114 meses. Com relação ao estudo radiográfico, esclerose óssea foi a alteração mais encontrada, seguida de osteólise e anormalidades da lâmina dura. A mandíbula foi mais afetada que a maxila. Conclusão: através deste estudo, foi concluído que pacientes com OM associada ao uso de bisfosfonatos, sem exposição óssea, apresentaram alterações radiográficas importantes, enfatizando a importância de uma análise radiográfica criteriosa em pacientes que fazem o uso de drogas antirreabsortivas, na tentativa de prevenir ou diagnosticar precocemente as alterações ósseas.


Introduction: osteonecrosis of the jaw associated with the use of bisphosphonates (OMMBF) is an important oral complication. With the report of atypical clinical cases, which presented the OMMBF with absent rupture of the adjacent oral mucosa raised discussion about this clinical variant. Objective: the aim of this study was to evaluate cases of Medication-related osteonecrosis of the jaw (OM) without clinical bone exposure, given the paucity of research on this clinical variant. Methods: following the favorable decision of Ethics Committee, it was evaluated, retrospectively, medical records and panoramic radiographs of patients diagnosed with OM. The study included only cases of OM without clinical bone exposure. Patients treated by head and neck radiation therapy or presenting clinical bone exposure were excluded. Through the selected records were collected the following information: age and gender of the patient, type of systemic disease, type of bisphosphonate, time of use and its administration. Radiographic analysis was performed using panoramic radiograph. The jaws were divided into sextants to assess the presence of: osteolysis, bone sequestration, bone sclerosis, periosteal reaction, abnormalities in the lamina dura, presence of pathological fracture. Results: only five patients were included in this study, all of oncological and female. The average age was 57.6 months, the type of bisphosphonate Zometa was administered intravenously with the average time of 114 months. Regarding the radiographic study, bone sclerosis was the most frequent finding, followed by osteolysis and abnormalities of the lamina dura. The mandible was more affected than the maxilla. Conclusion: through this study, it was concluded that patients with OM without bone exposure, present significant radiographic changes, emphasizing the importance of radiographic analysis in patients who make use of antiresorptive drugs in an attempt to prevent or diagnose early bone changes.

ABSTRACT

As doenças inflamatórias intestinais (DIIs) compreendem, principalmente, a doença de Crohn (DC) e a retocolite ulcerativa (RU), ambas idiopáticas, porém relacionadas a uma resposta imunológica anormal à microbiota bacteriana da luz intestinal. Na RU a inflamação é difusa, restrita à mucosa e inespecífica, com comprometimento contínuo da parede, principalmente do reto, enquanto na DC as lesões são descontínuas, podem comprometer todas as camadas da parede e afetar qualquer parte do trato gastrointestinal. O quadro clínico é comum e compreende diarreia, febre e dores abdominais, podendo cursar também com manifestações extraintestinais. O diagnóstico é feito através dos dados clínicos, achados radiológicos e histológicos, sem haver, no entanto, nenhuma característica que isoladamente feche o diagnóstico de DII específica.


Inflammatory bowel diseases (IBDs) comprise mainly Crohn?s disease (CD) and ulcerative colitis (UC), both are idiopathic but believed to be related to an abnormal immune response to bacterial microbiota in the intestinal lumen. In RU diffuse inflammation is restricted to the mucosa and is nonspecific, with continued commitment that stars at rectum?s wall. In DC, the injuries are discontinuous, involve all layers of the intestinal wall and can affect any part of the gastrointestinal tract. The clinical picture of both is diarrhea, fever, abdominal pain, and may present with extraintestinal manifestations. The diagnosis is made by the junction of clinical, radiological and histological findings, without having, however, a feature alone that leads to a diagnosis of a specific IBD.

ABSTRACT

BACKGROUND: We assessed the plain radiographic characteristics of 10 cases of osteosarcomas during the initial painful period that had been overlooked by a primary physician. In addition, we evaluated chronologic changes in radiographic findings from initial symptomatic period to the time of accurate diagnosis. METHODS: The clinical records were reviewed for clinical parameters including age, sex, location, presenting symptoms, initial diagnosis, duration from initial symptoms to definite diagnosis, and initial and follow-up plain radiographic findings of the lesion. RESULTS: Initial clinical diagnoses included a sprain in 6, growing pain in 2, stress fracture in 1, and infection in 1 patient. Initial plain radiographic findings were trabecular destruction (100%), cortical disruption (60%), periosteal reaction (60%), and soft tissue mass (10%). Intramedullary matrix changes were osteosclerosis in 6 and osteolysis in 4 patients. On progression, 4 cases with minimal sclerosis changed to osteoblastic lesion in 3 patients and osteolytic lesion in 1. Four cases with faint osteolytic foci transformed into osteolytic lesion in 3 and mixed pattern in 1. CONCLUSIONS: Notable plain radiologic findings of incipient-stage osteosarcoma include trabecular disruption along with faint osteosclerosis or osteolysis. In symptomatic patients with trabecular destruction, additional imaging study including magnetic resonance imaging should be performed to exclude osteosarcoma in the incipient phase, even without radiologic findings suggesting malignant tumor, such as cortical destruction or periosteal reaction.

Objetivo: Avaliar 235 laudos de urografia excretora, descrevendo as principais alterações relacionadas com o trato urinário, testar a associação entre as alterações encontradas segundo sexo e faixa etária e discutir a utilidade do método radiológico de imagem. Métodos: Estudo transversal, descritivo realizado a partir de levantamento de 235 laudos de urografia excretora realizados no período de 14/09/2004 à 21/12/2005, no Serviço de Radiologia do Hospital Nossa Senhora da Conceição, Tubarão - SC. Resultados: Do total de 235 exames, 127 (54,04%) apresentaram alterações radiológicas, 93 (39,57%) foram normais e 15 (6,38%) foram excluídos por falha técnica, por serem inconclusivos, por ter idade inferior a 18 anos ou pelo exame não corresponder ao proposto pela avaliação do estudo. A alteração mais frequente foi a de litíase (76,3%), seguida da dilatação do sistema pielocalicial (26,8%). Observamos que 60 exames (47,24%) eram pacientes do sexo masculino e 67 (52,75%), do sexo feminino. A média de idade do estudo foi de 42,19 anos e a faixa etária que apresentou maior número de alterações foi a de idosos (70,58%), seguido do grupo de adultos (56,54%) e adultos jovens (44,44%). Conclusão: Com relação ao sexo, a única alteração com relevância estatística, foi a Estenose de JUP - presente, em sua totalidade, no sexo feminino - porém, com relação às faixas etárias estudadas, não foi observado nenhum achado radiológico com significância estatística. Apesar do advento de novas técnicas diagnósticas de imagem como o ultra-som, a tomografia computadorizada e a ressonância magnética, a urografia excretora continua tendo importante papel diagnóstico.


Background: To evaluate 235 reports of intravenous urography, analyzing the main alterations related with urinary tract, to test the association between the joined alterations according to sex and age, and to discuss the utility of the radiological method of image. Materials and Methods: Descriptive cross-sectional study based on data of 235 intravenous urography reports which was made between the period from September 2004 to December 2005, at the Radiological Service of Hospital Nossa Senhora da Conceição, Tubarão - SC. RESULTS: Of all the 235 exams, 127 (%) had presented radiologic alterations, 93 (%) were normal and 15(%) had been excluded by imperfection technique, for being inconclusive, being age lower than 18 years or for the exam not to correspond to the considered for evaluation of the study. The alteration most frequent was lithiasis (76,3%), followed by dilatation of the calyceal system (26,8%). It was observed that 60 exams (47,24%) were of the masculine sex e 67 (52,75%), of the feminine sex. The average age of the patients was 42,19 years and the age group which presented greater number of alterations was of aged (70,58%), followed by the group of adults (56,54%) and young adults (44,44%). Conclusions: With regard to the sex, the only alteration with relevance statistics, was stenosis of uretero-pelvic junction - present in its totality in the feminine sex - however, with regard to the studied ages, no radiological finding with statistic significance was found. Although the advent of new diagnostic techniques of image as the ultrasound, the computerized cat scan and the magnetic resonance, intravenous urography continues having important diagnostic paper.

ABSTRACT

Congenital lobar emphysema (CLE) is a life-threatening but potentially reversible lung abnormality of unknown etiology that can present a diagnostic dilemma. Imaging procedures are helpful for early diagnosis. This paper aims to present the scintigraphic features of CLE in a two-year-old male in correlation with other diagnostic modalities. Lung perfusion scintigraphy revealed globally diminished perfusion to the left lung. High Resolution Computed Tomography (HRCT) of the chest showed intrinsic narrowing of the distal left mainstem bronchus. CT scan of the Pulmonary Arteries (CTPA) showed a small left main pulmonary artery. Lung perfusion scintigraphy in CLE is characterized by markedly attenuated vascularity of the involved lobe with consequent decreased perfusion, and increased to normal perfusion in the unaffected lobe. It plays a vital role in the early diagnosis and management of pediatric pulmonary diseases presenting with a diagnostic dilemma, particularly in cases where it can preclude more invasive diagnostic procedures.

ABSTRACT

Os autores relatam o caso de um paciente de 14 anos de idade com imperfuração bilateral das coanas ao exame físico e que, à tomografia computadorizada, apresentou sinais compatíveis com atresia das coanas bilateralmente do tipo misto. Na prática clínica, é comum o encontro de atresiaunilateral de coanas em crianças e adolescentes; já a atresia bilateral pode ser potencialmente fatal e geralmente é diagnosticada em neonatos. O objetivo deste relato é demonstrar as características tomográficas dessa doença, incomum na faixa etária do paciente, e fazer uma revisão da literatura dos aspectos clínicos, métodos de diagnóstico e aspectos de imagem encontrados nos pacientes acometidos.


The authors report a case of a 14-year-old child with choanal bilateral imperforation observed in clinical examination. A multidetector computed tomography scan revealed bilateral choanal atresia. No other congenital anomaly was noted. With this relatewe aim to describe the clinic and radiographic aspects of the disease,associated with a review of the iterature.

ABSTRACT

Purpose: The purpose of our study is to examine early radiographic findings and clinical signs of failure of the microstructure surface HA-coated acetabular cup. Material and methods: This study included 41 revisions of 204 THA with hydroxyapatite-coated acetabular cup from April 1992 to November 1996. Radiolucent line around cups, change in the cup angle and osteolysis were evaluated in serial radiographs. Acetabular cup was defined as loosening if any movement occurred at the bone and socket interface by manual rotation and extension force intraoperatively or migration of more than 2 mm and change of the cup angle by more than 5 degrees in radiographs. Results: The radiolucent line was observed in 12 of the 41 hips at an average of 55.4 months postoperatively and was most commonly located in zone III. All of the 12 hips showing radiolucent lines were classified as loosening by radiographically in 8 cases and intraoperatively in 4 cases. Radiographic loosening occurred at an average of 28.8 months after the appearance of a radiolucent line. Inguinal pain was noted in 18 of the 19 cases revised for acetabular cup loosening. Conclusion: The most important radiographic finding for the early diagnosis of loosening was progressive radiolucent lines, which occurred most frequently in zone 3.

OBJECTIVE: The purpose of this study was to evaluate the whole spine of the patients with chronic low back pain. METHOD: The cervico-thoraco-lumbar spine were evaluated in 128 patients with chronic low back pain. We analyzed radiologic and clinical findings of the cervico-lumbar spine and electrodiagnostic findings. RESULTS: Most of all cases showed abnormal findings in plain radiography and electrodiagnostic study. The radiologic findings were as follow: cervical X-ray with straightening of cervical lordotic curve in 85 cases; thoracic X-ray with scoliosis in 55 cases; lumbar X-ray with disc space narrowing in 85 cases. The electrodiagnostic study revealed lumbosacral radiculopathy in 87 cases. There were significant positive relationship between increased lumbosacral angle and straightening of cervical lordotic curve, and between lumbar scoliosis and thoracic scoliosis. CONCLUSION: Cervico-thoracic spinal abnormalities were shown in most of the patients with chronic low back pain. Therefore, The evaluation of whole spine would be needed comprehensive rehabilitation approach for the patients with chronic low back pain.

ABSTRACT

PURPOSE: The chest radiograph is useful and reliable in assessing the severity and progression of neonatal respiratory distress syndrome. To evaluate the effect of exogenous surfactant r eplacement therapy, we performed sequential observation of chest radiogratphic findings in neonates with respiratory distress syndrome. METHODS: Two groups of infant with RDS in mechanical ventilation therapy were studied. Surfactant(S-Tx) group(n=36) was treated with Surfactant-TA and control group(n=19) was without use of surfactant. Observation of gestational age, birth weight, clinical severity by ventilatory index, sequential change of radiographic findings and radiological complications in RDS patients were performed. RESULTS: In RDS patients, there was correlation in the radiologic grading and clinical severity. In surfactant treated infants, change of chest radiographic finding was significantly improved than control group. Incidence of pneumothorax and or pulmonary interstial emphysema in S-Tx group was less than that of control group. CONCLUSIONS: Exogenous pulmonary surfactant replacement therapy may contribute the improvement of the clinical and radiological severity and reduction of the incidence of clinical complications. We suggest that surfactant replacement therapy is effective and important useful therapeutic method in neonates with respiratory distress syndrome.
