ABSTRACT

To clarify the research progress and existing problems in the clinical research on oral Chinese patent medicine in the treatment of breast cancer. National drug catalogs and databases were searched for oral Chinese patent medicines against breast cancer. Chinese and English databases were searched for clinical trials on oral Chinese patent medicines in the treatment of breast cancer. The main functions,drug composition,literature basic characteristics,intervention measures, and outcome indicators of Chinese patent medicines were comprehensively analyzed and evaluated by a scoping review based on visual charts. In 68 Chinese patent medicines included,53% of them were potent in activating blood, resolving stasis, tonifying the kidney, and strengthening the spleen. In terms of frequency, the top drugs were Ligustri Lucidi Fructus, Astragali Radix, Moutan Cortex, Poria, and Olibanum,indicating the importance of the therapeutic principles of activating blood, resolving stasis, tonifying the kidney, and strengthening the spleen for breast cancer treatment. In 161 included clinical trials,70.71% of the intervention measures were Chinese patent medicine combined with chemotherapy. Literature studies mainly reported Xihuang pills and Huaier granules,which may be related to the combination of chemotherapy and the inclusion in the medical insurance. The outcome evaluation was mainly based on immune indexes,tumor markers,Karnofsky score,safety report and objective response rate,while the survival time and characteristic outcome indicators of traditional Chinese medicine (TCM) were insufficient. The application and research on oral Chinese patent medicine in the treatment of breast cancer have advanced in recent years,but most of the clinical research evidence is not robust,and the evaluation of outcome indicators is not standardized,which needs to be improved.

ABSTRACT

OBJECTIVE: To perfect China's pediatric knowledge database of rational medication use, to reduce the incidence of medication errors and to improve the safety and effectiveness of medication therapy in children. METHODS: Construct the pediatric prescription knowledge database by accessing relevant handbooks, guidelines, online databases and researches, and embed it into the prescription pre-audit intelligent decision system. Based on this, the system could automatically recommend dosage and usage information to the physicians during prescribing, it could also pre-screen and intercept improper medical orders and prescriptions according to different security levels. RESULTS: From November 2018 to April 2019, the prescription Pre-audit Intelligent Decision System reviewed a total of 906,945 inpatients orders. Due to the use of the Pediatric Prescription Knowledge Database, the inpatients' precaution, relative contraindication and absolute contraindications orders presented a remarkably decline, from 71.71%, 5.45% and 0.12% respectively, to 35.86%, 3.51% and 0.08%. The physician's fitness to the system and rationality of medication increased month by month. CONCLUSION: The use of the Pediatric Prescription Knowledge Database significantly reduces the incidence of medication errors in hospitalized children and improves the level of medical homogenization. It is a medical service model worthy of consideration.

ABSTRACT

Objective To understand the application situation of Chinese patent medicine during 2014-2017 in Changshu No.1 People's Hospital (hereinafter referred to as "our hospital"); To provide references for clinical rational medication. Metheds The data of Chinese patent medicine used in our hospital in 2014-2017 were retrieved from our hospital HIS. The consumption sum, annual growth rate, DDDs, sorting ratio and DDDc of Chinese patent medicine were analyzed statistically according to the defined daily dose recommended by WHO. Results During 2014-2017, the consumption sums of Chinese patent medicine were 49.155 2 million yuan, 51.352 7 million yuan, 49.463 7 million yuan and 45.650 4 million yuan, respectively. The consumption sums of Chinese patent medicine of 2015, 2016 and 2017 increased by 4.47%, -3.68% and-7.71%, respectively. The proportions of consumption sum of Chinese patent medicine in all medicines were 13.63% in 2014, 13.36% in 2015, 12.83% in 2016 and 12.44% in 2017. Of all the dosage forms of Chinese patent medicine, capsule (32.67%), injection (25.21%) and tablet (14.71%) ranked the top 3 on the list of consumption sum. Of all the single kinds of Chinese patent medicine, blood rationing medicine, heat-clearing medicine and tonifying medicine ranked the top 3 on the list of consumption sum. The blood rationing medicine, gynecologic medicine and tonifying medicine ranked the top 3 on the list of DDDs. The first varieties of single item of consumption sum were salvianolate injection, Pudilan Xiaoyan Oral Liquid and Xueshuantong Injection (freeze-dried). The first varieties of single items of DDDs were Yinxing Tongzhi Dispersible Tablet and Pudilan Xiaoyan Oral Liquid. Conclusion The use of Chinese patent medicine in our hospital is becoming more reasonable, and the role and advantages in the prevention and control of diseases are constantly emerging. However, there are still problems about irrational medication, which need to be further strengthened.

ABSTRACT

TCM incompatibility means that the combinations of some Chinese materia medica would produce or enhance side effects, or even reduce or damage medicine efficacy. Therefore, clinical incompatibility should be avoided. The core idea of TCM incompatibility is "Eighteen Clashes" and "Nineteen Incompatibilities". China Pharmacopoeia clearly states that "Eighteen Clashes" and "Nineteen Incompatibilities" belong to incompatibility. This article summarized the incompatibility in TCM patent medicine used in Guang'anmen Hospital of Chinese Medicine Academy of Sciences, with a purpose to provide references for clinical doctors and pharmacists.

ABSTRACT

Objective To understand the present situation and regularity of the application of TCM prescriptions in Chongqing Hospital of Traditional Chinese Medicine (hereinafter referred to as "our hospital"); To provide references for TCM pharmacy management and rational medication in clinic. Methods 8756 TCM prescriptions in our hospital from January to December in 2016 were randomly selected and the method of subtotals was used to collect statistics. The basic rules and existing problems of the TCM prescriptions were analyzed. Results The TCM prescriptions issued more in the famous doctor's center (27.67%), preventive treatment of disease center (13.67%) and oncology department (13.35%); elderly patients took the lead (45–75 years old accounted for 65.90%); prescriptions with 16–20 Chinese herbs added up to 4326 (49.41%); prescriptions within 3–7 dosages were 4921 in total (56.20%);single prescription cost within 100–300 yuan were 2777 in all (31.71%); prescriptions of single dosage cost 21–80 yuan were 5500 altogether (62.81%). Conclusion The application of TCM prescriptions in our hospital is basically rational, but problems such as too many herbs in one prescription, overdose use of some prescriptions and higher amount still exist.

ABSTRACT

Objective To analyze the clinical utility of TCM decoction pieces for reinforcing deficiency in the First Affiliated Hospital of Xi'an Jiaotong University (hereinafter referred to as our hospital).Methods By using Microsoft Office Excel 2013 software, use frequency, classifications of pharmacological actions and doses of the TCM decoction pieces for reinforcing deficiency from 2013 to 2015 were analyzed.Results From 2013 to 2015, some TCM decoction pieces for reinforcing deficiency showed relatively high use frequency in our hospital (>30%). In top 15 TCM decoction pieces for reinforcing deficiency, the amounts of medicine for tonifying qi were more than 40%, whose average doses exceeded the upper limit of pharmacopoeia dosage reaching 30%. The total over-use rate of TCM decoction pieces for reinforcing deficiency>50% and the not exceeded part accounted for 36.0%.Conclusion It is suggested that the use of TCM decoction pieces should be careful differentiation of symptoms, and the dose should also meet the specifications.

OBJECTIVE: Through the public platform of WeChat, promote the practice of obstetrics and gynecology medication, research the spreading methods of information and put forward feasible proposal for the propaganda of rational medication. METHODS: Promote the rational drug use information of obstetrics and gynecology by established WeChat platform. Analyze our hospital by the condition of WeChat platform. RESULTS: Up to March 2017, the totality of users who followed our WeChat public platform reached to 1 113 and the totality of readership reached to 71 461. CONCLUSION: S Promote rational medication by WeChat platform. Adjust the content of the information to be promoted by data analysis. We can provide better service to the people in the information age with effective transmission.

ABSTRACT

Objective:To analyze the adverse drug reactions(ADR)and clinical application of thalidomide to provide useful reference for rational medication in clinics. Methods:The case reports and literatures from foreign countries on the clinical medication of thalidomide were analyzed and summarized. Results:The ADR of thalidomide included gastrointestinal reaction, hematotoxicity,cadiovascular toxicity,neurotoxicity,skin lesion,pulmonary embolism and so on. Its new medication methods were widely used in clinics. Conclusion:Clinicians and pharmacists should pay more attention to the medication risks and rational use of thalidomide in order to assure the safety and effectiveness of clinical drug use.

Objective To understand the clinic usage of TCM decoction pieces in the First Affiliated Hospital of Xi’an Jiaotong University (hereinafter referred to as our hospital).Methods By using Microsoft Office Excel software, the medicine doses, classification of functions and clinic application of the TCM decoction pieces, which were among the top 50 most frequently used TCM decoction pieces were analyzed.Results In 2014, 60% of the top 50 most frequently used TCM decoction pieces in our hospital exceed the medicine dosage inChinese Pharmacopoeia. According to the functions, the top 3 TCM decoction pieces were medicine for reinforcing deficiency, clearing heat and promoting qi, respectively. Medicine for reinforcing deficiency was mainly used in gastroenteropathy, menopathy and nephropathy. Medicine for clearing heat was mainly used in gastroenteropathy, menopathy and infection of the upper respiratory tract. Medicine for promoting qi was mainly used in gastroenteropathy, tumor and disease of cardiovascular system.Conclusion TCM decoction pieces in our hospital exceed the medicine dosage in Chinese Pharmacopoeia, which requires attention. The top 3 TCM decoction pieces are respectively medicine for reinforcing deficiency, clearing heat and promoting qi, whose clinic applications are basically rational.

ABSTRACT

OBJECTIVE:To explore how to establish good procedure,system and mode of drug management,dispensing and application,in order to provide reference for risk management and rational use of rabies vaccine. METHODS:The use of rabies vaccine and storage temperature monitoring in our hospital during 2013-2014 were analyzed statistically,and risk management and use of rabies vaccine in our hospital were analyzed,and management measures and attentions were put forward. RESULTS:The amount of vaccinum rabiel (Vero cell) and human rabies immunoglobulin in our hospital were increased in 2014,compared to 2013. The position labeled with #1 in storehouse and that of labeled with #1 and #2 in dispensing store could meet the storage condi-tion of rabies vaccine. Risk management could be carried out in fields of drug requisition,cold chain management,drug manage-ment and drug dispensing. ADR should be paid attention. CONCLUSIONS:It is of significance to develop risk management and ra-tional medication guidance of rabies vaccine.

ABSTRACT

Objective To understand the rules and characteristics of adverse drug reaction ( ADR) in Xinhua Hospital of Xinhua Medical Group ( Huainan City Anhui Province )′s traditional Chinese medicine ( TCM) injection ca-ses,and to provide reference for rational and safety usage of TCM injection .Methods Reorganization was preformed on 663 cases of ADR report from January 2010 to November 2013 in this hospital .We selected 103 cases of TCM in-jection′s ADR reports,and made statistics and analysis according to patients′age,sex,drug source,report source,eval-uation,clinical features,system and organ ADR involved,etc.Results 103 cases of ADR reports involved in 26 kinds of drugs.All of them were vein medication .87.37%of the above cases were happened in 30 minutes with gender ratio of 60:43.54.36%were old patients over 60 with the highest incidence rate .33.98%were skin and appendages dis-orders.14.56%were Breviscapine ADR reports which was the most one .Conclusion It must insist on monitoring ADR of TCM injection and re-evaluation of market products ,to use TCM injection normative and rational ,and guaran-tee the medication security .

ABSTRACT

OBJECTIVE:To investigate the drug use in the inpatients of our hospital and evaluate its rationality.METHODS:By using PASS-nested "Army Health No.1" hospital information system,the drug utilization in these patients was analyzed.RESULTS:Among 32 834 medication orders for 586 inpatients,732(1.50%)were warning ones.CONCLUSION:The drug utilization in the inpatients of our hospital was rational on the whole.PASS satisfies clinical doctors' needs,and the physicians feel satisfied with the system.Nevertheless,there are some problems remain to be tackled.
