ABSTRACT

En algunos estudios se ha asociado a la terapia de reemplazo hormonal (TRH) con estrógenos y progestinas a un mayor riesgo de cáncer de mama que la terapia con estrógenos solos. Sin embargo, dependiendo de su naturaleza algunas progestinas serían más seguras que otras. Se buscaron y analizaron artículos atingentes al tema en las bases de datos Google Scholar, PubMed, Science, SciELO y Cochrane, introduciendo los siguientes términos: terapia de reemplazo hormonal y cáncer de mama, progestinas y cáncer de mama, receptor de progesterona. Específicamente se ha asociado a las progestinas sintéticas acetato de medroxiprogesterona, noretisterona y levonorgestrel con un mayor riesgo de cáncer de mama, no así a la progesterona natural, a la progesterona oral micronizada ni a la didrogesterona. La progesterona natural, progesterona micronizada y didrogesterona serían más seguras en TRH para evitar el desarrollo de cáncer de mama, lo que estaría dado por la mayor especificidad en su acción.


In some studies, hormone replacement therapy (HRT) with estrogens and progestins has been associated with a higher risk of breast cancer than therapy with estrogens alone. However, depending on their nature, some progestins may be safer than others. This article analyzes the mode of action of progesterone in breast tissue and also the role of some progestins in the development of this pathology. Articles related to the subject were searched for and analyzed in Google Scholar, PubMed, Science, SciELO and Cochrane databases, introducing the following terms: hormone replacement therapy and breast cancer, progestins and breast cancer, progesterone receptor. Specifically, synthetic progestins medroxyprogesterone acetate, norethisterone, and levonorgestrel have been associated with an increased risk of breast cancer, but not natural progesterone, micronized oral progesterone, or dydrogesterone. Natural progesterone, micronized progesterone and dydrogesterone would be safer in HRT to prevent the development of breast cancer, which would be due to the greater specificity of their action.

ABSTRACT

Resumen ANTECEDENTES Las malformaciones arteriovenosas uterinas se reportan con poca frecuencia, en la bibliografía existen documentados alrededor de cien casos; por esta razón su incidencia no puede establecerse. Afecta física y emocionalmente a las mujeres; se manifiesta con hemorragia uterina anormal de difícil control. La identificación oportuna y el tratamiento eficaz pueden cambiar el pronóstico y disminuir la morbilidad y mortalidad. CASO CLÍNICO Paciente de 22 años, con diagnóstico ultrasonográfico e histeroscópico de malformación arteriovenosa uterina. Debido al deseo de embarazo se trató con un modulador selectivo de receptores de progestágenos, sin respuesta adecuada. Se practicó la fistulectomía transabdominal con una técnica innovadora en la que transoperatoriamente se delimitó, con auxilio de ultrasonido Doppler, el área que abarcaba la malformación arteriovenosa uterina para su resección quirúrgica sin afectación de la mayor cantidad de tejido sano con un resultado exitoso.


Abstract BACKGROUND Uterine arteriovenous malformation (AVM) are a rare condition with approximately 100 cases reported in literature. This pathologic change affects women´s emotional and physical condition, showing as difficult control abnormal uterine bleeding. Early diagnosis and proper treatment can lead to morbimortality reduction and better prognosis. CLINICAL CASE We present the case of a female 22 years old, diagnosed with a uterine AVM by hysteroscopy and ultrasound. Management were initially done with selective progesterone receptor modulator due to the fertility desire, without good response. A fistulectomy was realized with an innovating technique involving the use of Doppler ultrasound to guide de procedure limited to the affected area, preserving healthy tissue with a successful response.

El comportamiento reproductivo de los ovinos varía entre las diferentes razas. Dentro de los factores que inciden en la capacidad reproductiva, el nivel de expresión de receptores de estrógenos y progesterona en su tracto genital parece tener un rol relevante. En el endometrio, oviducto y ovario los estrógenos y progesterona, regulan la expresión de numerosas proteínas comprometidas en su desarrollo morfofuncional. Factores genéticos como la raza estarian relacionados con los niveles de expresión de estos receptores. Las ovejas de raza Texel tienen elevados índices de fertilidad, son muy prolíficas y presentan un alto porcentaje de gestación múltiple lo cual podría tener relación con la expresión de estos receptores en el tracto reproductivo. El objetivo del presente estudio fue evaluar la expresión de receptores de estrógenos y progesterona en tracto genital de ovejas de raza Texel de alta prolificidad. La expresión de la proteína receptora de ambos receptores se detectó mediante análisis inmunohistoquímico y el nivel de expresión de los transcritos por RT-PCR en Tiempo Real Cuantitativo. Los resultados muestran expresión inmunohistoquímica del receptor de estrógeno principalmente en zonas glandulares y carunculares del endometrio. Se destaca además una menor expresión de ambos receptores en ovario, epitelio del oviducto y cervix. La expresión del receptor de progesterona a nivel inmunohistoquímico es bastante menor en cuanto a la señal destacándose marcas débiles en endometrio y ovarios. El nivel de expresión de los transcritos mantiene la misma distribución que las señales inmunohistoquimicas para ambos receptores. Concluimos que ambos receptores son expresados en el sistema reproductivo de ovejas de raza Texel en una distribución similar a lo encontrado en otras razas de ovejas quedando por definir si los niveles de expresión son similares en las distintas razas.


Reproductive performance of sheep varies between different races. Among the factors that affect the reproductive capacity, the level of expression of estrogen and progesterone receptors in the genital tract appears to have a relevant role. In the endometrium, oviduct and ovarian estrogen and progesterone regulate the expression of numerous proteins involved in morphofunctional development. Genetic factors such as race would be related to expression levels of these receptors. The Texel ewes have high fertility rates, are very prolific and have a high percentage of multiple births which could be related to the expression of these receptors in the reproductive tract. The aim of this study was to evaluate the expression of estrogen and progesterone receptors in the genital tract of ewes and Texel high prolificacy. The expression of the receptor protein of both receptors was detected by immunohistochemistry and the level of expression of the transcripts by Quantitative RT-PCR Real-Time. The results showed immunohistochemical expression of estrogen receptor mainly in areas of glandular and caruncular endometrium. It also highlights a lower expression of both receptors in ovarian tissue, epithelium of the oviduct and cervix. Expression of progesterone receptor immunohistochemical level is much lower with weaker signal in endometrium and ovaries. The expression level of transcripts remains the same distribution as immunohistochemical signals for both receptors. We conclude that both receptors are expressed in the reproductive system Texel ewes in a distribution similar to that found in other breeds of sheep, must be defined if the levels of expression are similar in different races.

ABSTRACT

Los efectos de la nutrición en reproducción ovina han sido estudiados por numerosos autores. Principalmente, el interés se ha centrado en los efectos de la calidad de la dieta ya que en ovinos sometidos a planos nutricionales bajos se ha observado anormalidades del embrión, fase lutea inadecuada, deficiencia en el aporte de progesterona al útero y falla en los mecanismos que comprenden el reconocimiento materno a la preñez. Existen trabajos sobre la relación alimentación-hormonas sin embargo la información existente sobre los receptores de hormonas esteroidales endometriales, indispensables para que ellas actúen, es muy escasa. El objetivo del presente estudio fue analizar comparativamente la expresión del receptor de estrógenos (RE) en el endometrio de ovejas en ciclo alimentadas con dieta suplementada y dieta normal. La expresión de la proteína receptora y del transcrito se detectó mediante análisis inmunohistoquímico y RT-PCR en tiempo real respectivamente. Los resultados muestran expresión inmunohistoquímica en zonas glandulares y carunculares, destacándose una intensa inmunorreacción en núcleos de células estromales y del epitelio glandular. Se detecto mayor expresión del transcrito del RE en endometrio de ovejas alimentadas con suplemento respecto a las que no recibieron suplemento alimenticio. Se discute el posible uso de esta información para aplicarla en programas de mejoramiento reproductivo en ovinos.


The effects of nutrition on sheep reproduction have been studied by many authors. The interest has focused on the effects of diet quality in sheepin in which low plans n have low plane nutrition can cause abnormalities of the ovum or the embryo, luteal inadequancy and failure of the supply of progesterone to the uterus or failurein the mechanisms involved maternal of pregnancy. There are papers about the relation food-hormones, however, the existing information on endometrial steroid hormone receptors, which are essential for them to act, is very scarce. The aim of this study was to comparatively analyze the expression of estrogen receptor (ER) in endometrial cycle of sheep fed diet supplemented and normal diet. The expression of the receptor protein and the transcript were detected by immunohistochemical analysis and RT real-time PCR, respectively. The results showed immunohistochemical expression of RE in glandular and carunculares areas, especially in an intense inmunorreacción stromal cell nuclei and glandular epithelium. Detected increased expression of transcripts of RE in sheep fed a supplement with respect to not receiving a dietary supplement. We discuss the possible use of this information for application in breeding sheeps programs.

ABSTRACT

Hormone receptor and Her2 protein overexpression evaluated by immunohistochemistry (IHC) is widely validated as a predictive factor in breast cancer. The quality of the IHC reaction is influenced by tissue fixation and processing. Over- and underfixation deeply affect IHC results. Antigen retrieval may improve IHC but it does not recover tissue from autolysis or overfixation. The choice of primary antibody for IHC as to its sensitivity and specificity in relation to therapeutic response represents an important stage. Apart from mouse monoclonal antibodies, new rabbit monoclonal antibodies are commercially available, such as clones anti-ER SP1 and B644, anti-PR SP2 and B645 and anti-Her2 SP3 and 4B5. They represent an alternative to hormone receptor and Her2 evaluation by IHC. New polymeric non-biotinylated detection systems are also available and allow accurate and strong marking with no stromal and no non-specific cytoplasmic staining due to endogenous biotin. The most recommended cut off for estrogen and progesterone receptors (ER and PR) is more than 1 percent of positive cells with moderate or strong staining intensity (Allred's scoring system). New guidelines for Her2 evaluation by IHC show a cut off of more than 30 percent of positive cells with strong intensity (3+) that correlates better with gene amplification. The 2+ cases are now considered indeterminate and should be confirmed by fluorescence in situ hybridisation (FISH) or chromogenic in situ hybridisation CISH. A quality control of pre-analytical, analytical and post-analytical phases of IHC is recommended in order to optimize results.


A superexpressão de receptores hormonais e Her2 avaliada pela imuno-histoquímica (IHQ) é amplamente validada como fator preditivo em câncer de mama. A qualidade da reação imuno-histoquímica é influenciada pela fixação do tecido e seu processamento. A fixação insuficiente ou demasiada afeta profundamente os resultados da IHQ. A reativação antigênica pode melhorar os resultados da IHQ, porém não recupera tecidos com autólise ou com excessiva fixação. A escolha do anticorpo primário para a IHQ, considerando sua sensibilidade e sua especificidade de acordo com a resposta terapêutica, representa uma importante etapa. Além de anticorpos monoclonais de camundongo, novos anticorpos monoclonais de coelho são comercialmente disponíveis, tais como clones SP1 e B644 anti-RE, SP2 e B645 anti-RP, e SP3 e 4B5 anti-Her2. Eles representam uma alternativa para avaliação de receptores hormonais e Her2 através da IHQ. Novos sistemas de detecção poliméricos não-biotinilados também são disponíveis e permitem marcação exata e forte sem marcação estromal ou citoplasmática inespecífica devido à biotina endógena. O cut off mais recomendado para receptor de estrogênio (RE) e receptor de progesterona (RP) é acima de 1 por cento de células positivas com marcação moderada ou forte (sistema de escore de Allred). Novas recomendações para avaliação de Her2 através da IHQ apontam um cut off de mais de 30 por cento de células positivas com marcação forte (3+), que melhor se relaciona com amplificação gênica. Os casos 2+ são agora considerados indeterminados e devem ser confirmados por hibridação in situ por fluorescência (FISH) ou hibridização in situ colorimétrica (CISH). Um controle de qualidade de fases pré-analítica, analítica e pós-analítica da IHQ é recomendado para a otimização dos resultados.
