ABSTRACT

Since December, 2019, lots of corona virus disease 2019 (COVID-19) cases have emerged. The situation of prevention and control is grim. Rehabilitation hospitals are different from general hospitals in prevention and control of infection. Therefore, supported by the Committee of Rehabilitation Institution Management of Chinese Association of Rehabilitation Medicine and so on, experts in rehabilitation and infection control formulated this Consensus of Prevention and Control Strategies in Rehabilitation Hospital under Epidemic of Coronavirus Disease 2019, to guide the working procedures of preventing COVID-19 in rehabilitation hospitals.

ABSTRACT

Objective To evaluate the status of rehabilitation resource configuration and rehabilitation service of health system in Guangdong province,and to provide basis for the development of rehabilitation institutions. Methods From November to December in 2015, an online survey was conducted among rehabilitation hospitals in Health and Family Planning Commission of 21 cities in Guangdong province by using self-administered ques-tionnaire.The deadline was December 30th,2015. Results There were 38 rehabilitation hospitals in Guangdong province including 18 non-profit and 20 for-profit ones. Only one was tertiary rehabilitation hospital.There were totally 4524 open beds,hospitals with 50 to199 beds ac-counted for 68.4%. There were 37,262 rehabilitation equipments worth more than 10,000 Yuan, in which less than 500,000 Yuan was the most.There were 776 rehabilitation doctors,1051 nurses,749 therapists,and 248 oth-er staff members. The annual hospital admissions were more than 55,000, hospital bed utilization rate was 74.6%,bed turnover rates was 12.75,the working time of bed was 272.3 days,and the average length of stay in hospital was 19.5 days. Conclusion These rehabilitation institutions cannot meet the need of people in Guangdong province.Many problems ex-isted,such as low hospital scales,unbalanced geographical distribution,low rates of hospital beds utilization and turnover,inadequate hospital management and imperfect health system.The service and management capabilities of rehabilitation institutions should be strengthened.

ABSTRACT

Objective To investigate the epidemiological characteristics and risk factors for healthcare-associated infection(HAI)in a newly built rehabilitation hospital.Methods A prospective survey was adopted to monitor HAI among all patients admitted to a newly built rehabilitation hospital between January 2011 and December 2014, related data were collected and analyzed statistically.Results A total of 22 126 patients (male,n=12 023;female, n=10 103)were monitored,age were 4 months-100 years old.HAI rate was 3.25%(n=720),HAI case rate was 3.34% (n=738).Patients in department of spinal rehabilitation had the highest HAI rate (9.76%);the main HAI site was lower respiratory tract (n = 388,52.57%);a total of 186 pathogenic isolates were detected,154 (82.80%)were gram-negative bacteria,25 (13.44%)were gram-positive bacteria,and 7 (3.76%)were fungi. Multivariate logistic regression analysis showed that paralysis (OR,1 .77 [95% CI ,1 .24 - 2.53 ]),length of hospital stay ≥60 days(OR,4.62 [95% CI ,3.28 -5.10]),age ≤ 10 years old (OR,1 .55 [95% CI ,1 .33 -2.93]),age ≥60 years old (OR,4.59 [95% CI ,1 .02-20.59]),chronic underlying diseases(OR,1 .56 [95% CI , 1 .37-11 .34]),invasive procedure (OR,3.33 [95% CI ,1 .21 -6.86]),and coma(OR,6.77 [95% CI ,5.41 -7.05])were major risk factors for HAI in patients in the rehabilitation hospital.Conclusion Investigation on newly built rehabilitation hospital is helpful for realizing the occurrence status of HAI,health care workers can carry out the corresponding prevention and control measures.

ABSTRACT

PURPOSE: This study was to examine the relationships between nurse's perception of patient safety culture and performance for safety nursing activities at rehabilitation hospitals. METHODS: This study applied a descriptive research design. Participants were 194 nurses who have provided nursing services for more than 6 months at 4 rehabilitation hospitals located in B metropolitan city. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffé test, Pearson's correlation coefficients and multiple regression. RESULTS: There was a positive correlation between the awareness of patient safety culture and safety nursing activity. Multivariate analysis showed that hospital work environment, experience of education, hospital climate, frequency of reported events, and marital status were significantly associated with the safety nursing activity. Overall, approximately 23.1% of total variability in the safety nursing activity could be explained by the 5 variables (R²=0.231, p<.005). CONCLUSION: Nurses at rehabilitation hospitals are relatively positive about patient safety culture. Therefore, we need to develop safety education programs at the level of organization in order to improve patient safety through performing effective safety nursing activities in addition to increase awareness of patient safety culture among nurses. Furthermore, we need hospital's strategies at the system level for open communication and outcome reports regarding patient safety.

Methods 43 nursing students of grade 2011 were as control group, 53 ones of grade 2012 as intervention group. The intervention group was introduced the definition,the harm of needle stick injury and the basic protection knowledge, shown the scene of needle stick injuries often occurred, and trained to prevent the needle stick injury a day before internship. The control group was not given any intervention. Results 48.84% and 20.75% of students of the control and the intervention groups occurred needle stick injury during the internship, and the frequencies were 1.36 and 0.42 per person-year, respectively (P<0. 05). Conclusion Scene simulation teaching can effectively reduce the incidence of needle stick injuries of nursing students, that is valuable for the occupational protective education of nursing students.

Este artigo pretende mostrar como ocorre o acompanhamento terapêutico, as dificuldades encontradas e o cruzamento entre prática e teoria, a partir da experiência com um paciente fisioterápico. No texto discutiremos um pouco sobre transferência, contratransferência, cinco estágios sobre a morte e o morrer que foram cunhados por Kübler-Ross, e verificados no contexto hospitalar em pacientes fisioterápicos. Pensaremos os significados criados pela paciente a respeito da sua vida e corpo, a partir de uma visão antropológica de sua história. O nosso relato será embasado e correlacionado às experiências de vida que levaram a paciente à angustia atual.


This article intends to show what happens in a therapeutic nursing practice, expliciting the difficulties and the connections between practice and theory. Through the experience of nursing a patient, we are going to explain what can be considered nursing. In this text we are going to discuss about transference and countertransference during psychotherapy, five stages about the death, and to die considering Kübler-Ross's works, and verify them in a rehabilitation hospital's context. We are going to regard the patient's thoughts and how she deals with her life and her own body, concerned about her background. Our study is based and related to the patient's experiences that leads him to his state now.

@#Objective To explore the effect of home-based rehabilitation on cerebral palsy.Methods 60 children with cerebral palsy were assigned to hospital-based rehabilitation group and hospital-home-based rehabilitation group and treated for 1 year.Every child was evaluated before and after the treatment with Bayley Scales of Infant Development(BSID).The Mental Development Index(MDI) and Physical Development Index(PDI) were compared between these two groups.Results After one year,the MDI and PDI improved in both groups(P<0.05),while those of the hospital-home-based rehabilitation group was better than of the hospital-based rehabilitation group(P<0.05).Conclusion Home-based rehabilitation plays an important role in the rehabilitation of cerebral palsy children.

ABSTRACT

Recently the numbers of persons with disabilities and aged are rapidly increasing every year. And the scope of persons with disabilities is much more diversified. According to the recent report of the desire for the persons with disabilities, it is noted that medical rehabilitation service is most insufficient. But compared to the numbers and needs of persons with disabilities, the numbers of medical rehabilitation facilities are still much insufficient. The important factors to be considered in rehabilitation hospital are accessibility, comprehensiveness, quality, continuity and efficiency. In Korea, the current conditions of medical rehabilitation service are as follows: 1) increase of medical rehabilitation demand, 2) shortage of rehabilitation hospital beds, 3) insufficient medical rehabilitation insurance system, 4) absence of continuum care, 5) accessibility problem etc. Considering these aspects, the government is planning to build the regional rehabilitation center each district and meet the need for persons with disabilities by the second five-year plan of the welfare for persons with disabilities. Finally I suggest some a plans to establish medical service provision system for the disabled. First of all, regional rehabilitation center should be established in each province, so as to provide better and comprehensive medical services to the disabled. Second, medical services in public and private health sector should be expanded. Third, the need for the community rehabilitation committee that links the hospital and the community should be emphasized.

ABSTRACT

BACKGROUND: A rehabilitation hospital has the characteristics of longer hospital stay, a variety of medical and paramedical personnels involved in treatment, and more probability of the primary care giver being a nonmedical person. The purpose of this study is to describe the characteristics of nosocomial infection in a rehabilitation hospitals METHODS: Surveillance was done in a rehabilitation hospital affiliated university hospital from June to August 1996, May to June 1997, and July to September 1999. Nosocomial infection was investigated by an infection control practitioner (JCP) according to the Center for Disease Control and Prevention (CDC)'s definitions visiting the rehabilitation wards regularly. RESULTS: The overall NI rates were 3.83 in 1996, 4.42 in 1997 and 1.91 in 1999 per 1,000 patient days, and the urinary tract infection (UTI) rates were 2.82 in 1996, 2.87 in 1997 and 1.07 in 1999 per 1,000 patient days. Both of overall NI rate and UTI rate in 1999 decreased significantly compared with that in 1996 and 1997 (P<0.001). Comparing the ward specific NI rate, the average NI rate of the spinal cord injury ward was the highest (5.67 per 1,000 patient days) and that of the pediatric rehabilitation ward was the lowest (0.83 per 1,000 patient days). UTI constituted of the largest proportion of NIs. The distribution of UTI showed a decreasing tendency from 73.47% in 1996 to 56.00% in 1999, but the distribution of gastrointestinal infection showed an increasing tendency from 4.08% and 5.40% in 1997 and 1996 to 24.00% in 1999. The commonly isolated organisms were E. coli (33.63%), Enterococcus spp. (12.73%) and C. difficile (7.27%). It reflected that the main infection was UTI. CONCLUSION: The main NI in rehabilitation hospital was the UTI. It means that the infection control program of rehabilitation hospital should be focused on the control of UTI. Surveillance in the rehabilitation hospital enabled us to describe the characteristics of nosocomial infection in the rehabilitation hospital. NI rate was decreased through continuous communications and educations with factual datum and involvement of various medical and paramedical personnels. These results show that NI surveillance is crucial for an efficient infection control program.
