ABSTRACT

Objective@#To investigate the clinical characteristics and treatment strategies of diabetic hyperosmolar hyperglycemia (HHS) with rhabdomyolysis (RM).@*Methods@#The clinical data of 40 patients with HHS treated in the General Hospital of Shenyang Military Command from November 2013 to November 2017 were retrospectively analyzed.According to the serum levels of creatine phosphokinase and myoglobin, they were divided into RM group (12 cases) and non-RM group (28 cases). The clinical characteristics and treatment results of the two groups were compared.@*Results@#There were 12 cases in the RM group, 6 cases were diagnosed RM at the time of consultation, and 6 cases developed RM during the course of treatment.Compared with the non-RM group, RM group had lower systolic pressure[(98.3±17.8)mmHg vs.(128.0±18.1)mmHg, t=4.823, P=0.000], higher blood glucose level[(44.4±14.0)mmol/L vs.(32.6±8.1)mmol/L, t=2.717, P=0.016], and more acidosis, mainly manifested by lower pH[(7.16±0.15)vs.(7.32±0.13), t=3.355, P=0.002], lower bicarbonate[(12.92±5.23)mmol/L vs.(19.07±6.80)mmol/L, t=2.792, P=0.008], higher blood D-3 hydroxybutyric acid [(5.84±2.98)mmol/L vs.(2.55±2.13)mmol/L, t=4.012, P=0.000], and renal function was worse[creatinine (257.1±149.8)μmol/L vs.(148.1±85.3)μmol/L, t=2.925, P=0.006]. Individualized rehydration and low dose insulin were given to control blood sugar, and increasing blood pressure, kidney protection, correction of electrolyte disturbance, anti-infection and inhibition of gland secretion were given to the complications.Hydration and alkalization were given to 7 cases of RM, and continuous renal replacement therapy (CRRT) was given to 5 cases.In 10 cases of HHS with RM, creatine kinase decreased, renal function recovered, and 2 patients died.@*Conclusion@#It is very important to improve the understanding of RM in HHS patients, routinely monitor the dynamic changes of muscle enzymes, make a good early diagnosis and prevention of RM.Urine hydration and alkalization should be given in time after RM occurs, and CRRT treatment as early as possible can improve the survival rate of diabetic patients.

ABSTRACT

O aumento da temperatura interna (Ti) é considerado importante causa da fadiga durante exercícios prolongados realizados no calor. Dentre as estratégias empregadas para atenuá-la, a reposição hídrica é a que mais se destaca por sua praticidade e baixo custo. Por outro lado, pouco se sabe a respeito da influência que a temperatura dos repositores hídricos exerce sobre respostas termorregulatórias e o desempenho aeróbio durante exercícios prolongados em ambientes quentes. Teoricamente, as bebidas em baixa temperatura poderiam conferir vantagem fisiológica, agindo como dissipadores de calor ou proporcionando uma sensação agradável, levando à manutenção da ativação do drive central. Combinados, esses mecanismos podem diminuir os efeitos deletérios da elevação da Ti ao desempenho. Mesmo assim, o número de estudos investigando tal hipótese é escasso. Logo, o objetivo deste Ponto de Vista foi examinar se as evidências existentes apoiam a hipótese de modulação da Ti e melhora do desempenho aeróbio a partir da ingestão de bebidas em baixas temperaturas durante os exercícios prolongados realizados no calor. Encontramos grande heterogeneidade na metodologia dos estudos, sobretudo no que diz respeito 1) ao baixo número amostral; 2) à ausência de soluções controle; 3) à falta de padronização do momento de administração dos repositores hídricos; e 4) ao protocolo de exercício utilizado. Isso dificulta o estabelecimento de conclusões definitivas sobre o assunto, e, portanto, mais estudos são necessários. Contudo, evidências oriundas de poucos estudos bem controlados sugerem que repositores hídricos em baixa temperatura podem atenuar o aumento na Ti e melhorar a capacidade aeróbia durante a realização de exercícios prolongados no calor....(AU)


The increase in internal temperature (Ti) is considered a major cause of fatigue during prolonged exercise in the heat. Among the strategies employed to attenuate it, the fluid replacement is the most used due its cost-effectiveness and practicality. On the other hand, little is known about the influence that the beverage temperature exerts on the thermoregulatory responses and aerobic capacity during prolonged exercise in hot environments. Theoretically, fluid replacers at a low temperature may provide a physiological advantage by acting as a 'heat sink', and providing a pleasant sensation, leading to increased central drive activation. These mechanisms would mitigate the side effects of an increased Ti on performance. Nevertheless, the number of studies dedicated to investigate this hypothesis is scarce. Thus, the aim of this Point of View was to examine whether the existing evidence support the modulation of Ti and improved aerobic capacity hypothesis from fluid replacers at low temperatures during prolonged exercise in hot environments. We have found substantial heterogeneity in the methodology inherent in these studies with regards to the 1) low sample size; 2) absence of control solutions; 3) lack of standardization of the moment of administration of the fluid replacers; and 4) exercise protocol. This causes difficulty in establishing definitive conclusions on this topic, and therefore more studies are required. However, the few existing evidence from well-controlled studies suggest that the fluid replacers at low temperatures can attenuate the increase in Ti and improve aerobic capacity during prolonged exercise in the heat....(AU)

ABSTRACT

RESUMEN Objetivo Evaluar los costos de las terapias de rehidratación oral (TRO) y de rehidratación nasogástrica (TRN) comparadas con la terapia de rehidratación endovenosa (TRE) para corregir la deshidratación por diarrea en niños. Metodología Análisis de minimización de costos desde la perspectiva del Sistema de Salud colombiano comparando TRO (seguida de TRN ante falla de la TRO), con la TRE. El horizonte temporal fue la duración de la rehidratación. La medida de efectividad se extrajo de una revisión sistemática de literatura. Para determinar costos, se construyó un caso típico y un árbol de decisiones, a partir de revisión de guías e historias clínicas, validado con expertos. Los costos unitarios se obtuvieron de bases de datos colombianas. Costos fueron calculados en pesos colombianos (COP) y dólares americanos (USD). Se realizaron análisis de sensibilidad de una y dos vías. Resultados La TRO y la TRE son similares en efectividad para prevenir hospitalización y lograr rehidratación. En el caso base, el costo de la TRO fue $91,221COP (40.5 USD) y para TRE $112,944COP (50.14USD), es decir, un ahorro de $21,723 COP (9.64 USD). En los análisis de sensibilidad por regímenes de aseguramiento y complejidad del hospital, la TRO suele ser la estrategia menos costosa. Discusión Ambas intervenciones son similares en efectividad, pero la TRO, seguida de TRN ante falla de la primera resulta menos costosa que la TRE. La TRO es recomendable como primera opción para corregir la deshidratación. Deberían continuarse esfuerzos por implementar TRO y TRN en los servicios de salud en Colombia.(AU)


ABSTRACT Objective To evaluate the costs of oral rehydration therapy (ORT) and nasogastric rehydration therapy (NRT) compared with intravenous rehydration therapy (IRT) to treat dehydration in children under 5 years of age with diarrhea. Methodology Cost-minimization analysis from the perspective of the Colombian Health System, comparing ORT, (followed by NRT when ORT fails), with IRT. The time horizon was the duration of rehydration. The effectiveness measure was obtained from a systematic review of the literature. To determine costs, a typical case was created based on current guidelines and medical records; this case was validated by experts. Unit costs were obtained from Colombian databases and were provided in Colombian pesos (COP) and US dollars (USD) for 2010. One- and two-way sensitivity analyzes were performed. Results ORT and ERT are similarly effective to prevent hospitalization and to achieve rehydration. In the base case, the expected cost of ORT was $91,221 COP (40.5 USD) and for IRT was $112,944 COP ($50.14 USD), saving $21,723 COP ($9.64 USD) per case. In the sensitivity analyzes by health insurance and hospital level, ORT is often the least costly strategy. Discussion Both interventions are similarly effective, but ORT, followed by NRT when ORT fails, is less costly than IRT. ORT is recommended as the first option to treat dehydration since it is effective and less expensive. Efforts should be continued to implement TRO and NRT in the health services of Colombia.(AU)

RESUMO A administração de fluidos em tempo adequado é crucial para a manutenção da perfusão tissular nos pacientes com choque séptico. Entretanto, a questão da escolha do fluido a ser utilizado para ressuscitação no choque séptico ainda é um assunto em debate. É crescente o corpo de evidência que sugere que o tipo, a quantidade e o momento da administração de fluidos durante a evolução da sepse podem afetar os desfechos do paciente. Os cristaloides têm sido recomendados como fluidos a serem administrados em primeira linha na ressuscitação do choque. No entanto, à luz da natureza inconclusiva da literatura disponível, não se podem fazer recomendações definitivas quanto à solução cristaloide mais apropriada. A ressuscitação de pacientes críticos sépticos e não sépticos com cristaloides não balanceados, principalmente a solução salina a 0,9%, tem sido associada a uma maior incidência de desordens do equilíbrio ácido-base e a distúrbios eletrolíticos, além de poder se associar à maior incidência de lesão renal aguda, à maior necessidade de terapia de substituição renal e à mortalidade. Foi proposto o uso de soluções cristaloides balanceadas como uma alternativa às soluções de cristaloides não balanceados, para mitigar seus efeitos deletérios. Entretanto, a segurança e a eficácia dos cristaloides balanceados para ressuscitação do choque séptico necessitam ser mais bem exploradas em estudos clínicos bem delineados, randomizados e controlados, multicêntricos e pragmáticos.


ABSTRACT Timely fluid administration is crucial to maintain tissue perfusion in septic shock patients. However, the question concerning which fluid should be used for septic shock resuscitation remains a matter of debate. A growing body of evidence suggests that the type, amount and timing of fluid administration during the course of sepsis may affect patient outcomes. Crystalloids have been recommended as the first-line fluids for septic shock resuscitation. Nevertheless, given the inconclusive nature of the available literature, no definitive recommendations about the most appropriate crystalloid solution can be made. Resuscitation of septic and non-septic critically ill patients with unbalanced crystalloids, mainly 0.9% saline, has been associated with a higher incidence of acid-base balance and electrolyte disorders and might be associated with a higher incidence of acute kidney injury. This can result in greater demand for renal replacement therapy and increased mortality. Balanced crystalloids have been proposed as an alternative to unbalanced solutions in order to mitigate their detrimental effects. Nevertheless, the safety and effectiveness of balanced crystalloids for septic shock resuscitation need to be further addressed in a well-designed, multicenter, pragmatic, randomized controlled trial.

ABSTRACT

En esta nueva edición de la Ronda Clínica y Epidemiológica analizamos cuatro estudios que consideramos importantes para la práctica clínica. El estudio SOME, en el cual Carrier y colaboradores evaluaron la eficacia de la tamización para el cáncer oculto en pacientes con primer episodio no provocado de tromboembolia venosa. El estudio de Freedman y colaboradores muestra que el uso de jugo de manzanas y líquidos elegidos libremente no es inferior a las soluciones hidroelectrolíticas en la terapia de rehidratación oral en pacientes pediátricos con gastroenteritis aguda de bajo riesgo. Gágyor y colaboradores investigaron sobre el uso de ibuprofeno comparado con el de fosfomicina en el tratamiento sintomático de las infecciones urinarias no complicadas. Finalmente, el metaanálisis de Martindale y colaboradores consolida una información valiosa respecto a las pruebas que se deben hacer en el diagnóstico de falla cardíaca aguda.


In this new edition of Ronda Clínica y Epidemiológica, four studies that we consider important for clinical practice are analyzed. The SOME study, in which Carrier et al., evaluated the efficacy of a screening strategy for occult cancer in patients with a first unprovoked venous thromboembolism. The study by Freedman et al., shows that the use of apple juice is not inferior to oral electrolyte maintenance solution in children with mild gastroenteritis. Gágyor et al., investigated about the use of ibuprofen versus fosfomycin for treating symptoms of uncomplicated urinary tract infection. Finally, the meta-analysis of Martindale et al., provided valuable information about the tests that should be done in the diagnosis of acute heart failure.


Nesta nova edição da Ronda Clínica e Epidemiológica analisamos quatro estudos que consideramos importantes para a prática clínica. O estudo SOME, no qual Carrier e colaboradores avaliaram a eficácia da tamisação para o câncer oculto em pacientes com primeiro episódio não provocado de tromboembolia venosa. O estudo de Freedman e colaboradores mostra que o uso de suco de maçãs e líquidos elegidos livremente não é inferior às soluções hidroeletrolíticas na terapia de reidratação oral em pacientes pediátricos com gastroenterite aguda de baixo risco. Gágyor e colaboradores investigaram sobre o uso de Ibuprofeno comparado com o de Fosfomicina no tratamento sintomático das infeções urinárias não complicadas. Finalmente, a meta-análise de Martindale e colaboradores consolida uma informação valiosa com respeito às provas que se deve fazer no diagnóstico de falha cardíaca aguda.

ABSTRACT

Purpose: To analyze the effects of water intake on heart rate variability (HRV) during and after exercise. Methods: Thirty-one young males performed three different procedures: I) a maximal exercise test to determine the load for the protocols; II) the control protocol (CP); III) the experimental protocol (EP). The protocols comprised 10 minutes at rest, 90 minutes of treadmill exercise (60% of VO2 peak), and 60 minutes of recovery. No rehydration beverage consumption was allowed during the CP. During the EP, the participants were given water every 15 minutes, being that the amount of water to be ingested by each volunteer was divided over the time of exercise and recovery. For the analysis of HRV data, time and frequency domain indices were investigated in the following moments (M): M1 (5 minutes final of rest), M2 (25-30 minutes of exercise), M3 (55-60 minutes of exercise), M4 (85-90 minutes of exercise), M5 (5-10 minutes of recovery), M6 (15-20 minutes of recovery), M7 (25-30 minutes of recovery), M8 (40-45 minutes of recovery), and M9 (55-60 minutes of recovery). Results: There was no statistically significant difference between the groups that were examined during the exercise. HRV values for the CP and EP at the end of the exercise period were as follows SDNN (5.95±1.89 vs 6.89±2.11); RMSSD (3.29±0.78 vs 3.39±0.95); HF [nu] (24.06±14.26 vs 19.10±11.94); LF [nu] (75.93±14.26 vs 80.89±11.94); HF [ms2] (0.90±1.16 vs 0.94±0.89); LF [ms2] (4.65±5.94 vs 5.35±5.09); and LF/HF ratio (4.64±3.15 vs 6.95±5.57). HRV values for the CP and the EP at the end of the recovery period were as follows: SDNN (39.63±19.63 vs 49.95±17.79); RMSSD (21.63±15.55 vs 36.43±22.73); HF [ms2] (285.51±392.27 vs 699.29±699.66); LF [nu] (74.55±13.25 vs 61.19±19.27); and HF [nu] (25.44±13.25 vs 38.8±19.27). Conclusion: The hydration with water was not enough to significantly influence the linear HRV indices during exercise; however, it promoted faster recovery of these variables after exercise

ABSTRACT

OBJECTIVE: To report effects of the pre-procedural rehydration for reduce thromboembolic complications in acute phase aneurysmal subarachnoid hemorrhage coil embolization. MATERIALS AND METHODS: From January 2009 to December 2013, 190 patients with ruptured aneurysmal subarachnoid hemorrhage (aSAH) treated by coil embolization at our institution were consecutively enrolled in this study. In period 1 (from January 2009 to June 2012, n = 122), pre-procedural fluid was not supplied. In period 2 (from July 2012 to December 2013, n = 68), depending on the state of the patient's body weight and degree of dehydration, intravenous fluid was started with infusion of approximately 7 mL/kg of 0.9 percent saline (minimum 300 to maximum 500 mL) over 30 minutes. RESULTS: A total of 190 patients were hospitalized due to aSAH and underwent coil embolization for five years between January 2009 and December 2013. Of these, 122 patients underwent coil embolization based on the old protocol before June 2012 (period 1) and 68 underwent the procedure based on the new protocol after the period 2. Neck size, width, maximum diameter of the aneurysm and procedure time were associated with procedure related thromboembolic complications in entire periods (multivariate analysis, p < 0.05, in respectively). The frequency of thromboembolism showed a drastic decrease in period 2 (re-hydration period), from 18.0% (22/123) to 4.4% (3/67), which was also statistically significant (p = 0.007, Chi-square test). CONCLUSION: Pre-procedural administration of a sufficient dose of fluid considering the patient's dehydration reduced the frequency of thromboembolism in cases of emergency coil embolization for ruptured aneurysm, without increasing additional specific complications.

RESUMOIntrodução:O número de participantes em programas de treinamento de futsal tem crescido como alter-nativa de prática esportiva entre adolescentes. A aptidão física e o perfil hídrico de adolescentes praticantes de futsal são escassos na literatura científica.Objetivo:Determinar a composição corporal e as característi-cas metabólicas de adolescentes do sexo masculino participantes de programa de treinamento de futsal e relacioná-los com a posição tática.Métodos:Foram avaliados 10 adolescentes do sexo masculino com idades entre 12 e 13 anos e nível de maturação sexual de 3,7 ± 0,7 para pilosidade e 3,6 ± 1,0 no desenvolvimento do genital externo. Os adolescentes foram submetidos à avaliação antropométrica (peso corporal, estatura e composição corporal), neuromotora (teste de impulsão vertical e horizontal e shuttle run) e de demanda metabólica (distância percorrida, gasto calórico, número de passos e variação hídrica).Resultados:Os resul-tados dos atletas foram analisados pelas 4 posições de jogo (goleiros, fixos, alas e pivôs) utilizando análise de variância Anovaone-way e post-hoc Scheffé com uma margem de erro de 5%, demonstrando que os goleiros apresentaram maior porcentagem de gordura que as demais posições de jogo, não havendo diferenças entre elas. A massa magra apresentou tendência inversa, com menor valor de muscularidade nos goleiros que nas posições de linha, onde os alas e pivôs apresentaram valores superiores aos fixos. Os goleiros apresentaram demanda metabólica menor em função da intensidade analisada durante os jogos comparado com as demais posições, corroborando com outros estudos similares. Para todos os grupos, a ingestão de água ad libitum foi suficiente para manter o estado de hidratação.Conclusão:Assim pudemos concluir que existem diferenças na demanda metabólica entre as posições de jogo, o que sugere prescrição especifica do treinamento buscando evitar o excesso de estÍmulo ou estímulos abaixo das necessidades dos adolescentes.


ABSTRACTIntroduccion:The increase in the number of adolescents attending indoor soccer training programs has been markedly observed as an alternative for sports practice. Physical fitness and the hydration profile in indoor soccer adolescents athletes are rare in scientific literature.Objective:To determine the body composition and metabolic characteristics in male adolescents attending indoor soccer training program, making a relation among them and the game position.Methods:We evaluated 10 male adolescent aged 12 to 13 years, with sexual maturational level of 3.7 ± 0.7 for pubic hair, and 3.6 ± 1.0 for external genital development. The adolescents were submitted to an-thropometric measurement (weight, height and body composition), neuromotor test (vertical and horizontal thrust and shuttle run test) and metabolic demand (distance covered, energy expenditure, number of steps and hydration variation).Results:The results were analyzed by the 4 game positions (goalkeepers, full-backs, wing backs and centre--forwards) using the ANOVA One Way and post-hoc Scheffé's variation analysis, with an accuracy margin of 5%. Data showed that goalkeepers presented higher percentage of body fat than other game positions, with no differences among them. The lean body mass showed a inverse trend, with lower muscular values for goalkeepers than in field positions, and higher values for wing backs and centre-forwards than full-backs. Goalkeepers presented lower me-tabolic demand taking into account the intensity analyzed during the games, compared to other game positions, corroborating with other similar studies. For all groups, ad libitum water intake was sufficient to maintain the state of rehydration.Conclusion:We can, thus, conclude that there are differences in metabolic demand according to the game positions, suggesting a specific training prescription aiming at avoiding over or under stimuli for adolescents.

La aptitud física y el perfil hídrico de adolescentes practicantes de futsal son escasos en la literatura científica.Objetivo:Determinar la composición corporal y las características metabólicas de adolescentes del sexo masculino participantes de programa de entrenamiento de futsal y relacionarlos con la posición táctica.Métodos:Fueron evaluados 10 adolescentes del sexo masculino con edades entre 12 y 13 años y nivel de madurez sexual de 3,7 ± 0,7 para pilosidad y 3,6 ± 1,0 en el desarrollo del genital externo. Los adolescentes fueron sometidos a evaluación antropométrica (peso corporal, estatura y composición corporal), neuromotriz (test de impulsión vertical y horizontal y shuttle run) y de demanda metabólica (distancia recorrida, gasto calórico, nú-mero de pasos y variación hídrica).Resultados:Los resultados de los atletas fueron analizados por las 4 posiciones de juego (arqueros, fijos, laterales y pivós) utilizando análisis de variancia Anova one-way y post-hoc Scheffé con un margen de error de 5%, demostrando que los arqueros presentaron mayor porcentaje de grasa que las demás posiciones de juego, no habiendo diferencias entre ellas. La masa magra presentó tendencia inversa, con menor valor de muscularidad en los arqueros que en las posiciones de línea, en donde los laterales y pivós presentaron valores superiores a los fijos. Los arqueros presentaron demanda metabólica menor en función de la intensidad analizada durante los partidos comparado con las demás posiciones, corroborando con otros estudios similares. Para todos los grupos, la ingestión de agua ad libitum fue suficiente para mantener el estado de hidratación.Conclusión:Así, podemos concluir que existen diferencias en la demanda metabólica entre las posiciones de juego, lo que sugiere prescripción específica del entrenamiento buscando evitar el exceso de estÍmulo o estímulos por debajo de las ne-cesidades de los adolescentes.

A queimadura é dos acidentes mais frequentes em Pediatria, com altas taxas de mortalidade.As complicações mais frequentes são o choque hipovolêmico, a desnutrição progressiva e as infecções. Este artigo faz uma revisão sobre a abordagem do choque hipovolêmico na criança queimada em relação à sua fisiopatologia e as diversas modalidades de tratamento, discutindo as suas vantagens e desvantagens.


Burns are the most frequent accidents in Pediatrics, with high mortality rates. The most frequent complications are a hypovolemic shock, progressive malnutrition, and infections.This article is a review of the hypovolemic shock approach in the burned child in relation to his pathophysiology and the various treatment modalities, discussing their advantages and disadvantages.

La deshidratación es una de las principales causas de mortalidad en la población pediátrica en países en vía de desarrollo. Por ello, se han estudiado alternativas de manejo para la rehidratación oral, intravenosa e intraósea, de fácil aplicación y bajo costo. La hipodermoclisis es la técnica para la administración subcutánea de líquidos con el fin de prevenir o tratar la deshidratación leve o moderada; olvidada por muchos años debido a la falta de experiencia y protocolos bien estructurados. Recientemente resurge el interés en investigar e implementar esta técnica gracias a sus beneficios y menores riesgos frente a las otras opciones, entre las que se encuentran facilidad de aplicación, mantenimiento de la vía, comodidad del paciente y disminución de complicaciones por daño endotelial, demostrados en diferentes estudios como es el caso del Infuse Peds 1 y 2. Para favorecer la absorción subcutánea se puede implementar la hialuronidasa recombinante humana, enzima encargada de disolver el ácido hialurónico para alterar, temporalmente, la permeabilidad del tejido, que es el objeto de análisis dentro de estos estudios. En Colombia esta técnica aún no ha sido completamente difundida para aplicarla en la población pediátrica. Es imperativo divulgar opciones de manejo que sean efectivas y de bajo costo para la atención de las enfermedades más prevalentes. La hipodermoclisis cumple con estos objetivos y cuenta, además, con beneficios como la satisfacción de los padres y mejora índices de morbimortalidad infantil.


Dehydration is one of the main causes of mortality in pediatric population in developing countries. Thus, we have studied alternative measures that are low-cost and easy to administer for oral, intravenous, and intraosseuos rehydration. Hypodermoclysis is the technique for subcutaneous administration of liquids in order to prevent or treat mild or moderate dehydration. This method has been forgotten for many years due to the lack of experience and well-structured protocols. Recently, there has been increasing interest in researching and implementing this technique due to the benefits and lower risks compared to other options, including easy administration, pathway maintenance, patient comfort, and reduced complications due to endothelial damage, demonstrated in different studies, such as INFUSE-Peds I and II. Human recombinant hyaluronidase can be implemented in order to favor subcutaneous absorption; this enzyme is in charge of dissolving hyaluronic acid to temporarily alter the permeability of the tissue, which is the object of analysis of these studies. This technique has not been completely disseminated in Colombia to administer in pediatric population. It is imperative to divulge treatment options that are effective and low-cost to treat the most prevalent diseases. Hypodermoclysis meets these objectives and also has benefits such as parent satisfaction and improves indexes of infant morbidity and mortality.

Por isso, se estudaram alternativas de manejo para a reidratação oral, intravenosa e intraóssea, de fácil aplicação e baixo custo. A hipodermóclise é a técnica para a administração subcutânea de líquidos com o fim de prevenir ou tratar a desidratação leve ou moderada; esquecida por muitos anos devido à falta de experiência e protocolos bem estruturados. Recentemente ressurge o interesse em investigar e implementar esta técnica graças a seus benefícios e menores riscos frente às outras opções, entre as que se encontram facilidade de aplicação, manutenção da via, comodidade do paciente e diminuição de complicações por dano endotelial, demostrados em diferentes estudos como é o caso do Infuse Peds 1 e 2. Para favorecer a absorção subcutânea se pode implementar a hialuronidase recombinante humana, enzima encargada de dissolver o ácido hialurônico para alterar, temporalmente, a permeabilidade do tecido, que é o objeto de análise dentro destes estudos. ABSTRACT

OBJECTIVE: To describe knowledge and perceptions regarding the use of oral rehydration solution (ORS) for the management of diarrheal diseases among formal and informal health care providers and community caregivers in the Guatemalan department of Santa Rosa, and to recommend strategies to increase ORS use for management of diarrhea in children. METHODS: From July to September 2008, in-depth, semi-structured interviews were conducted with formal health care providers; open-ended interviews were conducted with informal health care providers; and focus group discussions and pile sorting were carried out with community caregivers. RESULT: The study participants attributed episodes of diarrhea in children to culturally recognized or folk ailments (empacho, cuajo, and varillas) that are primarily treated by traditional healers. There were knowledge deficits about 1) dehydration as a manifestation of diarrhea, and 2) management of dehydration, including the use of ORS and the need to continue feedings during diarrheal episodes. Caregivers perceived bottled/ready-made ORS products and the more expensive over-the-counter antidiarrheal medications as superior to ORS packets in the treatment of diarrhea. CONCLUSIONS: In Guatemala, folk etiologies of disease differ from those of the biomedical establishment and influence the decisions made by caregivers when treating ill children, including those related to the use of ORS. Public health campaigns addressing the treatment and management of diarrheal diseases in Santa Rosa should recognize the ailments known as empacho, cuajo, and varillas and target them for ORS use by community caregivers as well as health care providers in both the formal and informal health sectors.


OBJETIVO: Describir los conocimientos y las ideas que tienen las personas que prestan atención de salud en el sector convencional, el sector no convencional y los cuidadores de la comunidad con respecto al uso de las soluciones de rehidratación oral en el tratamiento de las enfermedades diarreicas, en el departamento guatemalteco de Santa Rosa y recomendar estrategias encaminadas a aumentar la utilización de estas soluciones en el tratamiento de la diarrea en los niños. MÉTODOS: De julio a septiembre del 2008 se llevaron a cabo entrevistas exhaustivas semiestructuradas a los profesionales de salud y entrevistas con preguntas abiertas a los proveedores no convencionales de atención sanitaria y se organizaron debates en grupos de opinión y ejercicios de ordenamiento de tarjetas con los cuidadores de la comunidad. RESULTADOS: Los participantes en el estudio atribuyeron la causa de los episodios de diarrea en los niños a dolencias culturalmente aceptadas o populares (empacho, cuajo y varillas), que tratan principalmente los curanderos. Se observaron deficiencias en los conocimientos acerca de 1) la deshidratación como una manifestación de la diarrea y 2) el tratamiento de la deshidratación, incluido el uso de las soluciones de rehidratación oral y la necesidad de continuar la alimentación durante los episodios diarreicos. Los cuidadores consideraron las soluciones de rehidratación embotelladas o preparadas y los medicamentos antidiarreicos de venta libre, que son más costosos, como mejores opciones para el tratamiento de la diarrea que las soluciones de rehidratación oral. CONCLUSIONES: En Guatemala, el concepto popular y el de las instituciones biomédicas sobre la causa de las enfermedades es diferente e influye sobre las decisiones que toman los cuidadores al tratar a los niños enfermos, por ejemplo, las relacionadas con el uso de soluciones de rehidratación oral. Las campañas de salud pública que abordan el tratamiento y el manejo de las enfermedades diarreicas en Santa Rosa deben incorporar las dolencias conocidas como empacho, cuajo y varillas y fomentar el uso de las soluciones de rehidratación oral en estos casos, por parte de los cuidadores de la comunidad, los profesionales de salud del sector convencional y los proveedores de atención del sector no convencional.

ABSTRACT

The World Health Organization and the United Nations Children’s Fund recommend using a new oral rehydration solution (ORS) plus zinc supplementation for 10-14 days for the treatment of diarrhoea in children aged less than five years. The Social Marketing Plus for Diarrhoeal Disease Control: Point of Use Water Disinfection and Zinc Treatment (POUZN) project in Nepal was one of the first zinc-promotion projects to move beyond pilot efforts into a scaled-up programme with national-level reach. This study used data from a survey conducted in 26 districts in Nepal in 2008 to examine zinc-use behaviour, knowledge, and beliefs of caregivers of children aged less than six years, other diarrhoea-treatment practices, and recollection of project communication messages. The results of the survey indicated that, by six months following the onset of a zinc-promotion campaign, the majority (67.5%) of children (n=289), aged less than six years, with diarrhoea were treated with ORS, and 15.4% were treated with zinc. Over half (53.1%) of all caregivers (n=3,550) interviewed had heard about zinc products; most (97.1%) of those who had heard of zinc knew that zinc should be used for the treatment of diarrhoea. Zinc-related knowledge and behaviours were positively associated with recall of communication messages. Children whose caregivers recalled the mass-media message that zinc should be used for 10 days [odds ratio (OR)=2.02, 95% confidence interval (CI) 1.85-2.19] and whose caregivers perceived that zinc is easy to obtain (OR=1.76, 95% CI 1.49-2.09) were more likely to be treated with zinc for 10 days, along with ORS. The findings demonstrated that mass media play an important role in increasing caregivers’ knowledge about zinc and encouraging trial and correct use. Future efforts should also focus on understanding the factors that motivate providers to continue recommending antibiotics and antidiarrhoeals instead of zinc. These findings are being used for informing the design and implementation of zinc programmes in other developing countries with a high prevalence of diarrhoea.

ABSTRACT

Antimicrobial peptides represent an important component of the innate immune defenses of living organisms, including humans. They are broad-spectrum surface-acting agents secreted by the epithelial cells of the body in response to infection. Recently, L-isoleucine and its analogues have been found to induce antimicrobial peptides. The objectives of the study were to examine if addition of L-isoleucine to oral rehydration salts (ORS) solution would reduce stool output and/or duration of acute diarrhoea in children and induce antimicrobial peptides in intestine. This double-blind randomized controlled trial was conducted at the Dhaka Hospital of ICDDR,B. Fifty male children, aged 6-36 months, with acute diarrhoea and some dehydration, attending the hospital, were included in the study. Twenty-five children received L-isoleucine (2 g/L)-added ORS (study), and 25 received ORS without L-isoleucine (control). Stool weight, ORS intake, and duration of diarrhoea were the primary outcomes. There was a trend in reduction in mean±standard deviation (SD) daily stool output (g) of children in the L-isoleucine group from day 2 but it was significant on day 3 (388±261 vs 653±446; the difference between mean [95% confidence interval (CI) (-)265 (-509, -20); p=0.035]. Although the cumulative stool output from day 1 to day 3 reduced by 26% in the isoleucine group, it was not significant. Also, there was a trend in reduction in the mean±SD intake of ORS solution (mL) in the L-isoleucine group but it was significant only on day 1 (410±169 vs 564±301), the difference between mean (95% CI) (-)154 (-288, -18); p=0.04. The duration (hours) of diarrhoea was similar in both the groups. A gradual increase in stool concentrations of ß-defensin 2 and 3 was noted but they were not significantly different between the groups. L-isoleucine-supplemented ORS might be beneficial in reducing stool output and ORS intake in children with acute watery diarrhoea. A further study is warranted to substantiate the therapeutic effect of L-isoleucine.

Diarrhoeal infections are the fifth leading cause of death worldwide and continue to take a high toll on child health. Mushrooming of slums due to continuous urbanization has made diarrhoea one of the biggest public-health challenges in metropolitan cities in India. The objective of the study was to carry out a community-based health and nutrition-education intervention, focusing on several factors influencing child health with special emphasis on diarrhoea, in a slum of Delhi, India. Mothers (n=370) of children, aged >12-71 months, identified by a door-to-door survey from a large urban slum, were enrolled in the study in two groups, i.e. control and intervention. To ensure minimal group interaction, enrollment for the control and intervention groups was done purposively from two extreme ends of the slum cluster. Baseline assessment of knowledge, attitudes, and practices on diarrhoea-related issues, such as oral rehydration therapy (ORT), oral rehydration salt (ORS), and continuation of breastfeeding during diarrhoea, was carried out using a pretested questionnaire. Thereafter, mothers (n=195) from the intervention area were provided health and nutrition education through fortnightly contacts achieved by two approaches developed for the study—‘personal discussion sessions’ and ‘lane approach’. The mothers (n=175) from the control area were not contacted. After the intervention, there was a significant (p=0.000) improvement in acquaintance to the term ‘ORS’ (65-98%), along with its method of reconstitution from packets (13-69%); preparation of home-made sugar-salt solution (10-74%); role of both in the prevention of dehydration (30-74%) and importance of their daily preparation (74-96%); and continuation of breastfeeding during diarrhoea (47-90%) in the intervention area. Sensitivity about age-specific feeding of ORS also improved significantly (p=0.000) from 13% to 88%. The reported usage of ORS packets and sugar-salt solution improved significantly from 12% to 65% (p=0.000) and 12% to 75% (p=0.005) respectively. The results showed that health and nutrition-education intervention improved the knowledge and attitudes of mothers. The results indicate a need for intensive programmes, especially directed towards urban slums to further improve the usage of oral rehydration therapy.
