ABSTRACT

Introducción. La nueva era de la cirugía es cada vez más dependiente de la tecnología, y un ejemplo de ello es el uso generalizado de electrocauterio como parte primordial de la práctica quirúrgica. El humo quirúrgico es un subproducto de la disección y la coagulación de los tejidos producidas por los equipos de energía, que representa múltiples riesgos potenciales para la salud del grupo quirúrgico, sin embargo, se han minimizado los peligros causados por la exposición de manera frecuente y acumulativa a este aerosol. Métodos. Se realizó un análisis crítico, desde una posición reflexiva de la información disponible, estableciendo los posibles riesgos relacionados con la exposición al humo quirúrgico. Discusión. Es visible la necesidad imperativa de establecer directrices nacionales, pautas normativas y recomendaciones estandarizadas para cumplir con las exigencias dadas por los sistemas de gestión en salud ocupacional y seguridad del trabajo, cuyo objetivo principal es hacer efectivo el uso de mascarillas quirúrgicas apropiadas, la implementación de programa de vigilancia epidemiológica ambiental en sala de cirugía, la priorización del uso constante de aspiradores y sistemas de evacuación, y la ejecución de programas educativos de sensibilización dirigidos al personal implicado. De igual manera, se abre la inquietud de la necesidad de nuevos estudios para definir con mayor precisión el peligro de este aerosol. Conclusión. Se recomienda de manera responsable utilizar todas las estrategias preventivas existentes para intervenir en salas de cirugía los riesgos minimizados y olvidados del humo quirúrgico.


Introduction. The new era of surgery is increasingly dependent on technology, and an example of this is the widespread use of electrocautery as a primary part of surgical practice. Surgical smoke is a byproduct of the dissection and coagulation of tissues produced by energy equipment, which represents multiple potential health risks for the surgical group; however, the dangers caused by cumulative exposure have been minimized. Methods. A critical analysis was carried out from a reflective position of the available information, establishing the possible risks related to exposure to surgical smoke. Discussion. The imperative need to establish national normative guidelines and standardized recommendations to comply with the demands given by the occupational health and work safety management systems, whose main objective is to make effective the use of appropriate surgical masks, implementation of environmental epidemiological surveillance program in the operating room, prioritizing the constant use of vacuum cleaners and evacuation systems, and carrying out educational awareness programs aimed at the personnel involved. Likewise, there is concern about the need for new studies to more precisely define the danger of this aerosol. Conclusion. It is recommended to responsibly use all existing preventive strategies to intervene in operating rooms to minimize the forgotten risks of surgical smoke.

ABSTRACT

BACKGROUND@#Owing to shortage of surgical and N95 filtering facepiece respirators (FFRs) during the COVID-2019 pandemic, various masks were developed to prevent infection. This study aimed to examine the inward leakage rate (ILR) of sealed face masks and modified surgical masks using a quantitative fit test and compared it with the ILR of unmodified N95 FFRs.@*METHODS@#We conducted paired comparisons of ILRs of bent nose-fit wire masks, double masks, and N95 FFRs from October to December 2021. To measure the protective effectiveness of masks, participants wore masks, and the number of particles outside and inside the mask were measured. The ILR was based on the percentage of particles entering the mask using a fit tester.@*RESULTS@#We enrolled 54 participants (20 men and 34 women) in this study. The median ILR for surgical masks without and with a W-shaped bend in the nose-fit wire were 96.44% and 50.82%, respectively. The nose-fit wire adjustment reduced the ILR of surgical masks by a mean of 28.57%, which was significantly lower than the ILR without adjustment (P < 0.001). For double masks, with surgical or polyurethane masks on top of the W-shaped mask, the ILR did not differ significantly from that of N95. Although the filtration performance of double surgical masks matched that of N95 masks, their ILR was notably higher, indicating that double masks do not provide equivalent protection.@*CONCLUSIONS@#Wearing N95 masks alone is effective in many cases. However, surgical mask modifications do not guarantee consistent effectiveness. Properly selected, sealed masks with a good fit overcome leakage, emphasizing their crucial role. Without evidence, mask-wearing may lead to unexpected infections. Education based on quantitative data is crucial for preventing adverse outcomes.

ABSTRACT

Objetivo: investigar la prevalencia de lesiones cutáneas y factores asociados con el uso de respiradores N95 en profesionales de la salud de Brasil. Método: estudio transversal realizado con 11.368 profesionales de la salud en el que se empleó un método de muestreo dirigido por encuestados adaptado para entornos en línea. Se ejecutaron análisis univariados y multivariados para investigar la asociación entre la variable "lesiones cutáneas por utilizar respiradores N95" y sexo, categoría profesional, lugar de trabajo, capacitación, diagnóstico de COVID-19 y disponibilidad de suficiente provisión de Equipo de Protección Personal de buena calidad. Resultados: la prevalencia de lesiones cutáneas fue del 61,8%. Las mujeres fueron 1,203 veces (IC 95%: 1,154-1,255) más propensas a desarrollar lesiones que los hombres. La probabilidad de lesiones cutáneas en psicólogos (RP=0,805; IC 95%: 0,678-0,956) y dentistas (RP=0,884; IC 95%: 0,788-0,992) fue menor en comparación con la encontrada en los profesionales de Enfermería. Los profesionales con diagnóstico positivo de COVID-19 y que trabajan en la Unidad de Cuidados Intensivos son más propensos a sufrir lesiones cutáneas (RP=1,074; IC 95%: 1,042-1,107); (RP=1,203; IC 95%: 1,168-1,241), respectivamente, Conclusión: la prevalencia de lesiones cutáneas causadas por utilizar respiradores N95 fue del 61,8% y estuvo asociada al sexo femenino, a la categoría profesional, al lugar de trabajo, a la capacitación, a diagnóstico de COVID-19 y a la disponibilidad de suficiente provisión de Equipo de Protección Personal de buena calidad.


Objective: to investigate the prevalence of skin lesions and factors associated with the use of N95 respirators among health professionals in Brazil. Method: cross-sectional study conducted with 11,368 health professionals using a respondent-driven sampling method adapted for online environments. Univariate and multivariate analyses were performed to investigate the association between the "skin lesions with the use of N95 respirators" variable and gender, professional category, workplace, training, COVID-19 diagnosis, and availability of sufficient and high-quality Personal Protective Equipment. Results: the prevalence of skin lesions was 61.8%. Women were 1.203 times (95% CI: 1.154-1.255) more likely to develop a lesion than men. The chances of skin lesions in psychologists (PR=0.805; 95% CI: 0.678-0.956) and dentists (PR=0.884; 95% CI: 0.788-0.992), were lower when compared to Nursing professionals. Professionals with a positive COVID-19 diagnosis and working in the Intensive Care Unit have an increased chance of presenting skin lesions (PR=1.074; 95% CI: 1.042-1.107); (PR=1.203; 95% CI: 1.168-1.241), respectively. Conclusion: the prevalence of skin lesions caused by the use of N95 respirators was 61.8% and was associated with female gender, professional category, workplace, training, COVID-19 diagnosis, and availability of sufficient and highquality Personal Protective Equipment.


Objetivo: investigar a prevalência de lesões de pele e fatores associados ao uso de respiradores N95 entre profissionais de saúde no Brasil. Método: estudo transversal realizado com 11.368 profissionais de saúde por meio de um método de amostragem dirigido por respondentes adaptado para ambientes online. Análises univariadas e multivariadas foram realizadas para investigar a associação entre a variável "lesão de pele com uso de respirador N95" e sexo, categoria profissional, local de trabalho, treinamento, diagnóstico de COVID-19 e disponibilidade suficiente de equipamentos de proteção individual de qualidade. Resultados: a prevalência de lesões cutâneas foi de 61,8%. As mulheres foram 1,203 vezes (IC 95%: 1.154-1.255) mais propensas a desenvolver uma lesão do que os homens. As chances de lesão de pele em psicólogos (RP=0,805; IC 95%: 0,678-0,956) e dentistas (RP=0,884; IC 95%: 0,788-0,992) foram menores quando comparados aos profissionais de Enfermagem. Profissionais com diagnóstico positivo para COVID-19 e que trabalham em Unidade de Cuidados Intensivos têm maior chance de apresentar lesões de pele (RP=1,074; IC 95%: 1,042-1,107); (RP=1,203; IC 95%: 1,168- 1,241), respectivamente. Conclusão: a prevalência de lesões de pele causadas pelo uso do respirador N95 foi de 61,8% e esteve associada ao sexo feminino, categoria profissional, local de trabalho, treinamento, diagnóstico de COVID-19 e disponibilidade suficiente de equipamentos de proteção individual de qualidade.

ABSTRACT

Context : During COVID-19 Pandemic, frontline Health Care Worker (HCW) in hospitals were mandated to Personal Protective Equipment (PPE), while caring for suspected or confirmed COVID-19 patients, which involved the donning of close-fitting N95 Face Masks, Protective Eyewear, Gowns, Surgical Gloves and the use of Powered AirPurifying Respirators (PAPR). Aims : This study is to know the challenges faced during use of PPE among frontline HCW. Methods and Material : This is a cross-sectional study among HCW at our Tertiary Institution who were working in high-risk hospital areas during COVID-19. All respondents completed a self-administered questionnaire Statistical analysis used : Data were entered in Microsoft Excel and analyzed using SPSS version 23. Baseline characteristics were described using frequency and percentages. Association between predictors of PPE associated symptoms were assessed using Chi-square test with p-value of <0.05 considered as significant. Results : Total of 190 Health Care Workers participated in the study. Doctors- contributed most [143/189 (75.2%)]. Majority of the respondents reported usage of Masks, Eyewear, Shield and Gown [126/189 (66.7%)], in which most of them donned N-95 mask [152/189(80.5%)], and Goggles [110/189 (58.2%)] average for 6.32 (2.40) hours a day and 18.15(8.65) days in a month. 83 respondents reported a new onset headache associated with usage of PPE. Majority of the respondents localized Headaches as frontal (69.9%) which was statistically significant. Other symptoms were Tiredness (73.5%), Excess Sweating (45.4%) and Giddiness (20.6%). Conclusions : Prevalence and characteristics of PPE- associated symptoms in HCW working in high-risk areas in Tertiary Care Centers necessitates better measures and strategies for designing PPE and reducing the exposure time in HCW and also the impact on their work performance

Resumo Objetivo Identificar os fatores associados ao uso de máscara N95 relacionados a alterações de pele entre profissionais de saúde do Norte do Brasil durante a pandemia de COVID-19. Métodos Estudo multicêntrico realizado na região Norte do Brasil no período de outubro a dezembro de 2020, com 1.684 profissionais de saúde que atuaram na assistência à saúde durante a pandemia da COVID-19. Os participantes foram convidados via mídias sociais e as informações coletadas eletronicamente foram armazenadas na plataforma Survey Monkey. Foi usada estatística descritiva para caracterização da amostra, testes de associação (Qui-quadrado), com nível de significância estatística em p < 0,05. Os fatores associados ao uso de máscara N95 relacionados a alterações de pele foram determinados por Regressão Logística Binária, nível de significância de 5% (α = 0,05). Resultados Profissionais de saúde do sexo masculino apresentaram 1,708 mais chances de ter alterações de pele. Outros fatores como, o período de troca de máscara N95, ter atuado em hospital de campanha para COVID-19, terapia intensiva, urgência e emergência e ter recebido alguma capacitação ou curso sobre o COVID-19 foram considerados protetores ao desenvolvimento de alterações de pele, enquanto os principais motivos de troca de máscara N95 foram a umidade, perda de vedação, contaminação, danificação (rasgo). Conclusão Concluiu-se que profissionais de saúde do sexo masculino apresentaram mais chances de ter alterações de pele relacionadas ao uso de máscaras N95. Destacaram-se os fatores protetores e torna-se relevante a realização de estudos de intervenção a fim explorar medidas para prevenção destes tipos de lesões.


Resumen Objetivo Identificar los factores asociados al uso de la mascarilla N95 relacionados con las alteraciones en la piel entre profesionales de salud del norte de Brasil durante la pandemia de COVID-19. Métodos Estudio multicéntrico realizado en la región Norte de Brasil, en el período de octubre a diciembre de 2020, con 1.684 profesionales de salud que actuaron en la asistencia a la salud durante la pandemia de COVID-19. Se convocó a los participantes a través de las redes sociales y la información recopilada electrónicamente fue guardada en la plataforma Survey Monkey. Se utilizó la estadística descriptiva para la caracterización de la muestra, pruebas de asociación (Ji-cuadrado), con un nivel de significación estadística de p < 0,05. Los factores asociados al uso de la mascarilla N95 relacionados con alteraciones en la piel fueron determinados por regresión logística binaria, nivel de significación del 5 % (α = 0,05). Resultados Profesionales de salud de sexo masculino tuvieron 1,708 más posibilidades de presentar alteraciones en la piel. Otros factores como el intervalo de tiempo entre los cambios de mascarillas N95, haber trabajado en hospitales de campaña para COVID-19, cuidados intensivos, urgencias y emergencias y haber realizado alguna capacitación o curso sobre COVID-19 fueron considerados protectores del surgimiento de alteraciones en la piel, mientras que los principales motivos para el cambio de la mascarilla N95 fueron la humedad, la pérdida del sellado, la contaminación y daños (rotura). Conclusión Se concluye que los profesionales de la salud de sexo masculino presentaron más probabilidades de tener alteraciones en la piel relacionadas con el uso de mascarillas N95. Se destacaron los factores protectores y resulta importante la realización de estudios experimentales con la finalidad de explorar medidas para la prevención de este tipo de lesiones.


Abstract Objective To identify the factors associated with N95 mask use related to skin changes among health professionals in northern Brazil during the COVID-19 pandemic. Methods This is a multicenter study carried out in northern Brazil, from October to December 2020, with 1,684 health professionals who worked in health care during the COVID-19 pandemic. Participants were invited via social media and the information collected electronically was stored on the Survey Monkey platform. Descriptive statistics were used to characterize the sample, association tests (chi-square), with statistical significance level at p < 0.05. Factors associated with N95 mask ruse elated to skin changes were determined by Binary Logistic Regression, significance level of 5% (α = 0.05). Results Male health professionals were 1.708 more likely to have skin changes. Other factors such as the N95 mask change period, having worked in a COVID-19 field hospital, intensive care, urgency and emergency and having received some training or course on COVID-19 were considered protective against the development of skin changes, while the main reasons for changing the N95 mask were humidity, loss of seal, contamination, damage (tear). Conclusion We concluded that male health professionals were more likely to have skin changes related to N95 mask use. The protective factors were highlighted and it is relevant to carry out intervention studies in order to explore measures to prevent these types of injuries.

Objetivo: identificar as percepções dos trabalhadores do centro cirúrgico quanto ao uso da máscara N95 como Equipamento de Proteção Individual em relação à fumaça cirúrgica. Método: pesquisa qualitativa mediante entrevistas áudio-gravadas, com nove trabalhadores do Centro Cirúrgico de um hospital no norte do Paraná, Brasil. Questionou-se: qual a percepção dos trabalhadores expostos à fumaça cirúrgica em relação ao uso das máscaras N95? Foi utilizada a técnica de análise de conteúdo. Resultados: o estudo revela a fragilidade do conhecimento dos profissionais sobre a utilização da máscara N95 e sobre os riscos que estão expostos durante sua jornada de trabalho, quando expostos à fumaça cirúrgica. Assim como o desconforto pelo uso da máscara. Conclusão: sugere-se capacitação aos profissionais da saúde expostos à fumaça cirúrgica, onde possa ser esclarecido sobre o uso de Equipamento de Proteção Individual de forma correta e sua importância na prevenção para o desenvolvimento de doenças ocupacionais.


Objetivo: identificar las percepciones de los trabajadores del centro quirúrgico en cuanto al uso de la máscara N95 como Equipo de Protección Individual en relación al humo quirúrgico. Método: investigación cualitativa mediante entrevistas audio-grabadas, con nueve trabajadores del Centro Quirúrgico de un hospital en el norte de Paraná, Brasil. Se preguntó: ¿cuál es la percepción de los trabajadores expuestos al humo quirúrgico con respecto al uso de las máscaras N95? Se utilizó la técnica de análisis de contenido. Resultados: el estudio revela la fragilidad del conocimiento de los profesionales sobre la utilización de la máscara N95 y sobre los riesgos que están expuestos durante su jornada de trabajo, cuando están expuestos al humo quirúrgico. Así como la incomodidad por el uso de la máscara. Conclusión: se sugiere capacitación a los profesionales de la salud expuestos a humo quirúrgico, donde pueda ser esclarecido sobre el uso de Equipo de Protección Individual de forma correcta y su importancia en la prevención para el desarrollo de enfermedades ocupacionales.


Objective: to identify the perceptions of surgical center workers regarding the use of the N95 mask as Personal Protection Equipment in relation to surgical smoke. Method: qualitative research through audio-recorded interviews with nine workers from the Surgical Center of a hospital in northern Paraná, Brazil. It was questioned: what is the perception of workers exposed to surgical smoke in relation to the use of N95 masks? The technique of content analysis was used. Results: the study reveals the fragility of professionals' knowledge about the use of the N95 mask and the risks they are exposed during their working day, when exposed to surgical smoke. As well as discomfort from wearing the mask. Conclusion: training for health professionals exposed to surgical smoke is suggested, where it can be clarified about the use of Personal Protective Equipment correctly and its importance in preventing the development of occupational diseases.

ABSTRACT Objective: In view of the current COVID-19 pandemic, the objective of this study was to determine, through a systematic review and meta-analysis, whether the use of N95/FFP2 masks during aerobic exercise has a significant impact on HR, RR, SpO2, and blood pressure (BP) in healthy individuals. Methods: We searched the MEDLINE database for studies published in English between 2005 and 2021. To reduce bias and increase reliability, only randomized controlled trials and randomized crossover clinical trials were considered for inclusion. The selected outcomes included HR, RR, SpO2, and BP, with perceived exertion being evaluated by means of the Borg scale. Results: Eight controlled trials were included in the meta-analysis. Seven evaluated HR (p > 0.05), five evaluated RR (p > 0.05), five evaluated SpO2 and BP (p > 0.05 for both), and six evaluated perceived exertion, presenting controversial results such as risk ratios that were grouped for each variable. Conclusions: This study suggests that N95 and FFP2 masks do not have significant effects on HR, RR, SpO2, and BP during aerobic exercise in healthy individuals.


RESUMO Objetivo: Diante da atual pandemia de COVID-19, o objetivo deste estudo foi determinar, por meio de uma revisão sistemática e meta-análise, se o uso de máscaras N95/PFF2 durante o exercício aeróbico tem impacto significativo na FC, FR, SpO2 e pressão arterial (PA) em indivíduos saudáveis. Métodos: Buscamos no banco de dados MEDLINE estudos publicados em inglês entre 2005 e 2021. Para reduzir o viés e aumentar a confiabilidade, foram considerados para inclusão no estudo somente ensaios clínicos controlados randomizados e ensaios clínicos cruzados randomizados. Os desfechos selecionados foram FC, FR, SpO2 e PA; a percepção de esforço foi avaliada por meio da escala de Borg. Resultados: Oito ensaios controlados foram incluídos na meta-análise. Sete avaliaram FC (p > 0,05), cinco avaliaram FR (p > 0,05), cinco avaliaram SpO2 e PA (p > 0,05 para ambas) e seis avaliaram a percepção de esforço, com resultados controversos (razões de risco agrupadas para cada variável, por exemplo). Conclusões: Este estudo sugere que as máscaras N95 e PFF2 não têm efeitos significativos na FC, FR, SpO2 e PA durante o exercício aeróbico em indivíduos saudáveis.

ABSTRACT

Desde o início da pandemia, no final de 2019, até a retomada das atividades presencias de atendimento ao público nos cursos da área de saúde em Minas Gerais, especificamente nos cursos de odontologia; foram elaboradas notas técnicas, recomendações e manuais de biossegurança. Documentos esses que serviram como referencial para a padronização de protocolos de atendimento, incluindo o uso de EPI's mais eficientes no controle da transmissibilidade da Covid-19. A pesquisa de revisão foi realizada na base de dados PubMed/MEDLINE e Scielo, World Health Organization, Ministério da Saúde, Associação Brasileira de Ensino Odontológico, Conselho Federal de Odontologia, Conselhos Regionais de Odontologia dos Estados de São Paulo e Minas Gerais e instituições de ensino superior. Priorizando os conteúdos publicados entre os meses de janeiro a setembro de 2020. Período correspondente ao início da pandemia até o retorno das atividades de atendimento nos cursos de saúde em Minas Gerais. Buscando os documentos que nortearam esse retorno e que justificaram a adoção do uso desses EPI's pelos graduandos.


Since the beginning of the pandemic, at the end of 2019, until the resumption of face-to-face activities to serve the public in health care courses in Minas Gerais, specifically in dentistry courses; technical notes, recommendations and biosafety manuals were prepared. These documents served as a reference for the standardization of care protocols, including the use of more efficient personal protection equipment to control Covid-19's transmissibility. The review search was carried out in the PubMed/MEDLINE and Scielo databases, World Health Organization, Ministry of Health of Brazil, Brazilian Association of Dental Education, Federal Council of Dentistry, Regional Councils of Dentistry of the States of São Paulo and Minas Gerais and college. Prioritizing the contents published between January and September 2020. Period corresponding to the beginning of the pandemic until the return of care activities in health courses in Minas Gerais. Seeking the documents that guided this return and that justified the adoption of the use of this personal protective equipment by college students.

ABSTRACT

A pandemia de COVID-19 já causou mais de 399 milhões de infecções e custou a vida de mais de cinco milhões de pessoas no mundo, até 3 de março de 2022. Para reduzir a taxa de infecção, uma série de medidas de prevenção indicadas pela Organização Mundial da Saúde (OMS) foram adotadas pelos países, entre elas, o uso de máscara. O objetivo deste estudo é descrever a utilização de máscara na população brasileira, através da análise de dados do EPICOVID19-BR, um estudo de base populacional realizado em 133 cidades do país, em quatro fases entre março e agosto de 2020. A proporção de indivíduos que preferiram usar máscara quando saíam de casa foi de 97,9% (IC95%: 97,8-98,0). O entrevistador não visualizou a máscara do entrevistado em 50% (IC95%: 49,9-51,1) dos casos no momento da entrevista, no entanto, entre a fase uma e quatro da pesquisa, observou-se uma diminuição de 4,4 pontos percentuais na proporção de entrevistados que não usaram máscara no momento da entrevista. A não visualização da máscara foi mais observada em mulheres, participantes com idade entre 10-19 e 20-29 anos, de cor de pele indígena, preta, e parda, entre as pessoas com Ensinos Fundamental e Médio e na Região Centro-oeste. O uso de máscara de tecido foi predominante 91,4% (IC95%: 91,2-91,5) com um aumento de 4,9 pontos percentuais entre as fases 1 e 4. Os resultados do estudo trazem informações importantes para reforçar as políticas de controle de COVID-19 no Brasil. O alto percentual de pessoas sem máscara na hora da entrevista sugere que ainda é importante reforçar o aspecto preventivo e de autocuidado, não fazendo do uso da máscara algo apenas ligado à obrigatoriedade.


By March 3, 2022, the COVID-19 pandemic has caused more than 399 million infections and claimed the lives of more than five million people worldwide. To reduce infection rates, a series of prevention measures indicated by the World Health Organization (WHO) were adopted by countries, including the use of masks. This study aims to describe mask use in Brazil via data analysis from the EPICOVID19-BR, a population-based study conducted in 133 cities in the country in four phases between March and August 2020. The proportion of individuals who reported wearing a mask when they left their homes was 97.9% (95%CI: 97.8-98.0). The interviewer did not see interviewees' mask in 50% (95%CI: 49.9-51.1) of the cases at the time of the interview. However, between phase one and four of the survey, we observed a 4.4% decrease in the proportion of interviewees who failed to wear masks at the time of the interview. Mask non-visualization was more prominent in women, participants aged 10-19 and 20-29 years of indigenous, black, and brown skin color, and those with elementary and high school education and in the Central-West Region. The use of cloth masks showed a 91.4% predominance (95%CI: 91.2-1.5) with a 4.9% increase between phases 1 and 4. The results of the study bring important information to reinforce COVID-19 control policies in Brazil. The high percentage of people who failed to wear masks at the time of the interview suggests that it is still important to reinforce prevention and self-care, rather than relating mask wear to a mandatory measure.


La pandemia del COVID-19 ha provocado más de 399 millones de infecciones y se ha cobrado la vida de más de cinco millones de personas en todo el mundo hasta el 3 de Marzo de 2022. Para reducir la tasa de contagios, los países adoptaron una serie de medidas de prevención indicadas por la Organización Mundial de la Salud (OMS), entre ellas el uso de mascarillas. El objetivo de este estudio es describir el uso de mascarillas en la población brasileña, utilizando el análisis de datos de EPICOVID19-BR, un estudio de base poblacional realizado en 133 ciudades del país, en cuatro fases entre marzo y agosto de 2020. La proporción de personas que informaron usar mascarillas al salir de casa fue del 97,9% (IC95%: 97,8-98,0). El entrevistador no vio la mascarilla del entrevistado en el 50% (IC95%: 49,9-51,1) de los casos al momento de la entrevista, sin embargo entre las fases uno y cuatro de la investigación se observó una disminución de 4,4 puntos porcentuales en la proporción de los encuestados que no llevaban mascarilla durante la entrevista. Se observó una mayor visualización de falta de uso de mascarillas en las mujeres, en participantes con edades entre 10-19 y 20-29 años, de color de piel indígena, negra y parda, entre personas con educación primaria y secundaria y en la Región Centro-oeste. Hubo un mayor predominio de uso de mascarillas de tela en el 91,4% (IC95%: 91,2-91,5) con un aumento de 4,9 puntos porcentuales entre las fases 1 y 4. Los resultados muestran la importancia de fortalecer las políticas de prevención del COVID-19 en Brasil. El alto porcentaje de personas sin mascarilla al momento de la entrevista sugiere que es importante reforzar la prevención y el autocuidado en general no solo relacionado a la obligatoriedad en el uso de mascarillas.

RESUMEN El objetivo de este trabajo fue identificar el impacto de la introducción del cubrebocas contra el COVID-19. Se realizó una revisión de la literatura disponible en una base de datos especializada (PubMed), la cual se ajustó a las directrices enmarcadas en la metodología PRISMA (1). Se definieron ecuaciones de búsqueda bajo los términos MeSH: humanos (MeSH D006801), COVID-19 (MeSH D000086382), máscaras (MeSH D008397), SARS-CoV-2 (MeSH D000086402) y diseño de equipos (MeSH D004867), utilizando el carácter booleano AND. Con estas ecuaciones se buscaron artículos publicados en 2020: se recuperaron 676 registros en total, de los cuales se eligieron 82 que abordan el tema de interés y se incluyeron en la revisión final 49 artículos que cumplían con todos los criterios de inclusión y exclusión definidos. Los resultados sugieren que la introducción de mascarillas tuvo un impacto positivo para reducir el riesgo de COVID-19 y el uso inadecuado del cubrebocas es un factor de riesgo.


ABSTRACT The objective of this work was to identify the impact of the introduction of the mask against COVID-19. A review of the literature available in a specialized database, PubMed, was carried out, which was adjusted to the guidelines framed in the PRISMA methodology (1). Search equations were defined under the terms MeSH: human (MeSH D006801), COVID-19 (MeSH D000086382), masks (MeSH D008397), SARS-CoV-2 (MeSH D000086402) and equipment design (MeSH D004867), using the Boolean AND character. With these equations, articles published in 2020 were searched, recovering 676 records in total, choo-sing 82 that address the topic of interest and including in the final review 49 articles that met all the defined inclusion and exclusion criteria. The results suggest that the introduction of masks had a positive impact on reducing the risk of COVID-19 and the inappropriate use of masks is a risk factor.

ABSTRACT

The study assessed the Clinicopathological changes of Wistar rats exposed to municipal landfill gaseous particulate emissions. Four groups of Wistar rats were exposed to the different sites of landfill emission with use of filters for filtering landfill gaseous emission while the control group was not exposed. Performance of the animals was evaluated weekly, blood and lung, liver, kidney and heart tissues were taken for haematological and histopatholgical examinations. At the end of six weeks exposure, the peak weight was recorded as 108.96±0.45 of Group B as compared to 126.352±0.88 of the Control Group, as well as the decrease in the body weight of Group A-D compared to the Control Group. There was leucocytosis, lymphocytosis and neutrophilia in the rats that were exposed to landfill emission. There were pulmonary, hepatic, and renal lesions while the heart was spared. Changes were observed on the use of filters for filtering landfill gaseous emission. The 3M 6001 CN Gas Mask Carnister Cartridge which filters 50% of landfill emission together with black polythene nylon restricting external air into the filtered cage was not sufficient to reduce the effect of landfill gaseous emission at the end of the exposure period. The screening with PM2.5 material which filters 100% of PM2.5 emission was only able to screen PM2.5 emission but could not screen other landfill gaseous emission. This indicates that landfill gaseous emission is capable of inducing respiratory problems and tissue damage on dumpsite workers if there is sufficient and continuous exposure to landfill gaseous emissions. To prevent the hazard that may arose from dumpsite activities; gas nose mask respirator is recommended for an individual who works as scavengers and landfill workers before entering the dumpsite environment..

ABSTRACT

The SARS-CoV-2 pandemic has become a global health problem causing severe human respiratory infections. Countries have had to establish strategies to avoid the collapse of health systems. There has been reports describing that the human-to-human transmission is through droplet spread and contact routes as through hands and contaminated surfaces. Social distancing, personal protective equipment, hand washing often, and surface disinfection play a fundamental role in disease control. Some procedures and situations aerosolize the SARS-CoV-2, so protection measures must be extreme. Hard work is underway to develop and implement a vaccine that would provide immunity to the population, but it will take some time. Preventive measures must incorporate good epidemiological monitoring that guarantees adequate control of cases and contacts in order to isolate them from the rest of the population, whether hospitalized or at home.


La pandemia de SARS-CoV-2 se ha convertido en un problema global de salud provocando infecciones respiratorias severas en humanos. Los países han tenido que establecer estrategias para evitar el colapso de los sistemas sanitarios. Se ha descrito una transmisión de persona a persona facilitada por propagación de gotitas, manos o superficies contaminadas. El distanciamiento físico, los elementos de protección personal, el lavado de manos frecuente y la desinfección de superficies cumplen un rol fundamental en el control de la enfermedad. Algunos procedimientos y situaciones aerosolizan el SARS-CoV-2 por lo que se deben extremar las medidas de protección. Se trabaja arduamente para lograr una vacuna que otorgue inmunidad a la población, pero su desarrollo va a tomar algún tiempo. Las medidas preventivas deben incorporar una buena vigilancia epidemiológica que garantice el control adecuado de los casos y contactos, con el objeto de aislarlos del resto de la población, ya sea hospitalizados o en sus domicilios.

ABSTRACT

El SARS-CoV-2 es un tipo de coronavirus con un alto grado de contagio entre pacientes y personal de salud, por lo que ha provocado la emergencia sanitaria más grande de los últimos años. Los equipos de protección personal (EPP) tienen como indicación un único uso, sin embargo, la escasez de estos ha llevado a buscar métodos para descontaminarlos y reutilizarlos, asegurando su disponibilidad. La evidencia presenta múltiples procedimientos para descontaminar mascarillas con filtro respirador tipo N95 (MFR N95), pero el método ideal debe inactivar al patógeno, asegurar la integridad del filtro respirador, el ajuste de la mascarilla y la salud del personal que la utilizará. La descontaminación con peróxido de hidrógeno vaporizado, irradiación germicida ultravioleta e incubación con calor húmedo se presentan como los métodos más eficaces contra distintos patógenos. Ante la creciente demanda de EPP y MFR N95, se sugiere realizar más estudios clínicos, que evalúen la eficacia de los métodos de descontaminación específicamente contra SARS-CoV-2 y se generen protocolos que permitan reutilizar con total seguridad este vital elemento para el personal de salud.


SARS-CoV-2 is a type of coronavirus with a high risk of contagion among patients and health care workers (HCW), being the cause of the largest health emergency in recent years. Personal Protective Equipment (PPE) are indicated for a single use. However, shortage has led to research new methods to decontaminate and reuse them, ensuring its availability. Evidence presents multiple procedures to decontaminate Filtering Facepiece Respirators type N95 (FFR), despite this, the ideal treatment should inactivate the pathogen, to ensure filter integrity, mask adjustments and the safety of who use this elements. Decontamination methods as vaporized hydrogen peroxide, ultraviolet germicidal irradiation and moist heat are the most effective procedures against different pathogens. Facing the rising demand of PPE and FFR, it's suggested to conduct researches to evaluate decontamination methods efficacy, and specifically against SARS-CoV-2, to generate protocols in ways to let to HCW reuse FFR N95 totally safe.

ABSTRACT

OBJECTIVE:To test the efficacy and safety of PhilVent™, a pressure limited, time cycled, Philippine ventilator through animal and clinical studies. METHODS AND RESULTS: Animal study: Term, newborn piglets (N=8) were intubated and alternately cycled to the PhilVent™ or to a pressure limited, time cycled, commercial ventilator (Sechrist) at peak inspiratory pressures of 10, 13 and 15 cm H20 and rates of 15, 20, 25 breaths per min and constant FiO2 (0.40) and positive end expiratory pressure (+4). Blood gases and adverse events (pneumothorax, sudden deterioration, death) were monitored. Results show no significant difference in blood gases on either machine at the various ventilator settings. No adverse events occurred. Clinical study: Prospective, randomized, controlled trial of 90 preterm infants with respiratory distress, randomized either to PhilVent™ (N=45) or Sechrist (N=45). Ventilator settings were adjusted to achieve predetermined range of blood gases. Arterial blood gases and any adverse events e.g., pneumothorax, pulmonary hemorrhage were monitored. There were no clinically significant differences in the ventilator settings or blood gases of the infants on the PhilVent™ or Sechrist. No increase in adverse events were noted with the PhilVent™. CONCLUSION: In animal and clinical studies, the efficacy and safety of the PhilVent were comparable to the Sechrist. The PhilVent™ is an effective, alternative ventilator for the treatment of respiratory insufficiency in newborn infants.
