ABSTRACT

Objective:To investigate and analyze a human brucellosis (hereinafter referred to as brucellosis) in Fushan District, Yantai City of Shandong Province, and to provide scientific basis for study the main risk factors and doing a good job in brucellosis prevention and control.Methods:Collect the epidemic data of one human brucellosis in Fushan District, Yantai City in 2017. The information comes from the infectious disease report information management system, the epidemiological case questionnaire of brucellosis in Shandong Province, and the medical records of medical institutions. The epidemic situation, cases and epidemiological investigation results were analyzed retrospectively.Results:Serological tests were carried out among 48 close contacts of the reported case and workers in the same village. Among the 3 positive cases, 2 cases had symptoms and positive bacterial culture results, which were diagnosed as confirmed cases. One patient was asymptomatic and negative bacterial culture, and was diagnosed as recessive infection. All of the three confirmed cases and one recessive infection involved in the epidemic had a history of close contact with infected animals, and without protective measures. The serological examination of 70 key people in the district was carried out and the results were all negative. A total of 166 sheep blood samples were collected, including 21 positive samples.Conclusions:The infectious source of this outbreak is the non-quarantine infected sheep. The main exposure factor is personal unprotected exposure to infected sheep. The key of prevention and control is to strengthen the monitoring and quarantine management of brucellosis among livestock, to improve the personal protection consciousness and ability of key population, and to improve the diagnosis ability of medical institutions in low epidemic areas.

ABSTRACT

Objective To dynamic monitor drinking-water-borne fluorosis in Huzhu County and water improvement projects,to know the trend of the disease,and to evaluate the effect of prevention and control measures.Methods Seven townships in Huzhu County including 60 history drinking-water-borne fluorosis villages were selected as investigation sites in 2017,and demographic data were collected.In the villages of the diseased areas where the water had been changed,the operation of the water improvement project was investigated,and 1 tap water sample was collected to determine the fluorine content;for the villages in the diseased areas where the water had not been changed,one water sample was collected from the drinking water source in accordance with the five directions of east,west,south,north and middle to determine the fluorine content.More than 90% of the children aged 8-12 years old in the surveyed villages were examined and judged for dental fluorosis.According to historical data,1 village with severe illness of children was selected,50 people were selected who were over 25 years old and lived in the villages for more than 5 years.Clinical and X-ray examinations of skeletal fluorosis were performed.Urine samples from adults surveyed were collected for urinary fluoride detection.Results Six of the 60 villages in the 7 townships had been relocated.By 2017,a total of 54 villages in drinking-water-borne fluorosis areas had changed their water.A total of 16 water improvement project were surveyed,among them,there were small water improvement project 6,accounting for 37.50%,large water improvement project 10,accounting for 62.50%;normal operated project 16,accounting for 100.00%;water fluoride content of qualified project 16,the qualified rate was 100.00%,the water fluoride content ranged 0.10-0.66 mg/L,covering a population of about 160 thousand and 846 people.A total of 2 399 children aged 8-12 were examined,117 cases of dental fluorosis were detected,the detection rate of dental fluorosis was 4.88%,dental fluorosis index was 0.13.Among them,89 cases were extremely mild,accounting for 3.71% of all the children examined,21 cases were mild,accounting for 0.88% of all the children examined,7 cases were moderate,accounting for 0.29% of all the children examined,and no severe cases were found.A total of 68 adults over 25 years old were examined for skeletal fluorosis.Twelve patients were found to have clinical skeletal fluorosis.The detection rate was 17.65%.X-ray radiography was used to examine 46 people,and 1 case was detected,the detection rate was 2.17%.Clinical and X ray examinations showed no more than moderate patients.Fifty samples of adult urine were tested,the level of urine fluorine was 0.72 mg/L.Conclusions Water improvement project is working well in Huzhu County,and no serious children with dental fluorosis and adult skeletal fluorosis are detected.The prevention and control measures of drinking-water-bornefluorosis has achieved remarkable results in Huzhu County.We will continue to strengthen water improvement project and disease monitoring,and thoroughly control the prevalence of drinking water fluorosis.

ABSTRACT

Objective To analysis the on-site quality control assessment results of salt iodine content in Zhengzhou City,and to understand and master the actual professional abilities and level of grass-roots laboratory analyzers,and to search the problems and make improvement measures.Methods The total of 22 analyzers from 11 affiliated grass-roots laboratories of Zhengzhou City were gathered to take part in the on-site theoretical examination.The theoretical assessment was divided into five parts:measurement principle,application range,reagent preparation,key steps,and calculation formula of results.Parallel samples of standard materials were determinated in laboratory units,to evaluated operational skills,the problems during the process were recorded simultaneously.The content of the theoretical assessment and the rules for the assessment of on-site skills were based on the "Measurement of Iodine for the General Test Method of the Salt Industry" (GB/T 13025.7-2012).Results In the theoretical assessment,the correct rates of reagent preparation and key steps were 100%,and that of measurement principle,application range,and calculation formula of results were more than 80%.The main problems of operation skills were from liquid loading,sample weighing and titration operation,etc.One laboratory result of quality control sample exceeded the reference range and was judged to be unqualified.Conclusions The theoretical basis of some laboratory analyzers are not solid,the operational skills are lack of standardization.In view of the current situation,as the higher level technical guidance unit,it is necessary and responsible for us to formulate practical measures in training,guidance and assessment to help them improve business level and abilities.

ABSTRACT

Objective To investigate and analyze the epidemiological characteristics of an epidemic situation of human brucellosis epidemic in Xinchang County Zhejiang Province,and provide experience for prevention and treatment of brucellosis.Methods Retrospective analysis was performed to collect the data of epidemic situation,field case investigation,serum test and hospital diagnosis and treatment,and the situation of epidemic situation in 2018 of Xinchang County and field investigation were summarized and analyzed.Results A total of 3 confirmed cases were reported in this outbreak,one of them was a laboratory diagnosed case of brucellosis,2 cases were closely contacted with sick animals,and those 2 cases were confirmed by serological tests (n =30).The source of infection was the purchase and slaughter of unquarantined goats.A total of 10 goats blood samples were collected in the inter-animal investigation,4 of them were positive.Conclusion Unprotected contact with infected goats is a major exposure factor,raising awareness and promoting publicity about the path of infection with brucellosis is the key to preventing brucellosis.

ABSTRACT

Objective To analyze the epidemiological surveillance results of human brucellosis in Shanxi Province from 2012 to 2017,to know the epidemic status of brucellosis,and to provide evidence for prevention and control of brucellosis.Methods Incidence date and surveillance date of disease outbreaks in Shanxi Province from 2012 to 2017 were collected,the retrospective analysis method was used to analysis the "three distribution" of brucellosis,outbreak situation and the results of serological and pathogenic surveillance in the 4 surveillance stations.Results A total of 36 220 brucellosis cases were reported from 2012 to 2017,the average incidence was 16.62/100 000;8 540 brucellosis cases were reported in 2014,with incidence 23.53/100 000;a total of 23 197 cases of brucellosis were reported mainly in Datong,Shuozhou,Jinzhong and Xinzhou,accounting for 64.04% of the province total.The onset was seasonal,and the peak of the epidemic was from March to August,accounting for 67.23% (24 350/36 220).The brucellosis cases were mainly youth (23 084),male (28 317),farmers and herdsman (32 616).In the 4 surveillance stations of the brucellosis,39 140 cases were investigated,of which 10 536 cases did serological test,in which 585 were positive for Brucella (5.55%).The highest positive rate of serological test was 9.50% (226/2 738) which was found in Tianzhen.A total of 626 samples carried out pathogen culture,in which 107 strains of brucellosis were detected,the detection rate was 17.09%,and 106 strains Brucella were melitensis biovar 3 of the total strains except 1 mutant.Conclusions The reported incidence in Shanxi Province is in a decline tendency,but the situation of brucellosis epidemic is still relatively serious.It is suggested that the surveillance work should be strengthened;the epidemic situation of brucellosis should be mastered in time and effectively controlled.

ABSTRACT

Objective@#To analyze the results obtained from the proficiency testing program for determination of bovine blood lead and urinary cadmium, so as to evaluate the proficiency of the laboratories, and improve the detection and quality control capability of occapational health laboratory.@*Methods@#Prepared 3 levels of lead samples with bovine blood as base material, taking Technical Norm of Primary Reference Material as Operating standard, 60, 80, 180 μg/L were used as the standard concentration. All samples were made for 200 bottles. Normal urine were used to prepared 3 levels of cadmium samples with 5.0, 7.0, 10.0 μg/L as standard concentration, then 3 level of samples were made for 200 bottles respectively. Blood lead and urinary cadmium samples were sent to each laboratory in the form of sample pairs. One-way ANOVA was used to investigate the homogeneity. The results of each laboratory were statistically analyzed by robust statistical method, and the detection ability of the laboratory was evaluated by the Z ratio score method. By consulting the test reports and original records of each participating laboratory, the reasons for the unsatisfactory results were analyzed.@*Results@#The statistical F of the uniformity of blood lead 1.42, 1.37 and 2.85, F<3.13, P were 0.294, 0.314 and 0.059, respectively in 31 labs. There is no statistical significance in the difference of blood lead (P>0.05). And urine cadmium was less than the critical value (P>0.05). In all, 31 laboratories participated in the test of blood lead, and 28 loboratories paticipated in urine cadmium verification. The statistical F of the uniformity of urinary cadmium 1.90, 1.35 and 1.19, F<3.13. The homogeneity of the samples were in accordance with the requirements of proficiency testing program. The satisfactory rate of the results for blood lead was 84% (26/31) , and the satisfactory rate of urinary cadmium was 93% (26/28) . In the test of blood lead, 21 of the 31 laboratories were controlled by the standard material method. Totally, 20 of them get an satisfactory level, the rate was 95%.@*Conclusion@#Most of the laboratory test results were satisfactory, which showed that the detection capability for blood lead and urine cadmium was good. The main causes of unsatisfactory results were detection methods, tools of quality control ortraceability standards.

ABSTRACT

Objective To explore epidemiological characteristics of brucellosis in Shanxi Province during 2011-2015,and to provide evidences for making control and prevention strategy.Methods Data and surveillance data of disease outbreaks in Shanxi Province from 2011 to 2015 were collected.The retrospective analysis method was used to analyze the population distribution,regional distribution,time distribution,outbreak situation,serological and pathogenic test results of brucellosis.Results During 2011-2015,totally 33 697 brucellosis cases were reported,with an average annual incidence of 18.65/100 000,and one death case was reported.Brucellosis distributed in 119 counties (districts) of 11 cities.The area with most reported brucellosis cases was Datong City.The incidence of the disease had seasonal fluctuation and the incidence peak was from March to July.The male/female ratio of the cases was 3.69 ∶ 1.00 (26 515/7 182).Totally 75.77% (25 531/33 697) were at the ages of 40-< 70 years.The yearly proportion of the cases occurred in the age groups was significantly different (H =146.263,P < 0.05).Farmer was the predominant occupation group among the cases [82.90% (27 935/33 697)].Totally 16 cases of human brucellosis outbreaks were occurred during the five years.The epidemic spread to seven counties in five cities.Totally 144 136 cases received serological test,the positive rate was 5.85% (8 426/144 136),and 6 759 cases were found.Totally 164 strains of Brucella melitensis were isolated in human blood culture samples.All of which were Brucella melitensis biotype 3.Conclusions In recent years,the epidemic situation of brucellosis in Shanxi is severe,the total incidence has presented a rising tendency.There are outbreaks of human brucellosis in some countries.Brucella melitensis biotype 3 is the epidemic strain in Shanxi.

ABSTRACT

Objective To find out the distribution and status of drinking-water-borne fluorosis in Qinghai,and to provide basic data for prevention and control of the disease.Methods According to the "Action Plan of Endemic Fluorosis and Arsenic Poisoning in Qinghai (2016-2020)",10 counties were selected to carry out the epidemiological study in 2016.The operation of the improvment project was investigated.The fluoride content of water was determined with fluoride ion-selective electrode,and dental fluorosis of 8-12 years old children was diagnosed by the Deans method.Results A total of 97 villages have been investigated,and all water was improved.The range of water fluoride was 0.20-2.13 mg/L.Normal operation rate of water-improving project was 83.33% (50/60).The pass rate of water fluorine content was 96.67% (58/60).The prevalence rate of dental fluorosis of the children was 18.70% (774/4 140),and it was significantly lower than national standards (30%).Conclusions Water has been improved in all of the fluorosis villages.Most of the water-improving engineering projects to control drinking-water fluorosis has played a positive role,and drinking-water-borne fluorosis is under control.

ABSTRACT

OBJECTIVE: To discuss the problem of instrumental test method of clarity and degree of opalescence of liquids in the general notice 0902 of China Pharmacopeia(Ch.P) 2015, find out the causes and offer solutions. METHODS: The test methods of clarity in Ch.P 2015 and European Pharmacopeia (EP) 8.0 were compared, including instrumental types, applicability, sample requirements, and result evaluation. RESULTS: The primary opalescent suspension for the instrumental method is the same as the visual method, using the absorbance (A=0.12-0.15) at 550 nm to control the opalescence. Because the resolving power of the instrumental method is far higher than the visual method, the limit becomes interval distribution instead of simple point. The opalescent value (NTU) of the upper limit (A=0.15) is about 1.35 times of the lower limit (A=0.12). When the NTU value of the test liquid is in this interval, the result evaluation will be hard. CONCLUSION: The preparation of the primary opalescent suspension in Ch.P 2015 is different from EP8.0. For this reason, the limit set by Ch.P 2015 is actually stricter than that of EP8.0. The opalescent value of the standard solution used by Ch.P 2015 is about 75% of that used by EP8.0.

ABSTRACT

Trata-se de estudo de intervenção tipo antes e depois, no qual o sujeito é seu próprio controle, fator que permite identificar os efeitos na adesão ao tratamento e controle dos níveis glicêmicos. Teve como objetivo avaliar a contribuição da consulta de enfermagem na adesão ao tratamento do diabetes mellitus tipo 2, em uma Unidade Saúde da Família, de acordo com o "Protocolo de atendimento as pessoas com diabetes mellitus," em Ribeirão Preto, SP. A coleta de dados foi realizada no período de setembro de 2014 a janeiro de 2015. O trabalho foi aprovado pelo Comitê de Ética em Pesquisa da Escola de Enfermagem de Ribeirão Preto, SP, sob Parecer nº 648.970. Participaram 31 pessoas com diabetes mellitus, por meio de três consultas de enfermagem, na unidade de saúde e no domicílio, com intervalo de um mês entre as três consultas de todos os participantes. Foi utilizado um roteiro contendo variáveis sociodemográficos e clínicas e o teste de Medida de Adesão ao Tratamento. Para a análise da adesão, durante e após a intervenção, utilizou-se a estatística descritiva e o teste de Mann- Whitney; para a comparação do antes e após a intervenção, utilizou-se o teste de Wilcoxon; para análise de correlação com as variáveis numéricas, o coeficiente de correlação de Spearman e o teste Q de Cochran, para a comparação dos exames nos momentos anterior, durante e posterior à intervenção. Os resultados mostraram que os participantes tinham entre 33 e 79 anos, sendo 58,1% do sexo feminino; 71% tinham companheiro; renda familiar de 1 a 3 salários-mínimos (83,9%); 80,6% referiram ser profissionalmente inativos (aposentados, pensionistas ou do lar); média de 5,68 anos de estudo e predomínio de menos de 8 anos de estudo (67,7%). Em relação aos valores da pressão arterial sistêmica constatou hipertensão arterial sistêmica grau I em 25,8% das pessoas com diabetes mellitus, 90,3% com índice de massa corporal apresentando excesso de peso, quanto à circunferência abdominal, 32,2% dos homens estavam com valores maiores que 102 cm e 45,2% das mulheres com valores acima de 88 cm. A avaliação dos pés, com uso do monofilamento Semmes-Weinstein de 10g, apresentou 9,7% das pessoas com diabetes mellitus com pé em risco para ulceração e diminuição ou ausência de sensibilidade tátil pressórica protetora dos pés. O tempo de diagnóstico do diabetes mellitus tipo 2 variou entre 1 a 39 anos, predominando as comorbidades hipertensão arterial (83,9%), dislipidemia (58,1%) e obesidade (41,8%). Quanto aos exames laboratoriais, observa-se que, em 64,5% da população estudada, os níveis da glicemia de jejum estavam acima de 100 mg/dL , ocorrendo pequena redução para 61,3% nos casos de pessoas com diabetes mellitus durante a intervenção e se manteve após. No que se refere à glicemia pós-prandial, os casos das pessoas com diabetes mellitus com valores iguais ou acima de 160 mg/dL, antes da intervenção era de 45,2% e durante e após a intervenção caiu para 38,7%. Em contrapartida, aumentou o número de pessoas com diabetes mellitus durante e após a intervenção, com valores da glicemia pós-prandial abaixo de 160 mg/dL, de 54,8% para 61,3%. E, em relação à hemoglobina glicada, foi observado que em 61,3% das pessoas com diabetes mellitus os valores antes da intervenção eram iguais ou acima de 7%. Durante a intervenção, caiu para 19,3% e após a intervenção o número de pessoas com diabetes mellitus, com a hemoglobina glicada igual ou superior a 7%, chegou a 38,7%. Quanto aos valores abaixo de 7%, observou-se aumento de 38,7% antes da intervenção para 80,6 e 61,3% respectivamente, durante e após a intervenção, com diferença estatisticamente significante (p< 0,001). As pessoas com diabetes mellitus desse estudo, apresentaram 83,87% de adesão ao tratamento antes da intervenção, e esses escores subiram para 96,78% após a intervenção, fato corroborado pelo teste de Wilcoxon que mostrou escores estatisticamente significantes (p<0,001), entre antes e após a intervenção. Esse estudo contribui para ressaltar a importância do enfermeiro, enquanto integrante da equipe multiprofissional, seguindo as orientações do "Protocolo de atendimento ao indivíduo com diabetes", tanto no atendimento individual quanto em grupo, reorganizando o processo de trabalho, contribuindo para maior adesão ao tratamento e controle dos níveis glicêmicos, ao minimizar a fragmentação e assegurar a continuidade na assistência, por meio de abordagem integral ao diabético


This was a before- and after-intervention study, in which the subject serves as its own control, aimed to analyze the effects of intervention on treatment adherence and blood sugar control. The objective of the study was to evaluate the contribution of nursing care to treatment adherence of type 2 diabetes mellitus patients following the "diabetes care protocol" adopted by Family Health centers in Ribeirão Preto, São Paulo, Brazil. Data were collected between September 2014 and January 2015. The Research Ethics Committee of the Ribeirao Preto College of Nursing approved the study under ruling no. 648.970. From 31 participants with diabetes by way of three meetings with nurses, either at the health center or at home, with a month-long interval between appointments. We used an interview script to gather sociodemographic and clinical variables and gave participants a test to measure treatment adherence. Descriptive statistics and the Mann-Whitney test were used to analyze adherence during and after intervention; the Wilcoxon test was used for the before and after-intervention comparison; Spearman's Rank Correlation Coefficient was used to analyze the correlation between the numerical variables; and Cochran's Q test was used to compare tests before, during, and after intervention. Results showed that participants were between 33 and 79 years old, of which 58.1% were female; 71% had a partner; 83.9% had a family income of 1 to 3 minimum wages; and 80.6% referred to themselves as professionally inactive (retirees, pensioners, or homemakers). The mean education level was 5.68 years of formal education with a predominance of less than 8 years of formal education (67.7%). Regarding blood pressure values, stage 1 hypertension was detected in 25.8% of individuals with diabetes, and 90.3% had a body mass index indicating overweight. In terms of waist circumference, 32.2% of the men presented values greater than 102 cm and 45.2% of the women showed values greater than 88 cm. Foot assessment using 10g Semmes-Weinstein monofilaments indicated that 9.7% of diabetic individuals' feet were at risk for plantar ulcers and showed reduced or lack of protective sensation. Time of type 2 diabetes diagnosis varied between 1 to 39 years, and the most prevalent comorbidities were high blood pressure (83.9%), dyslipidemia (58.1%) and obesity (41.8%). In terms of laboratory tests, 64.5% of the studied population presented levels of fasting blood sugar levels above 100 mg/dL, which fell slightly to 61.3% during the intervention, and were then maintained after the intervention. Postprandial glucose tests revealed that 45.2% of individuals with diabetes presented values equal or greater than 160 mg/dL before the intervention, falling to 38.7% post-intervention. In turn, the number of individuals with diabetes with postprandial glucose levels greater than 160 mg/dL increased from 54.8% to 61.3%. Regarding glycated hemoglobin, 61.3% of diabetic patients presented pre-intervention values equal or greater than 7%. During the intervention, this value fell to 19.3%, and after the intervention, 38.7%. Patients with values below 7% corresponded to 38.7% before the intervention and 80.6% and 61.3% during and post-intervention, respectively, with a statistically significant difference (p< 0.001). Diabetic individuals in this study presented an 83.87% rate of treatment adherence before the intervention, a percentage that rose to 96.78% post-intervention, a finding corroborated by the Wilcoxon test that showed statistically significant scores (p< 0.001) between pre- and post-intervention. This study emphasizes the importance of nurses as members of multiprofessional teams following the guidelines set forth in the "diabetes care protocol," both in individual and group interventions, reorganizing work processes, and contributing to greater treatment adherence and blood sugar control by minimizing fragmentation and ensuring continuity of care for diabetic patients via a comprehensive approach

Objective To understand the status of drinking-water supply and the progress of waterimproving projects in drinking-water-borne endemic fluorosis areas in Henan,and to provide scientific data for related government departments to carry out prevention and surveillance plan for those areas.Methods Questionnaire survey was carried out in all the villages in drinking-water-borne endemic fluorosis areas in Henan in 2010.Each village was given one set of questionnaire.Results By the end of 2010,the total number of fluorosis villages in Henan was 25 434,among them 11 484 villages had been conducted water-improving projects,accounting for 45.15%.Currently,9267 water-improving projects worked properly,accounting for 36.4%(9267/25 434)of all villages surveyed,and 80.7% (9267/11 484)villages had water-improving projects.The projects in 2217 villages do not work properly,accounting for 19.3%(2217/11 484) of all projects.There were 5832 water-imp.roving projects conducted amnog 2005-2010,accounting for 50.8% (5832/11 484) of all water-improving projects.Among villages with no such projects,97.5%(15 769/16 167) of them used shallow groundwater,and in villages with water-improving projects,89.6% (8303/9267) of them used deep groundwater as drinking-water supply.Conclusions After 2005,the water improving progress was significantly speed up in drinking-water-borne endemic fluorosis areas in Henan,but progress of the water improving projects is relatively slow due to changing and expanding of fluorosis areas.The government should increase investment and improve the proportion of waterimproving defluoridation.

ABSTRACT

[Objective]To investigate the indications,complications and mid-term results of total knee arthroplasties using rotating hinge knee prosthesis. [Methods]Thirty total knee arthroplasties using rotating hinge prosthesis were performed in twenty-eight patients from July 1998 to July 2008.Seven patients were male and twenty-one patients were female.The average age was 62 years(42-75 years).The diagnosis included osteoarthritis in 8 patients,rheumatoid arthritis in 2 patients,traumatic arthritis in 3 patients,Paget's disease patient,serious genu valgum in 7 patients and 10 failure TKAs.All patients were followed up for 12 to 120 months.The patients were evaluated according to knee score system and the complications were observed.[Results]The pain was relieved and function was improved in all knees.The average knee pain score increased from 19.5 to 77.9,and knee function score from 22.5 to 50.5.Range of motion of knees increased from 58.4?to 72.8?.No revision was needed in all patients,The complications included anterior knee pain(8 cases),extension lag(5 cases),prosthesis broken(1 case) and patella tendon rupture(1 case).[Conclusion]The mid-term results of total knee arthroplasty using rotating hinge knee for severe knee deformity are satisfactory.Complications should be prevented carefully.

ABSTRACT

PBL teaching method is a new mode of teaching which is originated from the West and implemented into China in recent years with an expectation that it would mainly develop the students’ self-learning ability,and enhance their skills of comprehensive thinking and solving actual problems.The author summarizes the practical experience of using PBL teaching methods in the theory teaching in Department of Medical Microbiology and Human Parasitology,China Medical University in the past three years,and then proved this method is very helpful to improving the students’integrated thinking by analysis of sample.At the same time the results also suggested that the students showed high enthusiasm in discussing the cases.By this way,the students showed great subjective intiative in their studies.
