ABSTRACT

Objective:To explore the effect of SBAR (situation, background, assessment, recommendation) communication model combined with scenario simulation in clinical teaching of blood purification center.Methods:Sixty interns in the blood purification center from March 2019 to March 2020 were collected in the study, and randomly divided into experimental group ( n=30) and control group ( n=30). The two groups of students studied in two different treatment groups in the blood purification center. The control group used the method of traditional teaching, while the experimental group used SBAR communication combined with situational simulation for teaching on the basis of traditional teaching, with a total clinical practice time of two months. All trainees underwent theoretical examination, clinical operation examination, self-efficacy evaluation and teaching satisfaction survey after training. SPSS 22.0 was performed for t test. Results:Through SBAR communication model combined with situational simulation teaching, the scores of theoretical examination [(88.13±3.22) vs. (85.51±3.17)], clinical operation assessment [(91.31±3.48) vs. (84.17±4.94)], self-efficacy evaluation [(11.13±1.08) vs. (9.21±1.89)], teaching satisfaction [(95.71±2.87) vs. (91.18±3.08)], and communication ability [(12.27±1.13) vs. (8.63±1.45)] in the experimental group were higher than those in the control group; while there was no significant difference in the scores of theoretical assessment between the two groups ( P<0.05). Conclusion:SBAR communication model combined with situational simulation teaching can improve the learning ability, operation ability and comprehensive application ability of special training trainees, especially in communication ability, which is worthy of popularization and application in clinical teaching.

ABSTRACT

Objective:To analyze the role of wechat combined with SBAR communication in emergency nursing management.Methods:From December 2019 to December 2020, 100 cases of patients in the emergency department of No.904 Hospital, Joint Logistics Support Force of PLA were randomly selected and divided into two groups according to the computer random table method, 50 cases in the control group were treated with routine nursing management, 50 cases in the study group were treated with Wechat combined with SBAR communication mode for management. The goal achievement, nursing service quality and nursing satisfaction of the two groups were compared.Results:The score of Nurse′s Performancefor Goal Attainment (NPGA) in the study group was 196.32 ± 20.36, which was higher than (165.24 ± 18.14) of the control group ( t=8.06, P<0.05), and the score of Servqual evaluation questionnaire (94.32 ± 3.36) was higher than (89.24 ± 3.14) of the control group ( t= 7.81, P<0.05). The difference of nursing satisfaction between the two groups was significant, the study group (92.00%, 46/50) was higher than (64.00%, 32/50) of the control group ( χ2=11.42, P<0.05). Conclusions:In the emergency nursing work, using wechat combined with SBAR communication management can effectively improve the quality of nursing service, improve work efficiency, and increase patients′ recognition of nursing staff.

ABSTRACT

Objective:To explore the application effect of SBAR (situation, background, assessment and recommendation) standard communication mode in the training of morning shift handover ability of nursing interns in gynecology and obstetrics, so as to provide reference for improving the ability of morning shift handover of nursing interns.Methods:This study included in 30 nursing interns in Xuanwu Hospital in 2018 who were selected as the observation group, and 31 nursing interns in 2017 who were selected as the control group. The control group was trained in the traditional way of teaching, while the observation group was trained with SBAR standard communication mode on the basis of traditional teaching. After one month's teaching, the evaluation standard of nursing morning shift handover formulated by our hospital was used as the evaluation index to observe the effectiveness of SBAR mode in improving the morning shift handover ability of nursing interns in obstetrics and gynecology. The scores of morning shift handover of the nursing interns in two groups were compared by independent sample t test. Results:The total average score of nursing interns in the observation group was higher than that of the control group, with statistical significance ( P<0.05). Among them, the scores of shift handover environment, complete content, reasonable process and reasonable time control were higher and the difference was statistically significant ( P<0.05), but there was no significant difference in the scores of emphasis, specialty characteristics and language fluency ( P>0.05). The total average score of bedside handover ability in the observation group was significantly higher than that in the control group, with significant difference ( P<0.05). The scores of shift handover environment, complete content, outlining key points, reflecting specialty characteristics, reasonable process, material preparation and humanistic care were higher than those of the control group, with statistical significance ( P<0.05). Conclusion:SBAR mode can improve the bedside and collective shift handover ability of obstetrics and gynecology nursing interns, and nursing managers can use SBAR mode to cultivate nursing interns' ability of morning shift handover.

ABSTRACT

Objective To explore the effect of SBAR communication mode in handover between delivery room and neonatal department. Methods A total of 100 cases of neonates transferred from neonatal intensive care unit from March to June 2016 were selected as the control group.The traditional oral transfer mode was used. From June to September 2016, 106 cases of newborn infants who were transferred to neonatal intensive care unit were observed in the observation group. The newborns were transferred with SBAR structural framework. The differences of the integrity, the correct rate, the proportion of adverse events,the transfer time and the satisfaction of the medical staff in the 2 groups were compared. Results The integrity, the correct rate, the proportion of adverse events, the satisfaction of the medical and nursing staff on the integrity of the handover, the accuracy of the description of the disease,the practicality,and the operability were 99.06%(105/106), 99.06%(105/106), 15.09%(16/106), 93.33%(28/30), 90.00%(27/30), 96.67%(29/30), 93.33%(28/30) in the observation group, 89.00%(89/100), 94.00%(94/100),2.00%(2/100),63.33%(19/30),60.00%(18/30),73.33%(22/30),26.67%(8/30)in the control group,the differences were significant(χ2=4.008-27.778,P<0.05 or 0.01).There was no significant difference in the transit time (P> 0.05). Conclusions The design and application of the handover record based on SBAR communication mode in the delivery room and neonatal department improve the integrity of the patient′s condition,the correct rate,can detect adverse events in time.The staff were more comprehensive,more accurate description of the disease description, this form is simple and practical, operable, by the clinical staff of the trust,it is worth in the delivery room and the promotion of the use of newborns.

ABSTRACT

Objective To explore nursing safety management path of patients treat with modified electroconvulsive therapy(MECT) to improve the quality of nursing and reduce the adverse events. Methods According to the theory of Hazard Analysis and Critical Control Point(HACCP)and the SBAR standard communicative model, design and apply nurses handover check list for patients who treat with MECT(the following are referred to as MECT form).The nurses used MECT form to evaluate and manage the risk of patients before and after MECT treatment. This was also help to control the standardization criterion of nursing before and after treatment,meanwhile reduce the risk of treatment.Analyze the data of adverse event before and after applied this MECT form. Results After applied this form for one year,the number of above levle-0 adverse events decreased from 3.51% (191/5 441) to 1.05% (61/5 817) (x2=77.856, P 0.05). Conclusion The application of MECT form improves the nursing quality of MECT treatment in psychiatric department and secure the nursing safety.

@#The next step after the biopsychosocial information is collected from the extended history, extended examination, and extended investigation is the formulation of the 4P factors related to the patient’s problems (Predisposing, Perpetuating, Precipitating, and Protective factors). In this 4P formulation, attention is paid to the genogram, time-line, ROADS of the family, and the patient’s readiness to change. In certain situations, SBAR tool (Situation, Background, Assessment, Recommendation) may be needed to help get a better clarity of the situation and issues. Formulation helps us to be strategic in our psychosocial management.

ABSTRACT

Objective To explore the application and effect of SBAR communication model in clinical nursing teaching. Methods Delphi Method was used to determine SBAR clinical nursing teaching points and the clinical teaching program. A total of 80 baccalaureate and associated students were selected as the research subjects by the method of stratified sampling in the 2015-2016 academic year of Affiliated Hospital of Weifang Medical University.They were divided into experimental group and control group by random digits method.The control group was taught regularly,and the experimental group was given SBAR communication model. Both teachers and nursing students′ satisfaction, patient satisfaction, nursing students and patient communication ability questionnaires were used to observe the effectiveness of SBAR communication model in clinical nursing students teaching. Results Five key points of SBAR communication model in the application of clinical nursing teaching was established,and the corresponding teaching plan was formed.The data of two-way satisfaction survey,patient satisfaction survey, and evaluation of nurse patient communication ability of nursing students of experimental group and control group were analyzed, with statistical significant difference. Data showed that the relevant statistical indicators of the experimental group were better than those of the control group(t = 2.079 5-18.267 6, P<0.05). Conclusions The application of SBAR communication model in clinical nursing teaching can significantly elevate student nurses′communication ability.

ABSTRACT

Objective To observe the application of SBAR communication model in beside shift hand-over for the perioperative nursing of aneurysmall subarachnoid hemorrhage (aSAH). Methods A total of 72 cases who were diagnosed with aSAH were divided into the control group (36 cases)and observation group(36 cases).The traditional shift-change method was used in control group. The SBAR communication model was adopted in the observation group. To compare the time spent on handover, the incidence of care issues, the satisfaction of both patients and shift nurses and the incidence of complications between observation group and control group. Results Time consuming in shift change in observation group was (5.43 ± 0.44) min, lower than that in control group (7.81 ± 0.54) min, and the difference was significant (t=39.496, P<0.01). The incidence of care issues was 27.41%(37/135) in control group and 9.63%(13/135) in observation group, and there was significant difference (χ2=14.138, P<0.01). The satisfaction rate of shift nurses in observation group was higher than that in control group, and there was significant difference (t=-15.193--4.971, P<0.05). The satisfaction rate of caregivers and patients in observation group was higher than that in control group, and there was significant difference (t =--2.580--2.120, P<0.05) . Besides, the incidence of complications was 27.78%(10/36) in control group and 8.33%(3/36) in observation group, and there was significant difference (χ2=4.600, P<0.05). Conclusions The SBAR communication model in beside shift hand-over has improved the efficiency of shift change,reduced nursing risks and collaboration effect of nurse team.

Complexity goes beyond the concept of multiple medical co-morbidities to include multi-dimensional mental, social, and behavioral person-centric factors that interfere with routine care delivery. It is important for primary care to identify these patients and adopt a problem based, goal orientated, person-centric approach to improve the overall outcome and care experience for these patients. This unit illustrates the use of the SBAR4 framework to approach a complex case.

ABSTRACT

Patients who require a stay in a community hospital usually tend to be more complex, presenting not only with biomedical issues with complications, but also with a myriad of psychological and social issues as well. If they were to be discharged from an acute hospital directly to primary care and community, the patients and caregivers may feel helpless, overwhelmed and unsure of how to navigate the healthcare system to get their complex issues sorted out. Family physicians in the community hospitals need to hone their skills in such an area of care. The SBAR4 model can be effectively used to categorise the patients’ multiple bio-psycho-social issues, coordinate the multi-disciplinary team to bring hospital and community resources to help such patients, provide holistic care for such patients, and transit them safely into the care of our family physicians in primary care and community.

ABSTRACT

Providing comprehensive and continuing care to patients is the forte of family physicians. The burden of providing such care to patients with complicated co-morbidities is increasing rapidly in ageing populations. Primary care systems around the world are ill equipped to face such a challenge. Family physicians need to hone their skills in this area of care. In this article, we introduce the SBAR4 model and propose it as a framework for managing patients with complex co-morbidities. This model is easy to learn and use by family physicians as it is based on the familiar SBAR model of clinical communication and Pendleton’s 7 Tasks of consultation. We believe that the SBAR4 will assist the clinician to assess patients with complex co-morbidities and map out a comprehensive care plan that can be easily understood by a multidisciplinary team caring for such patients.

ABSTRACT

PURPOSE: The purpose of this study was to investigate effect of the SBAR-Collaborative Communication Program on nurses' communication skills and on collaboration between nurses and doctors. METHODS: From March 11 to November 11, 2013, data were collected from 180 hospital nurses working in a university hospital in Gyeonggi province. Outcomes were measured at three time intervals; before, three and six months after the program was completed. RESULTS: After participating in this program, there was a significant increase in nurses'communication skills but not in collaboration between nurses and doctors. None of the participants' general categories influenced nurses'communication skills at pre-test, but age, education level, total years of working and work department significantly influenced scores at 3 and 6 months. Work department was the only category for which there was a significant difference in collaboration between nurses and doctors at pre-test, and education level and work department were related to significant improvement at 6 months. CONCLUSION: Findings indicate that this program can improve communication skills for nurses and also, collaboration between nurses and doctors, especially for nurses under 25 years of age. Thus nursing and hospital managers should provide SBAR-Collaborative Communication Programs to new nurses in their job training.

ABSTRACT

Objective To identify the effect of SBAR (situation, background, assessment, recom-mendation) in OR (operating room)nursing teaching rounds. Methods According to internship turns, rou-tine group (even number, Group A:68 students) and SBAR group (odd number, Group B:70 students) were chosen by drawing lots. Group A used conventional mode for history report, while Group B used SBAR mode. Students' performance was evaluated by OR nurse students' rounds standard scale and the reporting time was recorded. Each nurse students filled in Communication Self-efficacy Evaluation Form on the fifth week in OR. The information was input into excel. The results of the two groups were compared with SPSS 11, and the data were analyzed by chi square test and t test. Results The average reporting quality score was 16.74 for Group A, and 18.66 for Group B. The average score of Group B was higher than that of Group A. Compared with the cases scores above 18 between the two groups, the result was statistically significant, P=0.039. The average score of communication Self-efficacy in Group A was 42.88, while it was 44.94 in Group B, which showed that Group B had better score than that of Group A. Significant differences was found between the two groups when compared the cases with score above 45, P=0.010. The average reporting time was (2.42±1.16) minutes for Group A and (2.32±1.21) for Group B, and there is no significant differences between the 2 groups. Conclusion SBAR is helpful for improving students communication Self-efficacy, and making progress on nursing round reporting quality,and it is worth promoting in clinical teaching.

ABSTRACT

PURPOSE: The purpose of this study was to develop Situation, Background, Assessment, Recommendations(SBAR) program and to test the effects of the program on communication clarity, clinical competence, and self-efficacy for clinical nurses in cancer hospitals. METHODS: This study applied a nonequivalent control group non-synchronized design. There were 28 participants in the intervention group and 27 in the control group. SBAR program consisted of 4 parts and applied for 6 weeks in the intervention group. Data were analyzed with χ² test, Fisher's exact test, t-test and ANCOVA using the SPSS program. RESULTS: Communication clarity, clinical competence and self efficacy were significantly increased in the intervention group compared to those in the control group. CONCLUSION: These results suggest that SBAR program may improve communication clarity, clinical competence and self-efficacy for clinical nurses in cancer hospitals. Therefore, it is required to actively take advantage of this program to improve communication clarity among medical staffs, clinical competence, and self-efficacy in clinical nursing practices.

ABSTRACT

Objective To design the disease transfer table of standardized communication between the maternity ward and the operating room,and explore the application effect of disease transfer table based on situation (S)-background (B)-assessment (A)-recommendation (R) communication in cesarean delivery of maternal. Methods 570 pregnant women and 30 medical staff from two wards of maternity ward were selected as research object, the first ward was the control group, the second ward was observation group. The control group used traditional way of disease transfer,and the observation group used SBAR transition table for disease transfer, compared the incidence of adverse events after caesarean section of puerperal and the satisfaction of disease transfer of medical staff from two groups. Results The incidence of wound bleeding, postpartum hemorrage caused by uterine inertia, wound infection of observation group was 2.06%(6/291), 1.71%(3/291), 1.37%(4/291), which was lower than 3.94%(15/279), 3.58%(10/279), 4.66%(13/279) of control group (χ2=4.41, 4.17, 5.31,P < 0.05). The satisfaction of comprehensive, state of the iuness, work acceptance, transfer efficiency of medical staff was (4.75 ± 0.12), (4.82 ± 0.55), (4.86 ± 0.67), (4.76 ± 0.22) points, which were higher than (3.04 ± 0.29), (3.11 ± 0.35), (3.09 ± 0.32), (3.01 ± 0.42) points of control group (t=2.72-3.12,P < 0.05). Conclusions The SBAR condition transition table can improve the efficiency and quality of disease transfer between maternity ward and operating room, ensure the care safety of cesarean section maternal.

ABSTRACT

<p><b>INTRODUCTION</b>The efficiency of postoperative handover of paediatric patients to the children's intensive care unit (CICU) varies according to institutions, clinical setup and workflow. Reorganisation of handover flow based on findings from observational studies has been shown to improve the efficiency of information transfer. This study aimed to evaluate a new handover process based on recipients' perceptions, focusing on completeness and comprehensiveness of verbal communication, and the usability of a situation, background, assessment and recommendation (SBAR) form.</p><p><b>METHODS</b>This was a prospective interventional study conducted in the CICU of KK Women's and Children's Hospital, Singapore. It comprised four phases: (1) evaluation of the current handover process through an audit and opinion survey; (2) development of a new handover process based on the opinion survey and hospital personnel feedback; (3) implementation; and (4) evaluation of the new handover process. The new handover process was based on a PETS (pre-handover, equipment handover, timeout and sign out) protocol with a 'single traffic communication' flow and a new SBAR handover document. It included relevant patient information, and the options 'not applicable' and 'none', to increase compliance and reduce ambiguity.</p><p><b>RESULTS</b>Significantly more recipients indicated that the new SBAR form was the most important handover tool and provided more useful information. Recipients' perceptions indicated improvement in information sufficiency and clarity; reduction of omission errors; and fewer inconsistencies in patient descriptions in the new process.</p><p><b>CONCLUSION</b>Dual customisation of the handover process, PETS protocol and SBAR form is necessary to meet the workflow and information demands of the receiving team.</p>

Ensuring the safety of patients is the the duty of medical workers. Nursing safety is the key factor to reflect the quality of nursing service, which is closely related to patient safety. The effective communication between physicians and nurses is a powerful guarantee for the safety of patients. The positive effect of SBAR communication model in clinical communication has been validated in many aspects, which gradually becomes an important communication model. Now reviews the origin, connotation, characteristics and application status of SBAR communication model, then put forward the matters needing attention when using the model, in order to provide theoretical reference for clinical application.
