ABSTRACT

Objective To investigate the relationship between lesion size of solitary bone cyst ( SBC) and pathological fracture of calcaneus, so as to provide references for the treatment of SBC. Methods The three dimensional (3D) finite element model of foot and ankle was established based on CT images. Four models with gradient spherical bone defects were constructed in the focal area to simulate different SBC lesion sizes, and the biomechanical characteristics of calcaneus in different gait phases were analyzed. Results With the increasement of SBC size, the kinematics of calcaneus did not change significantly, but the peak stress of calcaneus increased gradually. When the SBC size exceeded 75% of the calcaneal width, the stress in calcaneal sulcus and cortical bone below SBC increased by 1. 48 times and 7. 74 times, respectively. Conclusions The risk of pathological fracture increases when the SBC diameter exceeds 75% of the calcaneal width, and early surgical intervention should be recommended. The calcaneal sulcus and the cortex bone below SBC are stress concentration regions and can be used as important areas to evaluate pathological fractures.

ABSTRACT

Near infrared model established under a certain condition can be applied to the new samples status, environmental conditions or instrument status through the model transfer. Spectral background correction and model update are two types of data process methods of NIR quantitative model transfer, and orthogonal signal regression (OSR) is a method based on spectra background correction, in which virtual standard spectra is used to fit a linear relation between master batches spectra and slave batches spectra, and map the slave batches spectra to the master batch spectra to realize the transfer of near infrared quantitative model. However, the above data processing method requires the represent activeness of the virtual standard spectra, otherwise the big error will occur in the process of regression. Therefore, direct orthogonal signal correction-slope and bias correction (DOSC-SBC) method was proposed in this paper to solve the problem of PLS model's failure to predict accurately the content of target components in the formula of different batches, analyze the difference between the spectra background of the samples from different sources and the prediction error of PLS models. DOSC method was used to eliminate the difference of spectral background unrelated to target value, and after being combined with SBC method, the system errors between the different batches of samples were corrected to make the NIR quantitative model transferred between different batches. After DOSC-SBC method was used in the preparation process of water extraction and ethanol precipitation of Lonicerae Japonicae Flos in this paper, the prediction error of new batches of samples was decreased to 7.30% from 32.3% and to 4.34% from 237%, with significantly improved prediction accuracy, so that the target component in the new batch samples can be quickly quantified. DOSC-SBC model transfer method has realized the transfer of NIR quantitative model between different batches, and this method does not need the standard samples. It is helpful to promote the application of NIR technology in the preparation process of Chinese medicines, and provides references for real-time monitoring of effective components in the preparation process of Chinese medicines.

ABSTRACT

Se describen los casos de tres pacientes de sexo femenino a quienes se les hizo diagnóstico de síndrome de Budd Chiari. En una paciente la presentación del síndrome fue subaguda, pudiendo ser manejada exitosamente con la colocación de TIPS. Otra con mutación del factor V Leyden asociada desarrolló disfunción hepática progresiva y requirió de trasplante hepático. En dos de los tres casos se identificó una enfermedad hematológica como trastorno de base, y en uno el uso de anticonceptivos orales como factor de riesgo. Las tres pacientes fueron sometidas a terapia anticoagulante y el manejo quirúrgico fue definido de acuerdo a su condición clínica. Sin embargo, en un caso la presentación fue aguda con falla hepática y muerte.


This article describes the cases of three female patients who were diagnosed with Budd-Chiari syndrome. One patient was subacute and could be successfully managed by placement of a transjugular intrahepatic portosystemic stent (TIPS). Another patient who had the Factor V Leiden mutation developed associated progressive liver dysfunction and required liver transplantation. A hematologic disease was identified as the underlying disorder in two of the three cases. For one patient, the use of oral contraceptives was a risk factor. Since all three patients were undergoing anticoagulant therapy, surgical management was determined according to each patient’s clinical condition. Nevertheless, the one patient who that presented acute hepatic failure did not survive.

ABSTRACT

OBJECTIVE: Telemedicine uses common technologies that provide a conduit for information exchange between physicians nurses and patients. In addition to patient records, medical professionals can obtain vital signs and other reference data through telemedicine applications. also, recent advancement in RF technology and wireless communications has enabled the development of noble networks. Mobile telemedicine involves more than just communicating via mobile phone of PDA that sends and receives medical data on Portable Internet. METHODS: This paper describes the design of a mobile telemedicine PDA prototype based on Embedded Linux that can be used to monitor the medical information using a single board computer(SBC). RESULTS: The device hardware architecture consist of signal conditioning circuit, single chip micro-controller, LCD display and wireless network interface card(W-NIC). Embedded Linux was ported as its operating system and application software was developed QT/embedded 2.3.7. CONCLUSION: This system would be very useful for patients with chronic illness, especially in an emergent environment, and for containing medical cost in the aged society.
