ABSTRACT

@#The nasal skin is the most common site of malignancy in the face accounting for as much as 25.5 percent by virtue of its location and propensity for direct exposure to ultraviolet radiation from the sun.1-3 Among the various cutaneous malignancies, basal cell carcinoma is the most ommon, but other types of cancer such as squamous cell carcinoma, cutaneous malignant melanoma, and basosquamous carcinoma are also common.4 Following surgical resection of a malignant lesion, the defect calls for a reconstructive option that will restore aesthetics and function. We present a squamous cell carcinoma of the nasal alar skin which underwent excision and reconstruction of the defect using a superiorly - based nasolabial flap. CASE REPORT A 66-year-old man consulted at the outpatient clinic due to a nasal alar mass on the right. The mass started one year prior to consult as a pimple-like lesion on the right nasal ala. There was no history of manipulation or trauma to the aforementioned area. He consulted at a local hospital where he was given unrecalled antibiotics that did not cure the lesion. Instead, he noticed that it gradually enlarged, and a deep ulceration developed within the mass. This prompted consult at our outpatient clinic where a 3 x 2 cm ulcerating mass with crusting and necrotic areas was noted on his right nasal ala. (Figure 1) Anterior rhinoscopy showed an intact mucosa in the right nostril with no gross evidence of tumor involvement. There were no enlarged cervical lymph nodes palpated in the neck. A wedge biopsy revealed a well-differentiated squamous cell carcinoma. He claimed that he had no family history of cutaneous malignancy. However, he had a 20 pack-year history of smoking and was a heavy alcoholic beverage drinker. He previously worked as an electrician and denied chronic exposure to sunlight. He consequently underwent excision of the right nasal alar mass with 5-mm margin. (Figure 2A, B) A histologic evaluation of the margins revealed that the borders and tumor base were negative for malignancy. The alar cartilage was not involved by tumor. Reconstruction of the defect was done using a superiorly - based nasolabial flap on the right. (Figure 3A, B, C) Two weeks postoperatively, the patient came in for follow-up with a healed, aesthetically - pleasing, and well-coaptated wound. (Figure 4) He remains free of any evidence of recurrence after 1 year.

ABSTRACT

Objective:To investigate the relationship between base level of Cyfra21-1,SCCA,TK1 and lung cancer prognosis.Methods: A nested case-control study was conducted.721 lung cancer cases who had no distant metastasis were recruited baseline population from January 2010 to January 2013.About 2 years follow-up,364 cases of death or brain (or multiple) metastasis were identified as case group, and the other 357 cases were included in the control group.The level of serum Cyfra21-1,SCCA,TK1 was detected.The relationship between base level of Cyfra21-1,SCCA,TK1 and lung cancer prognosis were analyzed.Results: The age in the case group was (59.3±10.1),and the control group was (59.0±9.9).There were obvious differences in body mass index,smoking index,pathological type,clinical stage,lymph node metastasis and with chronic diseases between case group and control group(P0.05).There were differences in the base level of Cyfra21-1,SCCA,TK1;and there were differences in Cyfra21-1,SCCA,TK1 distribution between case group and control group(P0.05) in patients with different stages of lung cancer.There were differences in the base levels of Cyfra21-1 and SCCA(P0.05) in patients with different pathological types of lung cancer.Logistic regression analysis results showed that the OR value of SCCA,TK1 with lung cancer prognosis were respective 7.235(1.674-14.613),5.009(0.973-10.778),5.816(0.879-16.235).Conclusion: The baseline level of Cyfra21-1 can reflect the prognosis of lung cancer patients,while SCCA,TK1 not.

ABSTRACT

@#Laryngeal SCCA usually presents with hoarseness when the glottis is involved, dysphagia if the supraglottis is involved, and difficulty of breathing and stridor in subglottic invovlement. A neck mass as an initial presentation of laryngeal carcinoma is commonly linked to the involvement of the supraglottis due to its rich lymphatic drainage. About 70% of supraglottic tumours present with advanced disease (stages III-IV),1 while 75% of glottic tumours present with localized disease (stages I-II).1 Smoking and alcohol consumption are considered highly significant etiologic factors but evidence has suggested a possible role for human papilloma virus (HPV) infection, ras oncogene activation, and gastroesophageal reflux as well.2 To the best of our knowledge, laryngeal squamous cell carcinoma has not been associated with herpes simplex virus (HSV). We report a case of laryngeal squamous cell carcinoma with an unusual presentation and peculiar histopathology, and discuss its potential association with herpes simplex virus.

ABSTRACT

Objective:To investigate the applied value of serum Cyfra21-1,NSE,CRP and SCCA and pleural effusion Cyfra21-1, NSE in pathological types, clinical stages and combination detection for diagnosis of lung cancer.Methods: Based on chemiluminescence immunoassay,the levels of serum NSE,Cyfra21-1,SCCA,CRP and pleural effusion NSE,Cyfra21-1 in 100 patients with lung cancer (case group) and 50 patients with benign diseases(control group) were determined.Results:The positive rates of serum Cyfra21-1,NSE,SCCA and pleural effusion NSE,Cyfra21-1 were significantly higher in case group than those in control group ( P0.05).Conclusion:The serum NSE, Cyfra21-1, CRP, SCCA and pleural effusion NSE, Cyfra21-1 play an important role in the diagnosis of lung cancer and contribute to the pathological type and TNM stage of lung cancer, the combination detection of tumor markers is helpful for the early diagnosis of lung cancer.

ABSTRACT

Objective To study the expression levels of SCCA1 and SCCA2 mRNA in tissues of cervical squamous cell carcinoma. To investigate the role of this gene in the clinical diagnosis, evaluation of treatment and observation of prognosis of cervical squamous cell carcinoma. Methods Quantitative real-time RT-PCR was used to detect the expression of SCCA1 and SCCA2 mRNA in tissues of 60 cases of cervical squamous cell carcinoma and those of 30 cases of normal cervical tissues. Results The expression level of SCCA2 mRNA in tissues of 30 cases of cervical squamous cell carcinoma was higher than in those of 15 cases of normal cervical tissues (4.405 ± 2.310, 9.088 ± 2.195) (t =-6.513, P <0.001), while the expression level of SCCA 1 mRNA did not significantly differ between normal and malignant tissues (P >0.05). The expression of SCCA2 mRNA was relevant to FIGO stages and there was a tendency for this gene to increase with the stage getting worse (F =8.313, P <0.05). Moreover, the overexpression of SCCA2 mRNA was significantly correlated with lymph node metastases (t =2.853, P <0.05). The expression of SCCA2 mRNA was not correlated with age and pathological grading (P >0.05). However, the expression of SCCA1 mRNA was not correlated with age,FIGO stages, lymph node metastases and histological grade (P >0.05). Conclusion The expression of SCCA2 mRNA may provide help for more accurate diagnosis on the clinical stages and lymph node metastases of cervical squamous cell carcinoma.

ABSTRACT

A 48 year-old man was referred to our Department with a headache, and also presented with an elevated serum ACTH level, but without an elevated serum cortisol. Although there was no clinical evidence of Cushing's syndrome, a brain CT and MRI showed a 4x4.5 cm sized pituitary mass, which was successfully removed by a transsphenoidal approach (TSA). A histopathological examination revealed the mass to have an ACTH positive reaction. Therefore, through hormonal and pathological evaluation, a silent corticotroph-cell adenoma (SCCA), with an elevated serum ACTH level, was diagnosed. Although reports on SCCA have been recently increased, this case is reported because these kinds of tumor are still rare, and those SCCA with an elevated serum ACTH even more so

ABSTRACT

OBJECTIVE: SCC-Ag(Squamous Cell Carcinoma Antigen) is a tumor marker for patients with squamous cell carcinoma of the uterine cervix. It has shown that the SCC-Ag was well related with the response of cancer therapy and course of cervical cancer. The aim of this study is to investigate whether the presence of pelvic lymph node metstasis can be predicted by the measurement of the preoperative SCC-Ag levels in the patients with early staged squamous cell carcinoma of cervix. METHODS: The preoperative serum SCC-Ag levels were measured in 45 patients with stage I-II squamous cell carcinoma of cervix undergoing radical hysterectomy from September 1995 to December 1997. The serum SCC-Ag levels were analyzed for the clinicopathologic characteristics and other prognostic factors using univariate and multivariate analysis. RESULTS: The serum SCC-Ag levels of the patients exhibited pelvic lymph node metastasis were above 4.8ng/ml. An elevated preoperative serum SCC-Ag level, and tumor size were independent predictors for the presence of lymph node metastasis(p<0.01). CONCLUSION: The determination of the preoperative serum SCC-Ag levels provides a new prognostic factor in early staged cervical cancer.

ABSTRACT

With a PII-300 computer, a microcomputer system for SCCA marker detection is developed in this paper, which is based on Windows operating system and Visual Basic 6.0. More than 1,000 cases have proved that the results from the system completely accord with the one from artificial detection. With this system, such disadvantages are conquered as high labor intensity, low working efficiency and high error rate, and a quantitative detection method for SCCA marker comes into existence.
