ABSTRACT

RESUMEN Objetivo: Distintas alteraciones del electrocardiograma (ECG) han sido asociadas a disfunción sistólica ventricular izquierda (DSVI), si bien la asociación con el infradesnivel del segmento ST (IST) del plano frontal del ECG estándar no se encuentra establecida. El objetivo del presente trabajo fue evaluar si el IST de la derivación DI (IST-1) permite predecir la presencia de DSVI. Material y métodos: Se incluyeron de forma prospectiva pacientes portadores factores de riesgo o cardiopatías crónicas estables, con ECG basal y ecocardiograma que aportara evaluación de la fracción de eyección (FEVI), motilidad ventricular izquierda y evaluación dicotómica sobre la presencia de hipertrofia ventricular izquierda (HVI). Evaluamos la morfología del segmento ST en derivaciones DI y V6, definiéndose como anormal (IST-1; IST-6) al ST infradesnivelado (≥1mm a 80mseg del punto J) o descendente. Resultados: Se analizaron en forma prospectiva 691 pacientes, edad media 69,8 ± 12 años, 61,6% hombres. Se identificó IST-1 e IST-6 en 250 (36,2%) y 199 (28,8%) casos, respectivamente. La presencia de IST-1 e IST-6 se asoció a una FEVI significativamente menor comparado con la ausencia de dicho hallazgo: 44,8 ± 13,9% vs. 55,6 ± 8,9%, (p <0,0001) y 45,8 ± 14,1% vs. 54,1 ± 10,4% (p <0,0001) respectivamente. Ambos se asociaron a la presencia de DSVI, definida como FEVI <50%, aunque el IST-1 mostró mejor rendimiento diagnóstico que el IST-6 [área bajo la curva 0,72 (IC 95% 0,69-0,76) vs. 0,64 (IC 95% 0,610,68), p = 0,0001]. Conclusiones: Este estudio mostró que la depresión del segmento ST de la derivación DI permite predecir la presencia de DSVI mejor que IST-6. La potencial relevancia de dichos hallazgos debería situarse en el contexto actual de la emergente utilización de dispositivos wearables que analizan la información electrocardiográfica mediante una única derivación.


ABSTRACT Background: Different electrocardiographic abnormalities have been associated with left ventricular systolic dysfunction (LVSD), although the association with standard electrocardiographic frontal plane ST-segment depression (STD) has not been established. Objective: The aim of this study was to evaluate whether lead I STD (STD-I) allows predicting the presence of LVSD. Methods: Patients with risk factors or stable chronic heart disease, and baseline electrocardiogram (ECG) and echocardiogram that provided evaluation of left ventricular ejection fraction (LVEF), left ventricular wall motility, and dichotomous evaluation of left ventricular hypertrophy (LVH), were prospectively included in the study. ST-segment morphology in leads I and V6 was evaluated, defining horizontal (≥1mm at 80 ms from the J point) or downsloping STD as abnormal STD-I and STD-6. Results: A total of 691 patients; with mean age of 69.8 ± 12 years and 61.6% men, were prospectively analyzed. STD-I and STD-6 were identified in 250 (36.2%) and 199 (28.8%) cases, respectively. Presence of STD-I and STD-6 was associated with a significantly lower LVEF compared with the absence of this finding: 44.8 ± 13.9% vs. 55.6 ± 8.9% (p <0.0001) and 45.8 ± 14.1% vs. 54.1±10.4% (p <0.0001), respectively. Both were associated with the presence of LVSD, defined as LVEF <50%, although STD-I showed better diagnostic performance than STD-6 [area under the ROC curve 0.72 (95% CI 0.69-0.76) vs. 0.64 (95% CI 0.61-0.68), p = 0.0001]. Conclusions: This study showed that STD-I predicts the presence of LVSD better than STD-6. The potential relevance of these findings should be placed in the current context of the emerging use of wearable devices that analyze electrocardiographic information through a single lead.

ABSTRACT

Subcutaneous and topical epinephrine are widely used for hemostasis during dermatologic procedure. There was substantial variation in the amount of systemic absorption on the basis of patient's general condition, blood supply of wounds. Increased or unpredictable absorption after topical administration may increase anesthetic and cardiovascular risks. There have been no reports about cardiovascular complications developed during laser resurfacing. We experienced a case of transient hypertension, cardiac arrhythmia and ST-depression during whole face resurfacing by Erbium-Yag laser due to application of high concentration epinephrine gauze on the face. The hypertension and arrhythmia were normalized in 2 minutes, and ST-depression was sustained for about 4 hours.

ABSTRACT

BACKGROUND: The electrocardiogram may provide valuable information regarding the identity of the culprit coronary artery and the location of obstructing lesion within the artery, which may be of guidance in selecting the therapeutic modality. Previous studies have concluded that changes in lateral leads (I, aVL, V5, V6) are predictive of left circumflex coronary artery obstruction in inferior wall acute myocardial infarction. Elect-rocardiographic criteria for determining the location of the obstructing lesion, however, have not been well established. The purpose of this study is to investigate the patterns of ST segment depression in lateral leads in inferior wall acute myocardial infarction and the obstruction site of culprit artery according to ST segment depression in lateral leads. METHODS: We examined 78 patients with inferior wall acute myocardial infarction analizing their electrocardiogram and coronary angiography which performed during acute hospitalization. RESULTS: Of the fifty-five patients in which the culprit artery could be determined, 1)in 41 the culprit artery was the right coronary artery (19 proximal to the right ventricular branch and 22 distal), and in 14 the left circumflex coronary artery (7 proximal to the first obtuse marginal branch or involving a high first obtuse marginal branch, and 7 with distal obstruction). 2)Significant ST depression (ST< or =1 mm) in leads I and aVL was more common in right coronary artery obstruction (p<0.05 and p=0.01 respectively) than left circumflex artery. 3)It was difficult to define the location of obstruction with ST segment change of lateral precordial leads (V5, V6). CONCLUSIONS: In acute inferior wall myocardial infarction, ST segment depression in lateral limb leads (I, aVL) can be indicative of the right coronary artery obstruction and the ST segment depression pattern in lateral precordial leads was not indicative of the site of obstruction.

ABSTRACT

BACKGROUND: In aerometabolic process, the human heart mainly utilizes free acid as fuel. During anaerobic process, lactate production by the myocardium is increased and accumulates in the myocardium. Thus it decreases the contractility of myocadium. Therefore in patients with ischemic heart disease, lactate prodution must be increased by the myocardium during myocardial ischemia. During paroxysmal supraventricular tachycardia, patients frequently experience chest pain and ST segment depression suggesting acute myocardial ischemia. However it occurs on a physiologic basis independent of ischemia. The purpose of this study was to assess whether tachycardia induced by artial pacing produces myocardial ischemia in patients without evidence of ischemic heart disease. METHODS: Between May 28, 1996 and August 13, 1996, at the University of Keimyung, Dong-San Medical center, 15 patients(male 9, female 6, mean age of 38 years) with palpititation underwent electrophysiologic testing and had radiofrequency cather ablation. There were no evidence of ischemic heart disease. Right artrial pacing was done with lengths of 500msec, 400msec and 350msec in each 5 patients. A 12 lead electrdcardiogram, left ventricular enddiastolic pressure, blood from femoral artery and coronary sinus for lactate determinations and blood gas analysis were dbtained simultaneously. They were obtained at baseline, at 1, 5, 10 and 15 minute of atrial pacing and at 1, 5, 10 minute after cessation of pacing. RESULT: Significant changes were not observed in , , concentration of , pH and saturation. In all patients, mean percent lactate extraction was above 10% and not significantly changed during atrial pacing. However ST segment depression was significantly progressive during atrial pacing and according to decrease the cycle length(p<0.05), also left ventricular end-diastolic pressure was significantly decreased during atrial pacing(p<0.05). Conclusion: Therefore tachycardia induced by atrial pacing for 15 minutes did not produce myocardial ischemia in patients without evidence of ischemic heart disease. Depression of STsegment during supraventricular tachycardia or atrial pacing, in patient without underlying heart disease is necessary to inveestigate what makes this phenomenon.
