ABSTRACT

Introducción: El papiloma invertido es uno de los tumores benignos más frecuentes de la región nasosinusal. Los principales problemas en su manejo son la recurrencia y transformación maligna. Su tratamiento es la extirpación quirúrgica. Objetivos: Describir y analizar las caracterfsticas cínicas y epidemiológicas, así como el resultado del tratamiento endoscópico practicado en el Hospital Clínico Regional de Concepción. Material y método: Se realizó un estudio retrospectivo, descriptivo, de todos los pacientes con diagnóstico de papiloma invertido nasosinusal operados vfa abordaje endoscópico entre los años 1997 y 2012. Se analizaron variables epidemiológicas y clínicas, incluyendo análisis de las recurrencias y transformación/asociación a cáncer. Resultados: Se encontraron 57 casos de papiloma invertido nasosinusal, 55 de los cuales se abordaron endoscópicamente, 65,5% hombres, 54,7 años en promedio de edad. La obstrucción nasal fue el principal síntoma. La mayor cantidad se concentró en estadio 2 (47,3%) y 3 (32,7%) de Krouse. Complicaciones relevantes 7,8%, las cuales fueron resueltas sin secuelas. Seguimiento promedio de 4,1 años, con una recurrencia de 5 (9,1%) casos, una persistencia (1,8%). Asociación con carcinoma escamoso en el 5,5% de los casos. Conclusión: La experiencia con la resección endoscópica en el papiloma invertido nasosinusal ha demostrado ser efectiva en nuestro medio, con baja tasa de complicaciones y recurrencias.


Introduction: The inverted papilloma is one of the most common benign tumors of the sinonasal region. The main problems in their management are the recurrence and malignant transformation. Their treatment is surgical removal. Aim: To describe and analyze the clinical and epidemiological characteristics, as well as the results of endoscopic treatment in a tertiary hospital. Materials and methods: A retrospective, descriptive study of all patients with diagnosis ofsinonasal inverted papilloma operated by endoscopic approach between 1997 and 2012 was carried out. Epidemiological and clinical variables were analyzed, including analysis of recurrences and transformation to / association with cancer. Results: 57 cases of sinonasal inverted papilloma were found, 55 of which were endoscopically resected, 65.5% male, 54.7 years old on average. Nasal obstruction was the main symptom. Most of them were in Krouse stage 2 (47.3%) and 3 (32.7%); 7.8% of major complications, which were resolved without sequels. The follow up average was 4.11 years, with a recurrence of 5 (9.1%) cases, and (1.8%) persistence. Association with squamous-cell carcinoma was found in 5.5% of cases. Conclusion: The clinical and epidemiological characteristics do not differ greatly from those reported in the literature. Experience with endoscopic resection in sinonasal inverted papilloma has proven effective in our clinical practice, with a low rate of complications and recurrences, this approach currently being the first choice in most cases.

ABSTRACT

A 44-year-old man presented with a history of chronic epiphora, discharge from the right eye, and a palpable mass in the medial canthal area. Irrigation of the lacrimal system revealed bloody discharge. Orbital magnetic resonance imaging (MRI) showed a well-defined heterogeneous enhanced mass filling the lacrimal sac and upper nasolacrimal duct (NLD). A wide excision and surgical biopsy were performed. Histopathology showed the tumor to be an exophytic Schneiderian papilloma with moderate to severe dysplasia. Three months later, the mass was found to be invading the nasal cavity through the NLD. Endoscopic histopathological evaluation confirmed that it was identical to the originally identified papilloma.

ABSTRACT

Oncocytic Schneiderian papilloma (OSP) is a rare neoplasm of the nose and paranasal sinuses, and is often mistaken histologically for papillary adenocarcinoma or even rhinosporidiosis. Recently, we experienced a case of oncocytic Schneiderian papilloma of the left maxillary sinus developed in 53-year-old female patient. She complained of unilateral nasal obstruction, bloody rhinorrhea and frontal headache. The tumor mass was resected surgically through intranasal endoscopic and Caldwell-Luc's approach. We report its clinical manifestations and histologic characteristics.

ABSTRACT

Mutations in the p53 gene seem to be the most common genetic changes in human malignancies. Mutation or altered p53 expression is a common occurrence in many solid neoplasms, including head and neck carcinomas. Recent studies have also shown p53 alterations in several premalignant conditions of the colon, esophagus, lung, and brain. Preliminary data have suggested that p53 mutations may be involved in tumor progression. This study was performed to determine the incidence of p53 mutations in histologically 27 squamous cell carcinomas, 19 basal cell carcinomas, 18 Schneiderian papillomas, 3 Schneiderian papillomas with malignant transformation, and 15 pleomorphic adenomas of the head and neck region. The degree of p53 gene overexpression was also evaluated according to differentiation, histologic type of tumor, and tumor progression in the head and neck carcinomas. The results were as follows; 1) Eighteen of 27 squamous cell carcinomas, and 4 of 27 dysplasias adjacent to the squamous cell carcinoma of the head and neck expressed p53 protein, but none of the normal control specimens expressed detectable p53 protein. There was no relationship between differentiation of squamous cell carcinoma and p53 protein expression. 2) Twelve of 19 basal cell carcinomas expressed p53 protein; the adenoid type especially overexpressed p53 protein. 3) Nine of 15 pleomorphic adenomas expressed p53 protein especially in the epithelial components. 4) Thirteen of 18 Schneiderian papillomas and all Schneiderian papillomas with malignant transformation expressed p53 protein. The above results indicate that the p53 protein expression is a useful tool for the prediction of tumor progression in the head and neck tumor, but there was no relationship between the differentiation of the tumor and p53 protein expression.

ABSTRACT

Schneiderian papillomas are papillary lesions of the nasal cavity paranasal sinuses that arise from the Schneiderian membrane, a membrane of ectodermal origin embryologically derived from the nasal placodes. This membrane is characterized by transitional type epithelium, similar to bladder urothelium, with admixed microcysts or mucin droplets. It has a tendency to recur after removal and transform into malignancy even though rare. Total 27 cases of Schneiderian papillomas were examined in the Chosun University medical college, which were collected from 1978 to 1986. The results obtained were as follows: 1) There were 19 male and 8 female patients, and mean age at diagnosis was 50.5+/-12.0 (SD) year (27-74 years). 2) Nasal obstruction or perception of nasal mass was the most common presenting symptom; Mean duration of symptoms was 25.8+/-15.0 (SD) month (4-48 months). 3) Numbers of papillomas had predominantly endophytic (inverted, n=17) and predominantly exophytic (fungiofrom n=6) patients, and four had mixed pattern. 4) In 27 patients, recurrences developed in 9, local invasion in one (with an inverted papilloma), and epidermoid carcinoma in one (with an inverted papilloma).
