ABSTRACT

Objetivo: evaluar la asociación entre las categorías de clasificación de riesgo y el Modified Early Warning Score y los resultados de los pacientes con COVID-19 en el servicio de emergencia Método: estudio transversal, realizado con 372 pacientes hospitalizados con diagnóstico de COVID-19 atendidos en la Recepción con Clasificación de Riesgo en Urgencias. En este estudio, el Modified Early Warning Score de los pacientes se clasificó como sin y con deterioro clínico, de 0 a 4 y de 5 a 9, respectivamente. Se consideró que había deterioro clínico cuando presentaban insuficiencia respiratoria aguda, shock y paro cardiorrespiratorio. Resultados: el Modified Early Warning Score promedio fue de 3,34. En cuanto al deterioro clínico de los pacientes, se observó que en el 43% de los casos el tiempo de deterioro fue menor a 24 horas y que el 65,9% ocurrió en urgencias. El deterioro más frecuente fue la insuficiencia respiratoria aguda (69,9%) y el resultado fue alta hospitalaria (70,3%). Conclusión: los pacientes con COVID-19 que presentaban Modified Early Warning Score 4 se asociaron a las categorías de clasificación de riesgo urgente, muy urgente y emergente y tuvieron más deterioro clínico, como insuficiencia respiratoria y shock, y murieron, lo que demuestra que el Protocolo de Clasificación de Riesgo priorizó correctamente a los pacientes con riesgo vital.


Objective: to evaluate the association of the risk classification categories with the Modified Early Warning Score and the outcomes of COVID-19 patients in the emergency service Method: a crosssectional study carried out with 372 patients hospitalized with a COVID-19 diagnosis and treated at the Risk Classification Welcoming area from the Emergency Room. In this study, the patients' Modified Early Warning Score was categorized into without and with clinical deterioration, from 0 to 4 and from 5 to 9, respectively. Clinical deterioration was considered to be acute respiratory failure, shock and cardiopulmonary arrest Results: the mean Modified Early Warning Score was 3.34. In relation to the patients' clinical deterioration, it was observed that, in 43%, the time for deterioration was less than 24 hours and that 65.9% occurred in the Emergency Room. The most frequent deterioration was acute respiratory failure (69.9%) and the outcome was hospital discharge (70.3%). Conclusion: COVID-19 patients who had a Modified Early Warning Scores > 4 were associated with the urgent, very urgent and emergency risk classification categories, had more clinical deterioration, such as respiratory failure and shock, and evolved more to death, which shows that the Risk Classification Protocol correctly prioritized patients at risk of life.


Objetivo: avaliar a associação das categorias de classificação de risco com o Modified Early Warning Score e os desfechos dos pacientes com COVID-19 no serviço de emergência Método: estudo transversal, realizado com 372 pacientes internados com diagnóstico de COVID-19 atendidos no Acolhimento com Classificação de Risco no Pronto-Atendimento. Neste estudo, o Modified Early Warning Score dos pacientes foi categorizado em sem e com deterioração clínica, de 0 a 4 e de 5 a 9, respectivamente. Foram consideradas deteriorações clínicas a insuficiência respiratória aguda, choque e parada cardiorrespiratória. Resultados: o Modified Early Warning Score médio foi de 3,34. Em relação à deterioração clínica dos pacientes, observou-se que em 43% o tempo para deterioração foi menor de 24 horas e que 65,9% delas ocorreu no pronto-socorro. A deterioração mais frequente foi a insuficiência respiratória aguda (69,9%) e o desfecho foi o de alta hospitalar (70,3%). Conclusão: pacientes com COVID-19 que tiveram Modified Early Warning Score 4 foram associados às categorias da classificação de risco urgente, muito urgente e emergente e tiveram mais deterioração clínica, como a insuficiência respiratória e o choque, e evoluíram mais a óbito, o que demonstra que o Protocolo de Classificação de Risco priorizou corretamente os pacientes com risco de vida.

ABSTRACT

INTRODUCCIÓN. La sepsis es un estado de disfunción multisistémica, que se produce por una respuesta desregulada del huésped a la infección. Diversos factores influyen en la gravedad, manifestaciones clínicas y progresión de la sepsis, tales como, heterogeneidad inmunológica y regulación dinámica de las vías de señalización celular. La evolución de los pacientes depende del tratamiento oportuno, las escalas de puntuación clínica permiten saber la mortalidad estimada. OBJETIVO. Evaluar la mortalidad en la unidad de cuidados intensivos; establecer el manejo y la utilidad de aplicar paquetes de medidas o "bundlers" para evitar la progresión a disfunción, fallo multiorgánico y muerte. METODOLOGÍA. Modalidad de investigación tipo revisión sistemática. Se realizó una búsqueda bibliográfica en bases de datos como Google académico, Mendeley, ScienceDirect, Pubmed, revistas como New England Journal Medicine, Critical Care, Journal of the American Medical Association, British Medical Journal. Se obtuvo las guías "Sobreviviendo a la sepsis" actualización 2021, 3 guías internacionales, 10 estudios observacionales, 2 estudios multicéntricos, 5 ensayos aleatorizados, 6 revisiones sistémicas, 5 metaanálisis, 1 reporte de caso clínico, 4 artículos con opiniones de expertos y actualizaciones con el tema mortalidad de la sepsis en UCI con un total de 36 artículos científicos. RESULTADOS. La mortalidad de la sepsis en la unidad de cuidados intensivos, fue menor en el hospital oncológico de Guayaquil, seguido de Australia, Alemania, Quito, Francia, Estados Unidos de Norteamérica y Vietnan, La mortalidad más alta se observa en pacientes con enfermedades del tejido conectivo. DISCUSIÓN. La aplicación de los paquetes de medidas o "bundlers" en la sepsis, se asocia con una mejor supervivencia y menores días de estancia hospitalaria. CONCLUSIÓN. Las escalas SOFA, APACHE II y SAPS II ayudan a predecir la mortalidad de forma eficiente, en la detección y el tratamiento temprano en pacientes con enfermedades agudas y de alto riesgo.

Several factors influence the severity, clinical manifestations and progression of sepsis, such as immunological heterogeneity and dynamic regulation of cell signaling pathways. The evolution of patients depends on timely treatment, clinical scoring scales allow to know the estimated mortality. OBJECTIVE. To evaluate mortality in the intensive care unit; to establish the management and usefulness of applying bundlers to prevent progression to dysfunction, multiorgan failure and death. METHODOLOGY. Systematic review type research modality. A bibliographic search was carried out in databases such as Google Scholar, Mendeley, ScienceDirect, Pubmed, journals such as New England Journal Medicine, Critical Care, Journal of the American Medical Association, British Medical Journal. We obtained the guidelines "Surviving Sepsis" update 2021, 3 international guidelines, 10 observational studies, 2 multicenter studies, 5 randomized trials, 6 systemic reviews, 5 meta-analyses, 1 clinical case report, 4 articles with expert opinions and updates on the subject of sepsis mortality in ICU with a total of 36 scientific articles. RESULTS. The mortality of sepsis in the intensive care unit, was lower in the oncological hospital of Guayaquil, followed by Australia, Germany, Quito, France, United States of America and Vietnam, The highest mortality is observed in patients with connective tissue diseases. DISCUSSION. The application of bundlers in sepsis is associated with better survival and shorter days of hospital stay. CONCLUSIONS. The SOFA, APACHE II and SAPS II scales help to predict mortality efficiently in the early detection and treatment of patients with acute and high-risk disease.

Abstract Objective Commonly used methods for measuring proprioception have resulted in conflicting reports regarding knee proprioception with anterior cruciate ligament (ACL) rupture and the influence of ACL reconstruction. Methods One hundred subjects (50 patients with radiologically and arthroscopically confirmed unilateral ACL rupture and 50 normal controls) were assessed with regards to proprioception using dynamic single-leg stance postural stabilometry. Instrumented knee ligament laxity and knee outcome scores were also measured. Of the 50 patients in the ACL group, 34 underwent reconstruction and were reassessed postoperatively. Results There was a significant proprioceptive deficiency in the ACL group compared with their contralateral knee ( p < 0.001) and to the control group ( p = 0.01). There was asignificant improvement in knee proprioception following ACL reconstruction compared to preoperative findings ( p = 0.003). There was no correlation between ligament laxity measurements and outcome scores. A significant correlation was found preoperatively between outcome scores and proprioception measurements. This correlation was not found post-operatively. Pre-operative proprioception testing had asignificant correlation (r = 0.46) with post-operative proprioception ( p = 0.006). Conclusion Patients with an ACL rupture had a proprioceptive deficit which improved following ligament reconstruction. Knee outcome scores had a better correlation with proprioception than ligament laxity. Proprioception may be a superior objective measure than ligament laxity in quantifying functional knee deficits and outcomes in patients with ACL ruptures. Resumo Objetivo Os métodos comumente usados para medir a propriocepção resultaram em relatos conflitantes sobre a propriocepção do joelho com ruptura do ligamento cruzado anterior (LCA) e a influência da reconstrução do LCA. Métodos A propriocepção de 100 indivíduos (50 pacientes com ruptura unilateral do LCA confirmada à radiologia e artroscopia e 50 controles normais) foi avaliada por estabilometria postural dinâmica em apoio unipodal. A lassidão ligamentar do joelho instrumentado e suas pontuações de desfechos também foram medidas. Dos 50 pacientes do grupo LCA, 34 foram submetidos à reconstrução e reavaliados no período pós-operatório. Resultados O grupo LCA apresentou deficiência proprioceptiva significativa em relaçãoaojoelho contralateral ( p < 0,001) e ao grupo controle ( p = 0,01). A propriocepção do joelho melhorou de maneira significativa após a reconstrução do LCA em relação aos achados pré-operatórios ( p = 0,003). Não houve correlação entre as medidas de lassidão ligamentar e as pontuações de desfechos. Além disso, observamos uma correlação significativa entre as pontuações de desfechos e as medidas de propriocepção antes da cirurgia. Essa correlação não foi detectada no período pós-operatório. O teste pré-operatório de propriocepção teve correlação significativa ( r = 0,46) com a propriocepção pós-operatória ( p = 0,006). Conclusão Os pacientes com ruptura do LCA apresentaram déficit proprioceptivo que melhorou após a reconstrução ligamentar. As pontuações de desfecho do joelho tiveram melhor correlação à propriocepção do que a lassidão ligamentar. A proprio-cepção pode ser uma medida objetiva superior à lassidão ligamentar na quantificação de déficits funcionais e desfechos do joelho em pacientes com ruptura do LCA. Nível de Evidência III Estudo Terapêutico; Estudo de Caso-Controle Longitudinal Prospectivo.

ABSTRACT

Objective: Coronaviruses that cause respiratory infections in humans are known to be neuropathogenic. The purpose of this study is to determine whether there is an association between the severity of viral pneumonia and anxiety and depression levels in patients admitted to an intensive care unit (ICU) with coronavirus disease 2019 (COVID-19). Methods: Prospective study of 165 patients, patients were divided into two groups (intubated and non-intubated) according to whether they were intubated during follow-up. Results: Of 165 patients included, 70 were intubated after the first 7 days of follow-up. The Generalized Anxiety Disorder-7 (GAD-7) score was 10.37±3.75 (p < 0.001) in these patients. The length of ICU stay was 25±22.3 days (p < 0.001), and the length of overall hospital stay was 28.28±23.37 days, significantly longer than in patients who were not intubated (p < 0.001). The Acute Physiology and Chronic Health Evaluation II and Sequential Organ Failure Assessment Score scores, as well as GAD-7 and Patient Health Questionnaire-9, were positively associated with length of ICU stay and length of hospital stay (p < 0.01). Conclusion: The presence and exacerbation of anxiety and depression symptoms can provide information about the aggravation and progression of the underlying disease.

ABSTRACT

Los sistemas de ayuda al pronóstico constituyen uno de los grandes aportes de la medicina contemporánea al ejercicio de esta profesión, por su utilidad en la prevención, predicción, individualización y participación del enfermo en la toma de decisiones, incluso con propósito de auditoría. En el caso de las complicaciones posoperatorias, la aplicación de estos sistemas en apoyo del método clínico hace posible aumentar la certeza diagnóstica y, por tanto, la calidad de la asistencia médica. El objetivo de este artículo es precisar, mediante preguntas y respuestas las características, ventajas y desventajas de los sistemas de ayuda al pronóstico usados en el posoperatorio, para la predicción de dichas complicaciones. Se muestran las bondades de la aplicación de un sistema recientemente validado en un estudio multicéntrico, que se nutre de las mejores evidencias disponibles en beneficio de la seguridad de los pacientes.


Prognosis support systems are one of the contemporary medicine's great contributions to the practice of this profession, due to their usefulness in prevention, prediction, individualization and participation of the patient in decision-making, even for auditing purposes. In the case of postoperative complications, the application of these supporting systems to the clinical method makes it possible to increase diagnostic certainty and therefore the quality of medical care. The objective of this article is to specify, by means of questions and answers, the characteristics, advantages and disadvantages of the prognosis aid systems used in the postoperative period for the prediction of those complications, as well as the benefits of the application of a system recently validated in a multicentric study, which draws on the best available evidence for the benefit of patient safety.

PURPOSE@#The aim of this study was to analyze if any difference exists on the type of immobilisation (above elbow vs. below elbow) in the conservative treatment of distal end radius fractures in adults.@*METHODS@#The study was performed in accordance with Preferred Reporting Items for Systematic Reviews and Meta-analyses. An electronic literature search was performed up to 1st October 2021 in Medline, Embase, Ovid and Cochrane database using the search terms, "distal end radius fractures OR fracture of distal radius", "conservative treatment OR non-surgical treatment", "above elbow immobilisation" and "below elbow immobilisation". Randomized clinical trials written in English, describing outcome of distal end radius fractures in adults by conservative or non-surgical means using above elbow immobilisation or below elbow immobilisation were included and assessed according to the risk of bias assessment (RoB2) tool by Cochrane collaboration. Non-randomized clinical trials, observational studies, retrospective studies, review articles, commentaries, editorials, conference presentations, operative techniques and articles without availability of full text were excluded from this review. The meta-analysis was performed using Review Manager version 5.4.1 (The Cochrane Collaboration, Copenhagen, Denmark).@*RESULTS@#Six randomized clinical trials were included for quantitative review. High heterogeneity (I2 > 75%) was noted among all the studies. The standard mean difference (MD) between the disability of the arm, shoulder and hand scores in both the groups was 0.52 (95% CI: -0.28 to 1.32) which was statistically non-significant. There was no statistical difference in the radial height (MD = 0.10, 95% CI: -0.91 to 1.12), radial inclination (MD = 0.5, 95% CI: -1.88 to 2.87, palmar tilt (MD =1.06, 95% CI: -0.31 to 2.43) and ulnar variance (MD = 0.05, 95% CI: -0.74 to 0.64). It was observed that shoulder pain occurred more commonly as a complication in above elbow immobilisation and the values were statistically significant (above elbow: 38/92, 41.3%; below elbow: 19/94, 20.2%).@*CONCLUSION@#This two-armed systematic review on the above elbow or below elbow immobilisation to be used for conservative treatment of the distal end radius fracture in adults resulted in non-significant differences in terms of functional and radiological scores among the 2 groups but significant increase in the complication rates in the above elbow group.

ABSTRACT

ObjectiveTo investigate the value of combined determination of Model for End-Stage Liver Disease (MELD) score, albumin-bilirubin (ALBI) score, and β2-microglobulin in the diagnosis of liver cirrhosis with acute kidney injury (AKI). MethodsClinical data were collected from 258 patients with liver cirrhosis who attended The First Affiliated Hospital of Zhengzhou University from October 2019 to October 2022, and according to the presence or absence of AKI, they were divided into AKI group with 117 patients and non-AKI group with 141 patients. The changes in each index were compared between the two groups and between the patients with different stages of kidney injury. The independent samples t-test was used for comparison of normally distributed continuous data between two groups, and a one-way analysis of variance was used for comparison between multiple groups; the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups, and the Kruskal-Wallis H test was used for comparison between multiple groups; the chi-square test was used for comparison of categorical data between groups. The receiver operating characteristic (ROC) curve was plotted to evaluate the efficacy of each index in the diagnosis of liver cirrhosis with AKI. ResultsCompared with the non-AKI group, the AKI group had significantly higher age (t=2.307, P=0.022), proportion of patients with hepatic encephalopathy (χ2=18.064, P<0.001) or with spontaneous peritonitis (χ2=16.397, P<0.001), mortality rate (χ2=45.251, P<0.001), levels of creatinine (Z=-8.737, P<0.001) and β2-microglobulin (Z=-8.829, P<0.001), and scores of CTP (Z=-4.058, P<0.001), ALBI (t=2.563, P=0.011), and MELD (Z=-5.628, P<0.001), as well as a significantly shorter length of hospital stay (Z=-3.391, P=0.001). There were significant differences in creatinine, β2-microglobulin, MELD score, and ALBI score between the patients with stage 1, 2 or 3 AKI (P<0.05), while there was no significant difference in CTP score between these three groups (P>0.05). The combined determination of ALBI score, MELD score, and β2-microglobulin had an area under the ROC curve (AUC) of 0.837 (95% confidence interval [CI]: 0.782 — 0.892), with a sensitivity of 75.2% and a specificity of 90.8%; ALBI score combined with MELD score had an AUC of 0.700 (95%CI: 0.636 — 0.764), ALBI score combined with β2 microglobulin had an AUC of 0.823 (95%CI: 0.765 — 0.881), and MELD combined with and β2 microglobulin had an AUC of 0.835 (95%CI: 0.779 — 0.890), suggesting that combined determination of ALBI score, MELD score, and β2-microglobulin had a better diagnostic efficacy than ALBI score, MELD score, or β2-microglobulin used alone or in pairs, as well as a better diagnostic efficacy than creatinine. ConclusionCombined determination of ALBI score, MELD score, and β2-microglobulin has a relatively high value in the diagnosis of liver cirrhosis with AKI.

ABSTRACT

Objective To analyze the relationship between dietary and lifestyle inflammatory scores and metabolic syndrome, diabetes mellitus, cardiovascular disease, non-alcoholic fatty liver disease, tumor and other common metabolic diseases, and to explore the impact of dietary and lifestyle inflammatory potential on metabolic diseases, so as to provide new ideas for the diagnosis, treatment and prevention of metabolic diseases. Methods Databases such as CNKI, Wanfang , and PubMed were searched, and literatures related to the dietary and lifestyle inflammatory scores (DLIS) and metabolic diseases were reviewed . Results Seven articles showed that dietary and lifestyle inflammation scores had a positive correlation with metabolic diseases, and two articles showed that only lifestyle inflammatory scores had a positive correlation with metabolic diseases. Conclusion Dietary and lifestyle inflammatory scores may be positively correlated with metabolic diseases, but some results are still controversial. Further studies are needed to prove the correlation between DLIS and metabolic diseases.

ABSTRACT

La fragilidad determina una incapacidad para enfrentar estresores debido a la disminución de las reservas fisiológicas multisistémicas. El acto anestésico quirúrgico constituye un evento estresante y la presencia de fragilidad es un factor de riesgo independiente de morbimortalidad perioperatoria. Identificarla permitiría abordar los factores reversibles que la determinan con la intención de disminuir los riesgos inherentes a dicho acto. Su detección en la valoración perioperatoria aporta información relevante que no se obtiene con una evaluación tradicional. Este enfoque se ha convertido en un estándar en la valoración perioperatoria de personas mayores. El objetivo del estudio es valorar la prevalencia de fragilidad en la cirugía electiva de personas mayores en el Hospital de Clínicas. Es un estudio prospectivo y descriptivo. Fue aprobado por el Comité de Ética institucional. Se reclutaron 206 pacientes de 65 años y más, coordinados para cirugía electiva, entre marzo del 2019 y marzo del 2020. Se aplicó la Escala de Fragilidad de Edmonton Reportada (REFS) para la detección de la fragilidad. La prevalencia de fragilidad fue del 22,8% con un IC 16-29,6, por lo que decimos que es alta en esta población y similar al de otros contextos quirúrgicos y no quirúrgicos. Se encontraron un número significativamente mayor de hipertensión arterial, arritmias, diabetes, hipotiroidismo y tabaquismo entre los pacientes frágiles. Su prevalencia e impacto en la morbimortalidad operatoria constituyen razones de peso para su inclusión en la valoración perioperatoria en nuestro sistema de salud así como la capacitación de los anestesiólogos en la detección de la fragilidad a través del uso de herramientas prácticas, válidas y confiables.


Frailty determines an inability to cope with stressors due to decreased multisystem physiologic reserves. The surgical anesthetic act is a stressful event and the presence of frailty is an independent risk factor for perioperative morbidity and mortality Detection of frailty would allow for addressing reversible factors causing it, with the intention of reducing the risks that are inherent to anesthetic acts. Detection in the perioperative assessment provides relevant information that is not obtained in a traditional evaluation. This approach has become the standard in perioperative assessment of geriatric surgical patients. The study aims to assess the prevalence of frailty in elective surgery for the elderly at Clínicas Hospital. Method: prospective, descriptive study approved by the institutional Ethics Committee. 206 patients aged 65 years old and over who had been coordinated for elective surgery were recruited for the study between March, 2019 and March, 2020. The Reported Edmonton Frailty Scale (REFS) was applied to detect frailty. Prevalence of frailty was 22.8% with a CI of 16-29 in this population, rather high and similar to the frail patients percentages in other surgical and non-surgical settings. Significantly higher numbers of arterial hypertension, arrhythmias, diabetes and hypothyroidism cases and tobacco users were found among frail patients. Prevalence and impact of frailty on operative morbidity and mortality are compelling reasons for its inclusion in the perioperative assessment of our health system, as well as the training of anesthesiologists in the detection of frailty through the use of practical, valid and reliable tools.


A fragilidade determina uma incapacidade de lidar com estressores devido à diminuição das reservas fisiológicas multissistêmicas. O ato anestésico cirúrgico é um evento estressante e a presença de fragilidade é um fator de risco independente para morbimortalidade perioperatória. Identificá-lo permitiria abordar os fatores reversíveis que o determinam com o intuito de reduzir os riscos inerentes ao referido ato. Sua detecção na avaliação perioperatória fornece informações relevantes que não são obtidas com uma avaliação tradicional. Essa abordagem tornou-se padrão na avaliação perioperatória de idosos. O objetivo do estudo é avaliar a prevalência de fragilidade em cirurgias eletivas para idosos no Hospital de Clínicas. Realizou-se um estudo prospectivo e descritivo, aprovado pelo Comitê de Ética institucional. 206 pacientes com 65 anos ou mais, coordenados para cirurgia eletiva, foram recrutados entre março de 2019 e março de 2020. A Reported Edmonton Frailty Scale (REFS) foi aplicada para detectar fragilidade. A prevalência de fragilidade foi de 22,8% com um IC de 16-29,6, pelo que podemos dizer que é elevada nesta população e semelhante à de outros contextos cirúrgicos e não cirúrgicos. Números significativamente maiores de hipertensão arterial, arritmias, diabetes, hipotireoidismo e tabagismo foram encontrados entre os pacientes frágeis. A prevalência e impacto da fragilidade na morbimortalidade operatória são razões convincentes para sua inclusão na avaliação perioperatória em nosso sistema de saúde, bem como para o treinamento de anestesistas na detecção de fragilidade por meio do uso de ferramentas práticas, válidas e confiáveis.

Resumo Objetivo Analisar os preditores de mortalidade e o tempo médio de sobrevivência dos pacientes internados nas unidades de terapias intensivas. Métodos Coorte prospectiva, realizada no período de agosto de 2018 a julho de 2019, em quatro Unidades de Terapia Intensiva (UTI) de adultos, da rede pública e privada do Estado de Sergipe. Foram incluídos todos os pacientes adultos, desde que possuíssem o tempo de permanência mínima de 24 horas na unidade. O desfecho primário foi o óbito. Os desfechos secundários foram: diálise, lesão por pressão, lesão renal aguda, necessidade de ventilação mecânica invasiva por mais de 48 horas, infecção e o tempo de internação. Resultados Dos 432 pacientes, houve predomínio de óbito em pacientes do sexo masculino, com idade mais avançada e procedentes da unidade de emergência. A presença de insuficiência cardíaca, valores de creatinina >1,5 mg/dL na admissão, diabetes mellitus, doença hepática e tabagismo também tiveram associação com o desfecho óbito. Quanto aos demais preditores, destacaram-se o maior tempo de internação; maiores escores do Sequential Organ Failure Assessment (SOFA), Simplified Acute Phisiology (SAPS 3) e Nursing Activies Score (NAS), além do uso de noradrenalina. O uso do fentanil foi associado ao aumento do tempo de sobrevida e o tempo médio de sobrevivência geral foi 28 dias. Conclusão Os preditores de mortalidade dos pacientes internados em UTI de Sergipe foram o maior tempo de internação; os maiores escores de SOFA, SAPS-3 e NAS; creatinina >1,5mg/dl na admissão; uso de drogas vasopressoras e a necessidade de diálise.

Abstract Objective To analyze the predictors of mortality and the average survival time of patients hospitalized in Intensive Care Units. Methods This is a prospective cohort, carried out from August 2018 to July 2019, in four adult Intensive Care Units (ICU) from the public and private network of the State of Sergipe. All adult patients were included, provided they had a minimum length of stay of 24 hours in the unit. The primary outcome was death. Secondary outcomes were dialysis, pressure injury, Acute Kidney Injury, need for invasive mechanical ventilation for more than 48 hours, infection, and length of hospital stay. Results Of the 432 patients, there was a predominance of death in male patients, older and coming from the emergency unit. The presence of heart failure, creatinine values >1.5 mg/dL at admission, diabetes mellitus, liver disease and smoking were also associated with the death outcome. As for the other predictors, the longest hospital stay, higher Sequential Organ Failure Assessment (SOFA), Simplified Acute Physiology (SAPS 3) and Nursing Activities Score (NAS) scores, in addition to the use of noradrenaline, stand out. The use of fentanyl was associated with increased survival time and the overall median survival time was 28 days. Conclusion The mortality predictors of patients admitted to the ICU in Sergipe were longer length of stay; the highest SOFA, SAPS-3 and NAS scores; creatinine >1.5mg/dl on admission; use of vasopressor drugs and the need for dialysis.

@#Introduction: Poor nutritional status of female adolescents does not only have an impact on themselves but also have an impact on the next generation. Food intake and dietary quality are important to support nutritional status during adolescence. This study aims to determine the relationship between food intake and dietary quality with nutritional status of female adolescent in Tasikmalaya, West Java, Indonesia. Methods: This research was an observational study using a cross-sectional design. The subjects of this study were female adolescents aged 12-18 years with a total sample of 222 people. Data of food intake and dietary quality were measured using recall method, while nutritional status data were calculated using BMI for Age Z scores (BAZ). A linear regression model was used to predict the relationship of food intake and dietary quality to the nutritional status of female adolescents. Results: The average BAZ was 0.1±1.0, with an average dietary quality of 34.6±6.6%. The average energy intake was 1507.9±472.3 kcal, protein 32.4±11.0 g, carbohydrates 258.6±84.4 g, fat 44.9±19.9 g and fiber 3.88±2.7 g. Food intake (energy, protein, and fiber) and dietary quality were related to nutritional status (BAZ) in female adolescents (p<0.05). Conclusion: The balance of food intake and dietary quality is important to maintain the nutritional status of female adolescent properly. Nutritional interventions are needed to support female adolescent entering pregnancy with good nutritional status.

Background and objectives: The application of prognostic scoring systems to identify risk of death within 24 h of CICU admission has significant consequences for clinical decision-making. Previous score of parameters collected after 24 h was considered too late to predict mortality. As a result, we attempted to develop a CICU admission risk score to predict hospital mortality using indicators collected within 24 h. Methods: Data were obtained from SCIENCE registry from January 1, 2021 to December 21, 2021. Outcomes of 657 patients (mean age 58.91 ± 12.8 years) were recorded retrospectively. Demography, risk factors, comorbidities, vital signs, laboratory and echocardiography data at 24-h of patient admitted to CICU were analysed by multivariate logistic regression to create two models of scoring system (probability and cut-off model) to predict in-hospital mortality of any cause. Results: From a total of 657 patients, the hospital mortality was 15%. The significant predictors of mortality were male, acute heart failure, hemodynamic instability, pneumonia, baseline creatinine _x0001_1.5 mg/ dL, TAPSE <17 mm, and the use of mechanical ventilator within first 24-h of CICU admission. Based on Receiver Operating Characteristic (ROC) curve analysis a cut off of _x0001_3 is considered to be a high risk of inhospital mortality (sensitivity 75% and specificity 65%). Conclusion: The initial 24-h SCIENCE admission risk rating system can be used to predict in-hospital mortality in patients admitted to the CICU with a high degree of sensitivity and specificity,

ABSTRACT Introduction: The objective of this single-center study it to retrospectively analyze the relationship between transfusion and 30-day postoperative outcomes in patients undergoing isolated off-pump coronary artery bypass grafting. Methods: Perioperative data of 2,178 patients who underwent isolated off-pump coronary artery bypass grafting from 2018 to 2019 were collected. A 1:1 propensity score matching was performed to control for potential biases between patients who received blood transfusion and those who did not. After propensity score matching, we analyzed the clinical outcomes of transfusion and non-transfusion patients. Postoperative complications and the survival of patients within 30 days after surgery in both groups were analyzed. Kaplan-Meier survival curve and log-rank test were used for survival analysis. Results: The total blood transfusion rate of all patients was 29%, including red blood cell (27.6%), plasma (7.3%), and platelet (1.9%). Four hundred and forty patients in each group were compared after propensity score matching. There were no significant differences in the incidence of stroke, myocardial infarction, atrial fibrillation, acute kidney function injury, and sternal wound infection of both groups (P>0.05). However, higher incidence of postoperative pulmonary infection and more mechanical ventilation time and days of stay in the intensive care unit and postoperative in-hospital stay were associated with blood transfusion (P<0.05). The 30-day cumulative survival rate of the transfusion group was lower than that of the control group (P<0.05). Conclusion: Perioperative blood transfusion increases the risks of postoperative pulmonary infection and short-term mortality in off-pump coronary artery bypass grafting patients.

Background: identifying patients at risk for negative outcomes is key for performing a timely triage and adapting the care intensity for patients with COVID-19. Early warning scores are rules that alert to the risk of adverse outcomes during hospitalization. We sought to validate the modified NEWS, NEWS-2 and COVID-19 Severity Index (CSI). Methods: a prospective observational multicenter study of patients hospitalized for CO VID-19 at three quaternary care hospitals in Bogotá, Colombia, between April and November 2020. The operating characteristics and areas under the ROC curve were calculated. Results: 711 patients were included, in whom the AUC for death was 0.68, 0.58 and 0.68, and for ICU admission was 0.61, 0.63 and 0.66 for mNEWS, NEWS-2 and CSI, respectively. The CSI had the greatest sensitivity for ICU admission or death (87.6 and 90.0%) and NEWS-2 had the greatest specificity (76.8 and 75.5%). Conclusions: the three early warning scores had a low to moderate performance in pre dicting ICU admission or death in patients hospitalized for COVID-19. (Acta Med Colomb 2022; 47. Antecedentes: la identificación de los pacientes con riesgo de desenlaces negativos es clave para realizar un triage oportuno y adecuar la intensidad de los cuidados en los pacientes con COVID-19. Las puntuaciones de alerta temprana son reglas para advertir el riesgo de desenlaces adversos durante la hospitalización. Buscamos validar el NEWS modificado, NEWS-2 y COVID-19 Severity Index. Metodología: estudio observacional, prospectivo, multicéntrico con pacientes hospitalizados por COVID-19 en tres hospitales de cuarto nivel en Bogotá (Colombia), entre abril y noviembre de 2020. Se calcularon las características operativas y áreas bajo la curva ROC. Resultados: se contó con 711 pacientes entre los cuales el AUC para muerte fue 0.68, 0.58 y 0.68, y para ingreso a UCI de 0.61, 0.63 y 0.66 para NEWSm, NEWS-2 y CSI respectivamente. El CSI alcanzó la mayor sensibilidad para ingreso a UCI o muerte (87.6 y 90.0%) y la mayor especificidad fue el NEWS-2 (76.8 y 75.5%). Conclusiones: las tres puntuaciones de alerta temprana mostraron un desempeño bajo a moderado para la predicción del ingreso a UCI o muerte en pacientes hospitalizados por COVID-19. Abstract The Chilean Skill for Life (CSFL) is a school mental health program implemented by local agencies from the Education, Health, and Social Services public sectors. It represents an excellent opportunity to study inter-organizational collaboration and its advantages to public and state-subsidized schools. The propensity score matching technique was used to compare school performance in second grade (the most intensive treatment level) between schools participating in the program and those not participating and between schools with different types of sectoral collaboration to identify the best partner for the school. To select all Chilean schools' participant in CSFL and the comparative group of schools' non-participant, a sequential sampling was applied. The measures were obtained from government public data, considering annual school performance and other educational indicators. It was found that public schools that implement the CSFL obtain better school performance than those that do not implement it (ATT = .042; p < .05), for state-subsidized schools, the same was not found. It was also observed that when educational agencies implement the program, the gain is more significant (ATT = .046; p < .05). The importance of aligning program values and goals with local agencies and schools is discussed, analyzing the possibilities for better collaboration in school mental health.


Resumen Habilidades para la Vida (HPV) es un programa chileno de salud mental escolar multinivel que llega a millones de estudiantes en miles de territorios del país, y es considerado uno de los programas de salud mental escolar más grandes del mundo. Es implementado en el país por agencias locales de los sectores públicos de educación, salud y servicios sociales, lo que representa una excelente oportunidad para estudiar la colaboración interorganizacional y sus ventajas para las escuelas públicas y subvencionadas por el Estado. Este constructo ha recibido gran atención en los últimos años en el campo de la salud mental escolar, dado que destaca la importancia de generar procesos que permitan compartir una identidad colectiva, una agenda coordinada, tener una comunicación efectiva y una colaboración mutua entre los diversos dispositivos que buscan contribuir al logro de las metas de salud mental y a los objetivos educativos de las escuelas. A pesar de los grandes beneficios que traería la colaboración interorganizacional en la ejecución de programas de salud mental escolar, los estudios al respecto son más bien escasos y centrados en intervenciones que vinculan tan solo un tipo de agencia ejecutora con las escuelas. Por este motivo, el presente estudio pretende analizar el impacto de la colaboración interorganizacional en el rendimiento académico de aquellos estudiantes que reciben el programa HPV, teniendo dos hipótesis a la base: (1) que aquellos estudiantes que forman parte de las escuelas en las que se entrega el programa tendrán un mejor rendimiento académico que aquellos que no reciben la intervención; y (2) que el sector educación será el mejor partner o colaborador, puesto que comparte las metas educativas con la escuela, lo que impactaría en el rendimiento académico de aquellos estudiantes que reciben el programa. Para medir esto, se utilizó la técnica Propensity Matching Score, la cual sirvió para comparar el rendimiento escolar de los estudiantes en segundo grado (el nivel de tratamiento más intensivo) entre las escuelas participantes y no participantes del programa, y entre las escuelas con diferentes tipos de colaboración sectorial. Para seleccionar a los participantes de las escuelas chilenas en HPV y del grupo comparativo se aplicó un muestreo secuencial. Las medidas se obtuvieron de datos públicos del gobierno de Chile, y se consideró el rendimiento escolar anual, la vulnerabilidad social de las escuelas y otros indicadores educativos. Se encontró que las escuelas públicas que implementan el HPV obtienen un mejor rendimiento escolar que las que no lo implementan (ATT = .042; p < .05), resultados que no se repitieron en aquellas escuelas subsidiadas por el Estado. También se observó que cuando las agencias educativas implementan el programa, el aumento del rendimiento académico es mayor (ATT = .046; p < .05) respecto que aquellas que lo implementan mediante agencias locales de salud o de servicios sociales. Se discute la importancia de alinear los valores y las metas del programa con las agencias locales y las escuelas, analizando las posibilidades de una mejor colaboración en la salud mental escolar. Esto implica también considerar las metas educativas de cada centro en el cual se implementan estas intervenciones, ya que es posible encontrarse en ocasiones con modelos educacionales restrictivos y tradicionales, centrados por sobre todo en lo cognitivo. Por este motivo, experimentan dificultades al alinearse con programas de habilidades socioemocionales, lo que termina obstaculizando las posibilidades de colaboración interorganizacional con agencias interventoras de dispositivos orientados a esto. En este marco, se exponen una serie de consideraciones clave vinculadas a la importancia de preparar las condiciones y los recursos para el trabajo interorganizacional entre escuelas y entidades ejecutoras de programas de salud mental escolar.

Abstract Objective: In previous studies, smaller renal volumes were reported in prematurely born infants, however, these renal volumes were not corrected for body surface area, the main determinant of renal size. Given the rapid growth of the renal cortex after premature birth, the authors hypothesized that corrected volumes would not differ from healthy controls. Methods: Ambispective cohort study with prospective follow-up of prematurely born babies in a large specialized center and retrospectively recruited healthy control group. Children were assessed for renal length and renal volumes at age 5 by three independent ultrasonographers. Detailed anthropometry, blood pressure and renal function were also obtained. Age independent z-scores were calculated for all parameters and compared using descriptive statistics. Results: Eighty-nine premature study participants (median 32 weeks gestational age) and 33 healthy controls (median 38 weeks gestational age) were studied. Study participants did not differ in age, sex, Afro-Colombian descent, height, blood pressure, serum creatinine, or new Schwartz eGFR. Premature study participants had a significantly lower weight (17.65 ± 2.93 kg) than controls (19.05 ± 2.81 kg, p = 0.0072) and lower body surface area. The right renal volumes were significantly smaller (39.4 vs 43.4 mL), but after correction for body surface area, the renal volume and renal length z-scores were identical for both kidneys (mean right kidney -0.707 vs -0.507; mean left kidney -0.498 vs -0.524, respectively). Conclusion: Renal volumes need to be corrected to body surface area. After correction for body surface area, 5-year-old healthy and prematurely born children have comparable renal volumes.

Resumo O enfrentamento da covid-19 suscitou uma série de problemas na área da saúde, em razão do aumento da demanda de cuidados intensivos. Para solucionar a crise causada pela escassez de recursos de alta complexidade, a tomada de decisão tem se norteado por escores prognósticos, porém esse processo inclui uma dimensão moral, ainda que esta seja menos evidente. Mediante revisão integrativa, este artigo buscou refletir sobre a razoabilidade da utilização de indicadores de gravidade para definir a alocação de recursos escassos na saúde. Observou-se que o trabalho realizado em situações de escassez de recursos provoca sobrecarga moral, convergindo para busca por soluções padronizadas e objetivas, como a utilização de escores prognósticos. Conclui-se que seu uso isolado e indiscriminado não é eticamente aceitável e merece avaliação cautelosa, mesmo em situações emergenciais, como a da covid-19.


Abstract Facing COVID-19 caused many problems in the healthcare field, due to the rise in the intensive care demand. To solve this crisis, caused by the scarcity of resources of high complexity, decision-making has been guided by prognostic scores; however, this process includes a moral dimension, although less evident. With na integrative review, this article sought to reflect on the reasonability of using severity indicators to define the allocation of the scarce resources in healthcare. We observed that the work carried out on resource scarcity situations causes moral overload, converging to the search for standard and objective solutions, such as the use of prognostic scores. We conclude that their isolated and indiscriminate use is not ethically acceptable and deserves cautious evaluation, even in emergency situations, such as COVID-19.


Resumen La lucha contra el Covid-19 implicó una serie de problemas en el área de la salud, debido al aumento de la demanda de cuidados intensivos. Para solucionar la crisis provocada por la escasez de recursos de alta complejidad, la toma de decisiones estuvo orientada por puntuaciones pronósticas, pero este proceso incluye una dimensión moral aún menos evidente. A partir de una revisión integradora, este artículo buscó reflexionar sobre la razonabilidad de utilizar indicadores de gravedad para definir la asignación de recursos escasos en salud. El trabajo realizado en situaciones de escasez de recursos genera sobrecarga moral, llevando a la búsqueda de soluciones estandarizadas y objetivas, como el uso de puntuaciones de pronóstico. Se concluye que su uso aislado e indiscriminado no es éticamente aceptable y merece una cuidadosa evaluación, incluso en situaciones de emergencia, como la del Covid-19.

ABSTRACT

The COVID-19 pandemic lockdown of March-May 2020 affected lifestyles, availability of commodities, and dietary habits. This study examined the effect of lockdown on meal patterns and consumption of pro and anti-inflammatory foods by women. An online survey was conducted on 1545 women aged 18 and above, residing in India. Dietary changes before and during lockdown were assigned numerical scores. Data were analyzed using non-parametric statistical tests. Lockdown showed positive effects on diet patterns, with 37% increase in consumption of home-cooked meals, drastic decrease in ordering food via delivery apps from 49.9% pre-lockdown to 2.2%; decreased consumption of processed foods from 25.8 % to 11.4%; increased water intake (59.6%); increased consumption of anti-inflammatory foods like vegetables and fruits (48.4%) and nearly 40% decrease in consumption of pro-inflammatory foods like chaat, cakes, fried items. Three AYUSH guidelines were followed daily: 80.3% respondents used spices in cooking; 52.9% drank warm water and 37.2% drank “Golden” milk. The mean total score was 50.4 ± 12.6 out of a maximum score of 112. The favorable changes in the dietary patterns of the women could be due to unavailability of pro-inflammatory food items and closing of food deliveries during lockdown.

Objective:To investigate the application of fetal atria septal excursion index (ASEI) combining cardiovascular structure Z-scores in fetuses with redundancy foramen ovale flap (RFOF).Methods:Twenty-two fetuses with RFOF diagnosed by fetal echocardiography in Sir Run Run Shaw Hospital from December 2019 to September 2021 were selected as the RFOF group and 62 normal fetuses as the control group. The movement of the foramen ovale valve was observed in the four chamber view, and the maximum distance of the foramen ovale valve bulging, the total length of the atrial septum, the foramen ovale diameter, the diameters of the aortic valve, the pulmonary valve, the ascending aorta, the aortic isthmus, the descending aorta and the left and right ventricular areas were measured. ASEI, foramen ovale diameter, foramen ovale valve excursion distance, the ratio of foramen ovale diameter to total atrial septum length, aortic valvular annular diameter Z-score(AOV Z-score), pulmonary valvular annular diameter Z-score(PV Z-score), ascending aorta diameter Z-score(AOA Z-score), ascending aorta diameter Z-score (AOA Z-score), aortic isthmus Z-score (AOI Z-score), descending aorta diameter Z-score (AOD Z-score), diastolic right ventricular area Z-score (RV area Z-score) and diastolic left ventricular area Z-score(LV area Z-score) were compared between the two groups. RFOF fetuses were followed up to observe the prognosis.Results:There were significant differences in fetal ASEI, foramen ovale diameter, foramen ovale valve excursion distance, the ratio of foramen ovale diameter to total atrial septum length, AOV Z-score, PV Z-score, AOA Z-score, AOI Z-score, AOD Z-score and LV area Z-score between RFOF group and control group (all P<0.05), but there was no significant difference in RV area Z-score ( P>0.05). Postnatal follow-up outcomes in RFOF group indicated that 17 of 22 cases (77.3%) had live birth, five cases (22.7%) were lost at follow-up. Twelve of 17 live birth cases (70.5%) showed structurally normal heart by postnatal echocardiography, 3 cases (17.6%) showed mild coarctation of the aorta with nonsignificant hemodynamics under clinical follow-up, 2 cases (11.8%) were diagnosed with coarctation of the aorta, pulmonary artery dilation and atrial septal defect, and died after operation, of which one case died of infections pneumonia after operation. Another case was born prematurely at 36 + 4 weeks with a birth weight of 2 650 g, and neotal echocardiography showed ventricular septal defect, atrial septal defect and patent ductus arteriosus. Conclusions:ASEI can quantitatively evaluate the behavior of foramen ovale valve mobility and in combination with Z-scores of cardiovascular structures can be used to assess the morphological changes of fetal hearts. Although some fetuses with RFOF demonstrate left and right ventricular disproportion and coarctation of the aorta in fetal echocardiography, most fetuses have a good prognosis.

ABSTRACT

Objective:To explore the value of neutrophil to lymphocyte rate (NLR) combined with red blood cell distribution width to platelet count ratio (RPR) in evaluating the condition and prognosis of emergency elderly sepsis patients.Methods:A prospective research method was conducted to select 169 elderly patients with sepsis who visited the emergency department of Xuanwu Hospital of Capital Medical University from January 2020 to February 2022.After admission, blood routine examination, chest computerized tomography, biochemical examination, procalcitonin, and pathogenic examination were given, and the scores of acute physiology and chronic health evaluation (APACHE Ⅱ) were scored according to worst value of 24 hours. After 28 days of follow-up, the patients were divided into the survival group(125 cases) and the death group(44 cases) according to the prognosis. The differences of white blood cell count(WBC), NLR, PCT, RPR and APACHE Ⅱ scores were compared between the two groups. The correlation between NLR,RPR and APACHE Ⅱ scores were analyzed. The difference of area under receiver operating characteristic curve (ROC) of RPR, NLR, their combination and PCT in predicting mortality were compared in elderly patients with sepsis. The independent sample t test was used to compare the measurement data with normal distribution, and the χ 2 test was used to compare the enumeration data. The risk factors were analyzed by multiple logistic regression analysis. Results:There was no significant difference in WBC between the survival group and the death group ( P=0.361). The APACHE Ⅱ scores ((18.52±2.41) points), RPR (0.17±0.03), NLR (10.64±3.48), PCT ((2.55±1.14) μg/L) in the death group were higher than those in the survival group ((14.17±2.71) points, (0.14±0.03), NLR (7.67±3.33), (1.19±0.81) μg/L), the difference was statistically significant ( t values were 9.44,7.32,4.92, and 7.32, respectively; all P<0.001). RPR and NLR were positively correlated with APACHE Ⅱ scores ( r=0.393,0.368;both P<0.001). Multivariate logistic regression analysis showed that increased NLR ( OR=1.174,95% CI 1.041-1.325), procalcitonin ( OR=4.353,95% CI 2.382-7.954), RPR ( OR=14.247,95% CI 2.635-77.025) were independent risk factors for the prognosis of sepsis patients ( P values were 0.009,<0.001, and 0.002, respectively).The area under receiver operating characteristic curve (AUC) of PCT in predicting mortality was 0.859 (95% CI:0.801-0.917), the AUC of RPR was 0.755 (95% CI:0.665-0.845), and the AUC of NLR was 0.727 (95% CI: 0.643-0.812). The AUC of RPR and NLR was smaller than that of PCT ( P=0.033, 0.015), but the AUC of RPR combined with NLR was 0.799, which had no significant difference compared with PCT ( P=0.195). Conclusion:Both NLR and RPR had a certain predictive value for the condition and prognosis of elderly sepsis patients in emergency, and their combined evaluation value was similar to that of PCT.
