ABSTRACT

Aims: To compare operating time, early complication(s) and outcome of Gross & Ferguson and Mitchell-Banks repair techniques in children with congenital inguinal hernia. Materials and Methods: A prospective randomized study including boys aged 1-12 years, was conducted over one year. The patients were allocated in two interventional groups based on the repair viz. Gross and Ferguson and Mitchell-Banks group. After hernia repair, the patients were followed up on the day of surgery and at one week for early complications. Results: Fifty patients were enrolled. Mean operating time in Mitchell-Banks technique was significantly more than Gross and Ferguson technique (p=0.00012). The incidence of fever in two groups was similar. Scrotal edema was statistically significant in MB group on both Post-Operative Day -1 and 7 (p=0.023 and p=0.034). No patient in either group had wound infection or scrotal hematoma on POD-1. One subject (2%) had scrotal hematoma in MB group on POD-7. A total of 12 subjects (24%) had Surgical Site Infection on POD-7. There was statistically higher incidence of SSI on Day-7 in GF group (32%) as compared to MB group (16%) (p=0.018). Conclusion: GF technique had lesser operating time, less incidence of scrotal edema and hematoma though higher rate of surgical site infections. In this respect, GF technique can be used as a simple and safe procedure.

ABSTRACT

Background: Inguinoscrotal swellings are the most common presenting complaints in the surgical out patient department and surgeries for the same are the most commonly done elective procedures.However meticulous the surgeon is, there is always some amount of scrotal oedema. Our study is intended to use a simple and novel technique by applying a scrotal hitch after the surgery to prevent the scrotal oedema. The aim of the study was to evaluate the effectiveness of a scrotal hitch in reducing the scrotal oedema and pain postoperatively.Methods: In this study all subjects undergoing inguinal hernia surgeries at undergoing at R.L. Jalappa Hospital, Kolar from June 2019 to August 2019 were included. Of the 60 subjects, 30 in group A were given scrotal hitch and subjects in group B were given scrotal support alone. The incidence of scrotal oedema and postoperative pain and development of any complications will be compared in both the groups.Results: A total of 60 subjects were included in this study. Mean age of patients in group A is 45.63±21.745 and group B is 41.43±24.579. 2 patients with scrotal hitch developed scrotal edema whereas 6 patients developed scrotal edema in group B. The post-operative pain score was lower in patients with scrotal hitch.Conclusions: Scrotal hitch is a simple and effective technique in preventing post-operative oedema and pain. The results of this study would encourage surgeons to take up this simple procedure to prevent the commonest complication of inguino-scrotal surgeries.

ABSTRACT

Objective To evaluate the efficacy and safety of Aescuven forte on treating elderly patients with scrotal edema. Methods Eighty-two patients with scrotal edema were randomly divided into control group and treatment group(41cases for each group). Patients in the control group received physical therapy as well as oral Furosemide tablets for 20 mg and 2 times/ d,while in treatment group were given 450 mg oral Aescuven forte (2 times/ d),and 300 mg Aescuven forte at 10 d besides the treatment as the control group. The total treatment period was 20 days. The information of scrotal volume,score of discomfort or pain,scrotal ulcer healing time were collected. Results After 20 d treatment,the scrotal diameter in treatment group was(7. 81 ± 0. 64)cm,shorter than that before treatment(13. 30 ± 0. 73)cm,and the difference was statistically significant( t = 2. 201,P 0. 05). After treatment,the level of pain and discomfort,voiding symptoms,life quality score were(7. 35 ± 3. 08),(3. 43 ± 1. 91),(5. 89 ± 2. 11),lower than those in the control group((8. 47 ± 3. 07),(4. 52 ± 2. 19),(6. 96 ± 2. 13)),and the differences were statistically significant (t = 2. 260,2. 382,2. 319,P < 0. 05). The total effective rate of treatment group was 87. 8%(36 / 41),higher than that in control group(65. 8%(27 / 41)),and there was significant difference(χ2 = 4. 580,P < 0. 01). In the periods of treatment,no adverse reaction between two groups was occurred. Conclusion The use of Aescuven forte combine with diuretic has a synergistic effect and significantly improve the clinical symptoms of elderly patients with scrotal edema in terms of eliminating edema,promoting healing,relieving scrotal ulcer wound pain, improving voiding symptoms and the quality of life. And there is no side effect.
