ABSTRACT

@#Introduction: Adequate knowledge of antibiotics usage among users is one of the key factors preventing the spread of antibiotic-resistant strains’ infections. This study is conducted to determine the level of awareness on antibiotic usage and antibiotic resistance among the residents in Selangor, Malaysia. Methods: This study is a cross-sectional, questionnaire-based study that consisted of three parts: socio-demographic profile, antibiotic use awareness, and antibiotic resistance awareness. The questionnaire was distributed online using the snowball sampling method from January to March 2021. Results: A total of 239 respondents have participated in this study. The majority of the respondents involved were among the Malay youth residents in Selangor. Results showed that most residents in Selangor (65%) misunderstand that antibiotics can treat cold and flu. More than 44% of respondents also thought that antibiotic resistance could only occur to individuals who frequently take antibiotics and could not be spread to other people. Findings also showed associations (p<0.05) between the level of awareness on antibiotic resistance and the respondents’ socio-demographic profile (education level, household income and settlement category). A significant positive correlation was also observed between the level of awareness of antibiotic use and antibiotic resistance (r=0.42, p<0.05). Conclusion: In conclusion, the Selangor respondents’ antibiotic usage and antibiotic resistance level of awareness are moderate to high. However, there is still a need to increase awareness of antibiotic resistance and correct the misunderstandings among the public to help curb the antibiotic resistance issue from rapidly increasing in the future.

@#Breast cancer is one of the leading causes of mortality among women. The aim of the current study was to assess the factors affecting awareness and beliefs regarding breast cancer, among women living in the state of Selangor, Malaysia. A cross-sectional study was conducted among women living in Selangor aged 18 to 65 years old, with a total of 483 participants. A validated questionnaire regarding awareness and belief was used. The questionnaires consist of 25 questions comprising of socio-demographic, history of other diseases, awareness and belief. The prevalence of poor awareness among women was 63.4%. while the poor level of belief was 84.7%. There was a significant association between awareness and educational background, family history of breast cancer (P=0.001, P=0.032) respectively. The association between awareness and belief was significant (P=0.02). As a conclusion: There is a high level of poor belief and poor awareness among women in Selangor, Malaysia regarding breast cancer. Source of information such as the internet plays a major role in breast cancer prevention, and the majority of them do not know the technique of breast self-examination. More health promotion is needed to target general population through big campaign of awareness.

@#Dengue fever is one of the most dangerous vector-borne diseases. According to the World Health Organization (WHO), dengue fever is a mosquito-borne infection caused by virus serotype DEN-1, DEN-2, DEN-3 and DEN-4. In Malaysia, dengue fever cases are on the rise from 6,000 cases in 1995 to over 40,000 in 2010, and this number is still rising. In 2014, the increase of dengue fever cases was alarming. It was reported that up to the end of the year 2014, there were 108,698 notified cases, indicating an increment of 151% compared to the same period of time in 2013 with only 43,346 reported cases. Selangor was the highest contributor of dengue fever cases in 2014. The objective of this paper is to study the relationship between climatic factors namely temperature, rainfall and humidity to the prevalence of dengue fever in Subang Jaya and Sepang district, Selangor. Data on monthly average temperature, precipitation, relative humidity and dengue fever cases for each month in 2014 and 2013 were collected. Data collection was dealt with a few institutions such as Malaysian Meteorological Department, Subang Jaya and Sepang Municipal Council and health district offices. Data were analysed using SPSS (Statistical Package for the Social Sciences) Version 20. General linear model analysis was used to investigate the relationship between the climatic variables and dengue prevalence. Results and Discussion: Based on the general linear model, rainfall and humidity were found to have significant relationships to monthly dengue fever cases (p= <0.001, p= 0.002). Rainfall was identified as the most significant predictor because rainfall can provide more breeding places for Aedes mosquitoes. As for humidity, higher relative humidity had been associated with increased Aedes aegypti feeding activity, survival and egg development. Temperature was not significantly related to monthly dengue fever cases (p= 0.561) in this study. However, this could be due to the short period of study. Conclusion: Climatic factors play an important role in the prevalence of dengue fever. However, there are many other factors of dengue fever that should be considered such as urbanisation as well as community knowledge, attitude and practice.

ABSTRACT

Introduction: Pap smear screening may increase the quality of women’s health through early detection of cervical cancer thus providing a better prognosis for women with cervical cancer. While the benefit of Pap smear screening is well documented, the screening uptake among Malaysian women in general was unsatisfactory with only 47.3% being screened. This study aimed to determine the prevalence of Pap smear uptake and its associated factors among Orang Asli women who lives in rural areas in Selangor. Methods: A cross sectional study was conducted in Hulu Langat and Kuala Langat districts from April to July 2017. Through cluster sampling, five Orang Asli settlements in the two districts were chosen. All women in the selected village aged between 18 to 65 years were invited to participate and interviewed using a structured, pretested questionnaire. Data were analysed using SPSS Version 22. Univariate and bivariate analysis were conducted to identify factors associated with Pap smear uptake among the participants. All hypotheses tests were two-sided and level of significance was set at 0.05. Results: Out of the 147 respondents, 114 (77.6%) had undergone Pap smear for at least once in the past. Women aged more than 35 years (p=0.013) were shown to be significantly more likely to take Pap smear compared to women who were 35 years old and less. There were significant associations between marital status (p<0.001), knowledge (p<0.001), attitude (p<0.001) and use of oral contraceptive (p=0.001) with Pap smear uptake. Conclusion: This study showed a high prevalence of Pap smear uptake among Orang Asli women in Selangor.

ABSTRACT

Public and private hospitals in Kuala Lumpur and Selangor were evaluated in terms of their accessibility for the physically disabled. The research hypotheses for this study included the following: (1) Both types of hospitals are accessible for the physically disabled as measured by specifi c criteria but (2) the degree of accessibility is higher in the case of private hospitals as compared to public hospitals. A total of 23 private hospitals and 11 public hospitals in Kuala Lumpur and Selangor were invited to participate in the study. The 5 private hospitals and 5 public hospitals that agreed were evaluated for adequacy of facilities for the physically-disabled. For this purpose, 13 specifi c criteria were assessed and scored for each hospital. These criteria were also grouped into 5 categories, namely, parking, toilet, door and lift, corridor and ramp. Scores were compared between each hospital and then aggregated and compared for private hospitals versus public hospitals. It was found that none of the 5 private hospitals and 5 public hospitals studied satisfi ed 100% of the criteria evaluated. Looking at each hospital individually, the overall scores range from 32% to 92% for the criteria set. Only 4 of the 10 hospitals in our sample achieved overall scores of 80% or higher in terms of the evaluation criteria we used. With the exception of availability of ramps where public hospitals scored slightly higher ,for most of the individual criterion, private hospitals scored higher than public hospitals. Looking at each criterion across all hospitals, the scores range from 59.2% (adequacy of parking) to 85% (adequacy of corridors). The median score obtained by private hospitals and by public hospitals for all 13 criteria were analysed for any difference. The difference between private hospitals and public hospitals is not statistically signifi cant (Mann-Whitney U = 6.5, p-value = 0.099). There is no signifi cant difference between Kuala Lumpur/Selangor private and public hospitals in terms of accessibility for physically disabled people. However, some hospitals are more accessible for the physically disabled than other hospitals. These fi ndings indicate that there is room for improvement.

ABSTRACT

The objectives of this study were to determine the prevalence of the urban poor and its health related factors in Selangor. This cross-sectional study was conducted in the community of Selangor. Data was collected from January 2006 to June 2006. Three out of nine districts were selected to be involved in the study, which were Klang, Kuala Langat and Petaling. A Multistage Stratified Proportionate Systematic Random Sampling was used for this study. The sampling was carried out by the Statistics Department of Malaysia. The survey frame was based on the information gathered from the Population and Housing Census in year 2000. The respondents were interviewed by trained research assistants using a structured pre-tested standardized questionnaire prepared in Malay and English languages. A monthly income of RM706 and less was used to define urban poor based on the guidelines given by the Economy Planning Unit of Malaysia (2004). SPSS 16.0 version was used to analyze the data. Out of 2535 respondents interviewed in this study, 2491 respondents answered the questionnaire completely (response rate 98.3%). Out of 2491 respondents, 202 (8.1%) had a monthly income of RM706 and less. Analysis of the urban poor respondents found that majority were aged between 18 to 40 years old (55.9%), and 29.7% had diagnosed medical illnesses, where hypertension, diabetes and asthma were the most common illnesses at 12.4%, 10.9% and 5.0% respectively. About 10% of the urban poor respondents had physical disabilities such as blurring of vision (7.9%), hearing problems (1.0%) and other disabilities (1.0%).

ABSTRACT

A survey was conducted to investigate the level of consumption of ‘ulam’ in Selangor State among 252 adults (> 17years) (male 28.6%, female 71.4%) of major ethnics (Malays-51.6%; Chinese-30.5%; Indians-17.5%) with the mean age of 42.7 ± 13.9 years. Consumption data were collected using 24 hours duplicate samples together with questionnaire on perceptions of ‘ulam.’ Results showed that ‘ulam’ was preferred by majority of the subjects (82.1%), especially amongst Malays (92.3%). A total of 52% of the subjects consumed partially or boiled ‘ulam.’ Factors that affect their preferences on ‘ulam’ were the perception of therapeutic effects of the ‘ulam’ towards health, its good taste and unique aroma. The most consumed ‘ulam’ were cucumber (Cucumis sativus) (60.6%) ‘kacang botol’ (Psophocarpus tetragonolobus) (33%), Indian pennywort (Hydrocotyle asiatica) (31.5%), lettuce (Lactuca sativa) (27.6%), ‘petai’ (Parkia speciosa) (29%) and ‘ulam raja’ (Cosmos caudatus) (21.9%). The most preferred partially or boiled ‘ulam’ were tapioca shoot (Manihot esculenta) (31.5%), ocra (Hibiscus esculentus) (12.5%) and ‘jantung pisang’ (Musa sapientum) (20.1%). There was no significant difference (P > 0.05) amongst the three different ethnic groups on the consumption of ‘ulam’ and the median for total intake per day was within the range of 30-39 g/day. Ulam is a potential source for increasing vegetable consumption to meet recommendation by World Health Organization (WHO), which is 400 g per day.

ABSTRACT

This cross sectional study was conducted to determine the validity of three screening tools, Mini Nutritional Assessment Short Form (MNA-SF), Malnutrition Risk Screening Tool for Community (MRST-C) and Malnutrition Risk Screening Tool for Hospital (MRST-H) among elderly people at health clinics. The screening tools were validated against anthropometric and functional assessments. The anthropometric assessments that were carried out included body weight, height, arm span, body mass index (BMI), calf circumference (CC) and mid upper arm circumference (MUAC). A set of questionnaire on manual dexterity, muscular strength, instrumental activities daily living (IADL) and cognitive status was used to assess functional abilities. A total of 156 subjects were recruited from rural (38 subjects) and urban (118 subjects) health clinics at Sabak Bernam and Cheras respectively. Subjects’ age ranged from 60 to 83 years old, with 44.2% were men and 55.8% women. The prevalence of muscle wasting among the subjects assessed from MUAC and CC were both 7.0%. MNA-SF had the highest correlation with BMI (r = 0.497, p<0.001), followed by MUAC (r = 0.398, p<0.001), CC (r = 0.473, p<0.001), cognitive assessment (r = 0.229, p<0.001) and handgrip strength (r = 0.209, p<0.001). Whilst MRST-C had the highest correlation with IADL score (r = -0.320, p<0.001) and MRST-H had the highest correlation with the lock and key test (r = -0.325, p<0.01). Sensitivity was the highest for MNA-SF (93.2%), followed by MRST-H (52.5%) and MRST-C (25.8%). Specificity was the highest for MRST-H (97.3%), followed by MRST-C (90.8%) and MNA-SF (79.4%). Positive predictive value (PPV) for MRST-H, MNA-SF and MRST-C was 55.5%, 18.2% and 14.1%, respectively. In conclusion, among the screening tools being validated, MNA-SF is considered the most appropriate tool to be used in health clinics for identification of elderly individuals who are at high risk of malnutrition.
