ABSTRACT

Objective To investigate the influence of predeposited autotransfusion and intraoperative autotransfusion on immune function of patients with selective operation .Methods A retrospective analysis was made on 100 cases of patients with selective op-eration in our hospital from September 2014 to August 2016 ,and all cases were divided into control group and research group .Con-trol group was treated with allogeneic transfusion ,while research group was treated with combination of preoperative autologous blood donation and intraoperative autotransfusion ,and general indexes ,immune cells and immune protein levels between two groups were compared .Results The allogeneic transfusion volume and postoperative incision infection rate of research group were lower than those of control group ,and the difference was statistically significant (P<0 .05) .There was no significant difference in intrao-perative ,postoperative and total blood loss between the two groups (P>0 .05) .Before treatment ,immune cell level between two groups had no significant difference (P>0 .05) .At 1 d and 7 d after surgery ,CD3+ ,CD4+ ,CD4+ /CD8+ and NK cell levels of re-search group were higher than those of control group ,CD8+ level of research group was lower than that of control group ,and the difference was statistically significant (P<0 .05) .Before treatment ,immunoglobulin level between two groups had no difference (P>0 .05) .At 1 d and 7 d after surgery ,IgG ,IgM and C3 levels of research group were higher than those of control group ,and the difference was statistically significant (P<0 .05) .Conclusion Combination of predeposited autotransfusion and intraoperative au-totransfusion has fewer influence on immune function of patients with selective operation ,and has good safety .

ABSTRACT

Objective To investigate the application of preoperative autologous blood donation (PABD) in selective operation.Methods Retrospective investigations and analysis were carried out in clinical datas of 1 026 patients of PABD in selective operation from January 2016 to may 2017,comparing with those who had not performed PABD in the same time.Results ①The surgeries of PABD were mainly neurosurgery,urology,orthopedics,gynecology and hepatobiliary surgery,respectively,which accounted for 5.97%,4.90%,3.78%,3.76% and 3.55% of the patients in the same period;②The rate of transfusion of allogeneic red blood cells of selective operation in PABD group of neurosurgery,urology,orthopedics,gynecology and hepatobiliary surgery were 5.66%,2.51%,4.89%,0 and 4.88%,respetively,while not performed PABD group were 25.46%,28.58%,24.77%,10.62% and 17.54%,respetively;③The rate of transfusion of the group of hemoglobin (Hb)< 120 g/L is significantly higher than those Hb is (120-129.9) g/L,(130-139.9)g/L and ≥ 140 g/L(7.65%vs 2.44%,3.66% and 2.70%,P>0.05).Conclusion ①PABD is mainly carried out in neurosurgery,urology,orthopedics,gynecology and hepatobiliary surgery;②The group of Hb< 120 g/L has a high rate transfusion of allogeneic erythrocyte in selective operations;③Only the PABD is improved can patients make benefit from it.

ABSTRACT

Objective To explore the effect of time management on the on-time rate of implementation of the first selective operation.Methods Five hundred and two selective operations from January to May of 2013 without time management were set as control group, while another five hundred and twenty with time management as experiment group. The two groups were compared in terms of on-time rate of the first selective operation, time for nurse’s extra shift, number of nurses at extra shift, numbers of operations and satisfaction level.Results After time management, the experiment group was significantly better than the control in terms of the on-time rate of operations, time for nurse’s extra shift, number of nurses at extra shift, numbers of operations and satisfaction level (all P<0.01).Conclusion Time management used in the operation room can significantly improve the on-time rate of selective operations and doctors’satisfaction and work efficiency, thus decrease nurses’overworking time.

ABSTRACT

Objective To provide reference for the clinical application of preoperative autologous blood donation(PABD).Meth-ods 34 patients met the conditions of PABD,and underwent PABD were enrolled in the study.Five laboratory indicators include Hemoglobin(Hb),RBC,WBC,PLT)and hematocrit(HCT),and prepared blood volume before surgery,intraoperative and postop-erative reinfusion volume were recorded before blood collection and after reinfusion.Results In the patients who only underwent PABD(A group),compared between before blood collection and after blood transfusion(intraoperative or postoperative 24 h),all the five Hematology Index were statistically different(P 0.05).Con-clusion The effective of PABD application in surgical patients undergoing elective operation is good,which won′t cause harm to the patient′s body(reflected in the hematology indicators).However,in some cases preoperative assessment of blood loss is not accurate enough,result in the amount of blood collected before operaton not meeting the needs,which might impact the effect PABD.

ABSTRACT

ObjectiveTo discuss the effect of preoperative psychological communication on psychological state of elderly patients before destined operation.Methods136 cases of elderly patients underwent elective operation were divided into the observation group and the control group with 68 cases in each group,the control group was given routine nursing of general surgery,the observation group received targeted psychological communication based upon routine nursing for consecutive 3 days before surgery.The psychological state such as anxiety,depression and fear was compared between two groups.ResultsThe anxiety,depression and fear in the observation group was significantly improved compared with the control group.ConclusionsPreoperative appropriate and targeted psychological communication can alleviate the anxiety,depression and fear of the elderly patients with elective operation,and contribute to the smooth operation.

ABSTRACT

Objective:To observe the applications of damage control surgery(DCS) theory in selective abdominal operations.Methods:Two patients suffered from abdominal huge tumour were included in this clinical observation.DCS method was used in the first case when uncontrollable haemorrhage happened in the operation.In the second case,the DCS method was planed before operation.Results:Both the two patients were healed.The second one had no dangerous states after operation.Conclusion:In selective abdominal operation,perioperative DCS method should be considered if some life-threatening risks may happen in the operative procedure.
