ABSTRACT

Objective: There are some anthraquinones, anthraquinones and flavonones in Sennae Folium which exhibited significant acidity, such as sennoside A/B and sennoside C/D. The current strategies used in separating these components are mainly based on conventional column chromatography which is time consuming, laborious and costly. This study is aimed at exploring a method of precipitation extraction of acid components in Sennae Folium. Using alkaloid as a “hook”, it is reasonable to use the principle of “acid-alkali complexation” to "fish" the acidic components in Sennae Folium. Methods: Isothermal titration calorimeter (ITC) was used to measure the extraction efficiency of different alkaloids. Then, alkaloid determined by ITC was mixed with extracting solution of Sennae Folium to form complex. High performance liquid chromatography coupled with mass spectrometry (HPLC-MS2) was used to investigate the ingredients “fished” by berberine (Ber). The mechanism of “fishing” process was explained by ITC, optical activity, fluorescence spectrometry and scanning electron microscope. Results: The ITC results proved that the choice of “hook” was particularly important in the process of “fishing”. Among the hooks, the fishing efficiency of the isoquinoline alkaloids (Ber) was the highest, reaching 10.3%. Nine ingredients were detected and determined by HPLC-MS2, and the main components were sennoside A/B and sennoside C/D. Based on ITC test of Ber and sennoside A, the combination mechanism of the two ingredients was a chemical reaction with a nearly binding ratio (2:1). Fluorescence and optical properties of the active ingredients were changed after complexation. By scanning electron microscope, we found that two types of components had obviously self-assembled behavior during the formation process. Conclusion: Ber successfully “fished” the main acidic components, sennoside A/B and sennoside C/D, from Sennae Folium. Combined with different characterizations, the “fishing” process was determined as a chemical association reaction induced by electrostatic interaction or π-π stacking. Therefore, with special identification ability, the “fishing” process had the potential of practical application.

ABSTRACT

Objective: To investigate the clinical effect of Maren Soft Capsule (MSC) in the treatment of constipation induced by antipsychotic drugs. Methods: Totally 192 hospitalized psychiatric patients with constipation caused by antipsychotic drugs were randomly divided into three groups, there wer 64 cases in each group. The first group was treated with MSC, the second group was given Sennae Folium, and the third group was given Phenolphthalein Tablets. The exhaust, defecation time, abdominal pain, and abdominal distension were observed after the treatment. Results: The total effect rates of the three drugs were over 80%. There was no significant difference in the effect of three drugs (P > 0.05), but Sennae Folium and Phenolphthalein Tablets had different degrees of side effects such as diarrhea and abdominal pain. Occurrence of adverse reaction rate of MSC was significantly lower than that of Sennae Folium and Phenolphthalein Tablets (P < 0.01). Conclusion: The effect of MSC in the treatment of antipsychotic drugs-induced constipation is significant, and the incidence of adverse reactions is low.

ABSTRACT

OBJECTIVE:To evaluate the effects of Sennae Folium combined with mannitol for bowel cleaning on multi-slice spiral CT bowel imaging. METHODS:52 patients receiving multi-slice spiral CT imaging examination in our hospital during Jun. 2011 to Jun. 2012 were collected retrospectively,and then divided into control group(22 cases)and observation group(30 cases) according to the methods of bowel preparation. Both groups fasted for 12 h before examination. Sennae Folium 20 g soaked in boiled water 500 ml for half an hour was given to patients in control group 12 h before examination,and then was soaked in boiled water 500 ml again for drinking and other pure water 1 000 ml was also given to patients. Sennae Folium 10 g soaked in boiled wa-ter 500 ml was given to patients in observation group 1 d before examination and 2 h after breakfast;soaked in boiled water 500 ml again 2 h after lunch;10% mannitol 500 ml and pure water 1 000 ml were given to patients after supper. Both groups received multi-slice spiral CT bowel imaging until the patients defecated watery stool after medication. The effective rate of bowl cleaning, image quality and the incidence of ADR were compared between 2 groups. RESULTS:The effective rate of bowel cleaning was 100% in observation group,which was significantly higher than that of control group(81.82%),with statistical significance(P<0.05). The excellent and good rate of imaging quality was 96.67% in observation group,which was significantly better than that of control group(81.82%),with statistical significance(P<0.05). The incidence of ADR as abdominal distension,nausea,intestinal mucosa damage in observation group were significantly lower than those of control group,with statistical significance (P<0.05). CONCLUSIONS:Sennae Folium combined with mannitol is effective method to prepare enteric multi-slice spiral CT imaging exam-ination,and perform good bowl cleaning effect,high image quality and low incidence of ADR.
