ABSTRACT

Background: The link between varicocele and infertility was first reported by cessius in 1st century AD but it was not widely acknowledged until TULLOCH and colleagues reported the improvement of sperm parameters in 26 of 30 patients undergoing varicocelectomy. Varicocele is defined as excessive dilatation of pampiniform venous plexus of spermatic cord. varicocele is an important cause infertility which can be corrected by surgery. Several methods have been used for its treatment including open surgical ligation of spermatic veins as well as laparoscopic varicocelectomy. Open varicocelectomy has more risk of recurrence and complications whereas laparoscopic varicocelectomy is simple, has less risk of recurrence and complication in expert hands. Objectives: To compare Laparoscopic varicocelectomy with open varicocelectomy, in terms of postoperative pain, recurrence rate, hospital stay, cost, cosmetic and complication. To prove hospital stay was more in open group than laparoscopic group and also patient of laparoscopic group return to normal activities earlier than open group. To standardize the laparoscopic varicocelectomy procedure for varicocele. Materials and methods: It was a randomized clinical trial done in Department of General Surgery, Vinayaka Mission Medical College, Karaikal. Study was carried out from 1 st March, 2015 to 1st R. Bharathidasan, Reny Jayaprakash, Subith P. Bhaskar, G. Ambujam. Laparoscopic varicocelectomy now the gold standard procedure for varicocele - A comparative study with open technique based on our experience. IAIM, 2017; 4(7): 218-221. Page 219 March, 2016. A total 70 patients was taken in our study of its 36 patient undergone open varicocelectomy and 34 patient undergone laparoscopic varicocelectomy. Results: Recurrence rate 0% in Laparoscopic Varicocelectomy and 5.6% in open varicocelectomy. Wound complication was 0% in Laparoscopic varicocelectomy and 2.5% in open varicocelectomy. Post-operative pain was more in open group as compared to laparoscopic varicocelectomy. Laparoscopic varicocelectomy has less post-operative morbidity and early return to normal activity. Also there was improvement in seminal analysis in both groups. Conclusion: We would like to standardize laparoscopic varicocoelectomy and make it as a gold standard for Varicocele by replacing open technique provided there is good experienced surgical team and good instrumentation. Cost of the procedure is comparatively high.

ABSTRACT

Objective To observe the change characteristics of serum prealbumin and cholinesterase in the liver cirrhosis pa-tients ,and to discusses the determination of these two indicators in diagnosis ,treatment and prognosis of clinical significance in pa-tients with liver cirrhosis .Methods A total of 45 liver cirrhosis patients diagnosis in our hospital from July to December 2013 were recruited into study group ,at the same time 98 healthy people were recruited into control group .The Hitachi 7170 automatic bio-chemical analyzer was used to detected serum prealbumin and cholinesterase ,the former was detected by immunoturbidimetry meth-od ,the latter was detected by butyryl glucosinolates choline bottom method .The levels and abnormal rates of the two indicators be-fore and after treatment in the study group were compared with those of the control group .Results The serum prealbumin and cho-linesterase at different stage in the study group were significant lower than those of the control group(P0 .05) .Conclusion The serum prealbumin is a sensitive indicator of liver cell damage ,its change could reflect the condition of treatment and development status ,which has im-portant clinical significance on judging the prognosis of patients with liver cirrhosis .

ABSTRACT

Objective: To observe the effects of Fuzheng Xiaozheng Decoction (FXD) on serum prealbumin (PA) of patients with hepatocarcinoma after interventional therapy. Methods: A total of 107 cases of patients with advanced hepatocarcinoma were divided into experimental (59 cases) and control (48 cases) groups. The patients in the experimental group were treated by arterial perfusion + lipiodol embolism + FXD (po), and before transcatheter arterial chemoemblization (TACE) FXD was given for 20 d. The patients in the control group were treated by TACE without FXD, and both groups were treated with TACE for four times. The levels of serum PA of the patients in each group were determined before the TACE and 3, 10, and 25 d after the TACE treatment, respectively. Results: Compared with the control group, the serum PA levels of the patients in the experimental group 3 and 10 d after TACE treatment were increased obviously (P < 0.05). Conclusion: FXD could effectively increase the level of serum PA after TACE treatment and promote the recovery of liver function.

ABSTRACT

Objective To explore the occurrence of anemia, anaemia type, iron metabolism situation, variation of serum Prealbumin (PA), and the relationship among them in cancer patients. Methods Three hundred and seventy cancer patients, admitted from July 2007 to April 2010 to our hospital, were enrolled into the study. The clinical data of all subjects were analyzed retrospectively. Results The incidence of anemia in 370 patients was 55.67% (206/370). Among all 206 anemia cases,129 cases had mild anaemia and 77 cases had middle to severe anaemia. In the last group ( n = 77 ), we found significant decrease in serum iron level ( [ 8. 37 ± 6. 09 ] μmol/L) and increase in serum ferritin level ( [ 474. 57 ± 327. 58 ] μg/L); and the correlation between serum iron and the anemia degree( Ps < 0. 05 ). However, we found no significant differences of serum ferritin level between the groups with different degree of anemia(P >0. 05). Among all 206 anemia cases ,187(90. 77% ) patients had a low level of serum PA, but no relationship between the degree of anemia and the drop of serum PA ( P > 0. 05 ). Conclusion The anemia was very popular in cancer patients, which had correlation with iron metabolism situation but not PA.

ABSTRACT

PURPOSE: To evaluate the usefulness of serum prealbumin concentration for the nutritional assessment of acute diarrhea. METHODS: From February 1997 to August 1997, ninety patients with acute diarrhea(age, 4-24 months) were randomly assigned into two groups on the basis of their intakes for 2 days prior to admission : liquid diet group(N=56, clear liquid and/or oral electrolyte solutions), and formula and soft diet group(N=34). Serum prealbumin concentrations were measured by rate nephelometric microassay. RESULTS: Serum prealbumin concentration in the liquid diet group with acute diarrhea was 13.1+/-4.5mg/dL which was significantly lower than 15.5+/-5.0mg/dL of the formula and soft diet group (P<0.05) and 17.0+/-4.0mg/dL of the control group(P<0.05). Serum prealbumin concentration was less influenced by dehydration than serum albumin concentration. The duration of diarrhea and admission were not significantly shorter in the liquid diet group than in the formula and soft diet group. CONCLUSION: Serum prealbumin concentrations in patients with acute diarrhea reflected their current nutritional status and was less influenced by dehydration than serum albumin concentrations. Serum prealbumin concentration might be used to monitor short-term nutritional deprivation and the adequacy of dietary supply.
