ABSTRACT

Data were collected from national health statistics reports and grassroots health comprehensive reform monitoring reports to survey the workload, service efficiency and regional medical coordination in community health service centers in Shanghai Pudong New Area.The efficiency of basic medical services and basic public health services in Pudong New Area increased yearly from 2010 to 2016. The improved capacity of community health service institutions included: mutual recognition of investigation with superior hospitals, the sharing of test results and test equipment in the region, and achieving remote consultation with superior hospitals. With the continuous improvement, more medical services can be provided in community health centers in Shanghai Pudong New Area, resulting in the increased workload of medical staff. The coordination of regional institutions further promotes the implementation of tiered medical service system.

ABSTRACT

Objective To analyze problems in the development of medical service at private hospitals, and provide references for promoting the development of these hospitals and implementing policies for encouraging this sector′s growth. Methods The data were collected from the statistical yearbook published by the National Health Commission of the People′s Republic of China, and descriptive analysis method was used to analyze the medical service workload and medical service efficiency of private hospitals in China since the new healthcare reform. Results Private hospitals accounted for 56. 39% of the total number of hospitals in China, accounting for 21. 69% of beds by the end of 2016. From the perspective of medical service workload, the service volume of these hospitals had increased yet at a small pace. The number of patients and inpatients received accounted for 12. 90% and 15. 84% respectively of all the hospitals in China. From the point of service efficiency, by the end of 2016, the bed utilization ratio was 62. 8%, the daily medical visits to their doctors were 5. 5 persons-times, and the daily number of hospitalized beds per doctor was 2. 2 beds. These numbers lag far behind public hospitals. The development of private hospitals varied with regions in imbalance. Conclusions Improving the social image, attracting talents and improving service quality are key to improving the medical service capacity of private hospitals.

ABSTRACT

Objective To screen TCM dominant diseases from service efficiency, service quality and security by taking a three-A-grade TCM hospital as example.Methods According to the diagnosis related data of the TCM hospital, the common diseases in major disease categories (MDC) were screened out. Average cost of hospitalization, average hospitalization days, antibiotic use rates, blood use rates, and mortality rates were compared with the average level of tertiary general hospitals.Results Totally 27 common diseases were screened out. Three diseases had advantages in terms of service efficiency, security and service quality; 14 diseases had security advantage; 13 diseases had advantage in service quality.Conclusion Compared with three-A-grade general hospitals, most of the common diseases in the hospital has obvious advantages in security and service quality, but the average length of hospitalization in the hospital is longer, and the average cost of most of the common diseases is higher than the general hospital, without advantages in service efficiency.

ABSTRACT

Objective:To analyze the efficiency of 77 public hospitals' medical services and influencing factors in Tianjin.Methods:Using DEA method to measure the hospitals' medical service efficiency,establishing Tobit regression model to analyze the influencing factors of public hospitals medical service efficiency.Results:The overall efficiency of Tianjin public hospitals was low,most hospitals were not reasonable in the size,the significant influencing factors of Tianjin public hospitals' medical service efficiency including the per capita physician outpatient,the proportion of senior technical staffs,hospital gearing ratio,drug-income ratio and average fee per patient.Conclusion:Increasing the per capita physician outpatient and the proportion of senior technical staff could significantly enhance the overall technical efficiency,while decreasing the gearing ratio and the average fee of per patient were also beneficial to improve hospital efficiency.

ABSTRACT

Objective To analyze and compare the capacity and efficiency of county-level hospitals′medical service by using the diagnosis related groups ( DRGs ) method. Methods The homepage data of discharged inpatients from seven county-level hospitals in Wenzhou region in 2013 - 2015 period were analyzed, for measurement of the medical service capacity changes of such hospitals using the number of DRGs, total multiplicity of weight, and CMI value, and that of their medical service efficiency changes using expense consumption index and time consumption index. Results The study found in the seven hospitals 8. 49% increase of the total number of DRGs, 17. 34% increase of total multiplicity of weight, and 5. 06%increase of CMI value, with unchanged expense consumption index and 9. 82% decrease of the time consumption index. These facts evidenced enhancements of these hospitals in both service capacity and service efficiency in general. Conclusions DRGs as tools prove useful objectively and scientifically. Policies of Two emphases at primary ends and two enhancements have been implemented desirably.

Objective To investigate the application and feasibility of the single bed work efficiency in the evaluation of hospital beds efficiency,to establish the hospital beds efficiency evaluation model,in order to provide the basis for scientific and effective utilization and evaluation of beds.Methods Proposing the concept of single bed work efficiency,establishing a new evaluation model of bed efficiency,and analyzing the utilization of hospital beds in 2015.Results Single bed work efficiency is supenor to other indexes in evaluating the Utilization efficiency of hospital beds,and the new bed efficiency evaluation model is more objective and accurate.Conclusion The evaluation model of hospital beds utilization efficiency based on the single bed work efficiency is more comparable and operable,which can be widely used in hos pital delicacy management.

The authors probed into the application and feasibility of DRGs in improving the efficiency and quality of hospital medical services,in order to establish a lean management mode based on DRGs.DRGs are seen as different from other means of management,with such evaluation of medical services being more comparable and operable,and the evaluation results more trustworthy.Application of such practice can obviously improve the efficiency and quality of medical services,making it widely applicable to hospital′s lean management.

ABSTRACT

This article took the research work of 16 national clinical research base construction units as its background in order to dig the connotation of health service efficiency . Through data envelopment analysis , a comprehensive exposition was given on the inputs and outputs relation of medical and health service efficiency in order to propose appropriate policy recommendations. The study was guided by the scientific development concept in order to gradually improve the efficiency of health services and achieve a comprehensive , coordinat-ed and sustainable development of the hospital .
