ABSTRACT

In order to expand the breadth and depth of pharmaceutical services, in March 2022, a tertiary hospital opened a physician-pharmacist joint clinic based on clinical specialty clinics. The hospital formulated a fixed outpatient scheduling system, clarified service targets, established outpatient treatment processes and quality management systems, and standardized pharmacist communication models, to provide patients with " one-stop" standardized pharmaceutical services. As of December 2022, the pharmaceutical joint outpatient service had opened more than 100 consultations and served 1 709 patients. This practice provided reference for promoting the high-quality development of pharmaceutical services in medical institutions in China.

ABSTRACT

OBJECTIVE:To provide ideas for the development of electronic prescription (e-prescription)service mode under the background of “Internet+”. METHODS :Retrieving official websites and related online resources of the national and provincial governments,health committees ,medical products administration ,and medical security bureaus (up to Jun. 2020),relevant policies,supporting documents and implementation status of e-prescription service mode were sorted out and analyzed ,and the suggestions were put forward. RESULTS :Current e-prescription service modes included four modes ,such as pharmaceutical retail enterprise e-prescription service mode ,medical institution e-prescription information sharing service mode ,internet hospital e-prescription service mode ,and medical e-commerce e-prescription service mode. The pilot of e-prescription service mode of drug retail enterprises covered the widest range of provinces and cities ,but the participation of health departments and medical institutions was insufficient ,and the pattern of comprehensive power had not been formed. The medical institutions e-prescription information sharing service mode was mainly composed of three typical examples ,namely“Wuzhou mode ”,“Fuzhou mode ”and “Gansu mode ”. However ,it had not been unified in terms of technical standards ,access conditions ,exit mechanism and execution process,etc. At present ,this model was still in a small scope of exploration and pilot stage. The internet hospital e-prescription service mode was limited by the popularization of internet hospital medical insurance payment and the improvement of corresponding rules and regulations ,which was still at the stage of cautious pilot exploration. Medical e-commerce e-prescription service model was limited by the types of drugs within the legal service scope. Meanwhile ,the industry standards and regulations of e-prescription service were in the absence at present. CONCLUSIONS :As an innovative service mode ,the e-prescription service mode meets the real needs of the pharmaceutical retail industries ,medical institutions and patients. Each pilot mode conforms to the development trend of “Internet+”and shows a strong development momentum ,but overall it is still in the early stage of active exploration. It is urgent to further explore and research around the leading departments ,stakeholders and prescription transfer procedures.

ABSTRACT

Objective To explore the progress and prospect of preventive, predictive, personalized and participatory (4P) medicine in China in order to promote the “4P” medicine and provide reference for the government and medical institutions to strengthen health management. Methods An in-depth analysis and review of the “4P” medical service model was conducted through literature review. The prospect of the future development of the “4P” medicine was discussed. Results In recent years, with the advancement of human health concepts and the completion of the genome project, the human healthcare model has been gradually shifting to the “4P” medical service model, namely preventive-predictive-personalized-participatory integrated medicine. It can be seen that modern medical model has been in the process of continuous transformation, which is more human-oriented and emphasizes people’s initiative. Conclusions With the widespread and understanding of the “4P” medicine among healthcare workers, the value of the “4P” medicine in public health and clinical practice has been continuously proven.

ABSTRACT

Multi-disciplinary team(MDT) is a new mode of medical diagnosis and treatment service. Multidisciplinary discussion could provide patients with a scientific, standardized and effective individualized diagnosis and treatment plan to avoid over-treatment. This article is based on the application experience of the MDT information management platform designed by the First Affiliated Hospital of Zhejiang University during the past five years. It discussed how to build a MDT management system which could fit the medical environment of large general hospitals in China. The MDT management system simplifies the MDT process, improves the efficiency of MDT work, and enhances the overall medical quality of hospitals. Meanwhile, it also contributes to strengthen the disciplinary collaboration in such aspects as disease diagnosis and treatment, personnel training, and scientific research innovation, ultimately forming a new multi-disciplinary collaboration system in hospitals.

ABSTRACT

The issue on the acupuncture clinical service mode has been early proposed by the scholars. The acupuncture clinical service mode is still existed as the department of acupuncture in TCM hospital nowadays. But this pattern has brought the inconvenience and issues between physicians and patients. In the study, through reviewing briefly the history of acupuncture hospital since the foundation of the People's Republic of China, the authors explained why taking the Hospital of Acupuncture and Moxibustion, China Academy of Chinese Medical Sciences (CACMS) as the object. By integrating the relevant achievements of the scholars on the study of acupuncture hospital, the historical development of acupuncture-moxibustion hospital was reviewed. By comparing with the development mode of TCM hospital, the differences were generalized between the two hospitals in terms of the purpose of hospital establishment, the development mode of treatment, education and research, modernization mode, the relationship with western medicine and the hospital scale. It was analyzed by the authors that the development characteristics of acupuncture-moxibustion hospital were determined by the accidental and the inevitable factors. By the comprehensive analysis, the enlightenments to the future development of acupuncture-moxibustion hospital were explored.

Repeat prescription is adopted in Singapore as a convenient measure for patients with chronic diseases.On the one hand, it can avoid patients''inconvenience caused by frequent visits to hospitals.On the other hand, it reduces the waste of medical resources.In recent years, Singapore has also been using modern technology and measures to expand the medication refill service actively in order to enhance work efficiency, reduce waiting time and promote drug safety, thus to improve patients'' satisfaction.With the increasing medical pressure and the deepening of medical reform, the repeat prescription model in Singapore shows positive enlightenment to China.

ABSTRACT

KY3H is a traditional Chinese medicine health care service mode based on the existing problem,focusing on an interconnected and continuous integration of four key technological innovation,which promotes the service model transformation from passive "doctor-searching" to "asking yourself ".There are four features on KY3H mode.First,based on traditional Chinese medicine meridian and viscera theory,it can accurately identify the health status and take risk assessment of individuals through digital quantitative model.Then,the mode realizes the personalized health status recognition and precisious intervention through integrated technology products of dynamic monitoring and identification,assessment and intervention with independent intellectual property rights.The third is to improve the intelligence,realtime and accessibility of KY3H health care services through developing the Chinese-western medicine informationalized software system about health status identification,assessment and intervention.The last one is formulating traditional Chinese medicine health care standards,industry standards and service package design,which drive KY3H health service model spreading effectively and practical applying,achieving remarkable social and economic benefits.

ABSTRACT

Objective To analyze the service mode of electric manned climbing machine,to develop services of high-price electric assis-tive device(AD).Methods March to April,2013,and December,2014 to January,2015,33 stakeholders were interviewed about the service policy,utilization,the history of service modes and service elements of electric manned climbing machine 2012 to 2014.5W2H method was used to analyze the modes of service in term of resource,management,service,assurance,to study their pros and cons.Results There were three modes of service in climbing machine:subsidy adaptation,volunteer service from community and government purchasing service.The basic service included receiving demand,audit,service and evaluation,that is being adjusted to improve the service capabilities.Conclusion No service mode is the most optimal,but feasible.

Based on the advanced experience of foreign countries,com-bined with Chinese national conditions,ChineseMedical support integrationservice mode should adhere to the leading position of the government,introduce market competition mechanism,formulate strictMedical support integrationservices access stan-dards,and actively cultivate professional nursing staff,so as to achieve long-term development.

The first family doctor studio was established at 2011 in Shanghai,afterwards a wide variety of family doctor studios were set up over the country.Family doctor studio has been become a significant form and carrier to promote the family doctor service.However,there are still lacks of unified standards and perfect mechanisms for its operation and evaluation.To adapt the reform of community health service,it is urgent to standardize the family doctor studio,to promote the development of the community health service and the reform of the family doctor system.Based on Zhoujiaqiao mode,Changning District has kept exploring the operation mechanism,standardizing the management and service mode,and promoting brand building of the family doctor studio since 2012.The service mode and operation mechanism of family doctor studio with Changning characteristics has gradually formed.This article summarizes the construction,development and improvement of the family doctor studio.

ABSTRACT

Objective To construct integrated service mode of medical care in maternity department and discuss its effect. Methods 160 participants who had been registered and taken periodic inspection in our hospital were divided into two groups: observation group with 80 cases from June to December, 2014 while control group with 80 cases from January to May, 2015. Two groups were offered routine prenatal examination and pregnancy guidance, but observation group was also offered additional integrated service mode (including early pregnant assessment, second-trimester educations, one-to-one late pregnant directives, exchanges during the duration of hospital stay and sustained professional postpartum care). Two groups in mental state, delivery outcome, and pregnancy weight control and satisfaction rate were compared. Results Anxiety and depression score in observation group were (41.89 ± 7.50) points and (42.40 ± 7.40) points, and that in control group were (57.55 ± 9.68) points and (55.00±9.35) points, differences had statistical significance (t=-11.44,-9.42,P<0.05);vaginal delivery cases (48/80) in observation group were higher than control group (33/80) , differences had statistical significance (χ 2=5.63,P < 0.05). Body weight of antepartum and gestation were (67.80 ± 8.18) kg and (12.96 ± 3.44) kg in observation group, and those in control group were (70.98 ± 8.82) kg and (14.28 ± 3.68) kg,differences had statistical significance (t=-2.36,-2.35, P<0.05). Satisfaction rate of observation group was 90.00%(72/80), which was higher than 78.75%(63/80) of control group (U=592.00, P<0.01). Conclusions Integrated service mode of medical care can improve pregnant women′s negative mental state, control pregnancy weight, promote natural labour and enhance satisfaction rate effectively.

ABSTRACT

This paper compared the practice and service mode of general practitioners ( GPs) from a selection of typical countries and regions and discovered that GPs from most of the countries were self-employed, with various forms of employment, freedom to practice and work in different institutions;fixed practices were supplemented, and service types were transformed from individual services to the multi-disciplinary team of services. Result: GPs’ free-dom to practice in different institutions requires a certain social environment and circumstances. The multi-disciplina-ry team of practices has become the development direction for the service modes of GPs. GPs have played different roles in establishing the referral system in the health systems of different countries. Consequently, the paper suggests that establishing a diverse and relaxed practice environment to provide conditions for the transformation of the service mode of GPs. The construction of a multi-disciplinary team to promote changes in community service modes was must be strengthened. Health insurance regulations should be encouraged to establish the gatekeeper system and realize a grading of systems. Social capital should be encouraged to organize general medical services to promote the develop-ment of the health care industry.

Objective:To explore the clinical pharmaceutical care for cancer pain management. Methods: The intervention time of clinical pharmacists was determined. A comprehensive evaluation of cancer pain, physical function and life quality of patients was performed, the compliance of patients was scored, and individualized pharmaceutical service was carried out,consultation and sugges-tion on analgesics and adjuvant drugs were provided for doctors, and finally, the effect of pharmaceutical care was assessed. Results:Clinical pharmaceutical care could promote the rational drug use and improve the medical quality. Conclusion:Through the research of clinical pharmaceutical care for pain management,clinical pharmacists can play a practical and effective role in the pain treatment and management.

ABSTRACT

Objective: To explore the utilization and effects of informatization in perfecting the community health service mode. Method:Through purposive sampling, 12 community health service centers in Beijing, Shang-hai, Chengdu, and Zhengzhou were selected for site investigation. The heads of the health administrative department and community health service centers were interviewed, with a focus on the construction and utilization of the local community health informatization and a discussion of the role of informatization in perfecting the community health service mode. Results:The information systems can be divided into core systems, core auxiliary systems, and periph-eral support systems. Informatization provides strong support to perfecting the community health service mode in opti-mizing the service process, improving work efficiency, strengthening institutional cooperation, promoting quality regu-lation, optimizing performance appraisal, and strengthening doctor-patient interaction. Conclusion: Informatization plays a key role in perfecting the community health service mode. Recommendations were proposed to strengthen community health informatization construction and utilization:optimizing service processes, promoting the efficient co-ordination of safe health services, optimizing performance appraisals, and strengthening doctor-patient interaction.

ABSTRACT

Objective To explore the effects of high quality nursing service mode on nursing of patients with artificial joint replacement.Methods 82 patients from January to December 2010 were set as the control group,another 82 patients from January to November 2011 were named as the observation group.The observation group was given high quality nursing service mode,while the control group received conventional nursing.The satisfaction degree of patients to nursing,average hospitalization time and cost and incidence of complications were compared between two groups.Results The satisfaction degree of the observation group was 98.78%,significantly higher than 93.90% in the control group.The average hospitalization time was(14.68±5.88)days in the observation group,shorter than(19.22±6.78)days of the control group.The average hospitalization expense in the observation group was (25 665±528)yuan,less than (30 866± 732)yuan of the control group.The complication rate in the observation group was 21.95%,significantly lower than 46.34% of the control group.Conclusions Targeted high quality nursing service mode can effectively improve the quality of nursing,decrease the incidence of complications,reduce the patient's hospitalization time and cost,as well as improve the patients' satisfaction degree.
