ABSTRACT

Xiao Chaihu Decoction combined with Maxing Shigan Decoction is a classic herbal formula. All of them are derived from Treatise on Cold Damage(Shang Han Lun) by ZHANG Zhong-jing. This combination has the effects of harmonizing lesser yang, relieving exterior syndrome, clearing lung heat, and relieving panting. It is mainly used for treating the disease involving the triple-Yang combination of diseases and accumulation of pathogenic heat in the lung. Xiao Chaihu Decoction combined with Maxing Shigan Decoction is a classic combination for the treatment of exogenous diseases involving the triple-Yang combination. They are commonly used in exogenous diseases, especially in the north of China. This combination is also the main treatment strategy for coronavirus disease 2019(COVID-19) accompanied by fever and cough. Maxing Shigan Decoction is a classical herbal formula for treating the syndrome of phlegm-heat obstructing the lung. "Dyspnea after sweating" suggests the accumulation of pathogenic heat in the lung. Patients with mild symptoms may develop cough and asthma along with forehead sweating, and those in critical severe may develop whole-body sweating, especially the front chest. Modern medicine believes that the above situation is related to lung infection. "Mild fever" refers to syndromes rather than pathogenesis. It does not mean that the heat syndrome is not heavy, instead, it suggests that severe heat and inflammation have occurred. The indications of Xiao Chaihu Decoction combined with Maxing Shigan Decoction are as follows.(1) In terms of diseases, it is suitable for the treatment of viral pneumonia, bronchopneumonia, lobar pneumonia, mycoplasma pneumonia, COVID-19 infection, measles with pneumonia, severe acute respiratory syndrome(SARS), avian influenza, H1N1 influenza, chronic obstructive pulmonary disease with acute exacerbation, pertussis, and other influenza and pneumonia.(2) In terms of syndromes, it can be used for the syndromes of bitter mouth, dry pharynx, vertigo, loss of appetite, vexation, vomiting, and fullness and discomfort in the chest and hypochondrium. It can also be used to treat alternate attacks of chill and fever and different degrees of fever, as well as chest tightness, cough, asthma, expectoration, dry mouth, wanting cold drinks, feeling agitated, sweating, yellow urine, dry stool, red tongue, yellow or white fur, and floating, smooth, and powerful pulse, especially the right wrist pulse.

ABSTRACT

Wumei Pill is a classic insect repellent prescription, which is used for ascaris lumbricoides and diarrhea. It is considered to be a representative prescription for the treatment of simultaneous occurrence of cold and heat syndromes. Its prescription is mainly composed of sour drugs ( Mume Fructus and bitter wine) and pungent drugs ( Aconiti Lateralis Radix Praeparataaconite, Cinnamomi Ramulus, Sichuan pepper, Angelicae Sinensis Radix, Zingiberis Rhizoma, and Asari Radix et Rhizoma), supplemented by bitter drugs ( Coptidis Rhizoma and Phellodendri Chinensis Cortex) and sweet drugs ( Ginseng Radix et Rhizoma and honey). From the point of view of "Tang-Ye-Jing-Fa Map", the role of liver is to disperse, whose deficiency leads to limb syncope, and excess leads to the full of hypochondrium and abdominal pain. The pungent herbs can tonify the liver-deficiency, the sour herbs can dispel the liver-excess, and sweet herbs can relive them both. The role of spleen is to moderate, whose deficiency leads to fatigue and weakness, and excess leads to the vomiting and diarrhea. The sweet herbs can tonify the spleen-deficiency, the pungent herbs can dispel the spleen-excess, and bitter herbs can relive them both. Therefore, the function of Wumei Pills is located in the liver and spleen, giving consideration to the heart and lung. It is used for the deficiency and excess mixed syndrome of liver and spleen, which is mainly characterized by chest and flank abdominal pain, limb chills, diarrhea, muscle and pulse contracture and the symptoms of the parts along the liver meridian. It is commonly used in the treatment of ascaris, diarrhea, impotence, depression. At the same time, it can also be used for the treatment of diseases about heart and lung, and cough, anxiety and other diseases.

ABSTRACT

Objective@#To construct symptom-formula-herb heterogeneous graphs structured Treatise on Febrile Diseases (Shang Han Lun,《伤寒论》) dataset and explore an optimal learning method represented with node attributes based on graph convolutional network (GCN).@*Methods@#Clauses that contain symptoms, formulas, and herbs were abstracted from Treatise on Febrile Diseases to construct symptom-formula-herb heterogeneous graphs, which were used to propose a node representation learning method based on GCN − the Traditional Chinese Medicine Graph Convolution Network (TCM-GCN). The symptom-formula, symptom-herb, and formula-herb heterogeneous graphs were processed with the TCM-GCN to realize high-order propagating message passing and neighbor aggregation to obtain new node representation attributes, and thus acquiring the nodes’ sum-aggregations of symptoms, formulas, and herbs to lay a foundation for the downstream tasks of the prediction models.@*Results@#Comparisons among the node representations with multi-hot encoding, non-fusion encoding, and fusion encoding showed that the Precision@10, Recall@10, and F1-score@10 of the fusion encoding were 9.77%, 6.65%, and 8.30%, respectively, higher than those of the non-fusion encoding in the prediction studies of the model.@*Conclusion@#Node representations by fusion encoding achieved comparatively ideal results, indicating the TCM-GCN is effective in realizing node-level representations of heterogeneous graph structured Treatise on Febrile Diseases dataset and is able to elevate the performance of the downstream tasks of the diagnosis model.

ABSTRACT

Professor FU Ru-mei uses classical prescriptions to treat lung diseases, such as cough, asthma, the lung distension. Her treatment attaches importance to yin and yang, and emphasizes the six meridians. She pays attention to pulse and syndrome, and distinguishes evil qi and healthy qi and the primary and secondary symptoms. Her medication does not stick to classical cases. She considers pathogenesis carefully, gives and combines the prescriptions according to the symptoms. She usually combines classical prescriptions, seasonal prescriptions and local prescriptions, to make better clinical efficacy.

ABSTRACT

It has always been controversial about the theory of "the time of six meridians diseases tending to be cured" in Shang Han Lun. Many doctors pay more attention to the time of disease remission, while neglecting its appearance, aggravation and the meaning of treatment according to the rule of time. Based on the theory of "the time of six meridians diseases tending to be cured", this article made diagnosis and treatment of three clinical medical records about cough, mouth-dry and bitterness, and flushing from the time of Shaoyang, Taiyang, and Yangming meridians tending to be cured, and analyzed the clinical syndrome differentiation and characteristics of the theory of"the time of six meridians diseases tending to be cured".

ABSTRACT

Literature of TCM contains abundant information of Chinese medicine. Traditional Chinese medicine information is characterized by its high degree of knowledge-intensive and complex inter-knowledge, so it is difficult to integrate and share knowledge. TheShanghanlun is one of the Chinese medical classics, and it includes the rules and methods of syndrome differentiation and treatment and it showed good effects in the clinical application. However, it is difficult to master for beginners. As a kind of knowledge organization tool, topic maps can flexibly define the relationship between knowledge, which realizes the visualization of knowledge network, and integrates and shares the distributed knowledge. This paper focused on the current situation of topic maps researches. The topic maps was used to organize the original content ofShanghanlun, construct the concept model of it and explore the specific application of topic maps technology in the field of traditional Chinese medicine.

ABSTRACT

Sleep disorder is a common complication of dialysis patients, seriously affecting the life quality and prognosis of the patients. How to improve the quality of sleep of dialysis patients, their quality of life and survival rate, is still a major clinical problem. At present, the pathogenesis of dialysis patients with sleep disorders is not clear. Western medicine treatment is sedative hypnotic drugs, with obvious adverse reactions, and long-term use will reduce the efficacy and easy to produce dependencies. The effects of TCM on improving sleep disorders and life quality of dialysis patients is worth expectation. This article introduced the experience in using classic prescriptions and dialectical treatment for dialysis sleep disorder, with significant clinical efficacy.

ABSTRACT

Objective To provide references for clinical medication through the analysis on the dose-response relationship and compatibility medication rules of Cinnamomi Ramulus. Methods It systematically arranged the provisions containing Cinnamomi Ramulus inShang Han Lun. Relationship between the dose of Cinnamomi Ramulus and the related factors were analyzed by single factor logistic regression analysis and binary correlation analysis by SPSS20.0 statistical software. Results After screening, 40 prescriptions including Cinnamomi Ramulus were selected, accounting for 35.40% in the total prescriptions ofShang Han Lun. The taken dose of every time of Cinnamomi Ramulus had a close relationship with the number of medicine and water consumption, with statistical significance (P=0.000,P=0.004). However, it had no relationship with residual water, water consumption per time and use frequency, without statistical significance (P>0.05). Single factor logistic regression analysis showed whether Cinnamomi Ramulus as a mainmedicine had the close relationship with the taken dose of every time of Cinnamomi Ramulus and use frequency (P=0.008,P=0.043). When Cinnamomi Ramulus and Glycyrrhizae Radix et Rhizoma Praeparata cum Mella were in compatibility, compared with when Cinnamomi Ramulus was not the main medicine, the taken dose of every time of Cinnamomi Ramulus as the main medicine in the group was more frequently used, with statistical significance (P=0.046); however, the number of medicine was statistically lower than when Cinnamomi Ramulus was not main medicine (P=0.043). When Cinnamomi Ramulus and Ephedrae Herba were in compatibility, there was no statistical difference in the taken dose of every time of Cinnamomi Ramulus or the number of medicine (P>0.05). When Cinnamomi Ramulus was the main medicine, it was not in compatibility with Poria or Bupleuri Radix.ConclusionFrom the dose-response relationship, it can be found that the use and compatibility of Cinnamomi Ramulus inShang Han Lun are strict and flexible, and dose law can provide a basis for clinical application.

Methods It systematically arranged the provisions containing Glycyrrhizae Radix et Rhizoma in Shang Han Lun. It statistically analyzed the relationship among Glycyrrhizae Radix et Rhizoma and other various factors by using SPSS 20.0. Results After screening, 70 prescriptions were included, among which 2 (2.86%) contained raw Glycyrrhizae Radix et Rhizoma and 68 (97.14%) contained Glycyrrhizae Radix et Rhizoma Praeparata cum Melia. The taken dose of every time had a close relationship with the number of medicine, water consumption, residual water, water consumption per time and frequency of use, which had a statistical significance (P<0.01). Univariate logistic regression analysis showed whether Glycyrrhizae Radix et Rhizoma as a monarch medicine had the close relationship with the number of medicine (P=0.029). In 48(73.85%) prescriptions contained Glycyrrhizae Radix et Rhizoma Praeparata cum Melia as non-monarch medicine, the taken dose of every time and the number of drugs had statistical differences with that in the prescriptions which as monarch medicine (P=0.000). When Glycyrrhizae Radix et Rhizoma Praeparata cum Melia was in compatibility with other medicine, its dose had a certain rules with the number of medicine. When it was the basic medicine, there was no compatibility with Zingiberis Rhizoma Recens, Jujubae Fructus, and Ginseng Radix et Rhizoma. Conclusion It can be found from the dose-response relationship that Glycyrrhizae Radix et Rhizoma has a widely dose and combination efficacy in different prescriptions. Its flexibility, diversity and scientific nature provide the solid basis for clinical application.

ABSTRACT

Objective] The explanations of the six meridians theory in the book Shanghan Lun were always divergent, most of them explained it with medical theory of later ages. However, il ustrating it from the philosophic perspective was not commonly seen. Yi Jing, the book of changes, was the origin of traditional Chinese philosophy, and the six meridian system, which included three yin and three yang, was just derived from the four xiang(a traditional Chinese concept which refers to taiyang, shaoyin, shaoyang, taiyin). [Method] Through the eight trigrams and the images were analyzed with the Treatise on Six Classicsstate gasification combined with analysis Zhongjing Six Classics theoretical system and the principle of prescription. [Result] Found that eight trigrams can be good images of the corresponding six by the system, the difficult provisions in Tretise on Febrile Diseases were wel explained. [Conclusion]Through combining the images of Bagua(eight trigrams) in Yi Jing with six meridians in Shanghan Lun,il ustrating six meridians from philosophic aspect, and explaining the six meridians theory with philosophical ideas before Zhongjing ’s(the writer of Shanghan Lun) age, in order to provide new ways of thinking in comprehending the six meridians system.

ABSTRACT

We sought a connection between the half-exterior half-interior symptoms (hanpyo hanri sho), mentioned in the classic Chinese Shang han lun text and used in Kampo medicine, and embryology. An agreement between these shao yang (lesser yang) symptoms, and embryonic brachial arch domains was found. A part of the brachial arches strongly agrees with the ruling domain from the trigeminal nerve to the vagal nerve, and a connection was also seen to the vestibulocochlear nerve and partly to the accessory nerve. Thus we believe that half-exterior half-interior symptoms are a condition which relate back to part of the brachial arches, and are a result of so-called Heat reaction. Shang han lun Chinese medicine and embryology are two completely different fields. However, we consider an embryological view of nerve pathways to be helpful in understanding half-exterior half-interior symptoms, at least to a certain degree.

ABSTRACT

The adequacy of the Maoto decoction procedure described in the Shokan-ron text was investigated using the inhibitory activity of blended Ephedra herb on amylase enzyme. This enzyme is closely related to stress, and used clinically as an indicator of stress in humans.α-amylase in isolated mouse plasma was used in the experiment, and the enzymatic activity was measured according to the Caraway method. Two different decoction methods: the general process used today, and that described in the Shokan-ron (Ephedra herb alone is firstly infused, then another 3 crude drugs are added and it is further infused), were compared for yield of the extracts, inhibitory actions of the extracts, their crude polysaccharide fraction (one of the active components), and remaining fractions contained within each extract. The inhibitory activity of the extract was clearly promoted by the latter, ancient decoction method, compared with the former. Hence, the decoction method described in the Shokan-ron is presumed to have the aim of reducing stress to the body caused by Maoto itself, and mitigating the side effects of its component Ephedra herb. This biologically qualitative approach, reflected over the entire decoction process, is different from the simply physicochemically analytical approach which concerns itself merely with particular constituents, such as alkaloids. So, it may be beneficial to scientifically elucidate the significance of this decoction and its ingestion methods, as described by the ancient Shokan-ron and Kinki-yoryaku texts.

ABSTRACT

The form of modern Kampo Medicine is made after the influence of the schools at past time. This medicine was at first imitation of TCM in China and slowly had acquired the own Japanese form. At 16 century, Dosan Manase introduced the system of TCM from Ming-China to Japan successfully and made a great school called Goei-School. Late stage of 17 century, the influence of “Shang han Lun” research boom in China came to Japan and the new school called Koho-school which was based on this Classic has established. Especially Todo Yoshimasu researched the prescriptions in this book and invented new system named “Ho sho so tai”. After then, doctors had to compromise the thinking of both school. The form of eclectic school was so various that the doctors made individual medicines. After the Meiji Restoration, Kampo Medicine was out of political system but soon obtained revival. The several great doctors of Kampo Medicine have written “Practice of Kampo Medicine” at 1941 which made the base of the modern Kampo Medicine. Current Kampo Medicine adopted a lot of modern medical researches and there are new schools which are different from the past time.
