ABSTRACT

Fundamento: el carcinoma basocelular es un tumor maligno muy frecuente en el ser humano. Se considera invasivo de forma local, además de agresivo y destructivo. Objetivo: mostrar el comportamiento de los pacientes con carcinoma basocelular diagnosticados por dermatoscopia en consulta de dermatología del Hospital Universitario Miguel Enríquez, entre junio de 2020 y mayo de 2021. Método: se realizó un estudio descriptivo, de corte transversal, para mostrar el comportamiento de los pacientes con carcinoma basocelular que asistieron a la consulta de dermatología del Hospital Universitario Miguel Enríquez en el periodo de junio 2020 a mayo 2021. El universo de estudio se conformó por los pacientes mayores de 18 años que presentaron una o varias lesiones presuntivas de carcinoma basocelular menores de 3 cm de diámetro. Se utilizaron las variables: edad, sexo, fototipo de piel, formas clínicas, patrones dermatoscópicos, topografía de la lesión, ocupación relacionada o no con la exposición solar. Se trabajó con frecuencias absolutas, porcentaje, media, desviación estándar, valores máximos y mínimos, la razón, pruebas de ji-cuadrado y t de student, con significación p≤0,05. Resultados: predominó en ancianos (66,4±14,9 años), en hombres (54,1 %), con fototipo 2 (57,8 %), y en nariz el 33,9 %. La forma nodular fue más frecuente (41,3 %), ocupación expuesta al sol (66,9 %), tamaño promedio de la lesión 1,4 cm. La dermatoscopia mostró predominio de los vasos arborizantes (73,4 %). Por histología se confirmó el 90,8 % de los casos. Conclusiones: predominaron entre los pacientes los ancianos, con fototipo 2. La forma nodular fue más frecuente, el tamaño promedio de la lesión fue de 1,4 cm. La dermatoscopia mostró predominio de los vasos arborizantes. La dermatoscopía resultó ser una herramienta necesaria para el uso sistemático en dermatología.


Foundation: basal cell carcinoma is a very common malignant tumor in humans. It is considered locally invasive, also aggressive and destructive. Objective: to show the behavior of patients with basal cell carcinoma diagnosed by dermoscopy in dermatology clinic of the Miguel Enríquez University Hospital, between June 2020 and May 2021. Method: a descriptive, cross-sectional study was carried out to show the behavior of patients with basal cell carcinoma who attended the dermatology consultation at the Miguel Enríquez University Hospital from June 2020 to May 2021. The universe of study was made up of patients older than 18 years who presented one or more presumptive lesions of basal cell carcinoma less than 3 cm in diameter. The variables used were: age, sex, skin phototype, clinical forms, dermoscopic patterns, topography of the lesion, occupation related or not to sun exposure. We worked with absolute frequencies, percentage, mean, standard deviation, maximum and minimum values, the ratio, chi-square and student's t tests, with significance p≤0.05. Results: it prevailed in the elderly (66.4±14.9 years), in men (54.1 %), with phototype 2 (57.8 %), and in the nose 33.9 %. The nodular form was more frequent (41.3 %), occupation exposed to the sun (66.9 %), average size of the lesion 1.4 cm. Dermoscopy showed a predominance of arborizing vessels (73.4 %). By histology, 90.8 % of the cases were confirmed. Conclusions: the elderly prevailed among the patients, with phototype 2. The nodular form was more frequent, the average size of the lesion was 1.4 cm. Dermoscopy showed a predominance of arborizing vessels. Dermoscopy turned out to be a necessary tool for systematic use in dermatology.

ABSTRACT

En mayo de 2022 se reportó un aumento de casos de viruela símica (mpox en inglés) en el mundo, cuyo comportamiento epidemiológico y clínico, particularmente en pacientes con infección por VIH, condujo a la declaración del brote de mpox 2022 como emergencia de salud pública internacional. Se presenta el caso de un paciente con infección por VIH que cursó con mpox grave y fulminante, con placas necróticas en párpado y membrana inflamatoria sobre la superficie ocular; mucosa oral con lesiones blanquecinas y úlceras en lengua; induración de tejidos blandos y lesiones necróticas en los pies. Tras múltiples complicaciones, se convirtió en la primera víctima fatal reportada en Ecuador en 2022. En pacientes con infección por VIH, mpox puede presentarse como un agente oportunista, causando lesiones cutáneas graves, con o sin manifestaciones sistémicas.


In May 2022 several cases of mpox were reported worldwide, whose epidemiological and clinical outcome, particularly in patients seropositive for HIV, led to declaring the 2022 mpox outbreak as a public health emergency. We describe a case of a patient with HIV infection and severe and fulminant mpox, with necrotic plaques on the eyelid and an inflammatory membrane on the ocular surface; oral mucosa with whitish lesions and ulcers on the tongue; soft tissues induration, and necrotic lesions on the feet. After multiple complications, he became the first fatality reported in Ecuador in 2022. In HIV-infected patients mpox can be considered an opportunistic agent, with severe skin lesions with or without systemic manifestations.

Introducción: La histoplasmosis es una micosis sistémica que afecta a humanos, su agente Histoplasma capsulatum, hongo dimorfo, es ubicuo en la naturaleza. Frecuentemente se presenta como reactivación en personas con infección por VIH/SIDA, con manifestaciones polimórficas y diseminadas. Las lesiones mucocutáneas son una importante llave diagnóstica. Objetivo: Contribuir al conocimiento de esta patología a través del reporte de los diagnósticos de laboratorio de histoplasmosis realizados en Uruguay en los últimos 10 años. Materiales y Métodos: Se realizó un estudio observacional, retrospectivo de las histoplasmosis diagnosticadas en el laboratorio de referencia de Micología de Facultad de Medicina y dos laboratorios clínicos. Se enrolaron los registros clínicos y analíticos asociados. Resultados: Fueron 69 los diagnósticos de histoplasmosis. Más de 80% correspondió a personas con infección por VIH/SIDA. El 62,3% del total presentó lesiones de piel y/o mucosas y en 58% el diagnóstico se realizó mediante el estudio de estas. El 62,3% de los diagnósticos se realizaron mediante la visualización al microscopio óptico de frotis coloreados. Conclusiones: La mayoría de las histoplasmosis se vinculan a la infección por VIH/SIDA. El estudio micológico de las lesiones de piel y/o de mucosas, es accesible, mínimamente invasivo, rápido y presenta una excelente performance diagnóstica.


Background: Histoplasmosis is a systemic mycosis that affects humans, its agent Histoplasma capsulatum, a dimorphic fungus, is ubiquitous in nature. It frequently presents as reactivation in people with HIV/AIDS infection, with polymorphic and disseminated manifestations. Mucocutaneous lesions are characteristic and an important diagnostic key. Aim: To contribute to the knowledge of this pathology through the report of histoplasmosis laboratory diagnosis made in Uruguay in the last 10 years. Methods: We conducted an observational, retrospective study of diagnosed histoplasmosis in the Mycology reference laboratory of the Faculty of Medicine and two clinical laboratories. Associated clinical and analytical records were obtained. Results: There were 69 histoplasmosis diagnoses. More than 80% corresponded to people with HIV/AIDS infection. 62.3% of the total presented skin and/or mucosal lesions and in 58% the diagnosis was made by studying them. 62.3% of the diagnoses were initially made by viewing colored smears under an optical microscope. Conclusions: Most histoplasmosis is linked to HIV/AIDS infection. Exposure to a high fungal load is a constant in cases of immunocompetent individuals. The mycological study of skin and/or mucosal lesions is accessible, minimally invasive, fast and has excellent diagnostic performance.

Skin camouflage therapy is the use of specially formulated products to conceal skin diseases and disfigurements, with the goal of giving a temporarily normal appearance. It gives immediate relief in various dermatological disorders, especially those affecting the visible areas, which are known to be associated with profound psychosocial impact. Despite considerable evidence for its emotional benefits, its utilisation among dermatologists in clinical practice remains low. This article is intended to familiarise dermatologists with the use of skin camouflage therapy including its availability, indications, contraindications, techniques, limitations and future trends

ABSTRACT

La pandemia por Covid-19 ha marcado cambio en paradigma de la atención médica, generando la necesidad del uso de equipo de protección individual (EPI), para salvaguardar la salud. Lo que ha originado manifestaciones cutáneas secundarias asociadas al uso de los mismos. Se realizó una investigación descriptiva de corte transversal con el objetivo de caracterizar las lesiones cutáneas asociadas al uso de EPI e infecciones por Staphylococcusaureus en 283 enfermeros, el diagnóstico dermatológico fue realizado por especialistas; el cultivo bacteriológico se realizó a partir de hisopados de algodón estériles o frotis de aposición sobre la lesión de piel. Para el aislamiento del género Staphylococcus, las muestras se sembraron en agar salino manitol rojo de fenol, incubándose de 24 a 48 horas a 37°C. La identificación de S. aureus se efectúo por la fermentación del manitol en el agar selectivo, la reacción positiva de la prueba de la coagulasa. El S. aureus se diferenció del Staphylococcus coagulasa negativo con el empleo de la prueba de la Dnasa. Se precisó LC-EPI en 75,618% de los enfermeros, de los cuales 92,523% presentaron síntomas sugestivos a infección bacteriana secundaria, ratificada en su totalidad por cultivo microbiológico, identificando a S. aureus. Se estimó que más del 60% de las LC-EPI eran de presentación única catalogada grado I, afectando mayormente las regiones: orbitaria, temporal, nasal, infraorbitaria y frontal, asociándose con el uso de lentes/protectores faciales, gorros y mascarillas como causantes de las lesiones. Se recomienda la definición de protocolos de prevención de LC-EPI(AU)


The Covid-19 pandemic has marked a paradigm shift in medical care, generating the need for the use of personal protective equipment (PPE) to safeguard health. This has caused secondary skin manifestations associated with their use. A descriptive cross-sectional investigation was carried out with the objective of characterizing the skin lesions associated with the use of PPE and Staphylococcus aureus infections in 283 nurses. The dermatological diagnosis was made by specialists; the bacteriological culture was performed using sterile cotton swabs or apposition smears on the skin lesion. For the isolation of the genus Staphylococcus, the samples were seeded in phenol red mannitol saline agar, incubating for 24 to 48 hours at 37°C. The identification of S. aureus was carried out by the fermentation of mannitol in the selective agar, the positive reaction of the coagulase test. S. aureus was differentiated from coagulase-negative Staphylococcus using the DNase test. LC-EPI was required in 75.618% of the nurses, of which 92.523% presented symptoms suggestive of secondary bacterial infection, fully confirmed by microbiological culture, identifying S. aureus. It was estimated that more than 60% of the LC-EPI were of a single presentation classified as grade I, mainly affecting the regions: orbital, temporal, nasal, infraorbital, and frontal, associated with the use of glasses/face protectors, hats, and masks as causes of the injuries. The definition of LC-EPI prevention protocols is recommended(AU)

Se presenta el caso de un paciente varón de 28 años, heterosexual, con VIH negativo que desarrolló erupción cutánea intensa caracterizada por la presencia de vesículas, pústulas y pápulas umbilicadas confluentes en el pene y región púbica con escasa diseminación en el resto del cuerpo, sin adenopatía ni manifestaciones sistémicas, cuatro días después de haber tenido relaciones sexuales con una prostituta aparentemente sana. El hisopado de las lesiones dérmicas revelaron la presencia de ADN del virus MPOX por PCR en tiempo real. Las pruebas inmunológicas fueron las siguientes: ELISA cuarta generación para VIH y Western Blot negativas, anticuerpos IgM para herpes simplex1=1.2 U/ml, anticuerpos IgM para herpes simplex2=1.9 U/ml, anticuerpos IgM para varicela zóster=0.5 S/CO, FTA-ABS (IgM)=negativo. Se resalta la intensidad de las lesiones cutáneas en el pene y la necesidad de realizar pruebas para descarte de MPOX en prostitutas.


We present the case of a 28-year-old male patient, heterosexual, with negative HIV who developed an intense skin rash characterized by the presence of confluent vesicles, pustules, and umbilicated papules on the penis and pubic region with little spread to the rest of the body, without lymphadenopathy or systemic manifestations, four days after having sexual relations with an apparently healthy prostitute. The swabbing of the dermal lesions revealed the presence of MPOX virus DNA by real-time PCR. The immunological tests were the following: negative fourth-generation ELISA and Western Blot for HIV, IgM antibodies to herpes simplex1=1.2 U/ ml, IgM antibodies to herpes simplex2=1.9 U/ml, IgM antibodies to varicella zoster=0.5 S/CO and negative FTA-ABS (IgM) for syphilis. The intensity of skin lesions on the penis and the need to perform tests to rule out MPOX in prostitutes are highlighted.

RESUMEN La leucemia cutánea es una patología muy poco frecuente y se puede presentar en diferentes tipos de leucemias asociadas o no a síndromes genéticos. Es una forma muy poco común de presentación inicial de malignidad. Por sus lesiones inespecíficas en la piel y la similitud con diversas patologías cutáneas representa un gran desafío diagnóstico para el dermatólogo pediátrico. Se presenta el caso de un niño de 5 años con antecedentes de leucemia linfocítica aguda con lesiones papulosas descamativas de aspecto liquenoide diseminadas en tegumento cutáneo. Se realizó una biopsia bajo la sospecha de infiltración a piel o leucemia cutis. Se discutió el caso para llegar a la confirmación diagnóstica de leucemia cutánea. Se inició el tratamiento en el Servicio de Hematología. Esta enfermedad hematológica se manifiesta ocasionalmente en la piel y cuando lo hace es necesario reconocerla para completar su diagnóstico y tratamiento y salvar la vida del paciente afectado como en este caso. El interés de esta presentación radica en que la aparición de lesiones cutáneas, aunque sean inespecíficas, en un paciente con leucemia, debe alertar al equipo médico tratante para su rápido estudio y así orientar la conducta terapéutica y por su baja incidencia de presentación.


ABSTRACT Cutaneous leukemia is a very rare pathology and can occur in different types of leukemia associated or not with genetic syndromes. It is a very uncommon form of initial presentation of malignancy. Due to its non-specific skin lesions and the similarity with various skin pathologies, it represents a great diagnostic challenge for the pediatric dermatologist. We present the case of a 5-year-old boy with a history of acute lymphocytic leukemia with scaly papular lesions of a lichenoid appearance disseminated in the cutaneous integument. A skin biopsy was performed on suspicion of skin infiltration or leukemia cutis. The case was discussed to reach diagnostic confirmation of cutaneous leukemia. Treatment was started in the Hematology Service. This hematological disease occasionally manifests itself on the skin and when it does, it is necessary to recognize it to complete its diagnosis and treatment and save the life of the affected patient, as in this case. The interest of this presentation lies in the fact that the appearance of skin lesions, even if they are non-specific, in a patient with leukemia, should alert the treating medical team for their rapid study and thus guide therapeutic behavior and due to their low incidence of presentation.

RESUMEN Introducción: el nuevo coronavirus SARS-CoV-2, agente causal de la COVID-19, fue hallado en Wuhan, China, en diciembre de 2019. Desde entonces, la pandemia generada ha puesto en vilo al mundo, dada su alta contagiosidad y capacidad para dañar los órganos y sus sistemas, incluyendo la piel. Esta se ha visto afectada tanto por la acción directa del virus como por los nuevos estilos de vida que han sido adoptados para evitar el contagio. Objetivo: caracterizar las lesiones cutáneas originadas por los nuevos estilos de vida durante la COVID-19. Materiales y métodos: se realizó un estudio observacional, descriptivo y prospectivo de los pacientes que acudieron a consulta de Dermatología en el Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández, de Matanzas, con lesiones cutáneas de reciente aparición sin causa aparente conocida, entre septiembre de 2020 y febrero de 2021, y que dieron su consentimiento informado para participar en la investigación. El universo estuvo integrado por 14 pacientes portadores de lesiones sin etiología precisada. Todas las lesiones fueron sometidas a examen clínico y algunas, además, fueron tributarias de examen microbiológico. Resultados: predominaron las féminas con un 78,6 %, una media de edad de 34 años y la piel blanca, para un 71,4 %. Las pápulas y pústulas fueron las lesiones de mayor incidencia, siendo más afectados los que usan la mascarilla más de cuatro horas. Los sitios anatómicos más vulnerables fueron el mentón, las mejillas y la zona peribucal. Conclusiones: el maskné y las discromías postinflamatorias son enfermedades que se atribuyen en la actualidad al uso continuo del nasobuco.


ABSTRACT Introduction: the new SARS-CoV-2 virus, causal agent of COVID-19, was found in Wuhan, China, in December 2019. Since then, the pandemics generated has put the world in suspense, given its high contagiousness and capability to damage the organs and their systems, including the skin. The skin has been affected both by the direct action of the virus and by the new life styles that have been adopted to avoid contagion. Objective: to characterize the lesion skins originated by the new life styles adopted during the COVID-19 pandemics. Materials and methods: an observational, descriptive and prospective study was carried out with the patients who assisted the consultation of Dermatology in the Clinical-surgical Teaching Hospital Faustino Perez Hernandez, from Matanzas, with skin lesions recently appeared with apparently unknown cause, between September 2020 and February 2021, and who gave their informed consent to participate in the research. The universe was formed by 14 patients bearing lesions without precise etiology. All lesions undergone clinical examination and some of them also underwent microbiological examination. Results: females predominated with 78.6 %, aged an average 34 years and white skin with for 71.4 %. Papules and pustules were the lesions of higher incidence, being the most affected those who wear masks for more than four hours. The most vulnerable anatomic sites were the chin, cheeks and peribuccal area. Conclusions: maskne and post-inflammatory dyscromies are diseases that are currently attributed to the continued use of face masks.

ABSTRACT

Multisystem inflammatory syndrome in children is a serious condition that occurs due to postinfectious immune-mediated hyperinflammatory reaction seen in children, which develops after 4–6 weeks of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection; it is rare in neonates. We present here a case of a term newborn with fever, respiratory distress, and necrotic skin lesion that gradually progressed to multisystem dysfunction. Reverse transcription polymerase chain reaction for SARS-CoV-2 was negative for both mother and the baby. SARS-CoV-2 IgG titer was negative in the mother but was found to be positive in the newborn. The diagnosis of multisystem inflammatory syndrome in neonate was made and was successfully treated with intravenous immunoglobulin and corticosteroids.

Abstract: Sulfur mustard is one of the chemical warfare agent. It rapidly reacts with the cutaneous tissues and other tissues, leading to various devastating long-term effects on human health. Mustard-exposed veterans suffer from its chronic skin problems, including itching, burning sensation, and eczema. We aimed to evaluate the protective effects of Myrtus communis L. (myrtle) on chronic skin lesions and quality of life of sulfur mustard-exposed veterans. In this randomized, double-blind clinical trial, 60 sulfur mustard-exposed patients were evaluated. Thirty patients received myrtle essence 5% cream (case group) and 30 patients received Eucerin cream (placebo group) twice in a day for one month. Then, We assessed the chronic skin problems and itching-related parameters (such as the itching time, severity, distribution, frequency, and calculated itching score), duration of sleep, number of waking up at night, and quality of life in the both groups. Our analysis of data revealed that application of myrtle cream effectively decreased skin problems including; itching and burning sensation. Additionally, myrtle markedly decreased skin lesion symptoms such as excoriation in the case group as compared with before treatment. Noticeably, myrtle cream significantly improved quality of life of the patients in the case group. The present study provides more in-depth information regarding the protective role of myrtle on the sulfur mustard-induces skin complication. Also, myrtle effectively improved quality of life of the sulfur mustard-exposed veterans.

Objective:To investigate the relationship between psoriasis severity and clinical features in psoriatic arthritis (PsA).Methods:Patients were recruited from the Chinese REgistry of Psoriatic ARthritis (CREPAR) between December 2018 and June 2021, and data were collected including the baseline demographic characteristics, various clinical manifestations (including arthritis, nail disease, comorbidities), laboratory tests[including erythrocyte sedimentation rate(ESR), C-reactive protein (CRP)], health assessment questionnaire (HAQ). Body surface area (BSA) and psoriasis area and severity index (PASI) were selected for the tools of assessment of cutaneous psoriasis. Patients were divided to two groups, including the severe psoriasis group (BSA>10%) and the non-severe psoriasis group (BSA≤10%). Disease assessment included ankylosing spondylitis disease activity score (ASDAS), disease activity score 28 (DAS28) and disease activity in psoriatic arthritis (DAPSA).Results:1 074 eligible patients with PsA were recruited, and 106 (9.9%) had severe psoriasis. Compared with non-severe psoriasis group, the severe psoriasis group had more peripheral joint involvement (including patients with ever or current peripheral arthritis, 94.3% vs. 85.6%), more polyarticular joint involvement (including patients with current peripheral arthritis, 74.0% vs. 58.2%), more axial joint involvement (51.4% vs. 39.9%), more nail disease (72.6% vs. 61.4%), more frequency of smoking (20.2% vs. 18.7%), and higher proportion of hypertension (23.4% vs. 14.4%). In addition, the severe psoriasis group had higher level of ESR [33(10, 70) mm/1h vs. 20(9, 38) mm/1h] and CRP [18.6(5.0, 60.8) mg/L vs. 7.0(2.4, 18.1) mg/L], higher values of DAS28-ESR (4.5±1.7 vs. 3.7±1.5), DAS28-CRP (4.2±1.5 vs. 3.4±1.4), ASDAS-ESR (3.5±1.4 vs. 2.6±1.2), and ASDAS-CRP(3.4±1.6 vs. 2.5±1.2), higher scores of HAQ [0.6(0.1, 1.0) vs. 0.3(0.0, 0.8)].Conclusion:Patients with PsA with severe psoriasis bore a heavier disease burden. Therefore, clinicians were supposed to pay more attention to them. In addition to skin lesions, they should also focus on examination of other clinical manifestations, such as joints and nails.

ABSTRACT

Allyl alcohol is an intermediate widely used in industrial production activities, which has caused many occupational damage in China. In this paper, the data of a case of skin damage and clinical cure of inhalation pneumonia caused by occupational allyl alcohol exposure were analyzed and summarized. The patient accidentally inhaled large amounts of allyl alcohol at work and had skin exposure, which was mainly manifested as aspiration pneumonia, respiratory failure, eye damage and skin bullae. After short-term hormone shock therapy, mechanical ventilation, infection prevention, local treatment and other comprehensive treatment, the patient was clinically cured. This case suggests that chemical pneumonia and respiratory failure may be caused by inhalation of allyl alcohol, and skin bullae may be caused by skin contact. Early treatment and hormone use should be emphasized.

ABSTRACT

We investigated the local and systemic effects of hyperbaric oxygen therapy in BALB/C mice, exposed to two different exposure times, under 2.4 atmosphere (ATM). Fifteen animals were divided into three groups (GI, GII and Control) and underwent a surgical excision of a skin fragment of approximately one square centimeter of the dorsal region. The wounds were treated and monitored for 21 days. In the control group, the wound was cleaned once a day with sterile 0.9% NaCl solution. GI and GII mice were submitted to daily hyperbaric oxygen therapy of 30 or 60minutes sessions, respectively. The wounds were photographed every three days and their surfaces were analyzed by an image analyzer. At 21 days, all animals were euthanatized for histopathological analysis of the skin, lungs and liver in order to identify eventual alterations in wound healing or in the analyzed organs. Animals belonging to GI showed a faster skin wound healing in comparison to the other groups. Animals from GII, however, showed a delayed wound healing process and exhibited lung and microcirculatory alterations. These findings allow us to conclude that the exposure time to the oxygen in hyperbaric environment is crucial and can help or disturb skin wound healing or even be deleterious to other organs.(AU)


Investigaram-se os efeitos locais e sistêmicos da oxigenoterapia hiperbárica em camundongos BALB / C, submetidos a dois tempos de exposição diferentes, sob atmosfera 2,4 (ATM). Quinze animais foram divididos em três grupos (GI, GII e controle) e submetidos à excisão cirúrgica de fragmento de pele de aproximadamente um centímetro quadrado da região dorsal. As feridas foram tratadas e acompanhadas por 21 dias. No grupo controle, a ferida foi limpa uma vez ao dia, com solução estéril de NaCl 0,9%. Camundongos GI e GII foram submetidos à oxigenoterapia hiperbárica diária de 30 ou 60 minutos de sessões, respectivamente. As feridas foram fotografadas a cada três dias, e suas superfícies analisadas por um analisador de imagens. Aos 21 dias, todos os animais foram submetidos à eutanásia para análise histopatológica da pele, do pulmão e do fígado, em busca de eventuais alterações na cicatrização da ferida ou nos órgãos analisados. Animais pertencentes ao GI apresentaram cicatrização mais rápida de feridas cutâneas em comparação aos outros grupos. Já os animais do GII apresentaram retardo na cicatrização da ferida e alterações pulmonares e microcirculatórias. Esses achados permitem concluir que o tempo de exposição ao oxigênio em ambiente hiperbárico é fundamental e pode auxiliar ou atrapalhar a cicatrização de feridas cutâneas ou mesmo ser deletério para outros órgãos.(AU)

INTRODUCTION@#The occurrence of malignant tumors associated with leprosy has been observed, with lymphoma being the most commonly associated non-epithelial malignant tumor and may be due to the depressed immunologic surveillance. The converse where leprosy manifests in a lymphoma patient undergoing chemotherapy has also been mentioned in a few articles.@*CASE SUMMARY@#A 49-year-old female was diagnosed to have peripheral T-cell lymphoma after an initial presentation of enlarged lymph nodes, generalized asymptomatic papules and plaques on the trunk, and pancytopenia. Two weeks after initiation of chemotherapy with cyclophosphamide, doxorubicin, vincristine, prednisolone (CHOP), her skin lesions progressed to become violaceous to slightly hyperpigmented. Further query revealed a year-long history of hypoesthesia of the lower extremities. Histopathologic examination revealed nodular histiolymphocytic infltrates surrounding blood vessels, adnexal structures, and nerves, consistent with lepromatous leprosy. Fite-Faraco stain was positive. Due to lymphoma-related pancytopenia, the patient was given monthly rifampicin, ofoxacin and minocycline (ROM) chemotherapy, alternatively. Erythema nodosum leprosum reaction developed for which clofazimine was given, resulting in improvement.@*CONCLUSION@#Leprosy is a great mimicker and dermatologists need to be adept at diagnosing skin conditions in immunocompromised patients, especially since disease course and manifestation may be modifed in this subset of patients. Leprosy must be considered when granulomatous lesions arise in lymphoma patients before ascribing them to the underlying disease. Management may also be challenging due to the comorbidities, which may limit treatment options. Careful history, clinical clues, histopathologic correlation, and prudent therapeutic approach are important tools in addressing these cases.

RESUMEN Introducción: La larva migratoria cutánea, también conocida como miasis emigrante, es una entidad dermatológica producida por la penetración de helmintos a la piel. Caso: Varón de 53 años, quien consulto por 5 días de evolución de lesiones en extremidades de tipo eritematoso y pruriginoso,de crecimiento progresivo, que comenzaron como pápulas pequeñas con un diámetro de 0.5 cm y posteriormente aumentaron de tamaño aproximadamente 2.5 cm de longitud. Se hizo el diagnóstico presuntivo de larva migratoria cutánea, tanto por su clínica como por la exposición a factores de riesgo. Conclusiones: La prevalencia en los países de América del Sur varía entre el 0,2 y el 4,4% en la población general y alcanza hasta el 14% en niños menores de 5 años. El diagnóstico de larva migratoria se presume ante la exposición a factores de riesgo y la presencia de lesiones cutáneas con lesiones típicas serpiginosas, muy pruriginosas. Estos se ubican en áreas donde el grosor de la piel es más delgado. Una anamnesis dirigida a preguntar al paciente sobre los diferentes factores de riesgo nos ayudará a establecer el diagnóstico. El manejo de esta infección se basa en la administración de ivermectina, la cual tiene alta eficacia.


ABSTRACT Introduction: The cutaneous migratory larvae, also known as emigrant myiasis, is a dermatological entity produced by the penetration of Helminths to the skin. Case: 53-year-old male, who consulted for 5 days of evolution of lesions in the limbs of the erythematous and pruritic type, of progressive growth, which began as small papules with a diameter of 0.5 cm and subsequently increased in size approximately 2.5 cm in length. It was considered that he was studying cutaneous larva migrans, due to his symptoms and exposure to risk factors. Conclusions: The prevalence in the countries of South America varies between 0.2 and 4.4% in the general population and reaches up to 14% in children under 5 years of age. The diagnosis of larva migrans is fundamentally clinical, according to the evidence of skin lesions with typical serpeginous lesions, and very itchy. These are located in areas where the thickness of the skin is thinner. An anamnesis aimed at asking the patient about the different risk factors will help us to establish the diagnosis. Ivermectin treatment is highly effective.

Skin diseases in pigs can negatively impact the production. They cause losses related to the death of the affected pigs, to the cost with the treatment, growth retardation and condemnations in the slaughterhouses. This study was developed to determine the frequency and describe the histopathological findings of skin diseases in pigs in different age groups through a retrospective study from 2006 to 2018. A total of 154 conclusive cases were analyzed, including skin restricted diseases (allergic dermatitis, exudative epidermitis, vesicular dermatitis, pityriasis rosea, swinepox, follicular cyst, papilloma and scrotal hemangioma) or skin lesions secondary to systemic diseases (erysipelas, porcine dermatitis and nephropathy syndrome (PDNS), bacterial septicemia and multiple hemorrhages without definite cause). The skin lesions were classified as bacterial (46.1%), viral (26.6%), allergic (12.3%), neoplastic (1.3%) and others (13.6%). Swine erysipelas was the most frequent diagnosis (47/154), followed by PDNS (23/154), allergic dermatitis (19/154) and exudative epidermitis (15/154). Vesicular dermatitis (9/154), pityriasis rosea (9/154), septicemia with cutaneous manifestations (9/154), swinepox (9/154) and multiple hemorrhages without definite cause (7/154) were also observed. Follicular cyst (3/154), hyperkeratosis without definite cause (2/154), papilloma (1/154), and scrotal hemangioma (1/154) were less frequently described. Of the conclusive diagnosis, age was reported in 138 cases, with the highest frequency of skin lesions observed at the inspection process during slaughter (56/138).(AU)


As doenças de pele em suínos podem impactar negativamente a produção. Estas causam perdas relacionadas à morte dos acometidos, a custo com tratamentos, atraso no crescimento e condenações nos frigoríficos. Este trabalho foi desenvolvido para determinar a frequência e descrever os achados histopatológicos das doenças de pele em suínos nas diferentes faixas etárias, através de um estudo retrospectivo no período de 2006 a 2018. Foram analisados 154 casos conclusivos incluindo as doenças restritas a pele (dermatites alérgicas, epidermite exsudativa, dermatite vesicular, pitiríase rósea, varíola suína, cisto folicular, papiloma e hemangioma escrotal) e as secundárias a doenças sistêmicas (erisipela, síndrome dermatite nefropatia suína (SDNS), septicemia bacteriana e hemorragias múltiplas de causa não determinada). Estas foram classificadas em bacterianas (46,1%), virais (26,6%), alérgicas (12,3%), neoplásicas (1,3%) e outras (13,6%). A erisipela suína foi a enfermidade mais diagnosticada (47/154), seguida por SDNS (23/154), dermatite alérgica (19/154) e epidermite exsudativa (15/154). Observamos ainda dermatite vesicular (9/154), pitiríase rósea (9/154), septicemia bacteriana com manifestações cutâneas (9/154), varíola suína (9/154) e hemorragias múltiplas de causa não determinada (7/154). Em menor número, cisto folicular (3/154), hiperqueratose sem causa definida (2/154), papiloma (1/154) e hemangioma escrotal (1/154). Dos casos conclusivos, a idade foi informada em 138 casos, sendo a maior frequência das lesões de pele observadas na linha de inspeção, durante o abate (56/138).(AU)

Background: Clinical examination may suffice in making diagnosis of most dermatologic disorders but histopathological examination is often required to confirm the diagnosis and further categorize the lesions. Authors carried out this study to analyse the demographic and histomorphological characteristics of skin lesions, to determine the frequency of various dermatological disorders in the region and to evaluate the agreement between clinical and histopathological diagnosis.Methods: Punch biopsies of skin lesions received in histopathology section, were included in the study. Cases over a period of six months were analysed. Clinical details were recorded and histopathological analysis done. Special stains were applied wherever required.Results: Of the 120 cases studied, maximum cases fell in the category of 31-40 years, with male predominance.  Authors observed wide variety of non-neoplastic and neoplastic lesions. Infectious diseases were the most common of all pathologies. Leprosy was the most common histopathological diagnosis. Complete clinicopathologic correlation was seen in 51.67% of cases while partial correlation was noted in 23.33% making a total of 75%. 25% histopathological diagnosis were inconsistent with the clinical diagnosis.Conclusions: Histopathology is a gold standard investigation and plays a very important role in confirmation of clinical diagnosis of various skin lesions. Punch biopsy is a relatively easy outpatient procedure to perform.

Resumen: ANTECEDENTES: El liquen plano de vulva se manifiesta como una lesión erosiva, papuloeritematosa o hipertrófica aislada o con afectación extragenital concomitante. CASO CLÍNICO: Paciente de 41 años, sin antecedentes médicos de interés, que acudió a consulta por la aparición de lesiones cutáneas pruriginosas de dos meses de evolución. A la exploración se objetivaron múltiples pápulas eritematoescamosas, violáceas, hiperqueratósicas, concomitantes con fenómeno de Koebner y tendencia a la agrupación, ubicadas en la cara anterior de los antebrazos y las muñecas, el dorso del pie y el tronco; en los pliegues inguinales, región vulvar y perianal, y de forma asintomática en el surco interlabial izquierdo, una pápula no pruriginosa de 3 mm, de tonalidad violácea con reticulado blanquecino en la superficie (estrías de Wickham), y otras dos pápulas de 11 y 3 mm. Se estableció el diagnóstico de liquen plano pápulo-escamoso vulvar, variante hipertrófica cutánea. Puesto que la enfermedad aparece de forma espontánea, se decidió el tratamiento expectante y para el prurito se indicaron antihistamínicos por vía oral. Seis semanas después desaparecieron las lesiones cutáneas (sin cambios cicatriciales). CONCLUSIONES: En pacientes con procesos dermatológicos complejos se requiere la correcta anamnesis y exploración ginecológica, sobre todo cuando hay afectación de las mucosas, para evitar el infradiagnóstico. La biopsia de las lesiones es útil en caso de duda, sobre todo en las lesiones vulvares aisladas de tipo hipertrófico.


Abstract: BACKGROUND: Vulvar lichen planus is a subtype of dermatological pathology that is presented as erosive, papulo-erythematous, or hypertrophic lesions on the vulva. This lesion could appear in isolation or with concomitant extragenital involvement. CASE REPORT: A 41-year-old patient with no previous history of interest, who attended due to the onset of itchy skin lesions of two months of evolution. At medical examination, multiple erythematous squamous, violaceous, hyperkeratotic papules were observed, with Koebner phenomenon and a tendency to cluster at the forearms and wrists, dorsum of the feet and trunk. The same lesions were seen in inguinal folds, vulvar and perianal region. We also notice an asymptomatic non-itchy 3 mm papule with violet edge in left labia majora (it had a whitish reticulate on the surface called Wickham's striae) and other erosive papules 11 mm and 3 mm respectively, with violet edge. No vaginal or other mucosal lesions were seen. Diagnosis of vulvar papule-squamous lichen planus was established which coexists with a cutaneous hypertrophic form. Given the self-limited nature of this pathology, a wait-and-see approach and symptomatic treatment of pruritus with oral antihistamines was adopted. Six weeks later, disappearance of the cutaneous lesion without cicatricial areas was observed. CONCLUSIONS: It is essential to carry out an anamnesis and gynecological examination in dermatological preocedures, especially when mucosas are involvement to avoid under-diagnosis. The biopsy of the lesions will be useful when there are doubts in the diagnosis, and it is essential in isolated hypertrophic vulvar lesions.

Introduction: Cutaneous neoplasms are the most common cancers in the world, and have high morbidity rates. A definitive diagnosis can only be obtained after histopathological evaluation of the lesions. To develop an artificial intelligence program to establish the histopathological diagnosis of cutaneous lesions.Methods: A deep learning program was built using three neural network architectures: MobileNet, Inception and convolutional networks. A database was constructed using 2732 images of melanomas, basal and squamous cell carcinomas, and normal skin. The validation set consisted of 284 images from all 4 categories, allowing for the calculation of sensitivity and specificity. All images were provided by the Path Presenter website.Results: The sensitivity and specificity of the MobileNet model were 92% (95%CI, 83-100%) and 97% (95%CI, 90-100%), respectively; corresponding figures for the Inception model were 98.3% (95%CI, 86-100%) and 98.8% (95%CI, 98.2-100%); lastly, the sensitivity and specificity of the convolutional network model were 91.6% (95%CI, 73.8-100%) and 95.7% (95%CI, 94.4-97.2%). The maximum sensitivity for the differentiation of malignant conditions was 91%, and specificity was 95.4%.Conclusion: The program developed in the present study can efficiently distinguish between the main types of skin cancer with high sensitivity and specificity. ABSTRACT: Ornithonyssus sylviarum is a hematophagous mite present in wild, domestic, and synanthropic birds. However, this mite can affect several vertebrate hosts, including humans, leading to dermatitis, pruritus, allergic reactions, and papular skin lesions. This study evaluated the epidemiological characteristics of O. sylviarum attacks on poultry workers, including data on laying hens, infrastructure and management of hen houses, and reports of attacks by hematophagous mites. In addition, a case of mite attack on a farm worker on a laying farm in the Midwest region in Minas Gerais is presented. It was found that 60.7% farm workers reported attacks by hematophagous mites. Correspondence analysis showed an association between reports of mite attacks in humans with (1) presence of O. sylviarum in the hen house, (2) manual removal of manure by employees, and (3) history of acaricide use. The specimens collected from the location were confirmatively identified as O. sylviarum. O. sylviarum attacks ave not been reported when manure was removed in a shorter time interval and did not use acaricide in the sheds when the removal was done by manure conveyor belt. Parasitism by O. sylviarum should be considered a relevant occupational hazard affecting employees working in direct contact with commercial egg-laying hens in Minas Gerais. We emphasized the need to monitor hen infestations by this mite to improve the development of mite control strategies.


RESUMO: Ornithonyssus sylviarum é um ácaro hematófago de aves silvestres, domésticas e sinantrópicas. No entanto, este ácaro pode afetar vários hospedeiros vertebrados, incluindo humanos, levando a dermatites, prurido, reações alérgicas e lesões cutâneas papulosas. O presente estudo avaliou os aspectos epidemiológicos do ataque de O. sylviarum em trabalhadores de granjas avícolas e descreveu um relato de caso em uma granja de postura. Este estudo utilizou um banco de dados secundário com informações sobre as poedeiras, infraestrutura e manejo dos galinheiros e relatos de ataques por ácaros hematófagos. Foi apresentado um caso de ataque de ácaro a um trabalhador rural no Centro-Oeste em Minas Gerais. Houve uma frequência de 60,7% de trabalhadores que relataram ter sido atacados por ácaros hematófagos em fazendas. A análise de correspondência mostrou uma associação entre relatos de ataques de ácaros em humanos com (1) presença de O. sylviarum na fazenda, (2) remoção manual de esterco por funcionários da fazenda e (3) histórico de uso de acaricidas. Os espécimes coletados do local foram identificados como O. sylviarum. Não há ataques de O. sylviarum, quando a remoçãodo esterco é feita em menor intervalo de tempo, e não se utiliza acaricida nos galpões quando a remoção é feita por esteira transportadora de esterco. Conclui-se que o parasitismo por O. Sylviarum deve ser considerado como um risco ocupacional relevante, que afeta trabalhadores que trabalham diretamente com poedeiras comerciais em Minas Gerais. Ressaltamos a necessidade de monitorar as infestações de aves por esse ácaro, a fim de melhorar o desenvolvimento de estratégias de controle.
