ABSTRACT

Objective To design an air-cushioned belt for skin traction of lower extremities to reduce the incidence of deep vein thrombosis and pressure ulcers in patients with lower limb fractures. Methods The belt was composed of upper and lower leggings.The legging had an air cushion at its interior surface,and the air cushion consisted of several chambers.There was a connecting tube between every two chambers, and each tube was equipped with a pressure non-return valve. The chamber on the top of the cushion had a charging mechanism for inflation.Totally 100 patients from June to December 2016 were selected and divided equally into an experiment group and a control group.The experiment group used the developed air-cushioned belt,and the control group applied the traditional one.The incidence rates of deep venous thrombosis of lower limbs were compared in the two groups,and χ2test was carried out on the results.Results There were no cases of pressure ulcers and deep venous thromboses of lower limbs occurred in the experiment group,while 5 cases of deep venous thromboses of lower limbs and 4 cases of pressure ulcer happened in the control group,and there were significant differences between the incidence rates in the groups(P<0.05).Conclusion The air-cushioned belt has easy operation,relieves the patients'pains and the nurses'workload when used to prevent deep venous thrombosis of lower limbs and pressure ulcer,and thus is worthy promoting clinically.

ABSTRACT

Objective@#To explore the clinical effect of cotton pads under stitches when intradermal traction with sutures is used to repair of chronic wounds.@*Methods@#From January 2009 to August 2016, 21 patients with 31 wounds were treated by continuous intradermal traction. Wound edges were sutured with 2-0 silk thread in horizontal mattress fashion, but not tied. Then small cotton pads were put under stitches to protect the skin around the wound. After that, the sutures on both sides of the wound edges were pulled to the center of the wound with a certain force. And the wound was sutured finally when the skin on both sides of the wound was closed enough.@*Results@#All of 31 wounds healed without necrosis of the skin margin and 5 cases of lacerated wound caused by silk thread healed after dressing change. The healing time was (43.24±11.46) days, and there was scarcely hypertrophic scar at the stitches. This method was compared with the previous method of protecting stitches with silicone tubes( referring to past data). There was no significant difference in the healing time between the two methods (P=0.119), and less laceration occurred under the stitches with cotton pads (P=0.046).@*Conclusions@#It is a commendable method for treating chronic wounds to put cotton pads under the stitches while tracting the wound edges with sutures. It can effectively cushion the cutting force of the silk thread, protect the traction point and facilitate the replacement of the pads.

ABSTRACT

PURPOSE: The purpose of this study was to evaluate a new treatment method, which was using intraoperative skin traction and Steinmann(S)-pin for anatomically reduction by gentle manipulation to treat the displaced supracondylar fracture of the humerus with percutaneous pinning. MATERIALS AND METHODS: Clinical analysis was performed on thirty displaced supracondylar fractures (Gartland type III) of the humerus patients with a minimal three month follow up, who were treated percutaneous pinning after reduction with by intraoperative skin traction and S-pin. Clinical results were analyzed according to the Flynn grading system. RESULTS: According to the Flynn grading system, excellent results were obtained in 12 cases (43%), good in 14 case (47%), fair in 4 cases (10%), poor in 0 cases (0%) and we obtained all satisfactory results. There was one case which was reoperated for closed reduction and percutaneous pinning repeatedly due to reduction loss and no case with conversion to open reduction. CONCLUSION: To avoid the forceful manipulation, gentle closed reduction and percutaneous pinning using intraoperative skin traction and S-pin especially for rotational correction in the displaced supracondylar fracture of the humerus is considered to be useful method because showed satisfactory results.
