ABSTRACT

Objective To investigate the general situation, smoking status, smoking cessation status and one-month follow-up examination in Shanghai, providing scientific basis for further effective development of smoking cessation clinics in Shanghai. Methods A total of 841 smokers who visited the smoking cessation clinic from 2016 to 2018 were investigated and 725 patients were followed up one month later.The data thus obtained were analyzed by SPSS software. Results In general, of the visiting patients in 2018, 91.3% were male, 50.6% were 35-55 years old, 57.1% had educational background in primary school or below, and 60.2% were employees of enterprises, businesses and service industries.In terms of smoking, 44.1% of the patients smoked 10-20 cigarettes a day, 34.6% smoked for 10-20 years, and 38.6% smoked the first cigarette within 5 minutes after getting up.With regards to the situation of smoking cessation, 72.1% of the patients tried to quit smoking before going to the clinic; 14.8% of the patients used electronic cigarettes to help quit smoking; 33.9% learned about the smoking cessation clinic through the doctors; 61.4% decided to quit smoking because they were concerned about their health and the health of their family members.According to the follow-up, the rate of quitting smoking in one month in 2018 was 48.8%;44.0% of the patients believed that their own determination was the key to quit smoking; 46.2% of the patients who failed to quit smoking believed that if they tried to quit smoking again, the biggest obstacle was the onset of smoking addiction. Conclusion The establishment of smoking cessation clinics in Shanghai can help patients quit smoking.However, there are still some problems in this regard.In view of the characteristics of the patients going to the smoking cessation clinic, we should find out the weaknesses in the propaganda of the smoking cessation clinic in time.For example, it is found that there are fewer smoking women, less educated and less farmers visiting the clinic; according to the smoking situation of the patients, we should clearly focus on updating the service quality of the smoking cessation clinic; proceeding from the follow-up situation, we should find out the the approach to the construction and perfection of smoking cessation clinics in Shanghai.

ABSTRACT

Objective@#To evaluate the service competence of 99 smoking cessation clinics in Zhejiang Province, so as to provide reference for the improvement. @*Methods@#The questionnaire, prepared by Tobacco Control Office of Chinese Center for Disease Control and Prevention, was used to investigate all the smoking cessation clinics in Zhejiang Province, to score the basic and developmental indicators, and to assess the qualified rates ( basic indicators >50 points and total scores >60 points ). @*Results@#There were 99 smoking cessation clinics, with 52 in secondary hospitals and 47 in tertiary hospitals. The overall assessment scored from 11 to 96 points, with an average of ( 53.99±16.56 ) points. The smoking cessation clinics in secondary and tertiary hospitals scored ( 53.92±15.88 ) points and ( 54.06±17.45 ) points. The scores of basic and developmental indicators were ( 45.66±12.16 ) points and ( 8.33±5.39 ) points, with the scoring rates of 65.23% and 27.77%. The overall assessment of 43 smoking cessation clinics were qualified and the rate was 43.43%, which was 44.23% in secondary hospitals and 42.55% in tertiary hospitals. @*Conclusion@#The qualified rate of smoking cessation clinics in Zhejiang Province was 43.43%, which was similar between second hospitals and tertiary hospitals.

ABSTRACT

OBJECTIVES: This study was conducted to examine the factors implicated when people start smoking again after a 6 month cessation, and was carried out at the smoking cessation clinic of a public health center. METHODS: The study subjects were 191 males who had attended the smoking cessation clinic of a public health center for 6 months in an attempt to quit smoking. Data was collected, by phone interview, regarding individual smoking habits, if any, over the 6 month study period. The factors which may have caused an individual to smoke again were examined. This study employed a health belief model as it theoretical basis. RESULTS: Following a 6 month cessation, 24.1% of the study group began to smoke again during the 6 month test period. In a simple analysis, the factors related to individuals relapsing and smoking again included barriers of stress reduction, body weight gain and induction of smoking by surroundings among perceived barriers factor of our health belief model (p<0.05). In multiple logistic regression analysis for relapsed smoking, significant factors included barriers of stress reduction and induction of smoking by surroundings (p<0.05). The most important reason of for an individual to relapse into smoking was stress (60.9%) and the most likely place for a relapse to occur was a drinking establishment (39.1%). CONCLUSIONS: Our results indicate that both regular consultations and a follow-up management program are important considerations in a public health center program geared towards maintaining smoking cessation.

ABSTRACT

Introducción: El consumo de tabaco es uno de los principales factores de riesgo para enfermedades cardiovasculares. A pesar de que se conoce la relación entre el consumo de tabaco crónico y la dislipidemia, en la Clínica para Dejar de Fumar (CDF), "Dra. Carmen Gutiérrez de Velasco" del Instituto Nacional de Enfermedades Respiratorias (INER), Ismael Cosío Villegas, todavía no se ha descrito el perfil de lípidos y la prevalencia de las dislipidemias. Objetivo: Describir el perfil de lípidos y la prevalencia de dislipidemias en fumadores que acuden a la CDF. Métodos: Se revisaron 700 expedientes consecutivos de los fumadores que acudieron a la clínica CDF durante el periodo de tres años que comprendió de enero de 2003 a diciembre de 2005. Se investigó la prevalencia del perfil de lípidos aterógeno (PLA), definido como colesterol total y triglicéridos elevados y HDL-colesterol bajo, y la prevalencia de cada componente de PLA. Se estudió la diferencia del perfil de lípidos por género y por número de cigarrillos (cig) consumidos al día. Resultados: La prevalencia de PLA, hipercolesterolemia, hipertríglicerídemia y HDL-colesterol bajo fue de 21, 50, 56y 55%, respectivamente. El promedio de colesterol total, de tríglicéridos y HDL-colesterol fue de 204 ± 44 mg/dL, 208 ± 155 mg/dL y 46 ± 14 mg/dL, respectivamente. Los niveles de tríglicéridos se encontraron más altos y HDL más bajos en los que fumaban más de 20 cig/día en comparación con los que fumaban menos de 20 cig/día (225 ± 162 mg/dL vs 185 ± 134 mg/dL, p = 0.02; 44.2 ± 12 mg/dL vs 48 ± 14 mg/dL, p < 0.001, respectivamente). Las mujeres tuvieron niveles más altos de HDL en comparación con los hombres (51 ± 14 mg/dL vs 40.3 ± 11 mg/dL, p < 0.001). Conclusiones: La prevalencia alta de PLA en la población fumadora de nuestra CDF la hace muy vulnerable de presentar ateroesclerosis prematura. Este estudio resalta la importancia de la búsqueda de alteraciones lipfdicas en fumadores.


Background: Tobacco smoking is one of the major modifiable cardiovascular risk factors. The relationship between smoking and lipid disorders has been studied; however, its prevalence among smokers from our Smoking Cessation Clinic (SCC) "Dra. Carmen Gutiérrez de Velasco " at the Instituto Nacional de Enfermedades Respiratorias Ismael Cosío Villegas is unknown. Objective: To describe the lipid profile of smokers at our SCC and its prevalence in this population. Methods: 700 medical records from patients admitted to the clinic during a three- year period from 2003 to 2005 were studied. We analyzed the prevalence of their atherogenic lipid profile defined as high serum total cholesterol and triglycerides and low HDL-cholesterol. In addition, we studied the prevalence of each component of the lipid profile, according to a gender and a number of cigarettes (cig) smoked per day. Results: The prevalence of atherogenic lipid profile, hypercholesterolemia, hypertriglyceridemia and low HDL-cholesterol was 21%, 50% 56% and 55%, respectively. Their total serum cholesterol and triglycerides levels were high and HDL-cholesterol was low (CT 204 ± 44 mg/dL, TG 208 ± 155 mg/dL, HDL 46 ± 14 mg/dL). Those who smoked more than 20 cig/day had higher triglycerides and lower HDL-cholesterol than those smoking less than 20 cig/day (225 ± 162 mg/dL vs 185 ± 134 mg/dL, p = 0.02 and 44.2 ± 12 mg/dL vs 48 ± 14 mg/dL, p<0.001, respectively). Women had higher HDL-cholesterol than men (51 ± 14 mg/dL vs 40.3 ± 11 mg/dL, p < 0.001). Conclusion: The high prevalence of an atherogenic lipid profile in smokers makes them prone to develop premature atherosclerosis. This study underscores the importance of lipid disorders research in smokers.

ABSTRACT

BACKGROUND: In spite of harmfulness of smoking, the rate of adult man's smoking is high in Korea and that of young people and women has risen recently. Although the number of hospitals operating smoking-cessation clinic has risen year by year, there is little research on smoking cessation. In this study, we tried to find the factors associated with successful smoking cessation and have attempted to contribute to developing effective smoking cessation strategy and help to continue cessation state. METHOD: Three hundred smokers, who had visited the Dankook Univ. Hospital from May 1999 to December 1999, were consulted. After doing a survey with a basic questionnaire and smoking cessation consultation, we asked them to revisit on the date of appointment with continued use of nicotine patch as much possible. Six months later, we selected 272 people and analyzed the factors of difference between the two groups ; one is the successful group and the other is the failure group. RESULT: The success group included 77 people (28.3%), and the failure group 195 people (71.7%). In view of demographic profiles such as age, sex, marital status, education, and job, there was no significant difference between the two groups. In view of smoking pattern and factors that can influence them such as age of starting to smoke, duration of smoking, motive of smoking, reason of failure to stop smoking, reason of trying to stop smoking, the presence of smoking family number, confidence in smoking cessation, dependence on nicotine and the number of alcohol drinking, there was no significant difference between the two groups. In the results of univariate analysis, daily mean smoking amount, the presence of smoking-associated disease, expiratory CO level at initial visit to hospital, period of using nicotine patch and total times of visiting clinic were significant difference between the two groups. Therefore, the total number of visits to the clinic was the only significant factor according to multivariate analysis. CONCLUSION: The results of multivariate analysis has shown that the factor that is most associated with success of smoking cessation is the total number of visits to smoking-cessation clinic. This means that the more people who smoke consult with doctor and the longer the period of utilizing the smoking-cessation clinic is, the more successful they in smoking cessation.
