ABSTRACT

Abstract Objective: To compare the clinical efficacy of Ghana Shyam's TEFF space maintainer with band and loop space maintainer. Material and Methods: This split-mouth randomized clinical trial was conducted on 20 children (7-9 years), requiring bilateral/contralateral space maintenance therapy. After consent, all the selected children received both space maintainers and were followed up periodically every six months up to two years for clinical assessment according to the evaluation protocol. Instructions and motivation were given to the children. OHI (S) index was recorded before cementation of appliances. The following aspects were analyzed: 1) efficiency and function; 2) dislodgement due to cement loss; 3) breakage at soldered parts; 4) carious or gingival inflammation affecting abutment teeth; 5) distortion of band; 6) being embedded in gingival tissues / gingival slopping; and 7) initial and final S-OHIS scores. Statistical analysis was done using Fisher exact test for survival time and Chi-square test for chewing efficiency. Results: At 24th-month follow-up, 14 Ghana Shyam's TEFF space maintainers and 16 band and loop space maintainers were found to be intact. Conclusion: As both space maintainers demonstrated similar mean survival time Ghana Shyam's TEFF space maintainer with the added advantage of being functional can be recommended as an effective alternative to band and loop in premature loss of primary molar clinical situations.

ABSTRACT

The early loss of anterior primary teeth is frequent in the pediatric dentistry clinic and it is associated with extensive caries or dental trauma. The purpose of this study was to report two cases of aesthetic-functional rehabilitation due to early loss of anterior primary teeth, with the aid of fixed space maintainers of the tube-bar type. In the first case, a 4-year-old female child referred for dental treatment is reported. During clinical examination, several restorations in the posterior teeth and absent upper anterior teeth were observed. The treatment plan consisted of installing a tube-bar type upper fixed space maintainer, considering the ease of installation and adaptation for young children. The second case reports a 5-year-old male patient with the main complaint of dental caries. Clinical examination revealed extensive coronary destruction in the upper central incisors, as well as in the posterior teeth that presented carious lesions in several teeth. For the case planning, it was determined the need for the posterior teeth restorations and the exodontia of the anterior dental remnants. After oral rehabilitation, an aesthetic-functional space maintainer of the tube-bar type was made and installed. In both cases, prosthetic rehabilitation was fundamental to maintain the space until the eruption of the permanent successors, avoiding future damages, as well as to restore the children's self-esteem and introversion behavior. perda precoce dos dentes decíduos anteriores é frequente na clínica odontopediátrica e está associada a processos de cáries extensas ou traumas dentários. O objetivo do presente estudo foi relatar dois casos clínicos de reabilitação estético-funcional após perda precoce de dentes decíduos anteriores, com auxílio de mantenedores de espaço fixo do tipo tubo-barra. O primeiro caso refere-se a uma criança do sexo feminino, quatro anos de idade, encaminhada para atendimento odontológico. Durante exame clínico, observou-se restaurações nos dentes posteriores e ausência dos dentes anteriores superiores. O plano de tratamento consistiu na instalação de um mantenedor de espaço fixo superior do tipo tubo-barra, levando em consideração a facilidade de instalação e adaptação para crianças com pouca idade. O segundo caso reporta um paciente de sexo masculino, cinco anos de idade, tendo como queixa principal a presença de cáries dentárias. Ao exame clínico, foi notado extensas destruições coronárias nos incisivos centrais superiores, assim como nos dentes posteriores. Para o planejamento do caso, determinou-se a necessidade de restaurações dos dentes posteriores e as exodontia dos remanescentes dentários anteriores. Após adequação do meio bucal foi confeccionado e instalado um mantenedor de espaço estético-funcional do tipo tubo-barra. Em ambos os casos, a reabilitação protética foi fundamental para a manutenção do espaço até a irrupção dos sucessores permanentes, evitando prejuízos futuros, devolvendo a autoestima e a mudança do comportamento de introversão das crianças.(AU)

ABSTRACT Introduction: Space maintainers play an important role in orthodontic treatments involving premolar extractions, especially in cases with bad occlusion and marked crowding. The orthodontist must know several options of anchoring and choose the best method that would fulfill the treatment objectives for that patient. Objective: To compare the effects of maximum anchors like the double transpalatal arch (double ATP) and the transpalatal arch and Nance button combination (transpalanance) as space maintainers during cuspids retraction in patients undergoing upper bicuspid extractions. Methods: A universe of 100 patients, aged 14-25 years, was selected from among the patients presenting to the Orthodontics Specialty Clinic of the posgraduate School Dr. José Apolo Pineda, Pilot School of Dentistry, University of Guayaquil during the years of 2017 and 2018. Totally, 26 patients were recruited, of whom 13 were placed on transpalanance and the other 13 on double ATP, prior to the extraction of their first upper bicuspid. Post-extraction spaces were measured and compared with post-retraction spaces of upper cuspids. Results: With an average of 4-5 mm, transpalanance maintained the greatest amount of space at the end of canine retraction. It is expected that the results obtained in this study will be useful to orthodontists, providing them with information regarding the most effective space maintainers for treatments with extractions. Conclusion: The maximum anchor that showed the greatest efficacy was transpalanance since it maintained a greater amount of space during its time of use.


RESUMEN Los mantenedores de espacio desempeñan un papel importante en los tratamientos de ortodoncia que incluyen extracciones premolares, especialmente en casos de oclusión grave y aglomeración marcada. El ortodoncista debe conocer varias opciones de anclaje y elegir el mejor método que cumpla con los objetivos de tratamiento para ese paciente. Objetivo: comparar los efectos de los anclajes máximos como el arco transpalatino doble (ATP doble) y el arco transpalatino y la combinación del botón de Nance (transpalanance) como mantenedores de espacio durante la retracción de los cúspides en pacientes sometidos a extracciones de premolares superiores. Métodos: Se seleccionó un universo de 100 pacientes, de 14 a 25 años, entre los pacientes que acudieron a la Clínica de Ortodoncia de la Escuela de Posgrado de la Facultad Piloto de Odontología, Universidad de Guayaquil durante los años de 2017 y 2018. En total, se reclutaron 26 pacientes, de los cuales 13 fueron colocados en transpalanance y los otros 13 en doble ATP, antes de la extracción de su primer premolar superior. Los espacios posteriores a la extracción se midieron y compararon con los espacios posteriores a la retracción de los caninos superiores. Resultados: Con un promedio de 4 a 5 mm, el transpalanance mantuvo la mayor cantidad de espacio al final de la retracción de caninos. Se espera que los resultados obtenidos en este estudio sean útiles para los ortodoncistas, proporcionándoles información sobre los mantenedores de espacio más efectivos para los tratamientos con extracciones. Conclusión: el ancla máxima que mostró la mayor eficacia fue la transpalanidad, ya que mantuvo una mayor cantidad de espacio durante su tiempo de uso.

ABSTRACT

A cárie dentária na atualidade continua sendo um problema de saúde pública, apesar do grande avanço tecnológico e de novas técnicas para preveni-la. Esta, se caracteriza como uma doença multifatorial, infecciosa e dieta dependente. Além disso, se apresenta mais prevalente em crianças devido a maior ingestão de carboidratos e sacarose, além de falta de controle motor durante higienização. O tratamento é dividido em quatro fases: sistêmica, preparatória, reabilitadora e manutenção. Assim, este artigo tem como objetivo relatar o caso clínico de criança de 6 anos de idade acometida por cárie precoce, descrevendo o tratamento em fases, compreendendo a reabilitação com resina composta nas unidades acometidas pela cárie e mantenedor de espaço com intuito de substituir as unidades na qual foram perdidas. Ao final do tratamento proposto, a paciente demonstrou satisfeita com o resultado, o que foi possível devolver a qualidade de vida da mesma. (AU).


Currently, caries is still a major problem in public healthy, despite the great technological development and new prevention methods. The caries is a multifactorial, infectious, transmissible and diet dependent disease. It is more prevalent in kids because of their higher intake of carbohydrates and sucrose, and the lack of motor control during the toothbrushing. The treatment is divided into four stages: systemic, preparatory, rehabilitative and maintenance. Thus, this article aims to report a case about a six-year-old child affected by early childhood caries, describing the treatment phases, comprising rehabilitation with composite created in units and space maintainer to replace the missing units. At the end of the treatment, the patient showed satisfaction with the outcome, restoring the quality of her life. (AU).

ABSTRACT

Space reduction after the premature loss of the second primary molars is one of the important factors affecting the occurrence of malocclusion, often increasing the demand for orthodontic treatment. It has great significance to select and apply appropriate appliances to maintainthe space soon after the premature loss of the second primary molars. The space should be maintained until the adjacent teeth and the successors erupt successfully. This review summarizes the selection and clinical application of the space maintainer for the premature loss of the second primary molars in different periods, to improve dentists′ awareness of the importance of space maintainers and to provide advises for clinical choices.

ABSTRACT

We aimed to evaluate the effects of onlay-type grafted human freeze-dried corticocancellous bone block (FDBB) and deproteinized bovine bone with collagen (DBBC) loaded with Escherichia coli-produced recombinant human bone morphogenetic protein-2 (ErhBMP-2) on space maintenance and new bone formation in rat calvaria. Collagen sponge (CS), FDBB, or DBBC disks (8×4 mm) with ErhBMP-2 (2.5 µg) were implanted onto the calvaria of male Sprague-Dawley rats, whereas CS with buffer was implanted onto the calvaria as controls (n=20/carrier). Rats were killed at 2 or 8 weeks post-surgery for histologic and histomorphometric analyses; total augmented area, new bone area, and bone density were evaluated. At both time-points, all ErhBMP-2 groups showed significantly higher new bone area and bone density than the control group (p<0.05). ErhBMP-2/FDBB and ErhBMP-2/DBBC groups showed significantly higher total augmented area than ErhBMP-2/CS group (8 weeks), and ErhBMP-2/FDBB group showed significantly higher new bone area and bone density than ErhBMP-2/DBBC group (p<0.05). ErhBMP-2/CS group showed the highest bone density (p<0.05). Combining ErhBMP-2 with FDBB or DBBC could significantly improve onlay graft outcomes, by new bone formation and bone density increase. Moreover, onlay-grafted FDBB and DBBC with ErhBMP-2 could be an alternative to autogenous block onlay bone graft.

ABSTRACT

The aim of this study was to evaluate the effect of human freeze-dried bone block (FDBB) and deproteinized bovine bone with collagen (DBBC) on bone formation when applied as an onlay graft in rat calvariums. Thirty male Sprague-Dawley rats received collagen sponge (control), FDBB, or DBBC onlay grafts trimmed into 8-mm disks measuring 4-mm height. Each graft was secured onto the calvarium surface using horizontal mattress sutures. Rats in each group were killed at 2 (n=5) or 8 (n=5) weeks postoperatively for histologic and histomorphometric analysis. The total augmented area (mm²), new bone area (mm²), and bone density (%) were measured. The FDBB and DBBC groups showed significantly more new bone formation and bone density than the control group at 2 and 8 weeks. The increased new bone area was significantly greater in the FDBB group than in the DBBC group (p<0.05). The total augmented area was significantly higher in the FDBB and DBBC groups at 2 and 8 weeks than in the control group (p<0.05), and at 8 weeks, the area was significantly decreased in the DBBC group compared to that in the FDBB group and the area at 2 weeks (p<0.05). Within the limitations of the present study, we concluded that onlay FDBB and DBBC grafts caused new bone formation through an osteoconductive mechanism. In addition, compared to FDBB, DBBC had less capacity to form new bone and maintain the space.

Objective:To compare the bone dimensional changes following tooth extraction alone with extraction plus ridge preservation ( using deproteinized boving bone mineral Bio-Oss? and bioresorbable collagen mambrane Bio-Gide?) in periodontal compromised extraction sockets .Methods: Eighteen molars of sixteen subjects requiring tooth extraction because of periodontal destruction were enrolled in this study .The subjects were assigned to the control group ( extraction alone , EXT) or to the test group ( ridge-preservation procedure with Bio-Oss? and Bio-Gide?, RP) .Parallel periapical X-rays and cone-beam computed tomography ( CBCT ) scans were taken immediately after tooth extraction alone or plus ridge-preservation ( baseline ) and 6 months later .The changes of horizontal ridge width and vertical ridge height were assessed .Results:At the central buccal aspect , the ridge height increased 2 .9 mm in RP group, and reduced 1.0 mm in EXT group.At the distal buccal aspect , the ridge height increased 1.45 mm in RP group, and reduced 1.45 mm in EXT group.The differences between the groups reached statistical significance (P0.05).Conclusion:The ridge-preservation approach using Bio-Oss? in combination with Bio-Gide? can significantly increase vertical ridge height and horizontal ridge width after tooth extraction compared with extraction alone in periodontal compromised molars .

ABSTRACT

The loss of permanent tooth in adolescent patient makes it highly desirable to perform the necessary treatment as expeditiously as possible. When a tooth is lost, space maintenance should be provided immediately after extraction to prevent tipping, tilting, or rotation of the abutment teeth or eruption of the opposing teeth. To ascertain function and esthetics, immediate treatments include interim space maintainer approaches. So this report describes a new technique for the management of the lost mandibular second premolar with a fixed functional interim space maintainer in a 12-year-old boy. The technique offers provision of conservative and cost effective fixed interim space maintainer which can restore correct, harmonious, and nondestructive occlusal relationships.

ABSTRACT

Loss of anterior teeth is a psychological trauma both for parent as well as children. Children presenting with lost anterior teeth along with thumb sucking habit require not only attention for aesthetics, function and space maintenance but also an appliance for habit breaking. Depending on many clinical and economic factors, a course of treatment is decided by the dentist in consultation with parent and child. A new technique is presented in which a child with anterior teeth missing and concomitant ‘‘thumb sucking’’ habit was given a fixed functional space maintainer' with incorporated palatal crib.

ABSTRACT

Amelogenesis imperfecta (AI) is an inherited disorder which results in enamel defects. The main clinical characteristics are extensive loss of tooth tissue, poor esthetics and tooth sensitivity. Early recognition followed by appropriate preventive care and oral rehabilitation is essential in the successful management of AI. This clinical report describes the management of a 3 year-old girl with a hypocalcified type of Al. The patient presented early extensive tissue loss of the superior incisors and molars, with general dentinal sensitivity. The first phase of the treatment was preventive measures to improve dental and periodontal health. On the second phase the anterior teeth were extracted and the molars were endodontic treated and covered with stainless steel crowns. A functional appliance was made to rehabilitate the anterior extracted teeth on the third phase. The performed treatment achieved satisfactory esthetic and function, which resulted in an improvement in the child's quality of life.


Amelogênese imperfeita (AI) é uma doença hereditária que resulta em defeitos no esmalte. As principais características clínicas são a extensa perda de tecido dentário, estética prejudicada devido a coloração acastanhada dos dentes e sensibilidade dentária. O reconhecimento precoce seguido de cuidados preventivos adequados e reabilitação oral é essencial para o êxito nos tratamentos de AI. Este relato de caso clínico descreve o tratamento de uma criança de 3 anos com Al do tipo hipocalcificada. A paciente apresentava inicialmente extensas perdas de tecido nos incisivos superiores e molares, e uma sensibilidade dentinária generalizada. O tratamento reabilitador foi dividido em duas fases, na primeira fase do tratamento foram passadas à mãe e a criança medidas preventivas para melhorar a saúde dental e periodontal. Na segunda fase do tratamento os dentes anteriores foram extraídos e os molares foram tratados endodonticamente e cobertos com coroas de aço. Um aparelho funcional, visando manter o espaço dentário e oferecer uma estética de sorriso à paciente foi feito para reabilitar o arco anterior devido as extrações dentárias. O tratamento realizado foi satisfatório tanto na estética quanto na função, o que resultou em uma melhora na qualidade de vida da criança.
