ABSTRACT

Visceral leishmaniosis is a neglected tropical disease. We evaluated the spatial distribution of cases of visceral leishmaniosis in the state of Alagoas, Brazil. All cases of VL, registered by the health department, were analyzed and georeferenced. Results: Between 2008 and 2017, 97.1% of the municipalities presented sporadic classification of transmission. With temporal evolution, the incidence of cases of visceral leishmaniosis was concentrated in most municipalities in the microregion of Santana do Ipanema-AL. Space-time analysis, if considered, may promote the improvement of surveillance and control actions of visceral leishmaniosis.

Todos os casos de LV, registrados pela secretaria de saúde, foram analisados e georreferenciados. Entre 2008 e 2017, 97,1% dos municípios apresentaram classificação esporádica de transmissão. Com a evolução temporal, a incidência de casos de leishmaniose visceral se concentrou na maioria dos municípios da microrregião de Santana do Ipanema-AL. A análise espaço-tempo, se considerada, pode promover o aprimoramento das ações de vigilância e controle da leishmaniose visceral.

ABSTRACT Objective: To analyze the spatio-temporal dynamics of COVID-19 in the Rio de Janeiro state within the nine health regions, between March 2020 and December 2022. Methods: The Poisson model with random effects was used to smooth and estimate the incidence of COVID-19 hospitalizations reported in the Influenza Epidemiological Surveillance Information System (SIVEP-Gripe) to verify the synchronicity of the epidemic in the state. Results: The COVID-19 epidemic in the state is characterized by the presence of seven peaks during the analyzed period corresponding to seven found. An asynchrony in hospitalizations was identified, varying according to the different virus variants in the nine health regions of the state. The incidence peaks of hospitalizations ranged from 1 to 12 cases per 100,000 inhabitants during the pandemic. Conclusion: This spatio-temporal analysis is applicable to other scenarios, enabling monitoring and decision-making for the control of epidemic diseases in different areas.


RESUMO Objetivo: Analisar a dinâmica espaço-temporal de COVID-19 no estado do Rio de Janeiro nas nove regiões de saúde, entre março de 2020 e dezembro de 2022. Métodos: Utilizou-se o modelo de Poisson com efeitos aleatórios para suavizar a curva de incidência de hospitalizações por COVID-19 notificadas no Sistema de Informação da Vigilância Epidemiológica da Gripe (Sivep-Gripe) para verificar a sincronicidade da epidemia no estado. Resultados: A epidemia de COVID-19 no estado é caracterizada pela presença de sete picos no período analisado correspondentes a sete variantes encontradas. Identificou-se uma assincronicidade nas hospitalizações, variando de acordo com as diferentes variantes do vírus nas nove regiões de saúde do estado. Os picos de incidência das hospitalizações variaram de 1 a 12 casos por 100 mil habitantes no decorrer da pandemia. Conclusão: Essa análise espaço-temporal é extensível em outros cenários, sendo possível o monitoramento e a tomada de decisões de controle de doenças epidêmicas em várias áreas.

ABSTRACT

The aim of this research was to conduct a comprehensive spatial-temporal analysis of the population affected by congenital heart anomalies assisted at the Pediatric Cardiology Outpatient Department at the distinguished Western Paulista reference hospital. We conducted a retrospective study involving the analysis of electronic database records and patient medical charts for individuals diagnosed with congenital heart disease during the period from July 2013 to July 2018. A total of 298 medical records were selected for the analysis of variables encompassing the ICD-10 codes, gender, spatial distribution, and temporal trends. It was possible to observe that septal defects were the most prevalent congenital heart abnormalities, and there was no gender-based difference. An increase in diagnoses was noted from 2014, coinciding with the implementation of the "heart test," and 51% of the cases were from Presidente Prudente, with a higher concentration of cases in the industrial park area. There is an association between cardiac congenital malformations and an adverse environmental context. The findings can inform public health policies aimed at reducing the exposure of the most vulnerable population in pursuit of improving health indicators (AU).


O objetivo deste trabalho foi analisar a distribuição espaço-temporal dos pacientes com cardiopatias congênitas aten-didos no Ambulatório de Cardiologia Pediátrica do Hospital de referência do Oeste Paulista. Realizamos um estudo retrospectivo com análise de dados de base eletrônica e prontuários dos pacientes diagnosticados com cardiopatia congênita entre os períodos de julho de 2013 a julho de 2018. Foram selecionados 298 prontuários para análise das variáveis de CID-10, gênero, distribuição espacial e série temporal. Foi possível observar que os defeitos septais foram as cardiopatias mais prevalentes, não houve diferença entre os gêneros. Notou-se aumento do diagnóstico a partir de 2014, com implementação do teste do coraçãozinho e 51% dos casos eram da cidade de Presidente Prudente, com maior concentração de casos na região do parque industrial. Há uma relação na incidência das malformações cardíacas com o meio ambiente desfavorável. Os resultados encontrados podem guiar políticas de saúde pública, visando reduzir a exposição da população mais vulnerável, na busca da melhora nos índices de saúde (AU).

Introducción: Las comunidades de macroalgas marinas del Parque Nacional Guanahacabibes no han sido estudiadas desde el punto de vista cuantitativo, a pesar de la influencia que la vegetación submarina tiene sobre la estructura y el funcionamiento de los ecosistemas costeros, componente que hasta ahora ha sido subestimado en la región. Objetivo: Evaluar la variación temporal y espacial de los géneros de macroalgas en dos sitios de María La Gorda, sur del Parque Nacional de Guanahacabibes, y evaluar la salud del arrecife de acuerdo con la cobertura de los diferentes morfo-tipos de macroalgas predominantes. Metodología: El muestreo fue cuatrimestral entre febrero 2014 y marzo 2017 en Yemayá y Laberinto, mediante buceo autónomo a una profundidad de 10 m. La cobertura algal fue estimada siguiendo la metodología AGRRA. Se usaron ocho transectos de 20 m por sitio de muestreo de acuerdo con las condiciones del arrecife y la forma del fondo. Cada cinco metros se colocó un cuadrante de 25 x 25 cm. Se calculó el porcentaje de cobertura que aporta cada género y de los grupos morfofuncionales a la cobertura total. Resultados: Los géneros dominantes durante el periodo de estudio fueron: Dictyota, Lobophora y Halimeda. Las algas carnosas y calcáreas mostraron mayor cubrimiento en Laberinto (carnosas= 57.8 ±15.6; calcáreas = 8.3 ± 6.8) que en Yemayá, (carnosas = 47.3 ± 23.1; calcáreas = 8.5 ± 9.3). Las formas costrosas son más abundantes en Yemayá (17.1 ± 15.1) que en Laberinto (7.7 ± 10). El índice de las carnosas fue superior en Laberinto (225.7 ± 110.2). Conclusión: La disminución en los niveles de cobertura de algas costrosas y la dominancia de formas carnosas como Dictyota y Lobophora en la zona de estudio, evidencian el deterioro en ambos sitios, a pesar de las políticas de conservación de parques nacionales.


Introduction: The macroalgal communities of the Guanahacabibes National Park have been poorly studied from the quantitative point of view, despite the influence that underwater vegetation has on the structure and functioning of coastal ecosystems, a component that until now has been underestimated in the Cuba region. Objetive: To evaluate the temporal and spatial variation of the macroalgal genera in two sites of María La Gorda, south of the Guanahacabibes National Park, and evaluate the reef health according to the coverage of the different predominant macroalgal morphotypes. Methodology: Sampling took place quarterly between February 2014 and March 2017, in Laberinto and Yemayá by SCUBA diving at a depth of 10 m. The algal coverage was estimated following the AGRRA methodology. Eight 20 m transects per sampling site were used according to reef conditions and bottom shape. Every five meters a 25 x 25 cm square was placed. The coverage (%) contributed per genus and morpho-functional group to the total coverage found was calculated. Results: The dominant genera during the study period were: Dictyota, Lobophora and Halimeda. Fleshy and calcareous algae showed greater coverage in Laberinto (fleshy= 57.8 ± 15.6; calcareous = 8.3 ± 6.8) than in Yemayá (fleshy = 47.3 ± 23.1; calcareous = 8.5 ± 9.3). Crustose algae were more abundant in Yemayá (17.1 ± 15.1) than in Laberinto (7.7 ± 10). The fleshy index was higher in Laberinto (225.7 ± 110.2). Conclusion: The decrease in the levels of crustose algal cover, as well as the dominance of fleshy forms Dictyota and Lobophora in the study area, shows the deterioration in both sites, despite the conservation policies of national parks.

Objective:To analyze the epidemic status and spatial and temporal cluster distribution of visceral leishmaniasis (VL) in Gansu Province, and to provide evidence for making precise prevention and control measures and epidemic surveillance.Methods:The data of VL cases in Gansu Province reported in the China Disease Prevention and Control Information System Infectious Disease Reporting Information Management System from 2015 to 2021 were collected, descriptive epidemiology analysis was performed (three distribution of the disease). SaTScan v9.6 and ArcMap 10.7 were used for spatio-temporal cluster analysis.Results:A total of 368 VL cases were reported from 2015 to 2021, 89.13% (328/368) were concentrated in Longnan City and Gannan Tibetan Autonomous Prefecture. The number of VL cases increased year by year from 2015 to 2017, reaching a peak in 2017 (79 cases, accounting for 21.47%), and had decreased year by year since 2018. There were cases every month throughout the year, and the peak was in April in spring (38 cases). The ratio of male to female patients was 1.3 ∶ 1.0 (208/160). Scattered children aged less than 5 years old were more common (41.30%, 152/368). The spatio-temporal cluster analysis showed that both the spatial and temporal aggregation zones of VL in Tanchang County and Chengxian County were of statistical significance ( P < 0.05). Conclusion:The incidence of VL in Gansu Province is showing a low epidemic trend, having spatio-temporal aggregation, and precise monitoring and prevention and control should be strengthened in epidemic areas with different prevalence levels.

Objective@#To investigate the spatio-temporal clustering characteristics of influenza in Yinzhou District, Ningbo City, Zhejiang Province from 2017 to 2021, so as to provide insights into prevention and control of influenza. Methods Data of influenza in Yinzhou District from 2017 to 2021 were collected from the Chinese Disease Prevention and Control Information System. The software ArcGIS 10.8 was employed for spatial autocorrelation analysis, and SaTScan 10.1 was employed for spatio-temporal scanning to analyze the temporal and spatial clustering characteristics of influenza incidence in Yinzhou District. @*Methods@#Data of influenza in Yinzhou District from 2017 to 2021 were collected from the Chinese Disease Prevention and Control Information System. The software ArcGIS 10.8 was employed for spatial autocorrelation analysis, and SaTScan 10.1 was employed for spatio-temporal scanning to analyze the temporal and spatial clustering characteristics of influenza incidence in Yinzhou District.@*Results@#Totally 60 543 influenza cases were reported in Yinzhou District from 2017 to 2021, with an incidence of 0.76%. The incidence of influenza peaked in December 2019 (9.35%) and January 2020 (9.28%) during the period between 2017 and 2021. Spatial autocorrelation analysis showed that there was a positive spatial correlation of influenza incidence in Yinzhou District from 2018 to 2021 (all P<0.05), and a high clustering in 2019 and 2021. Zhonghe Street showed a low-high clustering from 2017 to 2020; Jiangshan Town showed a low-high clustering in 2017 and 2020, and a high-high clustering in 2019 and 2021; Shounan Street showed a high-high clustering from 2018 to 2020; Yunlong Street showed a high-high clustering in 2021. Spatio-temporal scanning analysis showed that the class Ⅰ clusters were located in the central region which centered in Dongqianhu Town, with aggregation time in August 2017, in the northwest region with aggregation time in December and January from 2018 to 2020, and in the west region with aggregation time in August 2021.@* Conclusion @#The incidence of influenza in Yinzhou District from 2017 to 2021 showed a spatio-temporal clustering in the northwestern region in winter and summer.

ABSTRACT

c-Myc protein encoded by c-Myc (cellular-myelocytomatosis viral oncogene) gene regulates the related gene expression through the Wnt/β-catenin signaling pathway, and has received extensive attention in recent years. The purpose of this study was to express Helicoverpa armigera c-Myc gene (Ha-c-Myc) by using prokaryotic expression system, prepare the polyclonal antibody, examine the spatio-temporal expression profile of Ha-c-Myc, and investigate the possible function of Ha-c-Myc in regulating H. armigera sterol carrier protein-2 (SCP-2) gene expression. The Ha-c-Myc gene was amplified by PCR and cloned into a prokaryotic expression plasmid pET-32a(+). The recombinant plasmid pET-32a-Ha-c-Myc was transformed into Escherichia coli BL21. IPTG was used to induce the expression of the recombinant protein. Protein was purified by Ni2+-NTA column and used to immunize New Zealand rabbits for preparing the polyclonal antibody. The Ha-c-Myc expression levels in different developmental stages (egg, larva, prepupa, pupa, and adult) of H. armigera and different tissues (midgut, fat body, head, and epidermis) of the prepupa were determined by real-time quantitative reverse transcription PCR (qRT-PCR). Ha-c-Myc siRNA was synthesized and transfected into H. armigera Ha cells. The relative mRNA levels of Ha-c-Myc and HaSCP-2 in Ha cells were detected by qRT-PCR. Results showed that the pET-32a-Ha-c-Myc recombinant plasmid was constructed. The soluble Ha-c-Myc protein of about 65 kDa was expressed in E. coli. The polyclonal antibody was prepared. Western blotting analysis suggested that the antibody had high specificity. Enzyme linked immunosorbent assay (ELISA) showed that the titer of the antibody was high. Ha-c-Myc gene expressed at all developmental stages, with high levels in the early and late instars of larva, and the prepupal stage. Tissue expression profiles revealed that Ha-c-Myc expressed in various tissues of prepupa, with high expression level in the midgut, but low levels in the epidermis and fat body. RNAi results showed that the knockdown of Ha-c-Myc expression significantly affected transcription of HaSCP-2, leading to a 50% reduction in HaSCP-2 mRNA expression level. In conclusion, the Ha-c-Myc was expressed through a prokaryotic expression system, and the polyclonal anti-Ha-c-Myc antibody was obtained. Ha-c-Myc may promote the expression of HaSCP-2 and play an important role in the lipid metabolism of H. armigera. These results may facilitate further study on the potential role and function mechanism of Ha-c-Myc in H. armigera and provide experimental data for exploring new targets of green pesticides.

ABSTRACT

Geographical and temporal weighted regression (GTWR) model is a local regression linear model, which indirectly reflects the spatio-temporal non-stationary characteristics of the study data by calculating the trends for changes in parameters with space and time. Recently, GTWR model has become one of the hot topics in the study on spatio-temporal heterogeneity of diseases. This review summarizes the basic principles and study methods of the GTWR model, and describes the applications of this model in epidemiology.

En la sociedad de hoy los delitos vienen incrementándose y particularmente en la ciudad de Bogotá, lo que ha causado muchos inconvenientes a la Policía Nacional de Colombia, así como también a los centros de seguridad ciudadana. Ante esta situación, se ha propuesto una predicción de tiempo-espacio en los puntos críticos de crímenes y delitos, con la ayuda de inteligencia artificial. Por consiguiente, este trabajo tiene como objetivo analizar, resumir, interpretar y evaluar las distintas técnicas de predicción espacio-temporal de la delincuencia con un panorama inteligente. Por la propia naturaleza de la investigación, se utilizó una metodología de enfoque descriptivo-cualitativo, con la cual se diseñaron fichas de observación estructurada para sistematizar información de cinco bases de datos: Scopus, Web of Science, IEEE, ACM, Springer; dichas publicaciones comprenden desde 2019 hasta junio de 2021. En consecuencia, se encontraron en total 3015 estudios, después del proceso de cribado y verificación de los criterios de exclusión e inclusión, se seleccionaron 132 artículos, luego se aplicaron preguntas Psicólogo Interno Residente (PIR), quedando así 18 artículos. Los principales hallazgos encontrados indican que los algoritmos de redes neuronales resultaron ser uno de los métodos más eficaces para la detección de puntos críticos de delincuencia, dado que los grandes avances de la tecnología coadyuvarían en los próximos años a predecir de forma rápida y eficaz los actos delictivos y los crímenes ubicados en cualquier región del continente latinoamericano.


In today's society, crimes are increasing, particularly in the city of Bogota, which has caused many inconveniences to the National Police of Colombia, as well as to the citizen security centers. Given this situation, a time-space prediction of crime and crime hotspots has been proposed with the help of artificial intelligence. Therefore, this paper aims to analyze, summarize, interpret and evaluate the various techniques of space-time prediction of crime with an intelligent view. Due to the very nature of the research, a descriptive-qualitative approach methodology was used, with which structured observation sheets were designed to systematize information from five da-tabases: Scopus, Web of Science, IEEE, ACM, Springer; these publications span from 2019 to June 2021. Consequently, a total of 3015 studies were found, after the screening process and verification of exclusion and inclusion criteria, 132 articles were selected, then questions were applied Psychologist Internal Resident (PIR), thus leaving 18 articles. The main findings indicate that neural network algorithms proved to be one of the most effective methods for the detection of crime hotspots, given that the great advances in technology would help in the coming years to quickly and effectively predict criminal acts and crimes located in any region of the Latin American continent.

Diante desta situação, foi proposta uma previsão tempo-espacial de hotspots de crime com a ajuda da inteligência artificial. Portanto, este documento visa analisar, resumir, interpretar e avaliar as diversas técnicas de previsão espaço-temporal do crime com uma visão inteligente. Devido à própria natureza da pesquisa, foi utilizada uma metodologia de abordagem descritiva-qualitativa, com a qual foram elaboradas fichas de observação estrutura-das para sistematizar informações de cinco bancos de dados: Scopus, Web of Science, IEEE, ACM, Springer; estas publicações abrangem o período de 2019 a junho de 2021. Consequentemente, foi encontrado um total de 3015 estudos, após o processo de triagem e verificação dos critérios de exclusão e inclusão, 132 artigos foram selecionados, depois foram aplicadas perguntas ao Psicólogo em Residência (PIR), deixando 18 artigos. As principais descobertas indicam que os algoritmos de redes neurais provaram ser um dos métodos mais eficazes para a detecção de hotspots de crime, dado que os grandes avanços na tecnologia ajudarão nos próximos anos a prever rápida e efetivamente atos criminosos e crimes localizados em qualquer região do continente latino-americano.

Abstract Objectives: to analyze the trend of morbidity and mortality indicators due to acute diarrheal diseases in children under five years old in Piauí. Methods: ecological study with data from the Information Technology Department at the Public Health System. The indicators of hospitalization rate and coefficient of mortality from the disease between 2000 and 2019 were calculated. A descriptive analysis of the indicators was carried out in the studied period and by the macro-regions in the State. For trend analysis, the simple linear regression model with log-transformation was used. Trends were classified as increasing, decreasing and stable, with a significance level of 5%. Results: the average on hospitalization rate was higher in the semi-arid macro-region (36.6/1000 children under five years old) and lower in Teresina (14.9/1000 children under five years old). The mean mortality coefficients were higher in the coastal macro-region (0.98/1000 live births) and lower in Teresina (0.47/1000 live births). The indicators showed a downward trend in all analyzed locations (p<0.05). A turning point was noted from 2009, with a significant reduction in hospitalization rates in the savanna and semi-arid macro-regions. Conclusion: indicators of morbidity and mortality due to acute diarrheal diseases in children under five years old showed a downward trend in Piauí between 2000 and 2019, with differences in trends between the evaluated macro-regions.


Resumo Objetivos: analisar a tendência de indicadores de morbimortalidade por doenças diarreicas agudas em menores de cinco anos no Piauí. Métodos: estudo ecológico com dados do Departamento de Informática do Sistema Único de Saúde. Calculou-se os indicadores taxa de internação e coeficiente de mortalidade pela doença entre 2000 e 2019. Realizou-se análise descritiva dos indicadores no período estudado e pelas macrorregiões do estado. Para análise da tendência, foi utilizado o modelo de regressão linear simples com log-transformação. As tendências foram classificadas como crescentes, decrescentes e estáveis, com nível de significância de 5%. Resultados: a média das taxas de internação foi maior na macrorregião semiárido (36,6/1000 menores de cinco anos) e menor em Teresina (14,9/1000 menores de cinco anos). A média dos coeficientes de mortalidade foi maior na macrorregião litoral (0,98/1000 nascidos vivos) e menor em Teresina (0,47/1000 nascidos vivos). Os indicadores mostraram tendência de redução em todos os locais analisados (p<0,05). Notou-se um ponto de inflexão a partir de 2009, com redução significativa das taxas de internação nas macrorregiões cerrados e semiárido. Conclusão: os indicadores de morbimortalidade por doenças diarreicas agudas em menores de cinco anos mostraram tendência de redução no Piauí entre 2000 e 2019, com diferenças das tendências entre as macrorregiões avaliadas.

ABSTRACT

Ao longo das três versões da Política Nacional de Atenção Básica (PNAB) alterações foram realizadas em relação à estruturação da Estratégia Saúde da Família (ESF), com destaque para modificações em relação ao caráter prioritário da ESF como estratégia de organização e de cuidado na atenção básica. O objetivo foi analisar tendências temporais de indicadores referentes à ESF sob o olhar das três versões da PNAB 2006, 2011 e 2017. Estudo descritivo de tendência temporal de indicadores selecionados a partir de modelo lógico construído por componentes referentes à ESF nas três versões da PNAB. O modelo lógico foi elaborado baseado nos componentes Território/Adscrição, Equipes, Processo de Trabalho, Planejamento e Gestão do Território, e Cuidados à Grupos Prioritários pelas Equipes de Saúde da Família, sendo cada um representado por indicadores selecionados. A construção das séries temporais entre 2007 e 2020, nacional e regionais, foi realizada utilizando o software Joinpoint. A maioria dos indicadores apresentou tendência de crescimento nos primeiros segmentos temporais identificados pelos modelos, seguidos por seguimentos de estabilidade ou queda, principalmente após o ano de 2017. Destaca-se o indicador Número de Agentes Comunitários de Saúde que apresentou queda após 2017 na maioria das regiões geográficas e no Brasil. A PNAB 2017 pode ter proporcionado um desestimulo à continuidade e ampliação da ESF como modelo prioritário da atenção básica, ao permitir e financiar novos arranjos e processos de trabalhos de equipes.


A lo largo de las tres versiones de la Política Nacional de Atención Básica (PNAB) alteraciones fueron realizadas en relación con la estructuración de la Estrategia Salud de la Familia (ESF), con destaque para modificaciones en relación con el carácter prioritario de la ESF como estrategia de organización y de cuidado en la atención básica. El objetivo fue analizar tendencias temporales de los indicadores referentes a la ESF desde la perspectiva de las tres versiones de la PNAB, 2006, 2011 y 2017. Estudio descriptivo de la tendencia temporal de los indicadores seleccionados a partir del modelo lógico construido por componentes referentes a la ESF en las tres versiones de la PNAB. El modelo lógico fue elaborado basado en los componentes Territorio/Adscripción, Equipos, Proceso de Trabajo, Planificación y Gestión del Territorio y Atención a Grupos Prioritarios por Equipos de Salud de la Familia, siendo cada uno representado por indicadores seleccionados. La construcción de las series temporales entre 2007 y 2020, nacional y regional, se realizó utilizando el software Joinpoint. La mayoría de los indicadores presentaron una tendencia de crecimiento en los primeros segmentos temporales identificados por los modelos, seguidos por segmentos de estabilidad o la caída, principalmente después del año 2017. Se destaca el indicador Número de Agentes Comunitarios de Salud que presentó una caída después de 2017 en la mayoría de las regiones geográficas y en Brasil. La PNAB 2017 puede haber proporcionado un desaliento a la continuidad y ampliación de la ESF como modelo prioritario de la atención básica, al permitir y financiar nuevos arreglos y procesos de trabajo de los equipos.


Throughout the three editions of the Brazilian National Primary Health Care Policy (PNAB), changes were made in relation to the structure of the Family Health Strategy (FHS), with emphasis on modifications concerning the priority nature of the FHS as an organization and care strategy in primary health care. The objective was to analyze temporal trends in indicators related to the FHS from the perspective of the three PNAB editions: 2006, 2011, and 2017. This is a descriptive study of the temporal trend of indicators selected from a logical model constructed by components related to the FHS in the three editions of the PNAB. The logical model was developed based on the components Territory/Enrollment, Teams, Work Process, Territory Planning and Management, and Care for Priority Groups by Family Health Teams, each one being represented by selected indicators. The construction of the national and regional time series between 2007 and 2020 was carried out using the Joinpoint software. Most of the indicators showed an upward trend in the first time segments identified by the models, followed by segments of stability or decrease, especially after the year 2017. The indicator Number of community health workers stands out, which decreased after 2017 in most geographical regions and in Brazil. The 2017 PNAB may have discouraged the continuation and expansion of the FHS as the priority model of primary health care, by allowing and financing new teamwork arrangements and processes.

ABSTRACT OBJECTIVE To analyze the trends of cervical cancer mortality in Brazilian Southeastern states, and to compare them to Brazil and other regions between 1980 and 2020. METHODS Time series study based on data from the Sistema de Informações de Mortalidade (Brazilian Mortality Information System). Death data were corrected by proportional redistribution of deaths from ill-defined causes and cervical cancer of unspecified portion. Age-standardized and age-specific rates were calculated by screening target (25-39 years; 40-64 years) and non-target (65 years or older) age groups. Annual percentage changes (APC) were estimated by linear regression model with breakpoints. The coverage of Pap Smear exam in the Unified Health System (SUS) was evaluated between 2009 and 2020 according to age group and locality. RESULTS There were increases in corrected mortality rates both in 1980 and in 2020 in all regions, with most evident increments at the beginning of the series. There was a decrease in mortality nationwide between 1980-2020; however, the state of São Paulo showed a discrete upward trend in 2014-2020 (APC=1.237; 95%CI 0.046-2.443). Noteworthy is the trend increment in the 25-39 year-old group in all study localities, being sharper in the Southeast region in 2013-2020 (APC=5.072; 95%CI 3.971-6.185). Screening coverage rates were highest in São Paulo and lowest in Rio de Janeiro, with a consistent decline from 2012 onwards at all ages. CONCLUSIONS São Paulo is the first Brazilian state to show a reversal trend in mortality from cervical cancer. The changes in mortality patterns identified in this study point to the need for reorganization of the current screening program, which should be improved to ensure high coverage, quality, and adequate follow-up of all women with altered test results.


RESUMO OBJETIVO Analisar as tendências da mortalidade por câncer de colo de útero nos estados da região Sudeste e compará-las com o Brasil e demais regiões entre 1980 e 2020. MÉTODOS Estudo de série temporal com base nos dados do Sistema de Informações de Mortalidade. Os dados de óbito foram corrigidos por redistribuição proporcional das mortes por causas mal definidas e por câncer de útero de porção não especificada. Foram calculadas taxas padronizadas por idade e específicas por faixas etárias alvo de rastreamento (25-39 anos; 40-64 anos) e não alvo (65 anos ou mais). Variações percentuais anuais foram estimadas por modelo de regressão linear com pontos de quebra. A cobertura do exame Papanicolaou no Sistema Único de Saúde (SUS) foi avaliada entre 2009 e 2020 segundo faixa etária e localidade. RESULTADOS Foram verificados aumentos das taxas de mortalidade corrigidas tanto em 1980 como em 2020 em todas as regiões, com incrementos mais evidentes no início da série. Houve queda da mortalidade em todo o país entre 1980-2020, entretanto, o estado de São Paulo apresentou discreta tendência de aumento em 2014-2020 (APC=1,237; IC95% 0,046-2,443). Destaca-se o incremento da tendência no grupo de 25-39 anos em todas as localidades de estudo, mostrando-se mais acentuado na região Sudeste em 2013-2020 (APC=5,072; IC95% 3,971-6,185). As taxas de cobertura de rastreamento foram mais elevadas em São Paulo e mais baixas no Rio de Janeiro, com queda consistente a partir de 2012 em todas as idades. CONCLUSÕES São Paulo é o primeiro estado brasileiro a apresentar inversão de tendência da mortalidade por câncer de colo do útero. As mudanças nos padrões de mortalidade identificadas neste estudo apontam para a necessidade de reorganização do atual programa de rastreamento, que deve ser aperfeiçoado para garantir alta cobertura, qualidade e seguimento adequado de todas as mulheres com exames alterados.

ABSTRACT

O objetivo deste estudo é analisar os padrões espaçotemporais da mortalidade nas primeiras 24 horas de vida e sua evitabilidade no estado de Pernambuco no período de 2000 a 2019. Trata-se de estudo ecológico misto, tendo os trimestres e os municípios como unidades de análise. Constituíram-se como fontes de dados os sistemas de Informações sobre Mortalidade e Nascidos Vivos. Foram incluídos todos os óbitos no estado nas primeiras 24 horas de vida e por causas evitáveis, caracterizados conforme fatores de riscos proximais, intermediários e distais. A análise de evitabilidade dos óbitos utilizou a Lista Brasileira de Causas de Mortes Evitáveis por Intervenções do Sistema Único de Saúde. Foram calculadas as taxas de mortalidade e de evitabilidade nas primeiras 24 horas de vida. A análise espacial foi conduzida por meio da estatística scan, e a temporal, segundo o Modelo Autorregressivo Integrado de Médias Móveis. No período de 2000 a 2019, foram registrados 13.601 óbitos nas primeiras 24 horas de vida, sendo 10.497 (77,19%) por causas evitáveis. Destes, 5.513 (40,53%) foram classificados como reduzíveis por adequada atenção à mulher na gestação. As variáveis do nível proximal idade gestacional < 22 semanas (RA = 855,01) e peso ao nascer < 1.500 gramas (RA = 204,03) representam uma parcela importante dos óbitos ocorridos nas primeiras 24 horas de vida. As variáveis idade gestacional, sexo, peso ao nascer, tipo de gravidez, tipo de parto, idade da mãe e escolaridade materna foram estatisticamente significantes (p < 0,01). A varredura espacial identificou o aumento de clusters para ambas as taxas. A análise temporal evidenciou que a taxa de mortalidade e a taxa de mortalidade por causas evitáveis apresentaram padrão estacionário no período e a previsão revelou tendência crescente. Os resultados mostraram que a mortalidade nas primeiras 24 horas de vida e por causas evitáveis se configura como um problema de saúde pública no estado. Áreas de maior risco de óbito foram apontadas e a previsão revelou um aumento nas taxas.


This study aims to analyze the spatio-temporal patterns of mortality in the first 24 hours of life and its preventability in the state of Pernambuco from 2000 to 2019. This is a mixed ecological study with the quarters and the municipalities as units of analysis. The information systems on Mortality and Live Births constituted the data sources. All deaths in the state in the first 24 hours of life and from preventable causes were included, characterized according to proximal, intermediate, and distal risk factors. The analysis of preventability of deaths used the Brazilian List of Preventable Causes of Death by Interventions of the Brazilian National Health System. Mortality and preventability rates in the first 24 hours of life were calculated. Spatial analysis was conducted using scan and temporal statistics according to the Autoregressive Integrated Moving Averages Model. In the period from 2000 to 2019, 13,601 deaths were recorded in the first 24 of life, of which 10,497 (77.19%) were due to preventable causes. Of these, 5,513 (40.53%) were classified as reducible due to adequate care for women during pregnancy. The variables at the proximal level: gestational age < 22 weeks (AR = 855.01) and birth weight < 1,500 grams (AR = 204.03) represent an important part of the deaths that occurred in the first 24 hours of life. The variables gestational age, sex, birth weight, type of pregnancy, type of delivery, mother's age and maternal education were statistically significant (p < 0.01). Spatial scanning identified the increase in clusters for both rates. The temporal analysis showed that the mortality rate and the mortality rate from preventable causes presented a stationary pattern in the period and the forecast reveals a growing trend. The results show that mortality in the first 24 hours of life and from preventable causes is configured as a public health problem in the state. Areas of greater risk of death were identified and the forecast reveals an increase in the rates.


El objetivo de este estudio es analizar los patrones espaciotemporales de mortalidad en las primeras 24 horas de vida y su prevención en el estado de Pernambuco (Brasil) en el periodo de 2000 a 2019. Se trata de un estudio ecológico mixto, con los trimestres y los municipios como unidades de análisis. Los sistemas de información sobre Mortalidad y Nacidos Vivos fueron las fuentes de datos. Se incluyeron todas las muertes en las primeras 24 horas de vida y por causas prevenibles en el estado y se caracterizaron según factores de riesgo proximal, intermedio y distal. El análisis de la prevención de las muertes utilizó la Lista Brasileña de Causas de Muerte Prevenibles por Intervenciones del Sistema Único de Salud. Se calcularon las tasas de mortalidad y de prevención en las primeras 24 horas de vida. El análisis espacial se llevó a cabo usando estadísticas de escaneo y análisis temporal de acuerdo con el modelo de promedios móviles integrados autorregresivos. En el período de 2000 a 2019 se registraron 13.601 muertes en las primeras 24 de vida, de las cuales 10.497 (77,19%) se debieron a causas evitables. De estas, 5.513 (40,53%) fueron clasificadas como reducibles por atención adecuada a la mujer durante el embarazo. Las variables a nivel proximal edad gestacional < 22 semanas (RA = 855,01) y peso al nacer < 1.500 gramos (RA = 204,03) representan parte importante de las muertes ocurridas en las primeras 24 horas de vida. Las variables edad gestacional, sexo, peso al nacer, tipo de embarazo, tipo de parto, edad de la madre y escolaridad materna fueron estadísticamente significativas (p < 0,01). El escaneo espacial identificó un aumento de conglomerados para ambas tasas. El análisis temporal mostró que la tasa de mortalidad y la tasa de mortalidad por causas evitables presentaron un patrón estacionario en el periodo, y el pronóstico revela una tendencia creciente. Los resultados muestran que la mortalidad en las primeras 24 horas de vida y por causas prevenibles se configura como un problema de salud pública en el estado. Se identificaron áreas de mayor riesgo de muerte, y el pronóstico revela un aumento en las tasas.

ABSTRACT

Background and objectives: we investigated the existence of a historical geographic overlap between the location of spotted fever group rickettsiosis human cases, a disease caused by the gram-negative bacterium Rickettsia sp., and that of mammalian reservoirs, specifically domestic horses and capybaras, in the urban perimeter of the city of Juiz de Fora, Minas Gerais, Brazil. Methods: cases of human rickettsiosis that occurred during a period of 17 years (2003-2020) were geolocated and the distribution of cases in time and geographic space was assessed using 1st and 2nd order geospatial association indicators. We also analyzed the overlap between the locations of human rickettsiosis cases and the area of occurrence of domestic horses and capybaras. Results: men were diagnosed more often than women, but a large proportion of affected women died. The results indicate an aggregation of human rickettsiosis cases in time (cases tend to occur close to each other at each epidemic event) and in geographic space (cases are concentrated in a specific geographic region of the urban perimeter). Human cases seem to be more associated with city regions with: i) higher local frequency of domestic horses and not capybaras; ii) lower rates of family development. Conclusion: it is suggested that, in the local epidemiological scenario, domestic horses appear to be the main sources of the rickettsia infecting humans, not capybaras.(AU)


Justificativa e objetivos: foi investigada a existência de uma sobreposição geográfica histórica entre a localização dos casos de riquetsiose humana do grupo da febre maculosa, um grupo de doenças causadas pela bactéria Gram-negativa Rickettsia sp., e dos reservatórios mamíferos, especificamente cavalos domésticos e capivaras, no perímetro urbano do município de Juiz de Fora, Minas Gerais, Brasil. Métodos: foram geolocalizados os casos de riquetsiose humana ocorridos durante um período de 17 anos (2003-2020), sendo avaliada a distribuição dos casos no tempo e espaço geográfico através de indicadores de associação geoespacial de 1a e 2a ordem. Também analisamos a superposição dos locais dos casos de riquetsiose humana com a área de ocorrência de cavalos domésticos e capivaras. Resultados: homens foram diagnosticados mais frequentemente que as mulheres, mas grande proporção das mulheres acometidas faleceu. Os resultados indicam uma agregação dos casos de riquetsiose humana no tempo (os casos tendem a ocorrer próximos entre si a cada evento epidêmico) e no espaço geográfico (os casos se concentram em uma região geográfica específica do perímetro urbano). Os casos humanos aparentam ser mais associados às regiões da cidade com: i) maior frequência local de cavalos domésticos e não das capivaras; ii) menores índices de desenvolvimento familiar. Conclusão: sugere-se que, no cenário epidemiológico local, são os cavalos domésticos que aparentam ser as principais fontes da riquétsia infectando os humanos, não as capivaras.(AU)


Justificación y objetivos: investigamos la existencia de una superposición geográfica histórica entre la localización de casos de rickettsiosis exantemáticas humana, enfermedad causada por la bacteria gramnegativa Rickettsia sp., y la de mamíferos reservorios, específicamente caballos domésticos y capibaras, en el perímetro urbano de la ciudad de Juiz de Fora, Minas Gerais, Brasil. Métodos: se geolocalizaron los casos de rickettsiosis humana ocurridos durante un período de 17 años (2003-2020), y se evaluó la distribución de casos en el tiempo y espacio geográfico utilizando indicadores de asociación geoespacial de 1er y 2do orden. También analizamos la superposición entre las ubicaciones de los casos de rickettsiosis humana y el área de ocurrencia de los caballos domésticos y capibaras. Resultados: los hombres fueron diagnosticados con más frecuencia que las mujeres, pero una gran proporción de mujeres afectadas fallecieron. Los resultados indican una agregación de casos de rickettsiosis humana en el tiempo (los casos tienden a ocurrir cerca uno del otro en cada evento epidémico) y en el espacio geográfico (los casos se concentran en una región geográfica específica del perímetro urbano). Los casos humanos parecen estar más asociados con regiones urbanas con: i) mayor frecuencia local de caballos domésticos y no de capibaras; ii) menores tasas de desarrollo familiar. Conclusión: se sugiere que, en el escenario epidemiológico local, los caballos domésticos parecen ser las principales fuentes de la rickettsia que infecta a los humanos, no los capibaras.(AU)

ABSTRACT

Abstract: In this study, the relationship between fish assemblage structure and environmental factors was analyzed in a bay in southern Brazil. Fish were collected every two months between February and December 2002 at six sampling sites using bottom trawl nets. Abiotic data (salinity, temperature, rainfall, and depth) and biotic data (number of individuals, biomass, and total length of individuals from each species) were obtained. In total, 56 fish species representing 27 families were collected. Assemblage structure varied with seasonality, as was evidenced by the variation in temperature and rainfall in each season. Catches showed a high abundance of demersal fishes, particularly Genidens genidens, Eucinostomus gula, and E. argenteus.


Resumo: Neste estudo, a relação entre a estrutura da assembleia de peixes e fatores ambientais foi analisada em uma baía no sul do Brasil. Os peixes foram coletados a cada dois meses entre fevereiro e dezembro de 2002 em seis locais de amostragem usando redes de arrasto de fundo. Dados abióticos (salinidade, temperatura, precipitação e profundidade) e dados bióticos (número de indivíduos, biomassa e comprimento total de indivíduos de cada espécie) foram obtidos. No total, 56 espécies de peixes representando 27 famílias foram coletadas. A estrutura da assembleia variou com a sazonalidade, conforme evidenciado pela variação da temperatura e precipitação em cada estação. As capturas mostraram grande abundância de peixes demersais, principalmente Genidens genidens, Eucinostomus gula e E. argenteus.

Abstract Using five cause-specific mortality data sourced by the Brazilian Ministry of Health, and over 17 years period, we applied Bayesian spatio-temporal models on 644 municipalities of the state of São Paulo, using logistic model to the binary outcome that specifies whether or not the death was from a specific cause. We modeled the temporal mortality effects using B-splines, while the spatial components were considered through Gaussian and Markov random field, and inference was based on Markov chain Monte Carlo simulation. The results demonstrate consistent downward trend in mortality from infectious and parasitic diseases and external causes, while those from neoplasms and respiratory are rising. Cardiovascular is the only cause-specific death that is kept constant in time. All the causes of death considered show heterogeneous spatial and temporal variations among the municipalities, which sometimes change considerably within successive years. Mortality from infectious diseases clustered around the Northwestern municipalities in 2000, but changes to the Southeastern part in 2016, a similar development as external death causes. The study identifies areas with increased and decreased odds mortality and could be useful in disease monitoring, especially if we consider small spatial units.


Resumo Usando dados do Ministério da Saúde do Brasil para cinco causa de mortes, e num período de 17 anos, aplicamos modelos espaço-temporais Bayesianos em 644 municípios do estado de São Paulo, utilizando um modelo logístico binário que especifica se o óbito foi (ou não) de uma determinada causa. Modelamos os efeitos temporais da mortalidade com B-splines, e os componentes espaciais foram estimados através de campos aleatórios de Gaussiano e Markov. Simulamos a inferência estatística com Monte Carlo via cadeias de Markov. Os resultados demonstraram tendência consistente de queda nas mortes por doenças infecciosas e causas externas, enquanto mortes por neoplasias e doenças respiratórias aumentaram no tempo. Cardiovascular foi a única causa de morte constante no tempo. As causas de morte apresentaram variações espaciais e temporais entre os municípios, com consideráveis mudanças em anos sucessivos. A mortalidade por doenças infecciosas se concentrou nos municípios do noroeste do estado em 2000, mas mudou para a parte sudeste em 2016, um desenvolvimento semelhante as causas externas de morte. Este estudo identificou áreas com maior e menor chances de morte entre diferentes causas, e pode ser útil no monitoramento de doenças, especialmente se considerarmos pequenas unidades espaciais.

ABSTRACT

Objective:To explore the feasibility and accuracy of three-dimensional (3D) modeling methods based on ultrasound imaging data for normal and abnormal fetal cardiac structures, and to construct a methodology system for 3D printing of fetal heart based on ultrasound.Methods:A total of 93 fetuses examined in Tangdu Hospital of Air Force Military Medical University from January to December 2019 were selected. Fetal echocardiography was obtained using spatio-temporal image correlation (STIC). Ninety-three hearts were 3D modeled by blood flow modeling, blood pool modeling and cavity modeling, and printed by stereolithography technique. The data measured on the 3D digital models and 3D printed solid models were compared with the corresponding fetal echocardiographic images respectively in order to evaluate the accuracy of the modeling methods.Results:The fetal cardiac blood flow models based on Doppler flow image data showed the malformation and trend of small blood vessels. The fetal cardiac structure models printed based on blood pool modeling displayed the malformation of heart and large blood vessels. Models printed based on cavity modeling method accurately displayed valve and structural defects.For 83 normal fetal hearts, the long diameters of left and right ventricles measured on echocardiography [(15.3±1.9)mm, (13.2±1.9)mm] were compared with those measured on digital models [(15.1±1.9)mm, (12.9±1.9)mm] and 3D printed models[(15.1±1.9)mm, (13.0±1.9)mm], respectively, and there were no significant differences between any two groups of them ( P>0.05). Bland-Altman showed good consistency for all measurements within and between operators. Conclusions:The three modeling methods, including blood flow modeling, blood pool modeling and cavity modeling, have their own advantages in displaying different types of fetal heart malformations. Appropriate modeling methods should be selected for 3D modeling and printing to make up for the limitations of single modeling method. The consistency between measurements on 3D models and those on echocardiography is high, and the repeatability between operators is good.

ABSTRACT

With the development of computer technology and the abundance of spatio-temporal data, Bayesian spatio-temporal model (BSTM) has been developed rapidly, and wildly used by academics to investigate the spatial epidemiological feature of infectious diseases. Hydatid disease is a global natural focus disease that seriously endangers human health. Its epidemic process is complex and affected by many factors. BSTM provides a new method for study of hydatid disease. By modeling, we can not only analyze the influencing factors of hydatid disease, but also can predict the epidemic trend and make the disease distribution map, which is of great significance to public health decisionmaking. Based on a comprehensive review of the literatures, this paper expounds the principles, types and application status of BSTM in hydatid disease.
