ABSTRACT

BACKGROUND: Oral food challenge (OFC) tests are conducted in both specialized institutions and general hospitals. We aimed to compare the severity of the conditions of the patients between these 2 types of institutions in order to consider the role of such institutions in society. OBJECTIVE: We evaluated the results of OFC tests for hen's egg, cow's milk, and wheat that were conducted in a specialized institution (Aichi Children's Health and Medical Center [ACHMC], n = 835) and in 4 general hospitals (n = 327) in Aichi prefecture, Japan. METHODS: The symptoms provoked were scored using the total score (TS) of the Anaphylaxis Scoring Aichi scoring system in combination with the total ingested protein dose (Pro) before the appearance of allergic symptoms. RESULTS: The total ingested dose of the challenge-positive patients in ACHMC was significantly less than that in the general hospitals (p < 0.01). The median TS of the provoked symptoms in ACHMC and the general hospitals did not differ to a statistically significant extent in the hen's egg or cow's milk challenges; however, the median TS in ACHMC was significantly lower than that in the general hospitals for the wheat challenge (p = 0.02). The median TS/Pro values in ACHMC were almost identical to the upper 25% of the TS/Pro values in the general hospitals, suggesting that the specialized institution usually managed more severe patients. CONCLUSION: The specialized institution performed OFC tests at a lower threshold dose, but provoked similar TSs to the general hospitals. This evaluation may help in optimizing the distribution of patients to general hospitals and specialized institutions.

Discipline construction is the core of the construction and development of hospital,the situation of discipline construction reflects the overall strength,academic position and core competitiveness of the hospital.The construction of clinical discipline mainly includes three contents:the construction of academic team,the determination of the subject direction,and the construction of the discipline platform.Scientific research is the main content of discipline construction.High level of research is the carrier of discipline construction.We have performed many management measures:improving the scientific management system,strengthening the dynamic management of the department,global planning,paying attention to personnel training and talent introduction,building reasonable talent echelon and advanced platform to create a good academic environment.According to these measures we have achieved fruitful results.

Objective: To summarize the experience of clinical pharmacy of traditional Chinese medicine in a special hospital. Methods: Combining the work of clinical pharmacists in the hospital, the service content of clinical pharmacy of traditional Chinese medicine in the special hospital was described and the improvement methods were explored. Results:The pharmaceutical care of clini-cal pharmacy of traditional Chinese medicine could improve safe and rational use of drugs. Conclusion: Pharmacists should enhance the study of pharmacy and traditional Chinese medicine to improve the pharmaceutical care of clinical pharmacy of traditional Chinese medicine.

OBJECTIVE: To provide some references for speciaslized hospital to apply for national drug clinical trial institution. METHODS: According to the state on related clinical test qualification of the relevant provisions, combined with the characteristics of specialist hospitals, we described in the application process of qualification and experience of the actual operation. RESULTS AND CONCLUSION: Specialized hospital should understand our own weaknesses, mining clinical advantages of resources, strengthen professional advantages, with rational use of external resources in order to make up for our lack.

ABSTRACT

OBJECTIVE:To explore an emergency drug supply mechanism for women and children’s special hospital during earthquake disaster. METHODS: The practice of drug supply in our hospital after "5?12" Wenchuan earthquake was analyzed. RESULTS: The daily and post-earthquake medicine supply of the hospital was guaranteed,including the drugs which supply to the medical treatment rescues team in the disaster area;and the principle and approach of emergency drug supply in women and children's special hospital during earthquake disaster has been established. CONCLUSIONS: The emer-gency drug supply in women and children’s special hospital should cover drugs used for wounded women and children in addition to those special drugs used for earthquake injuries. All the links including drug plan,drug purchase,quantity monitoring and drugs needed for rescue should be considered in detail to form a fast and smooth hospital emergency drug supply mechanism.

ABSTRACT

OBJECTIVE:To provide references for drug stock management in special hospital.METHODS:On the basis of the ABC analysis,the drugs underwent systematic analysis after being classified into special disease medication and non-special disease medication and then subclassified into common and non-common drugs.Then a periodic and quantitative purchasing schedule was made.RESULTS:Drug procurement based on the purchase schedule contributed to the reduction of both capital occupation and the frequency of procurement under the premise of guaranteeing drug supply,and it helped improve the economic returns as well as the work efficiency.CONCLUSION:The method is simple and feasible and suitable for drug stock management in special hospital.

OBJECTIVE: To provide reference for establishing and improving clinical pharmacists' working mode in women-children special hospital.METHODS: The work carried out by clinical-medical-nursing team and the situation of clinical pharmacists' involvement in clinical treatment were summarized retrospectively.RESULTS & CONCLUSIONS: The medication safety in women-children special hospital has specificity and urgency,and the establishment of clinical-medical-nursing team is an important approach to realize clinical safe and rational drug use.A platform for clinical pharmacists under unified management of organization and system requirement should be set up in hospital to explore a clinical pharmacy working mode with the characteristics of special hospital,carry out clinical pharmacy research and search for quality management and performance management method for clinical pharmacists.

ABSTRACT

In order for special hospitals to survive and develop in the fierce market competitions, it is imperative for them to enhance their competitiveness, the key to which lies in accelerating personnel training. For this reason, it is necessary to take the following steps: changing concepts, identifying goals, implementing personnel training strategies and taking the initiative in selecting talented personnel; making exertions in personnel training through expanding channels and conducting classifications; creating the right circumstances, optimizing the environment and making a rational use of personnel; adopting innovative mechanisms, perfecting systems and scientifically managing personnel. It is important to take specialty advancement as the guide, personnel training as the prop, and scientific research and teaching as the driving force in achieving the sustainable development of special hospitals.
