ABSTRACT

In the emergency department, open endotracheal suctioning for mechanically ventilated patients with endotracheal intubation will lead to the spread of respiratory droplets and aerosols, polluting the surrounding environment and medical staff. The traditional heat-and-moisture exchanger has the effect of warming and humidifying, and can block pathogenic microorganisms, but it does not have the function of inserting a sputum suction tube. When the heat-and-moisture exchanger is pulled out for sputum suction, it is easy to cause sputum splash, which pollutes the surrounding environment and medical personnel. The addition of closed sputum suction devices will increase the economic burden on patients. Thus, the medical staff of emergency department of the First People's Hospital of Tongxiang City of Zhejiang Province designed a new type of heat-and-moisture exchanger with anti-splash sputum suctioning function and obtained the National Utility Model Patent of China (ZL 2021 2 0017615.0). The new heat-and-moisture exchanger is mainly composed of a receiving cavity, a connecting tube, a sputum suction tube intubation tube, a sealing valve, etc. The disposable sputum suction tube can be used to insert sputum suction, and at the same time, it can prevent the secretion from splashing to ensure sealing. The patent combines the humidification and pathogen blocking functions of the heat-and-moisture exchanger with the anti-splash sputum suctioning function, which is suitable for use in the emergency and critical care medicine departments and has clinically practical value.

ABSTRACT

BACKGROUND@#Occupational exposure to blood and body fluids is a major risk factor for the transmission of blood-borne infections to healthcare workers. There are several primary studies in Ethiopia yet they might not be at the national level to quantify the extent of occupational blood and body fluid exposures (splash of blood or other body fluids into the eyes, nose, or mouth) or blood contact with non-intact skin among the healthcare workers. This systematic review and meta-analysis aimed to estimate the pooled prevalence of occupational blood and body fluid exposure of healthcare workers in Ethiopia.@*METHODS@#PubMed, Science Direct, Hinari, Google Scholar, and the Cochrane library were systematically searched; withal, the references of appended articles were also checked for further possible sources. The Cochrane Q test statistics and I tests were used to assess the heterogeneity of the included studies. A random-effects meta-analysis model was used to estimate the lifetime and 12-month prevalence of occupational exposure to blood and body fluids among healthcare workers in Ethiopia.@*RESULTS@#Of the 641 articles identified through the database search, 36 studies were included in the final analysis. The estimated pooled lifetime and 12-month prevalence on occupational exposure to blood and body fluids among healthcare workers were found to be at 54.95% (95% confidence interval (CI), 48.25-61.65) and 44.24% (95% CI, 36.98-51.51), respectively. The study identified a variation in healthcare workers who were exposed to blood and body fluids across Ethiopian regions.@*CONCLUSION@#The finding of the present study revealed that there was a high level of annual and lifetime exposures to blood and body fluids among healthcare workers in Ethiopia.

Objetivou-se, neste trabalho, quantificar o efeito da vinhaça no comportamento compressivo e nos atributos mecânicos, como consistência, umidade ótima de compactação e densidade máxima, de um Latossolo Vermelho-Amarelo (LVA); avaliar o efeito da vinhaça e do estado de compactação no salpicamento de partículas, da região de Lavras, MG. A vinhaça foi proveniente de um alambique de cachaça artesanal. Foram coletadas amostras deformadas e indeformadas nas profundidades de 0 a 3 cm e 60 a 63 cm. As amostras deformadas destinaram-se à caracterização do solo, determinação dos limites de consistência e realização do ensaio de Proctor Normal, após a incubação do material com vinhaça. As amostras indeformadas foram saturadas com vinhaça ou água destilada (controle), por 48 horas e destinaram-se aos ensaios de compressibilidade e resistência ao salpicamento, usando-se um minisimulador de chuvas. A vinhaça alterou os limites de consistência das camadas estudadas. Para a camada de 0-3 cm a vinhaça reduziu a faixa friável e para a camada 60-63 cm aumentou. Para ambas as camadas, a vinhaça aumentou a Dmáx e reduziu a Uótima. As camadas 0-3 cm e 60-63 cm, independentemente do tratamento com vinhaça, apresentaram a mesma capacidade suporte de carga. O estado de compactação das amostras influenciou a erosão por salpicamento. O aumento da pressão aplicada sobre as amostras resultou no aumento da quantidade de material salpicado. Na camada 60-63 cm, o tratamento com vinhaça reduziu a quantidade de material salpicado com o aumento da pressão aplicada, notadamente com as amostras na capacidade de campo.


The objective of this study was to quantify the effect of plonk on compressive behavior and mechanical attributes such as consistency, optimum moisture for compaction and maximum density of a Red-Yellow Latosol (Oxisol) to evaluate the effect of plonk and compaction state in splashed particles, from Lavras (MG) region. The plonk was obtained from an artisanal sugarcane brandy alembic. Undisturbed and disturbed soil samples were collected at 0 to 3 cm and 60 to 63 cm depths. Disturbed soil samples were used for soil characterization, determination of consistence limits and Normal Proctor essay after material incubation with plonk. Undisturbed soil samples were saturated with plonk or distilled water (control) during 48 hours for testing the compressibility and resistance to splash by using simulated rainfall. The plonk altered the consistence limits of studied layers. For the 0-3 cm layer, the plonk reduced the friable range, and for the 60-63 cm layer the effect was in the opposite direction. For both layers, the plonk increased Dmax and decreased Uoptimum. Regardless of the plonk treatment, both layers presented the same load support capacity. The compaction degree of samples influenced the splash erosion. The increase of the applied pressure over the samples resulted in increase of splash material quantity. At the 60-63 cm layer, the plonk treatment reduced the splash material quantity by increasing the applied pressure, mainly when the samples were at field capacity.

ABSTRACT

BACKGROUND: We wanted to determine the postoperative analgesic efficacy of preincisional caudal epidural block versus instillation (splash block) following inguinal herniorrhaphy in children. METHODS: Thirty children (age range: 1-7 years) who were scheduled to undergo inguinal herniorrhaphy were divided into 2 groups: the caudal block group and the splash block group with 15 children in each group. Tracheal intubation was performed. Fifteen children received caudal block with 1.0 ml/kg of 0.25% ropivacaine (Group 1). Caudal block was performed using the loss of resistance method via the sacral hiatus. Fifteen children in Group 2 received local instillation (splash block) in the surgical site with up to 0.4 ml/kg of 0.25% ropivacaine. The patients were observed for 90 minutes in the postanesthesia care unit and then they were transferred to the ward. The pain scores were taken 4 times. We assessed pain using the Faces pain scores. RESULTS: There were no significant differences between the groups regarding the pain scores at 10, 30 and 60 minutes upon entering the postanesthesia care unit. The pain scores of Group 1 were slightly lower at the last evaluation point when compared to that of Group 2. One patient in Group 1 required supplemental postoperative intravenous (IV) tramadol, while all the other patients in both groups did not require supplemental IV tramadol. The intraoperative requirement for sevoflurane was decreased in Group 1 as compared to that of Group 2. There were no major complications related to either type of block. CONCLUSIONS: We conclude that a splash block can have a similar analgesic effect as that of a caudal block for the postoperative herniorrhaphy pain of children.

ABSTRACT

The purpose of this study was to determine the incidence of blood splash to the eyes of obstetricians during Cesarean section. By systemic random sampling, 213 cases of all Cesarean sections that performed during the 6 months period at Siriraj Hospital were enrolled in the study. Before and after the operation, benzidine test of the swabs taken from 128 and 123 pairs of the protective eyeglasses wared by the assistants were used to determine the presence of blood contamination. The incidence of blood splash to the protective eyeglasses of the operators and the assistants were 56.3% and 25.2% while 52.8% and 70.9% of the contaminated protective eyeglasses were undetected by the operators and the assistants. The factors influencing the incidences were the indications and the urgency of the operations. Because of the high rats of blood splash during cesarean section, obstetricians should were protective eyeglasses during the operation to minimize the risk of exposure to potentially infectious blood.
