ABSTRACT

Introducción: El síndrome de uveítis-glaucoma-hifema (UGH) es una complicación infrecuente de las cirugías de catarata, debido a un roce mecánico ejercido por una lente intraocular (LIO) sobre el iris. Caso clínico: Un varón de 64 años, con antecedente de cirugía de catarata, presenta disminución de la agudeza visual y dolor en el ojo derecho. En el examen oftalmológico, se evidenciaron signos de uveítis anterior, presión intraocular (PIO) elevada, microhifema y una LIO de una pieza plegable en sulcus que provocaba un roce mecánico con la cara posterior del iris. El tratamiento médico fue insuficiente, por lo que se realizó una cirugía de explante de LIO de una pieza plegable y se reemplazó por una LIO de tres piezas plegables. La evolución posoperatoria fue favorable. Conclusión: Se debe sospechar de esta complicación, en pacientes con antecedente de cirugía de catarata, especialmente en casos en los cuales la LIO es de una pieza y ha sido implantada fuera del saco capsular.


Introduction: Uveitis-glaucoma-hyphema syndrome (UGH) is a rare complication of cataract surgery, due to mechanical chafing exerted by an intraocular lens (IOL) on the iris. Clinical case: A 64-year-old man with a history of cataract surgery, who presented decreased visual acuity and pain in the right eye. The ophthalmological examination revealed signs of anterior uveitis, elevated intraocular pressure (IOP), microhyphema, and a single-piece foldable IOL in the sulcus that caused a mechanical chafing with the posterior face of the iris. The medical treatment was insufficient; for this reason, a folding simple-piece IOL explant surgery was performed and replaced by a three-piece IOL. Postoperative evolution was favorable. Conclusion: Should be suspected this complication in patients with a history of cataract surgery, especially in cases in which the IOL is in single-piece and has been implanted outside the capsular bag.

ABSTRACT

We report the outcomes of a custom-designed toric piggyback intraocular lens in a patient with high postoperative residual astigmatism. A 60-year-old male patient underwent customized toric piggyback IOL for postoperative residual astigmatism of 13 D, with follow-up examinations for IOL stability and refractive outcomes. The refractive error stabilized at two months and remained stable at one year, with a correction of nearly 9 D of astigmatism. The IOP remained within normal limits, and there were no postoperative complications. The IOL remained stable in the horizontal position. To our knowledge, this is the first case report of correction of unusually high astigmatism by a novel smart toric design of piggyback IOL.

ABSTRACT

Purpose: To evaluate the outcome of glaucoma drainage device (GDD) insertion of tube through ciliary sulcus (CS) versus anterior chamber (AC) placement in the North Indian population Methods: This retrospective comparative case series included 43 patients in CS group and 24 in AC group, who underwent GDD implantation, from March 2014 to February 2020. The main outcome measures were intraocular pressure (IOP), number of anti?glaucoma medications, best corrected visual acuity (BCVA), and complications. Results: Sixty?seven eyes of 66 patients were included in study with mean follow?up of 25.04 months (range, 12–69 months) in the CS group and 17.4 months (range, 13–28 months) in the AC group. Preoperatively the two groups were similar except for postpenetrating keratoplasty glaucoma (PPKG) and pseudophakic patients, which were higher in the CS group (P < 0.05). Both groups showed statistically insignificant difference in postoperative IOP and BCVA at last follow?up (P = 0.173, P = 0.495, respectively). Postoperative complications were similar, except for corneal decompensation which was significantly higher in the AC group (P = 0.042). Conclusion: Our findings suggest that there was no statistically significant difference in mean IOP between the CS and AC groups at the last follow?up. CS placement of tube of GDD appears to be effective and safe technique. However, CS placement of tube resulted in lesser corneal decompensation and thus should be preferred in pseudophakic/aphakic patients, especially PPKG.

Introducción: El envejecimiento periorbitario se considera un proceso complejo y multifactorial que se relaciona con procesos intrínsecos y extrínsecos tales como los cambios degenerativos en la piel, la aparición de líneas finas, la pérdida de volumen, el descenso de tejidos, la piel flácida y el exceso de depósito de grasa. Objetivo: Evaluar los resultados de la corrección de las ojeras y del surco nasoyugal con Nanofat. Métodos: Se realizó un estudio longitudinal prospectivo en el Servicio de Cirugía Plástica y Caumatología del Hospital Clínico Quirúrgico "Hermanos Ameijeiras" desde agosto de 2019 a mayo de 2021. La muestra la constituyeron 37 pacientes. Los datos se resumieron en frecuencias absolutas y relativas. Resultados: La edad promedio fue 47,2 ± 10,7 años. El 89,2 por ciento de los pacientes eran mujeres. La media del tejido emulsionado infiltrado por cada lado fue de 1,40 ± 0,4 ml, con un volumen mínimo de 1 ml y un máximo de 2 ml. En el 83,8 por ciento de los casos se constató una disminución del surco nasoyugal y en el 16,2 por ciento se evidenció una gran mejoría. En el 100 por ciento de los pacientes hubo avance de la hiperpigmentación y mejoría de la calidad de la piel. Dentro de las complicaciones solo un paciente (2,7 por ciento) presentó nódulos por calcificación en la zona receptora. El resto de los pacientes en estudio no presentó eventos adversos. El grado de satisfacción postratamiento con Nanofat fue de 100 por ciento. Conclusiones: Los resultados de la corrección de las ojeras y del surco nasoyugal con Nanofat son muy buenos(AU)


Introduction: Periorbital aging is considered a complex and multifactorial process that is related to intrinsic and extrinsic processes such as degenerative changes in the skin, appearance of fine lines, volume loss, tissue descent, sagging skin, and excess fat deposition. Objetive: To evaluate the results of correction of dark circles under the eyes and the nasojugal sulcus with Nanofat. Methods: A prospective longitudinal study was performed in the Plastic Surgery and Caumatology Service of the Clinical Surgical Hospital "Hermanos Ameijeiras" from August 2019 to May 2021. The sample consisted of 37 patients. The data were summarized in absolute and relative frequencies. Results: The mean age was 47.2±10.7 years. The 89.2 percent of the patients were women. The mean emulsified tissue infiltrated per side was 1.40±0.4 ml, with a minimum volume of 1 ml and a maximum of 2 ml. In 83.8 percent of the cases there was a decrease in the nasojugal sulcus and in 16.2 percent there was a great improvement. In 100 percent of the patients there was improvement of hyperpigmentation and great improvement of skin quality. Among the complications only one patient (2.7 percent) presented nodules due to calcification in the recipient area. The rest of the patients in the study did not present adverse events. The degree of post-treatment satisfaction with Nanofat was 100 percent. Conclusions: The results of correction of dark circles and nasojugal sulcus with Nanofat are very good(AU)

ABSTRACT

Purpose: Anterior segment optical coherence tomography (AS?OCT) by a swept source can visualize the ciliary body. The study was performed for analyzing the feasibility of a new swept?source OCT (Anterion) device for measuring iris ciliary sulcus, ciliary body thickness, and iris thickness. Methods: It is a pilot study among 30 normal young participants and hospital employees with their consent. The ciliary body, iris thickness, and ciliary sulcus were measured and compared to the results in the literature. We obtained 28 good?quality results. The main outcome measures were iris thickness (near and away from the pupillary margin), ciliary body thickness, and iridociliary sulcus measurement. Results: The iris thickness 2.5 mm from the pupillary margin was 0.70 ± 0.10 mm in the right eye and 0.68 ± 0.11 in the left eye. Closer to the root, the iris thickness varied from 0.55 ± 0.16 mm (right eye) and 0.57 ± 0.12 (left eye). Ciliary body thickness: Right eye 0.59 ± 0.14mm (28 good images), left eye 0.58 ± 0.13mm; ciliary sulcus (inner angle): right eye 82.65 ± 26.6°, left eye 83.66 ± 30.1°. Conclusion: Anterion OCT is feasible in our setting for various measurements and can provide valuable inputs to ophthalmologists for the treatment of ocular diseases.

ABSTRACT

AIM: To determine the correlation among horizontal ciliary sulcus(HCS)diameter, vertical ciliary sulcus(VCS)diameter and corneal diameter in different anterior chamber depth(ACD)levels from patients with low to moderate myopia.METHODS: Retrospective study. A total of 78 patients(129 eyes)with low to moderate myopia who had preoperative examination of implantable contact lens(ICL)implantation in Shanghai Bright Eye Hospital from April 2021 to April 2022 were included. HCS, VCS and white to white(WTW)were measured and compared. Patients were divided into shallow ACD group(2.8 mm ≤ ACD ≤3.2 mm), medium ACD group(3.2 mm < ACD ≤3.4 mm)and deep ACD group(ACD > 3.4 mm)based on the ACD obtained by Pentacam. The HCS, VCS and WTW within and among groups were compared, and the correlation of WTW, HCS and VCS among each group was analyzed by Pearson and linear regression equation.RESULTS: There was no statistical difference in WTW and HCS overall and in groups with different ACD(P>0.05), while there was difference between VCS and both HCS and WTW(P<0.01). The WTW had a correlation with both HCS and VCS in each group, while correlation between WTW and VCS was weak in the deep ACD group(r=0.470, R2=0.221, P<0.01). Furthermore, the WTW, HCS and VCS in the shallow ACD group were statistically different from those in the medium and deep ACD groups(P<0.01).CONCLUSION: The WTW was suggested predicting HCS rather than perfectly replacing HCS before patients had the ICL operation among patients with low to moderate myopia. The predictability of VCS would decline as the ACD depth increasing, predicting VCS by WTW was not recommended.

ABSTRACT

AIM: To evaluate the efficacy of Ahmed glaucoma valve implanting through the ciliary sulcus in the treatment of intraocular lens(IOL)glaucoma.METHODS: Retrospective case series study. The clinical data of 18 patients(18 eyes)with IOL glaucoma from February 2020 to February 2022 in Liuzhou Red Cross Hospital were collected. All patients underwent Ahmed glaucoma valve implanting through the ciliary sulcus. The visual acuity, intraocular pressure, corneal endothelial cell density, corneal endothelial cell loss rate and complications were observed before operation and 1, 3, 6 and 12mo after operation.RESULTS: The mean intraocular pressure before operation was 43.41±5.33 mmHg. The mean intraocular pressure at 1, 3, 6 and 12mo after operation were 12.11±3.26, 13.82±4.12, 13.54±3.83 and 15.80±4.35 mmHg respectively. The mean intraocular pressure at each follow-up point after operation was significantly lower than that before operation(F=203.40, P&#x003C;0.01). The corneal endothelial cell density at each time point after surgery was lower than that before surgery(F=6.18, P&#x003C;0.01), but there were no significantly differences in the density and loss rate of corneal endothelial cell at 1, 3, 6 and 12mo after surgery(P&#x003E;0.05). Furthermore, the overall surgery success rate was 83%.CONCLUSIONS: Ahmed glaucoma valve implanting through the ciliary sulcus is safe and effective in the treatment of IOL glaucoma, and the damage to corneal endothelial cells is mild. The corneal endothelial cells density doesn't decrease with time after surgery.

ABSTRACT

Myopia is considered to be a disease occurred under the influence of genetic and environmental factors, and it can only be restored by corrective surgeries. The current surgical methods include laser surgeries and phakic intraocular lens(PIOL)implantation, and PIOL has the advantages of wide range of correction, repeatable procession and small damage to corneal tissue. Furthermore, good visual acuity can be achieved after implantation of intraocular lens. Implantable collamer lens(ICL)is the most common surgical method in this group. The vertical distance between the highest point of the posterior central surface of the implanted lens and the anterior surface of the crystalline lens is called vault. It will cause serious complications if the vault turns out to be abnormal. Up to now, there is no optimal prediction method. This research focuses on vault, discusses the ideal range of postoperative vault, the safety and effectiveness of this surgery, and analyzes the existing prediction methods to provide directions for future clinical research.

ABSTRACT

AIM: To compare the efficacy of 25-gauge(25G)vitrectomy combined with intraocular lens transciliary sulcus scleral suture fixation and sutureless scleral tunnel interlamellar fixation in the treatment of complete lens dislocation.METHODS: Retrospective case-control study. A total of 40 patients(40 eyes)diagnosed with complete lens luxation in the vitreous cavity in our hospital from May 2015 to September 2021 were selected, among which 21 eyes(suture group)underwent 25G vitrectomy combined with intraocular lens fixation via ciliary sulci scleral suture, and 19 eyes(sutureless group)underwent 25G vitrectomy combined with interlamellar scleral tunnel fixation of intraocular lens. The patients in both groups were followed up until 3mo after surgery to observe the operative time, best corrected visual acuity(BCVA), corneal endothelial cell count(CECC), central corneal thickness(CCT)and postoperative complications.RESULTS: The operation time was significantly shorter in the sutureless group than in the suture group(31.79±6.01min vs. 45.38±8.04min, P&#x003C;0.001). BCVA in both groups was significantly improved after surgery(all P&#x003C;0.05), and the BCVA(LogMAR)at 1wk after operation in the sutureless group was significantly better than that in the suture group(0.32±0.14 vs. 0.57±0.25, P&#x003C;0.001). At 3mo after surgery, CECC in both groups was lower than that before surgery(all P&#x003C;0.01). The CCT at 1wk after operation in the suture group was greater than that before operation and at 3mo after operation(all P&#x003C;0.01), and there was no significant change in CCT before and after surgery in the sutureless group. During follow-up period, the total complication rate in the sutureless group was lower than that in the suture group(26% vs. 38%, P&#x003E;0.05).CONCLUSION: 25G vitrectomy combined with intraocular lens sutureless scleral tunnel interlamellar fixation in the treatment of complete lens luxation has shorter operation time, faster postoperative visual acuity improvement and fewer postoperative complications.

ABSTRACT

Purpose: To study the use of sulcus placement of a 3?piece intraocular lens (IOL) with optic capture in patients with exfoliation with zonular weakness. Methods: Data on all exfoliation patients who had direct or indirect evidence of zonular weakness and had a 3?piece IOL implanted in the sulcus with optic capture over a 5?year period between January 2017 and January 2022 were included in this retrospective case series. Results: The study comprised of 35 eyes of 35 patients. The mean age at surgery of the 20 male and 13 female patients was 75.21 years ± 5.74 (standard deviation (SD)). The mean pupillary diameter was 5.77 ± 2.23 mm (range: 10 to 3 mm). A capsular tension ring (CTR) was used in 17 cases and iris hooks as a pupil?expanding device were used in 15 cases. No patients had an increase in inflammation after surgery and there were no late subluxation or dislocation of “in the bag” posterior chamber IOL or major complications. There was a significant improvement in visual acuity. Conclusion: The sulcus placement of 3?piece IOL with optic capture is the ideal technique in patients with clinical or intraoperative evidence of mild to moderate zonular weakness. It may also be more appropriate in relatively younger patients (in the fifties or sixties) with exfoliation with no overt zonulopathy to prevent late subluxation or dislocation of “in the bag” IOL

ABSTRACT

We report the early outcomes and describe an ab interno 21?G needle technique of sulcus placement of the Aurolab aqueous drainage implant (AADI) tube in nine pseudophakic eyes. IOP reduced from a preoperative mean (SD) of 28.33 (9.80) to 11.56 (2.65) mm Hg and the mean (SD) number of preoperative medications reduced from 3.0 (0.7) to 0.4 (0.9) at 3 months. There were no intraoperative complications noted. This technique of sulcus placement of the AADI tube is a precise technique of tube insertion. It may be an alternative to existing ab externo procedures of tube sulcus placement, limiting multiple blind entries.

ABSTRACT

Purpose: To analyze the postoperative visual and surgical outcomes following surgery for pediatric?absorbed cataracts and intraoperative difficulties. Methods: This prospective longitudinal study included 43 eyes (30 children) with absorbed cataracts aged between 6 months and 18 years (either sex). All children underwent best?corrected visual acuity (BCVA), anterior and posterior segment evaluation, rubella titer estimation, intraocular lens (IOL) power calculation, superior small incision cataract surgery with or without posterior capsulotomy/anterior vitrectomy/IOL implantation under general anesthesia, visual rehabilitation, and were followed up for 1 year. Results: The mean age was 7.89 ± 4.84 years. Preoperative BCVA distance: 39 eyes had either perception of light (PL) or counting finger close to face (CFCF); near BCVA: all eyes had either PL or N36. Postoperative (12 months) distant BCVA: a majority of the eyes (27) had 6/60–1/60, 11 eyes had 6/18–6/36; near: N18 in 19 eyes, N6 in 7 eyes. Anterior continuous curvilinear capsulorrhexis (ACCC) was possible in eight eyes. Cortical aspiration was difficult in 16 eyes (peripheral calcified ring). A majority (32 eyes) underwent in the sulcus implantation (in?the?bag: eight eyes); two eyes: no IOL, one eye: secondary IOL. Eleven eyes had early postoperative inflammation. At 12 months, one eye underwent membranectomy for visual axis opacification and 38 eyes had well?centered IOLs. Conclusion: Surgery in absorbed cataracts is challenging because of the anatomic disorganization of the absorbed lens. By adopting appropriate surgical methods and good visual rehabilitation, one can achieve satisfactory surgical and visual outcomes, highlighting the need for surgical intervention

ABSTRACT

@#Desmoid tumor is a rare, locally-invasive fibromatosis with a high recurrence rate and non-metastatic features. Here, we reported a 62-year male patient with desmoid tumor in the superior sulcus of left lung, complaining of cough, chest pain, limited-movement, and pain on the left upper arm. We performed extended resection of the tumor, including wedge resection of the left upper lobe, resection and anastomosis of partial left subclavian artery, resection of T1 nerve root, cauterization of adhesive pleura, and resection of the left first and second ribs. After surgery, the patient's symptoms were relived. The duration of hospital stay was 8 d. This is the first reported case of surgical treatment for the superior sulcus desmoid tumor.

Abstract: It is stated that peri-implanter diseases have destructive effects similar to periodontal diseases. This study aims to compare IL-1β and TNF-α levels in healthy and diseased areas. Forty non-smokers systemically health individuals (40 implants/40 teeth) (age:38-67) were included in the study. In clinical and radiographic examinations; plaque index (PIn), gingival index (GI), periodontal pocket depth (PPD), clinical attachment level (CAL) and marginal bone loss (MBL) values were recorded. The gingival crevicular fluid (GCF) and peri-implanter sulcus fluids (PISF) of the patients were collected, and IL-1β and TNF-α levels were measured by ELISA in the samples. The collected data were analyzed with the help of SPSS v.22 package program. Sample PPD score showed a statistically significant difference between the diseased and healthy groups and also sample CAL showed statistically significant higher in Periodontitis(P) compared to periodontally heathy(H) and Gingivitis(G) (p>0.05). IL-1beta and TNF-α levels in GCF/PISF; In the P and Peri-implantitis (PI) group, it was found statistically significantly higher than the H, Healthy-Implant (HI), besides P showed higher levels compared to G (p<0.05). Within the limitations of our study, it can be said that IL-1β and TNF-α among inflammatory cytokines in GCF/PISF will increase in periodontal and peri-implanter diseases, it can also be said that this increase in cytokines may indicate that periodontal and peri-implanter diseases have similar immunological structure. Peri-implant mucositis without periodontitis history has similarity to peri-implantitis and periodontitis in terms of IL-1β and TNF-α levels in GCF/PISF.


Resumen: Se afirma que las enfermedades peri-implantarias tienen efectos destructivos similares a los de las enfermedades periodontales. Este estudio tiene como objetivo comparar los niveles de IL-1β y TNF-α en zonas sanas y enfermas. Cuarenta individuos no fumadores con salud sistémica (40 implantes/40 dientes) (edad:38-67) fueron incluidos en el estudio. En los exámenes clínicos y radiográficos se registraron los valores de índice de placa (PIn), índice gingival (GI), profundidad de la bolsa periodontal (PPD), nivel de fijación clínica (CAL) y pérdida de hueso marginal (MBL). Se recogieron los fluidos creviculares gingivales (GCF) y los fluidos del surco peri-implantario (PISF) de los pacientes, y se midieron los niveles de IL-1β y TNF-α mediante ELISA en las muestras. Los datos recogidos fueron analizados con la ayuda del programa de paquete SPSS v.22. La puntuación PPD de la muestra mostró una diferencia estadísticamente significativa entre los grupos enfermos y sanos y también la muestra CAL mostró estadísticamente significativa más alta en la Periodontitis (P) en comparación con la salud periodontal (H) y la Gingivitis (G) (p>0.05). Los niveles de IL-1beta y TNF-α en GCF/PISF. En el grupo de P y Peri-implantitis (PI), se encontró estadísticamente significativo más alto que el H, implante sano (HI), además P mostró niveles más altos en comparación con G (p<0.05). Dentro de las limitaciones de nuestro estudio, se puede decir que IL-1β y TNF-α entre las citoquinas inflamatorias en GCF/PISF aumentarán en las enfermedades periodontales y peri-implantarias. También se puede decir que este aumento de citoquinas puede indicar que las enfermedades periodontales y peri-implantarias tienen una estructura inmunológica similar. La mucositis peri-implantaria sin antecedentes de periodontitis tiene similitudes con la peri-implantitis y la periodontitis en cuanto a los niveles de IL-1β y TNF-α en la GCF/PISF.

@#AIM:To evaluate the clinical effect of 25G pars plana vitrectomy(PPV)combined with intraocular lens(IOL)ciliary sulcus suture fixation and replacement of IOL capsular bag complex.<p>METHODS: A retrospective analysis of 21 patients(21 eyes)in our hospital who applied 25G PPV combined with IOL ciliary sulcus suture and fixation replacement in our hospital from January 2015 to January 2020 to treat the IOL capsular complex completely detached from the clinical data.<p>RESULTS: All the cases were successfully removed the dislocated IOL complex during the operation. There was no iatrogenic retinal damage. During the operation, 4 cases(4 eyes)were found to have the retina hole, 2 cases(2 eyes)were found to be retinal lattice degeneration area, which were treated with laser photocoagulation of the retina. Follow-up for 6-18mo, best corrected visual acuity(BCVA)(LogMAR)increased from 0.40±0.30 before operation to 0.33±0.25 after operation(<i>P</i>=0.040). The BCVA at the last follow-up of all cases reached the BCVA before surgery. The absolute value of the difference between the postoperative spherical equivalent power and the preoperative IOL refractive power prediction value is less than or equal to 0.75D. At the last follow-up, the position of the intraocular lenses in all cases was good, and there were no complications such as retinal detachment. <p>CONCLUSION: The technique of 25G PPV and the exchange of IOL ciliary sulcus fixation with intraocular IOL capsule complex is a safe and effective procedure for dislocation of IOL-capsular bag complex entirely into the vitreous cavity.

ABSTRACT

Background: Laparoscopic cholecystectomy is the most commonly performed laparoscopic surgery worldwide. Safe cholecystectomy is the priority to reduce the morbidity and mortality. There is a paradigm shift from extensive Calot’s dissection to identification of Rouviere’s sulcus and lesser dissection. Identification and analysis of Rouviere’s sulcus will help us doing a safe cholecystectomy and avoiding further injuries to bile ducts.Methods: The study included 160 cases of laparoscopic cholecystectomy, posted in elective OT and identified Rouviere’s sulcus during laparoscopy. Table visual inspection and analysis was done. And the collected data was analyzed for different types of sulcus, its position, morphology and content.Results: Of 160 cases, 147 cases had Rouviere’s sulcus. 13 cases did not have a sulcus. Open type sulcus was present in 99 cases, 35 had closed type, whereas 19 had slit type and only 7 had a scar like sulcus. The study showed 92% of our patients had Rouviere’s sulcus and of them 61.9% had an open type which was the most common type of sulcus of them 18 cases had a visible pulsating vessel in the floor of the sulcus i.e. posterior sectional pedicle in the sulcus.Conclusions: Present study showed, in 92% cases it is easy and approachable to visualise the Rouvier’s sulcus. So, it is feasible and beneficial to identify the sulcus and keep the dissection above this level to avoid common bile duct injury and further complication thereof.

ABSTRACT

Aim: The aim of the study was to evaluate the comparative effect of Ornigreat gel and Hexigel as a local drug delivery system in association with scaling and root planing (SRP) in patients with chronic periodontitis. Materials and Methods: A double-blind trial was conducted to test the comparative efficacy of the two commercially available Ornigreat gel and Hexigel at Indira Gandhi Govt. Dental College and Hospital, Jammu. These indices were recorded at baseline (“0 day”), 15th day, 30th day, 60th day, and 90th day in 40 sites, >4 mm pockets in 11 patients among which 20 sites received Ornigreat gel and other 20 sites receive Hexigel following SRP were compared. Results: The results of the study showed that the combination of SRP and Ornigreat gel therapy was more effective in reducing the mean values of gingival index and sulcus bleeding index though not statistically significant, but the values of plaque index showed statistical significance on the 60th day and 90th day, and the probing pocket depths showed statistically significant difference from the 15th day to the 90th day at P < 0.05 in comparison with Hexigel. Conclusion: The Ornigreat gel could be an efficient local drug delivery system when used in adjunct to SRP in comparison with Hexigel.

ABSTRACT

Objective: The objective of this study was to find out whether there was any shoulder instability prevailing in the shot put throwers due to the rotational activities achieved in the respective sport. Methods: A total of 76 shot put throwers were recruited for the study. The multidirectional instability of the shoulder joint was tested by performing special tests. Results: The study revealed that out of 76 shot put throwers, 49 individuals were found to be having shoulder instability, and 27 individuals were not having shoulder instability. This was confirmed by using a special test like the Sulcus sign and Rowe test followed by statistical analysis. The individuals diagnosed with multidirectional instability showed Sulcus sign and Rowe test positive, of which 30 individuals showed both the test positive and 19 individuals showed only Rowe test positive. Conclusion: About 64.47% of individuals were diagnosed with multidirectional instability in the shot put throwers while the rest 35.52% of individuals showed no signs of instability.

ABSTRACT

Objective@#To introduce a new surgical procedure for the treatment of neck benign tumors by endoscopic techniques.@*Methods@#Seventeen patients with neck benign tumor underwent surgery by endoscope through a concealed incision in Department of Oral and Maxillofacial Surgery, Qilu Hospital of Shandong University from January 2018 to August 2019 were analyzed, which included 3 cases of tumor in the submental area, 2 cases in submandibular region, 9 cases in lower pole region of parotid gland, 1 case in superior region of sternocleidomastoid muscle, 1 case in central region of sternocleidomastoid muscle, 1 case in inferior region of sternocleidomastoid muscle. All patients underwent routine preoperative examination and CT examination to evaluate tumor size, boundary, morphology and nature. According to the area where the tumor located, concealed incisions in different sites were designed. Lumps in the submental area and submandibular area were treated with oral vestibular sulcus incision. Benign tumors located in the lower pole region of parotid gland and the sternocleidomastoid muscle region were treated with approach of the short hidden postauricular incision. During the operation, the self-developed "maxillofacial suspension device" was used to provide the operating space. The tumors were completely removed with endoscope and all patients were followed up every 3 months.@*Results@#All surgical procedures were performed as expected. Visual analogue scale (VAS) was 9.3 on average at 3 months after operation, all the patients were satisfied with the incision design and the cosmetic effect. No recurrences were found in patients with a follow-up period ranged from 1-15 months.@*Conclusions@#These studies have shown that endoscope-assisted neck benign tumor resection is a surgical procedure with covert incision and good cosmetic results.

ABSTRACT

Objective @#To assess the safety and feasibility of incision in the retroauricular sulcus for removal of benign tumors in the lower pole of the parotid gland and to provide some ideas for aesthetic consideration of parotidectomy.@*Methods@# In total, 18 cases with benign tumors of the lower pole regions of the parotid gland were included in this study, including 9 pleomorphic adenomas, 2 myoepitheliomas, 5 Warthin tumors, 1 basal cell adenoma and 1 oncocytoma. Three months after the operation, facial paralysis and salivary fistula were assessed. A visual analog scale was used to score the cosmetic satisfaction of the surgical incision. Tumor recurrence was followed up 6 ~ 12 months after operation.@*Results@# In 18 patients with benign tumors the lower pole regions of the parotid gland, the tumor diameter ranges from 1.0 to 3.1 cm, with an average value of 2.5 cm. All patients experienced successful complete removal of the parotid mass with the minimally invasive retroauricular approach, and all wounds healed in one stage after operation. No serious complications, such as permanent facial paralysis and tumor recurrence, occurred 3 months after the operation. The patients were satisfied with the appearance of the surgical incision (aesthetic score 9.3 ± 0.4), achieving the expected aesthetic effect. @*Conclusion@#This approach is feasible and safe for most small benign parotid tumors located in the lower pole region of the parotid gland and in the posterior region of the jaw. The scar is located in the retroauricular sulcus, which significantly meets the aesthetic needs of the concealed incision for patients.
