ABSTRACT

@#Objective    To retrospectively analyze the surgical treatment of Stanford type A aortic dissection after coronary artery stenting, and to explore the surgical techniques and surgical indications. Methods    Clinical data of 1 246 consecutive patients who underwent operations on Stanford type A aortic dissection from April 2016 to July 2019 in Beijing Anzhen Hospital were retrospectively analyzed. Patients with Stanford type A aortic dissection after coronary artery stenting were enrolled. Results    Finally 19 patients were collected, including 16 males and 3 females with an average age of 54±7 years ranging from 35 to 66 years. There were 11 patients in acute phase, 15 patients with AC (DeBakey Ⅰ) type and 4 patients with AS (DeBakey Ⅱ) type. In AC type, there were 10 patients receiving Sun's surgery and 5 patients partial arch replacement. Meanwhile, coronary artery bypass grafting was performed in 7 patients and mitral valve replacement in 1 patient. Stents were removed from the right coronary artery in 4 patients. In this group, 1 patient died of multiple organ failure in hospital after operation combined with malperfusion of viscera. Eighteen patients recovered after treatment and were discharged from hospital. The patients were followed up for 30 (18-56) months. One patient underwent aortic pseudoaneurysm resection, one thoracic endovascular aortic repair, one emergency percutaneous coronary intervention due to left main artery stent occlusion, and one underwent femoral artery bypass due to iliac artery occlusion. Conclusion    Iatrogenic aortic dissection has a high probability of coronary artery bypass grafting at the same time in patients with Stanford type A aortic dissection after coronary artery stenting. Complicated type A aortic dissection after percutaneous coronary intervention should be treated with surgery aggressively.

ABSTRACT

Medicine is the subject of studying human and its inherent humanistic attribute endows medicine with the temperature it deserves. However, with the continuous improvement of medical testing technology and treatment technology, medical workers pay more attention to diseases and ignore humanistic care, which has become an important factor in the aggravation of patients’ burden and the tension relationship between doctors and patients. There are many virtues in Chinese traditional culture. Sun Simiao’s medical ethics thought of "great doctor with professionalism and sincerity" embodies the core value of the humanistic spirit of Chinese traditional medicine. In his medical ethics thought, the moral and ethics of "great doctor with professionalism and sincerity", the medical practice attitude of "being cautious and diligent", the values of "benevolence for the world" and the professional conduct of "honesty and truth" still have strong practical significance at present. It is the good material to cultivate the humanistic spirit of medical students. This is of great value to integrate Sun Simiao’s medical ethics thought into cultivate medical students with both "professionalism" and "sincerity", practice the original intention of medical and health undertakings, and carry forward cultural self-confidence.

ABSTRACT

Oil-in-water emulsion has promised values in food, pharmaceutical, drug, cosmetic and allied industries. The stable emulsion with long shelf life increases its utility. Many avouring agents are used to increase shelf life as preservative or commercial value. Food grade acids are used to in the emulsion preparation which enhances the taste or avour. The present study focuses the usage of acetic acid, phosphoric acid and oxalic acid. The emulsions for different acids are prepared ranging concentration from 0.001 to 0.003M using lecithin as emulsier and sunower oil. Experiments were done to study organoleptic properties with respect to basicity and dissociation constant values. The colour of the primary emulsion was creamy white and sustainable for acetic acid emulsion to 28 day at experimental temperature 10, 25 and 40 0C. Oxalic acid is recorded a low pH value 1.88 for 0.003 M emulsion solution in compare with acetic acid (3.63) and phosphoric acid (2.32).The relative conductance measurement for oxalic acid shows a very high value 5.6mS to acetic acid 0.257mS and phosphoric acid 0.247 mS. The rst dissociation constant value 5.6x10-2 of oxalic acid is larger relatively compare to phosphoric acid 7.5 x 10-3 and acetic acid 1.8 x10-5. Basicity of acids increases from acetic acid (mono basic), oxalic acid (dibasic) to phosphoric acid (tri-basic).These results strongly supports that basicity and dissociation constant values of acids conspicuously inuence the stability of the emulsion. Higher value of dissociation constant value and basicity, lower the stability of the emulsion.

ABSTRACT

Klara and the Sun is the first work published by Kazuo Ishiguro after he won the Nobel Prize for Literature. Its publication attracted widespread attention for he turned his attention to the gene-editing technology. By telling the story of Clara, an artificial friend (AF) designed to accompany "lifted" children, the author revealed the ethical problems caused by the application of gene-editing technology to "lifted" children. From the theoretical perspective of bioethics and its basic principles, this paper interpreted the ethical issues of gene-editing technology from three aspects that: "lifted" technology, "lifted" technology implementer, and "lifted" technology implementation consequence, and analyzed Kazuo Ishiguro’s technical ethics proposition.

ABSTRACT

The article summarized professor SUN Weizheng's clinical experience in treating multiple myeloma using the clearing-releasing therapy. It is believed that the mechanism of multiple myeloma is kidney essence insufficiency, pathogenic toxin erosion, and blinding of phlegm and stasis. The treatment should consider both the deficiency and the excess, and the root and branch. Thus, the clearing-releasing therapy is proposed. “Clearing” refers to the approach of supplementing deficiency and reinforcing health while advocating the use of clearing and supplementing products to replenish without generating additional pathogenic factors. Additionally, products that clear heat, resolve toxins, dispel stasis, dissolve phlegm, and eliminate masses is suggested to benefit for clearing away pathogenic toxins. “Releasing” means to replenish the normal yang qi with sweet-warm and acrid-warm products on the basis of “clearing” method, and to release the cold pathogen constraint in the muscles and bones. Based on the principle of clearing and releasing, the selfmade Jishi Beverage (济世饮) is formulated to supplement the kidney and secure essence, dispel phlegm and dissolve stasis, resolve toxins and dissipate masses. The prescription can be modified according to syndrome differentiation. It is also advocated to use multiple methods and pay attention to external therapies such as enema for relieving constipation and draining heat, and to combine acupuncture and medicine to relieve pain.

ABSTRACT

This paper introduces professor SUN Shen-tian's clinical thoughts and his characteristics of acupuncture techniques for the treatment of depression based on "psychosomatic medicine". Professor SUN, the master of traditional Chinese medicine, believes that depression refers to comorbidity of "heart mind" and "body", resulting from the "body-mind" disharmony, specially dominated by the emotional disorder. This disease is located in the brain, with the injury of mind and closely related to the heart and liver dysfunction. In pathogenesis, the dysfunction of brain mind and the unhealthy conditions of body and mind are involved. The treatment should focus on "regulating the mind, improving the intelligence, co-modulating the abdominal and brain functions and treating the physical and mental disorders". Baihui (GV 20), Ningshen (Extra) and emotional area on the head are selected as the main points to benefit the intelligence and calming down the mind; the abdominal region 1 and region 8 of "Sun's abdominal acupuncture" are used as the main points of the abdomen to regulate the brain functions. The point prescription is modified according to the symptoms and etiologies. The repeated transcranial acupuncture stimulation and electroacupuncture at low frequency (2 Hz) are crucial to the therapeutic effect. Reliving anxious emotions is specially considered before acupuncture, and the mind is protected and deqi is consolidated during acupuncture.

ABSTRACT

ObjectiveTo analyze the dynamic changes of the main chemical components and color characteristics of Polygoni Multiflori Radix(PMR) during the process of ancient classical nine-time repeating steaming and sun-drying, and to explore the correlation between the changes. MethodHigh performance liquid chromatography(HPLC) was used to determine the contents of gallic acid, stilbene glycoside, emodin-8-O-β-D-glucoside, physcion-8-O-β-D-glucoside, emodin and physcion in PMR during the process of nine-time repeating steaming and sun-drying, and electronic eye technology was used to objectively characterize the color of PMR during the process. Partial least squares(PLS) was used to analyze the correlation between the content of main components and the color characteristics. ResultDuring the nine-time repeating steaming and sun-drying process of PMR, the content of gallic acid gradually increased with the increase of steaming and sun-drying times, and the contents of other components showed fluctuating changes, with an overall decreasing trend in the contents of stilbene glycoside, emodin-8-O-β-D-glucoside and physcion-8-O-β-D-glucoside, and an overall increasing trend in the contents of emodin and physcion. The results of electronic eye analysis showed that the color of PMR became dark, red and blue in the process of nine-time repeating steaming and sun-drying. The results of hierarchical cluster analysis(HCA) and principal component analysis(PCA) of the contents of six main components and the colors showed that the samples steamed and sun-dried for one to four times could be grouped into one category, the other five categories of samples could be grouped into another category. Gallic acid and stilbene glycoside were significantly correlated with the color indexes of PMR during the process of nine-time repeating steaming and sun-drying. ConclusionThe general changes of material basis and color characteristics of PMR during the process of ancient classical nine-time repeating steaming and sun-drying have certain rules, and the establishment of the correlation model between color and index components can realize the judgment and quality evaluation of processing degree of PMR decoction pieces.

ABSTRACT

SUN gene is a group of key genes regulating plant growth and development. Here, SUN gene families of strawberry were identified from the genome of the diploid Fragaria vesca, and their physicochemical properties, genes structure, evolution and genes expression were also analyzed. Our results showed that there were thirty-one FvSUN genes in F. vesca and the FvSUNs encoded proteins were classified into seven groups, and the members in the same group showed high similarity in gene structures and conservative motifs. The electronic subcellular localization of FvSUNs was mainly in the nucleus. Collinearity analysis showed that the members of FvSUN gene family were mainly expanded by segmental duplication in F. vesca, and Arabidopsis and F. vesca shared twenty-three pairs of orthologous SUN genes. According to the expression pattern in different tissues shown by the transcriptome data of F. vesca, the FvSUNs gene can be divided into three types: (1) expressed in nearly all tissues, (2) hardly expressed in any tissues, and (3) expressed in special tissues. The gene expression pattern of FvSUNs was further verified by quantitative real-time polymerase chain reaction (qRT-PCR). Additionally, the seedlings of F. vesca were treated by different abiotic stresses, and the expression level of 31 FvSUNs genes were assayed by qRT-PCR. The expression of most of the tested genes was induced by cold, high salt or drought stress. Our studies may facilitate revealing the biological function and molecular mechanism of SUN genes in strawberry.

ABSTRACT

Increasing physical inactivity in children necessitates the exploration of physical activities that can confer health benefits. The purpose of this study was to review the literature regarding the physiological benefits of Suryanamaskar, a composite yogasana on physical fitness in children. Primary source articles in English, published between 1980 and 2020, in peer-reviewed journals were included. A literature review was carried out using PubMed, Cochrane-Library, Science Direct, and Google Scholar databases. Keywords used were “Suryanamaskar,” “physical fitness,” “sun-salutation,” “physiological effects,” “physical activity,” “health promotion,” “yoga,” and “children.” The methodological quality of the studies was assessed using the Physiotherapy Evidence Database scale. The effect of Suryanamaskar training on physical fitness components, namely flexibility, muscle strength, cardiorespiratory endurance, and cognition, was reviewed. Seven studies matched the inclusion criteria. All seven studies were randomized controlled trials. The level of evidence was of fair quality with a high risk of bias. Following Suryanamaskar intervention, improvement in musculoskeletal function—muscle strength and endurance, and the flexibility of hip joint, wrist joint, hamstrings, and dorso-lumbar fascia were reported. Improvements in cardiorespiratory variables such as peak expiratory flow rate forced vital capacity, forced expiratory volume, vital capacity, and reduction in blood pressure and heart rate are reported in children. Improvements in cognition were also observed following Suryanamaskar’s intervention. Suryanamaskar training confers health benefits and improves physical fitness components such as muscle strength, flexibility, cardiorespiratory endurance, and cognition. Suryanamaskar can emerge as a useful tool to increase physical fitness, deter physical inactivity, and promote healthy behavior (sanskar) in school-going children. High-quality longitudinal randomized control trials need to be undertaken to confirm the same.

ABSTRACT

The obesity worldwide has produced an increase in obesity-related diseases and can be associated with low concentrations of 25-hydroxyvitamin-D. Also obesity and low physical activity can decrease sun exposure, so the aim was to correlate vitamin D intake with serum 25-hydroxyvitamin-D levels and to assess sun exposure habits in schoolchildren with obesity. Materials and methods. A correlational study was performed from January 2017 to January 2018 on 103 children between 6-12 years of age, with a body mass index ≥+2SD for age and sex, according to the World Health Organization. Blood samples were taken to determine the serum concentrations of 25-hydroxyvitamin-D, a nutritional survey to determine the vitamin D intake and a sun exposure questionnaire were applied. A Spearman correlation coefficient analysis was performed. Results. Forty-seven percent of the children were girls. The median years of age was 10. The median serum 25-hydroxyvitamin-D levels were 35.5 ng/mL, 74.8% had sufficient levels, 25.2% had insufficient levels. The median vitamin D intake was 214.7IU in boys and 231.9IU in girls. Regarding sun exposure, most of the children had excessive levels of sun exposure and inadequate sun protection practices. A positive correlation between vitamin D intake and serum 25-hydroxyvitamin-D was identified only in boys (rho=0.276, p=0.041). Conclusion. A positive correlation between vitamin D intake and serum 25-hydroxyvitamin-D levels was found in obese boys and excessive levels of sun exposure with inadequate sun protection practices in boys and girls(AU)

ABSTRACT

Abstract The skin is regularly exposed to several environmental aggressions, including solar radiation, whose biological effects can induce sunburn, dyschromia, skin aging and cancer. Among the photoprotection measures, sunscreens comprise a relevant part of the strategy aimed to prevent solar radiation damage and, for effective action, the patient must adhere to the product use and the latter, in turn, must follow technical parameters to promote adequate protection. This review article brings together the most current and relevant concepts about photoprotection for dermatological use, including the challenges for their formulation, the risks of certain photoprotective active substances for individual and environmental safety and the importance of stringency in determining the product efficacy, considering the regulatory aspects, highlighting relevant differences between Brazil and other countries. Thus, when assessing a sunscreen, not only the visual aspects and sensory perception will be immediately evaluated, but also the quality and suitability of the vehicle, the chemical composition of the formulation, the environmental risks, the photostability of the screening system, and the measurement of its protection spectrum. Technical knowledge of sunscreens can help dermatologists in this important role of educating patients about the best photoprotective strategies in each situation.

ABSTRACT

Aims: Lovastatin is a cholesterol-lowering drug produced by several filamentous fungi as a secondary metabolite. Its concentration in mushroom can be affected by many post-harvest processes. In this study, fruiting bodies of Pleurotus ostreatus grown on corn cobs were used to evaluate the effect of conservation techniques on their lovastatin concentration. Methodology: acid and blanching treatments combined to different processes of fruiting bodies conservation like sun drying, oven drying and canning by autoclave cooking were tested to evaluate their effect on lovastatin concentration. Results: Sun drying, oven drying at 60°C/70°C and canning using autoclave cooking at 100°C did not significantly affect lovastatin concentration. On the contrary, oven drying at 80°C caused the reduction of this compound up to 45.4% with respect to fresh product irrespective of the precedent use of citric acid or blanching treatments. Also, during canning, the use of autoclave heat treatments at temperatures 110 and 121°C for 15 minute caused a significant reduction of lovastatin concentration of about 52.2% and 48.9% respectively compared to the control. In this regards, it can be concluded that processes that use dry thermal treatments higher than 80°C and autoclave heat treatments higher than 100°C will contribute to the reduction of lovastatin, the cholesterol-lowering compound in Pleurotus ostreatus.

ABSTRACT

In order to investigate functions of NOP2/Sun RNA methyltransferase 2 (Nsun2) in melanoma cells, shRNA lentiviral vectors were constructed to target Nsun2 in mouse melanoma B16 cells. After treated B16 cells with recombinant virus, the mRNA and protein levels of NSUN2 in the interference group were significantly reduced, and the knock down efficiency reached 80%. EdU staining assay showed significant inhibition of DNA synthesis in Nsun2 knock-down B16 cells. RNA-seq was used to systematically analyze the gene expression of the Nsun2 knock-down and the control cells. A total of 1062 differentially expressed genes (DEGs) were screened, of which 678 were up-regulated and 384 were down-regulated. DEGs were mainly enriched in chromosome, centromere region, and protein binding GO terms. KEGG analysis showed that DEGs were significantly enriched in cell cycle, DNA replication, cell senescence and other pathways. RT-qPCR and RNA-seq data demonstrated that Cdk2, Ccnal, Cdc25b and other genes related to enhance cell division were significantly down-regulated, while Gadd45g and Gadd45a and other genes arresting cell growth were significantly up-regulated. Collectively, this study indicated that NSUN2 affects melanoma cell proliferation by regulating cell cycle and DNA replication, and provided fundamental data for exploring the molecular mechanism of melanoma genesis and development.

ABSTRACT

Objective:To evaluate the efficacy of arch-clamping technique in Sun’s procedure to repair acute type A aortic dissection.Methods:20 consecutive patients[17 males with mean age of (49.7±10.9)years old] with acute type A aortic dissection who underwent total arch replacement and frozen elephant trunk implantation (TAR+ FET) from May 2019 to April 2020 were divided into group with using arch-clamping technique during operation (arch-clamping group) or group without (traditional group). Circulatory arrest time, cardiopulmonary bypass time, recovery time, ventilation time, platelet count during the first 3 days postoperatively, blood loss during the first 12 h postoperatively, death, cerebral infarction and acute renal failure were compared. Patients with advanced age(age>70 years old ), or malperfusion syndrome preoperatively or who did not receiving TAR+ FET surgery were excluded from this study. Results:Circulatory arrest time[(4.1±0.6 )min vs. (17.2±2.6)min, P=0.001] and cardiopulmonary bypass time[(158.4±6.8 )min vs. (198.2±12.6)min, P=0.01] were significantly lower in arch-clamping group, compared with traditional group. Postoperative recovery time[(8.9±2.6 )h vs. (16.0±7.3)h, P=0.94] and ventilation time[(13.6±2.2)h vs. (34.1±14.3)h, P=0.18] were non significantly lower in arch-clamping group. Although postoperative platelet counts were higher in arch-clamping group during the first three days, those difference did not reach statistical significance ( P>0.05). All patients were discharged alive. There was no significant difference among the two group with respect to drainage during the first 12 h postoperatively, death, cerebral infarction and acute renal failure. Conclusion:Arch-clamping technique decrease circulatory arrest time dramatically and provide good protection of brain, coagulation and renal function by shortening ischemia time, thereby reducing postoperative complications. It is a safe and feasible innovative approach to effectively improve surgical outcome of Sun’s procedure for repair of acute type A aortic dissection.

ABSTRACT

@#The Bornean sun bear, a subspecies of the endangered Malayan sun bear, resides only on Borneo Island and little is known about diseases or parasites that may impact their health. In 2019, blood and ticks were collected from 46 captive bears held at the Bornean Sun Bear Conservation Centre in Sabah, Malaysia during annual health examinations in response to previous blood smear analysis that revealed presumptive haemoparasites in more than half the resident bears. Polymerase chain reaction detected a unique Babesia sp. in one of the bears. Disease surveillance of mosquitoes trapped along the outer perimeter of the bears’ outdoor enclosure did not reveal any malaria parasites. This research marks the first documented case in Bornean sun bears of both a Babesia sp. and the Ixodes tick Haemaphysalis nr koningsbergeri. More research on incriminating the vector and the effects of Babesia infection on the health of Bornean sun bears is needed. Due to the zoonotic nature of babesiosis, mitigative actions should be taken to protect any humans that work with or come into close contact with these captive bears or their enclosures.

OBJECTIVE@#To compare the clinical effect of SUN 's abdominal acupuncture and conventional acupuncture in the treatment of depression after methamphetamine withdrawal.@*METHODS@#A total of 80 female patients with depression after methamphetamine withdrawal were randomly divided into an observation group (40 cases, 1 case dropped off) and a control group (40 cases, 2 cases dropped off). The control group was treated with conventional acupuncture at Baihui (GV 20), Yintang (GV 29), Taichong (LR 3), Shenmen (HT 7), Neiguan (PC 6), Danzhong (GV 17), and the observation group was treated with SUN 's abdominal acupuncture at area 1 of the abdomen and area 8 of the abdomen. Both groups were treated once a day, 30 min each time, 6 days as a course of treatment, 1 day rest between treatment courses, a total of 4 courses of treatment. The scores of withdrawal symptoms, Hamilton depression scale (HAMD), Pittsburgh sleep quality index (PSQI) scale and serum serotonin (5-HT) level were compared between the two groups before and after treatment.@*RESULTS@#After treatment, the scores of withdrawal symptoms, HAMD and the various scores and total score of PSQI scale in the two groups were all lower than before treatment (P<0.01), and the scores of withdrawal symptoms, HAMD and the sleep quality, time to fall asleep, sleep time scores and total score of PSQI in the observation group were lower than the control group (P<0.05, P<0.01). After treatment, the serum 5-HT level of the two groups was increased (P<0.01), and that in the observation group was higher than the control group (P<0.05).@*CONCLUSION@#SUN 's abdominal acupuncture can improve withdrawal symptom, depression and sleep quality, increase serum 5-HT content in treatment of depression after methamphetamine withdrawal, and has better effect than conventional acupuncture.
