ABSTRACT

1) A total of 73 incidents and accidents related to guidance provided to residents obtained from data published by the Japan Council for Quality Health Care were investigated.2) These events can be divided into the following three categories: (1) Events that occurred during direct guidance to a resident by a supervising physician, (2) Events that occurred when a resident performed a procedure by himself or herself without the guidance of a supervising physician, although the supervising physician was near the resident, (3) Events that occurred when a resident performed a procedure by himself or herself while a supervising physician was absent.3) Regarding measures to prevent recurrence, the present results suggests that, in addition to the individual efforts of supervising physicians and residents, there is a need for system development through increased cooperation between the postgraduate clinical training center, each section/department and the department of medical safety management.

ABSTRACT

In this study, we conducted a questionnaire survey among community pharmacists in Japan, to explore how they and the pharmacy’s management support a fellow pharmacist who has committed a medical error. Using stratified random sampling, we selected 900 pharmacies from national community pharmacies. The response rate for pharmacies was 31.7%, and we analyzed data from 572 pharmacists. It was observed that after a pharmacist committed a medical error, the subsequent treatment was relevant to the years of experience. Supervising pharmacists with little experience tended to answer that they “do not know how to get along with the colleague”. The backup to supervising pharmacists with little experience should be considerable to give care to pharmacist caused dispensing error. A few pharmacies have a support system for pharmacists concerned in an error; 38.9% of supervising pharmacists emphasized the significance of mental support. Therefore, it was suggested that concluded that head offices of pharmacies and the community pharmaceutical association should review their mental support system for pharmacists to work after committing an error.

ABSTRACT

Objective To investigate the role of supervising-nurse in self-protection of medical staffs treating patients with Ebola virus disease in Liberia. Methods Based on the Standard Procedure of Personal Protection, we designed a Chinese- English bilingual Inspection Table for putting on personal protective equipment (PPE) for Chinese People) Liberation Army medical team to Liberia and another one for takingoff PPE. The supervising-nurse gave supervising tips and psychologically positive implications to all medical staffs working in the observing room and treatment ward. Nurses also recorded and summarized the problems and mental status which have been prevented and stopped, and the efficacy of the supervision was evaluated. Results From Jan. 16, 2015 to Mar. 19, 2015, about 3 000 person-times and 126 000 supervision items for the Chinese and Liberia staffs working in the observing room and treatment ward were involved. A total of 1 532 potential problems were prevented by supervising-nurse. Conclusion A supervising-nurse can effectively ensure the self-protection of medical staffs involved in the frontline treatment of patients with Ebola virus disease, guaranteeing the “zero infection” of medical staffs.

ABSTRACT

Background and purpose:The incidence of liver cancer is high in China. Primary liver cancers usually occur in patients with liver cirrhosis, which is a challenge for the early diagnosis of liver cancer. Our purpose is to investigate the efifcacy of contrast-enhanced ultrasonography (CEUS) in the early identiifcation and diagnosis of small hepatocellular carcinoma (HCC) by regularly tracking and supervising the high risk population. Methods:A total of 320 high risk patients of HCC admitted in our hospital from February 2011 to November 2013 were enrolled in this prospective study. All patients underwent conventional ultrasound and hepatic CEUS. The differential diagnosis of malignant HCCs from benign ones was based on the enhancement patterns of hepatic lesions in different phases on CEUS. Results:Twenty patients were diagnosed as small HCC among 320 HCC high risk patients who were under regular surveillance using CEUS and all were pathologically conifrmed. Seven of the 20 HCC cases were smaller than 1.0 cm and 13 measured 1.1-2.0 cm. There were 6 (30.0%) HCCs presented as“early wash-in and slow wash-out”atypical pattern of HCC. The small size of the lesion and iso-echogenicity were the main factors of atypical pattern of HCC on CEUS.Conclusion:Ultrasonography and CEUS surveillance is a useful strategy for the early detection of small HCCs in high risk patients, which can help them to receive proper therapeutic management in time.

ABSTRACT

Introduction: The aim is to describe the teaching activities in the clinical context from the residents' perspective. Objectives: 1) To determine how frequently clinical professors perform teaching activities in the four areas: teaching, supervision, organization and interpersonal relationships, 2) to distinguish professors' performance by specialty 3) To inquire differences in the evaluation of professors according the residents' academic year. Materials and methods: The "Survey among PUEM's Students 2010" included 13 frequency questions and 2 of valuation in the section of teaching performance. Teaching activities from the four previously mentioned areas in ten specialties were considered. The sample was 1816 cases; significant associations (p <0.05) among variables were assessed. Results: In the answer choices: "always" and "almost always", interpersonal relationships obtained the highest score (>80%), the regular organization of academic and healthcare activities was mentioned in more than 2/3 of the answers. The frequency of teaching activities related to the curricular structure, and supervision were situated below average. The highest scores were for family medicine professors and the lowest for orthopedia. By academic grade, first and fifth year residents considered that teaching activities were more constant; while second year students considered them the least constant. Discussion: Personal relationships between clinical professors and residents are adequate. Students perceive themselves integrated to their wards; however the fulfillment of academic programs and supervision are not frequent enough.

ABSTRACT

Our institute was established in 1979 with the mission given by the Ehime prefectural government and Medical Association to supply traditional medical care under the leadership of MDs. We developed The concept of chronological analysis for researching individual health needs and also instituted associated systems such as chronological analysis care system and chronological audit system. Through these systems, our staff of therapists developed a common integrated cognitive perspective and we established the identity of our traditional medical care as a system in which supervising physicians offer leadership of a team care system of traditional medicine. In this way we have fulfilled the heavy mission given to us by the government and medical association.<BR>We were also given a heavy mission to research about the individuation of each traditional medical technique, by seniors such as Takeshi Itakura. For this purpose we adopted the chronological analysis care system as the field scientific method of case study. These systems have been useful to improve the care level of the staff and also useful for training program for post graduate medical and traditional medical students.
