ABSTRACT

Squamous cell carcinoma of supraglottis is a common head and neck malignancy, comprising 30% of laryngeal carcinoma. Supraglottic carcinoma commonly metastasizes to the Levels II, III, and IV cervical nodes, and distant metastases are uncommon. Delayed local and regional recurrences are known, local recurrence being more common. We report a case of carcinoma supraglottis with the advanced locoregional disease at initial presentation and treated with neoadjuvant chemotherapy followed by chemoradiotherapy with a complete response on post-treatment positron emission tomography-computed tomography (PET-CT) scan. Surveillance PET-CT showed an isolated lesion in the left post-styloid parapharyngeal space, where neurogenic tumors are more common and isolated nodal metastasis is unusual. We did ultrasound-guided FNAC and cytology confirmed the presence of metastatic poorly differentiated carcinoma.

ABSTRACT

In 1942, Stout and Murray first used the term hemangiopericytoma to describe a tumor which is distinguished histologically from other types of vascular neoplasm characterized by proliferation of pericytes. It is a rare neoplasm that was originally described as a vascular tumor derived from the pericytes. They account for 2%–3% of all soft tissue sarcomas in humans and they occur mainly in the musculoskeletal system. About 15%-30% of all hemangiopericytomas occur in the head and neck region. Hemangiopericytoma of supraglottis is very rare neoplasm with only nine cases reported in literature and ours is the tenth case overall and first case in pediatric age group. Herein, we are presenting an extremely rare case report of hemangiopericytoma of supraglottis in a 6-year-old male child who presented with stridor followed by which tracheostomy was done. Later, the patient was treated initially with radiotherapy followed by surgery, i.e., laryngectomy in view of residual disease postcurative radiotherapy. Hence, hemangiopericytoma is a very rare tumor overall and can present in pediatric age group and can be one most important differential diagnosis because many patients in this age group, stridor most commonly occurs due to the infectious causes such as influenza virus and diphtheria-induced croup, i.e., laryngotracheobronchitis

ABSTRACT

Korean Society of Thyroid-Head and Neck Surgery appointed a Task Force to develop clinical practice guidelines for the surgical treatment of laryngeal cancer. This Task Force conducted a systematic search of the EMBASE, MEDLINE, Cochrane Library, and KoreaMed databases to identify relevant articles, using search terms selected according to the key questions. Evidence-based recommendations were then created on the basis of these articles. An external expert review and Delphi questionnaire were applied to reach consensus regarding the recommendations. The resulting guidelines focus on the surgical treatment of laryngeal cancer with the assumption that surgery is the selected treatment modality after a multidisciplinary discussion in any context. These guidelines do not, therefore, address non-surgical treatment such as radiation therapy or chemotherapy. The committee developed 62 evidence-based recommendations in 32 categories intended to assist clinicians during management of patients with laryngeal cancer and patients with laryngeal cancer, and counselors and health policy-makers.

ABSTRACT

Adenoic cystic carcinoma (ACC) is a malignant tumor, occuring in the major and minor salivary glands. Laryngeal ACC is a rare malignancy that constitutes less than 1% of all laryngeal malignancies. ACC is characterized by slow growth, multiple recurrences and late distant metastasis. The treatment of the ACC is by wide excision or by combining wide excision with postoperative radiotherapy. A 33-year-old woman, who had been diagnosed with epiglottic cyst in 2007, complained of throat discomfort that had been aggravating for the past two weeks. The tumor was treated with transoral laser excision. The pathologic report revealed an adnoid cystic carcinoma on the epiglottis; it was composed of cribriform patterned nests and columns of tumor cells. No recurrence was detected until post-operative 16 months by endoscopic examination. Transoral laser excision was suggested for successful treatment for the adenoid cystic carcinoma.

ABSTRACT

Small cell carcinoma is primarily a lung malignancy occurring rarely in extra pulmonary sites such as the larynx, nasal cavity, paranasal sinuses, and oral cavity. The diagnosis was based on histological investigation expanded with immunohistochemical tests of a fragment of a tissue. The immunohistochemical stain was positive for synaptophysin, CD56 and cytokeratin. So we diagnosed small cell carcinoma. The treatment of small cell carcinoma diagnosis differed from the treatment of other neuroendocrine carcinomas and squamous cell carcinomas of the larynx. We report a case of 72-year-old man with a rare case of small cell carcinoma in the supraglottic region and a brief review of literature.

ABSTRACT

La laringe constituye un órgano anatómicamente complejo. Glotis, supraglotis y subglotis son, en orden de importancia relativa, los sitios más frecuentes de asentamiento de tumores, los cuales se distinguen clínicamente por el modo en que afectan las principales funciones de la laringe: fonación y deglución. Describir la frecuencia, características sociodemográficas y tumorales correspondientes a los casos incidentes de cáncer de laringe en Cuba según subsitio anatómico dentro de la laringe. Se realizó un estudio descriptivo a partir de los casos nuevos de cáncer de laringe reportados al Registro Nacional de Cáncer durante el período 1988-2003. Se analizó información sociodemográfica de los casos y otras variables relacionadas con el diagnóstico y la enfermedad tumoral. Se utilizó la prueba Chi-cuadrado de independencia para determinar asociación entre las variables en estudio con un nivel de significación de 0,05. El cáncer de laringe ocupa una de las primeras causas de incidencia y mortalidad por cáncer en Cuba, fundamentalmente en hombres entre 50-74 años de edad. Mientras que el cáncer de subglotis es poco frecuente, el cáncer de glotis es hasta tres veces más frecuente que el de supraglotis. El cáncer de glotis da síntomas tempranamente pudiendo ser detectado mediante un examen relativamente sencillo. Considerando las características del sistema nacional de salud cubano, este hecho puede abrir una expectativa al diagnóstico temprano, por lo que es recomendable evaluar la factibilidad y eficacia del examen otorrinolaringológico -como parte del examen periódico anual- fundamentalmente en hombres de 50 años y más, con historia de tabaquismo o alcoholismo, con el fin de detectar y tratar tempranamente la enfermedad.


Larynx is an anatomically complex organ. Glottis, supraglottis and subglottis are, in order of relative importance, the most frequent tumor locations, which are clinically characterized by the way they affect the main functions of the larynx: phonation and deglutition. Objectives the present paper describes the frequency, socio-demographic and tumorous characteristics of laryngeal cancer cases in Cuba. A descriptive study was conducted, taking the new laryngeal cancer cases reported to the National Register of Cancer from 1988 to 2003 as a basis. Social and demographic information of cases and other variables related to diagnosis and to the tumorous disease were analyzed. Chi-square test of independence determined the association of variables under study, with significance level equal to0,05. Laryngeal cancer is one of the first causes of incidence and mortality due to cancer in Cuba, mainly in males aged 50-74 years. Although subglottis cancer is rare, glottis cancer is up to three times as frequent as that of supraglottis. Glottis cancer shows early symptoms, so it can be detected on a relatively simple examination. Considering the Cuban health system characteristics, this event could open up possibilities for early detection, therefore, it would be advisable to evaluate the feasibility and efficacy of the otorhinolaryngology test -as part of the annual systematic examination -mainly in men aged 50 years and more, with history of smoking/alcoholism, in order to detect and treat the disease early.
