ABSTRACT

The reported incidence of synchronous multiple primary cancer (SMPC) is rare, and it is even less common to observe synchronous solid tumor with a hematological malignancy. We report five cases of solid tumor presented synchronously with hematological malignancy, all observed within a 2 year period at the oncology department of a university hospital in Shanghai, China. These individual cases included lung adenocarcinoma with chronic myelogenous leukemia, colon cancer with solitary plasmocytoma, gastric adenocarcinoma with diffuse large B cell non-Hodgkin's lymphoma, lung adenocarcinoma with multiple myeloma, and colon cancer with diffuse large B cell non-Hodgkin's lymphoma. It is challenging to therapeutically control the biological behavior of concurrent multiple primary tumors, and there is no standard treatment for such rare conditions. In this paper we discuss these five cases of SMPC and their treatments.

ABSTRACT

A 60-year-old female was admitted with epigastric pain lasting a month. Preoperative diagnosis was choledochal cyst with anomalous pancreaticobiliaryductal union (APBDU), C-P type. A papillary mass measuring 2.5 x 1.9 cm was found adjacent to the pancreaticocholedochal junction. Gallbladder (GB) cancer was also observed. Pyloric-preserving pancreaticoduodenectomy (PPPD) was performed. The patient received adjuvant chemotherapy/radiation therapy on the tumor bed. The gallbladder cancer showed serosal invasion, while the bile duct cancer extended into the pancreas. Although common bile duct (CBD) cancer lesion showed focally positive for p53 and the gallbladder cancer lesion showed negative for p53, the Ki-67 labeling index of the CBD cancer and GB cancer were about 10% and 30%, respectively. Nine months after curative resection, a stricture on the subhepatic colon developed due to adjuvant radiation therapy. Localized peritoneal seedings were incidentally found during a right hemicolectomy. The patient underwent chemotherapy and had no evidence of tumor recurrence for two years after PPPD.

ABSTRACT

An association of carcinoid tumors with another primary neoplasm has been reported in 17~53% of cases, with an especially high incidence of adenocarcinomas. However, simultaneous association of a gastric carcinoid tumor with a gastric adenocarcinoma as a synchronous double primary cancer is extremely rare. We report a case of a synchronous double primary cancer associated with a gastric carcinoid and early gastric cancer in a 63-year-old male patient.

ABSTRACT

PURPOSE: This study aimed to evaluate the incidence, nature and prognosis of multiple primary malignancies in renal cell carcinoma. MATERIALS AND METHODS: From June 1995 to April 2004, we retrospectively reviewed the records of 578 patients who underwent an operation for renal cell carcinoma at Yonsei University. The incidence of other primary malignancies, in addition to the renal cell carcinoma, was determined and classified as antecedent, synchronous or metachronous. We analyzed the influence of the other primary malignancies on the prognosis, and the overall survival rates of the patients with multiple primary malignancies were compared to the remaining patients. RESULTS: Of the 578 patients, 62 patients (10.7%) had at least one malignancy beside renal cell carcinoma. In these cases, renal cell carcinoma was commonly incidental, small or low stage (75.8%). 22 patients (35.4%) had gastrointestinal cancer and 11 patients (17.7%) had hepatobiliary cancer. The malignancies were antecedent in 25 (40.3%), synchronous in 24 (38.1%), and metachronous in 14 patients (22.2%). On the analysis of overall survival with using the log rank test, there was no statistically significant factor for the presence of other antecedent or synchronous malignancies. CONCLUSIONS: Primary malignancy associated with renal cell carcinoma in the Korean population shows a different pattern from that of Western countries. The expected survival rate of patients suffering from renal cell carcinoma with tumor involvement of other organs, if surgically treated, is similar to that of renal cell carcinoma alone, so aggressive surgical treatment of renal cell carcinoma should be performed.

ABSTRACT

Synchronous multiple primary neoplasms of the female reproductive tract are rare cases and has ranged from 0.54% to 2.1%. Rare cases of synchronous endometrial cancer and squamous cell carcinoma in situ of the cervix have also been discribed in the literature. Recently, it has been reported that the frequency of multiple primary neoplasms is increasing as a result of the lengthening average lifetime, progress in cancer screening techniques, and improved cancer therapy. We have experienced a case of synchronous endometrial cancer and squamous cell carcinoma in situ of the cervix in Department of Obstetrics and Gynecology and Pathology, College of Medicine Korea University and discribed our case with a brief review of literature.
