ABSTRACT
The syndrome differentiation of Yin and Yang has the function of controlling the other six principles in the eight principles syndrome differentiation, which is a higher level or general induction of the disease. In the clinical process of traditional Chinese medicine, syndrome differentiation of Yin and Yang runs through the whole process of disease diagnosis and treatment. For Parkinson's disease, syndrome differentiation of Yin and Yang is particularly important. Different symptoms, the transformation of pathogenesis during the development of the disease and the treatment of traditional Chinese and western medicine all reflect the characteristics of Yin and Yang opposition restriction, mutual root and mutual use,and the transformation of ebb and flow. This article discusses the background, application and value of Yin-Yang syndrome differentiation from three aspects:the origin and application of yin-yang syndrome differentiation, the basis of Parkinson's disease syndrome differentiation, and the status and role of Yin-Yang syndrome differentiation in Parkinson's disease. It is of great significance to guide the diagnosis and treatment of Parkinson's disease with "Yin-Yang as the key point".
ABSTRACT
ObjectiveTo explore the elements, distribution and characteristics of traditional Chinese medicine (TCM) syndromes in depressive episodes of bipolar disorder (BD). MethodsBasic information, along with the four examination information, the Hamilton Depression Scale and Young Mania Rating Scale scores, were collected from 293 outpatients with BD at Beijing Anding Hospital, Capital Medical University. The four examination information with an occurrence rate greater than 12% were retained. The R language “dist” function was used to calculate the distances between samples using the Euclidean distance method. The hierarchical clustering of the four examination information was performed using the “hclust” function and the squared Euclidean distance method. A team of five researchers was formed to determine the nature and location of the essential elements of TCM syndrome in BD based on the clustering results. The PC algorithm was used to construct a Bayesian network model of the essential elements. The working group combined the essential elements of TCM syndromes in the Bayesian network according to the reference model results, and then extracted common TCM syndromes. The score of each patient based on the essential elements was matched with the common TCM syndromes to determine the syndrome type of each patient. The working group then performs conformity and revision based on this, obtaining the final distribution of TCM syndromes for the patients. ResultsThere were 77 common TCM symptoms in BD with a frequency greater than 12%. The top 15 symptoms with higher frequencies were slippery pulse, mental fatigue and lack of strength, wiry pulse, excessive rumination, preference for solitude, vexation, agitation and irritability, dry mouth, palpitations, profuse dreaming, unwarranted worries, chest oppression, thin white coating, amnesia, frequent sighing, and poor appetite. TCM syndrome elements of BD can be grouped into 11 categories. The nature of disease-related essential elements included fire, qi deficiency, blood deficiency, qi counterflow, yin deficiency, dampness, heat, fire from constraint, and phlegm. The location of disease-related essential elements included heart, liver, spleen, stomach, kidney, bladder channel, and gallbladder. By constructing a Bayesian network model and considering the opinions from the experts, six common syndromes of BD were identified, among which the highest proportion was heart-stomach heat accumulation, accounting for 27.99% (82 cases), followed by heart-spleen deficiency (55 cases, 18.77%), non-interaction between the heart and the kidney (49 cases, 16.72%), liver constraint and blood deficiency (42 cases, 14.33%), heart qi deficiency (37 cases, 12.63%), and damp-heat in the liver and gallbladder (28 cases, 9.56%). ConclusionsThe nature of disease-related elements of BD are predominantly fire and heat, while the location of disease-related essential elements are primarily associated with the heart, liver, and spleen. The most common TCM syndromes are heart-stomach heat accumulation and heart-spleen deficiency.
ABSTRACT
Based on the theory of qi, blood and fluids, and taking into account of the pathogenesis evolution process from constraint to phlegm, stasis and then mass in pulmonary nodules, an attempt has been made to construct a three-dimensional differentiation system for pulmonary nodules from the dimensions of time and space. The temporal progression of the early, middle, and late stages of pulmonary nodules reflects the pathological changes from constraint to phlegm and then stasis in the metabolism disorders of qi, blood and fluid. The spatial structures such as size, density, and morphology of pulmonary nodules reflect the pathological states of the duration, severity, and primary and secondary conditions of qi, blood and fluid metabolism disorders. Based on the temporal progression, the therapeutic principles have been proposed, which are dispelling pathogenic factors and promoting the use of beneficial factors to interrupt the growth momentum in the early stage, removing turbidity and dispersing phlegm to reduce the degree of nodules in the middle stage, and dispersing nodulation and eliminating abnormalities in the late stage. Based on the spatial structures, the suggested therapeutic methods are using wind herbs, employing multiple approaches to treat phlegm, and promoting blood circulation to resolve stasis, so as to provide theoretical reference for the systematic diagnosis and treatment of pulmonary nodules in traditional Chinese medicine.
ABSTRACT
Data analysis models may assist the transmission of traditional Chinese medicine (TCM) experience and clinical diagnosis and treatment, and the possibility of constructing a “data-knowledge” dual-drive model was explored by taking gastric precancerous state as an example. Data-driven is to make clinical decisions around data analysis, and its syndrome-differentiation decision-making research relies on hidden structural models and partially observable Markov decision-making processes to identify the etiology of diseases, syndrome elements, evolution of pathogenesis, and syndrome differentiation protocols; knowledge-driven is to make use of data and information to promote decision-making and action processes, and its syndrome-differentiation decision-making research relies on convolutional neural networks to improve the accuracy of local disease identification and syndrome differentiation. The “data-knowledge” dual-driven model can make up for the shortcomings of single-drive numerical simulation accuracy, and achieve a balance between local disease identification and macroscopic syndrome differentiation. On the basis of previous research, we explored the construction method of diagnostic assisted decision-making platform for gastric precancerous state, and believed that the diagnostic and decision-making ability of doctors can be extended through the assistance of machines and algorithms. Meanwhile, the related research methods were integrated and the core features of gastric precancerous state based on TCM syndrome differentiation and endoscopic pathology diagnosis and prediction were obtained, and the elements of endoscopic pathology recognition based on TCM syndrome differentiation were explored, so as to provide ideas for the in-depth research and innovative application of cutting-edge data analysis technology in the field of intelligent TCM syndrome differentiation.
ABSTRACT
By exploring the core ideas of the "Xingnao Kaiqiao" theory, analyzing the relationship between the "Xingnao Kaiqiao" theory and the TCM encephalopathy, and exploring the necessity of acupuncture for the treatment of TCM encephalopathy, in order to clarify how to diagnose and treat brain diseases in acupuncture and moxibustion. The "Xingnao Kaiqiao" is the core of the theory of the "Tiaoshen", the acupuncture of "Xingnao Kaiqiao" was based on the principle of "Xingnao","Xingshen" and "Tiaoshen", the idea of the "Tiaoshen" was inseparable from the diagnosis and treatment of the TCM encephalopathy. Based on the special features of acupuncture therapy and complexity and particularity of TCM encephalopathy, acupuncture and moxibustion treatment of encephalopathy was supposed to have their own unique system of syndrome differentiation and treatment. During the practice of acupuncture, the diagnosis and treatment of TCM encephalopathy should be based on "brain", "Xingnao Tiaoshen" as a general rule, adding and subtracting from the main points of the "Xingnao Kaiqiao", and the different brain diseases are matched with appropriate treatments.
ABSTRACT
BACKGROUND:Knee osteoarthritis is a common disease in middle-aged and elderly people. It is a kind of disease that seriously affects the quality of life of patients and even has the risk of disability. Therefore, the pathogenesis and treatment of knee osteoarthritis have become the focus of research. In Chinese medicine, knee osteoarthritis is often treated as"biness,"which is closely related to"biness"caused by blood stasis and blood vessels blocking collaterals in the theory of"blood stasis"in traditional Chinese medicine.Iron overload is a kind of pathological state caused by iron metabolism disorder, which highly coincides with the pathogenic characteristics and clinical manifestations of the"blood stasis"theory of traditional Chinese medicine, and is a risk factor that promotes the development of knee osteoarthritis. OBJECTIVE:Based on the"blood stasis"theory,to summarize the effects of iron overload on cartilage metabolism and subchondral bone reconstruction,to lay a new theoretical foundation for the treatment of knee osteoarthritis with traditional Chinese medicine, and to explore the therapeutic effect of traditional Chinese medicine for promoting blood circulation after interfering with bone tissue. METHODS:CNKI,WanFang database,PubMed and Web of Science databases were searched for relevant literature. The Chinese search terms were"ferroptosis, iron, iron overload, osteoarthritis, blood stasis"and the English search terms were"ferroptosis, iron, iron overload, osteoarthritis,TCM."In the end,76 articles were included for further review. RESULTS AND CONCLUSION:First of all,we explored the potential of the"blood stasis"theory in treating knee osteoarthritis, and found that"blood stasis"is a crucial part in the progress of knee osteoarthritis, indicating that the"blood stasis"theory is the key to the treatment of knee osteoarthritis in traditional Chinese medicine.Secondly,"blood stasis"and iron overload have a high degree of similarity in pathogenic factors, clinical manifestations, and pathogenic characteristics, suggesting the possibility of"blood stasis"theory in treating iron overload. This finding reminds us that iron overload may be an important mechanistic basis for the"blood stasis"theory in the treatment of knee osteoarthritis. The extracts of blood-activating drugs can relieve iron overload and treat knee osteoarthritis, but the specific mechanism is still unclear. Therefore,we believe that the relationship between"blood stasis"theory and iron overload and related mechanisms are important research directions for knee osteoarthritis in the future. The related mechanism of"blood stasis"theory to alleviate iron overload and then treat knee osteoarthritis also provides a theoretical basis for the modernization of traditional Chinese medicine, such as the development of new drugs and innovative usage, and has certain guiding significance for clinical practice.

ABSTRACT
Objective To explore the therapeutic effect of traditional Chinese medicine (TCM) on subsolid nodule (SSN). Methods A practical randomized controlled study method, including 254 SSN patients was adopted. The patients were divided into the TCM (102 cases) and follow-up (152 cases) groups. The follow-up group received regular check-ups in accordance with the guidelines, and the TCM group received TCM syndrome differentiation treatment for 24 weeks. The two groups were compared in terms of the changes in their SSN diameter, SSN number, TCM symptom score, and overall therapeutic effect before and after treatment. Adverse reactions and safety indicators were also recorded. Results The TCM group showed a significantly higher effective rate of treatment (16.7%) than the follow-up group (2.6%) (P<0.01). Compared with their condition before treatment, the TCM group showed no significant changes in their SSN diameter and number but presented considerably reduced fatigue, yellow and red urine symptoms, and overall TCM symptom score (P<0.05). The follow-up group exhibited significantly increased diameter and number of SSN (P<0.01). The follow-up group showed the significantly higher increase in SSN diameter after treatment than the TCM group (P<0.05). Moreover, the follow-up group showed significantly higher fatigue, depression, yellow and red urine symptom scores, and overall TCM symptom score than the TCM group (P<0.05 or P<0.01). Conclusion TCM treatment for SSN has a distinct clinical efficacy, reduces the malignant risk of SSN and improves clinical symptoms of SSN patients, and is safe and feasible.
ABSTRACT
Due to the complexity of traditional Chinese medicine (TCM) interventions and the diversity of herbal components, single-omics technologies such as genomics, transcriptomics, proteomics, and metabolomics often cannot comprehensively elucidate the scientific connotations of TCM. Multi-omics technologies driven by system biology can analyze the theoretical connotations and application mechanisms of TCM from different levels such as genes, gene expression, proteins, and metabolites, in line with the holistic view of TCM, which helps to promote the modernization of TCM. By reviewing the literature on the application of omics technologies in the field of TCM, it is found that multi-omics technologies have been widely used in TCM for syndrome differentiation, evaluation of herbal quality, elucidation of pharmacological mechanisms, and drug toxicity assessment, providing comprehensive explanations of the mechanisms of action of TCM and overcoming the limitations of single-omics technologies, and having obtained significant achievements. However, multi-omics technologies also face challenges such as high cost, difficulties in data analysis due to large data volumes, and insufficient translation of research results. In the future, it is expected that through strengthening interdisciplinary cooperation, conducting long-term and dynamic clinical research, standardizing and normalizing data analysis processes, adopting appropriate and reasonable multi-omics integration patterns, establishing multi-omics databases for TCM, revealing the individualized characteristics, therapeutic mechanisms, and disease regulatory networks of TCM, the modernization of TCM will be promoted.
ABSTRACT
The essence of asymptomatic syndrome differenciation is that the disease is in a special stage or state. To perform traditional Chinese medicine (TCM) diagnosis when there was no symptoms for differentiation is a key scientific issue in current TCM diagnosis and treatment. The reasons include limitations in doctor-patient behaviors and relationships, as well as limitations in cognitive thinking. To address such situations, the following strategies can be adopted from a thinking perspective,i.e. grasping the occurrence and development of diseases based on a holistic concept, using dialectical thinking to summarize the laws of diseases, integrating clinical practice with experiential thinking, and introducing new thinking modes through interdisciplinary collaboration. These strategies help TCM practitioners make accurate diagnoses in the absence of obvious symptoms, thereby providing better medical services for patients.
ABSTRACT
Interpretation of syndrome differentiation is a necessary topic for the research on intelligent syndrome differentiation. The syndrome differentiation system of traditional Chinese medicine (TCM) is huge, with diverse and interrelated interpretations, and non-standardized linguistic expressions, while detailed interpretations involve complex basic theories and literature of TCM, resulting in a challenging organization of the system. From the perspective of text mining, this article analyzed the limitations of existing intelligent syndrome differentiation research, clarified the importance and necessity of constructing a syndrome differentiation interpretation system, and proposed the idea of constructing an interpretation system including conceptual system, logical system, evaluation system, and visualization presentation. Future research can continue to refine the structure, logical and semantic relationships, as well as evaluation methods of the interpretation system based on this ideas, present concrete examples, and build the interpretation corpus by utilizing various data sources such as textbooks, medical records, medical discourse, and papers, to provide technicians with standard reference and data support for TCM syndrome differentiation interpretation.
ABSTRACT
Emphasizing the correspondence between western medicine pathology and traditional Chinese medicine pathogenesis, ZHANG Cigong proposes “dual diagnosis and single treatment”. “Dual diagnosis” refers to the determination of pathology, etiology and disease names in western medicine as well as the syndromes of traditional Chinese medicine (TCM), while “single treament” is the use of Chinese herbal medicine (CHM). By analyzing the records of gastric disease cases by ZHANG Cigong and the related discussions, this paper summarized the “dual diagnosis and single treatment” of gastric disease, including the three-step diagnosis logic of gastric disease, the correspondence between pathology and pathogenesis, etiological diagnosis and TCM syndrome differentiation, as well as CHM for the treatment of acute gastritis, gastric ulcer and hyperacidity, duodenal ulcer, chronic gastritis, dyspepsia and gastrointestinal neurosis, and upper gastrointestinal bleeding. Under the thinking mode of organic integration of logical thinking, clinical practice, and scientific research, he corresponded the basic concepts of “Qi and blood, cold and hot, warm and cool” to “substance and function” in traditional Chinese and western medicine, exploring the essence of interoperability between Chinese and Western medicine, thereby expanding the scope of TCM syndrome differentiation and its efficacy to the microscopic physiological and pathological level. It is believed that ZHANG Cigong's academic ideas provide reference to the “win-win” approach of modern integration between Chinese and western medicine, and should follow the premise of learning from each other's strengths and complementing each other to fully highlight their advantages.
