ABSTRACT

Objective To compare the effects of combined ‘en bloc' liver-pancreas transplantation (LPT) with portal vein drainage and simultaneous combined kidney-pancreas transplantation (KPT) with systemic venous drainage on the pancreatic endocrine function and related metabolism.Methods Four LPT patients and 6 KPT ones with normal hepato-renal function, good quality of life and periodic follow-up received measurement of serum insulin, insulin provocation test, fasting glucose, oral glucose tolerance test, C-peptide, glycated hemoglobin, triglyceride and total cholesterol; and their laboratory test parameters were compared and analyzed.Results In KPT group, 2-h insulin level, C-peptide level and total cholesterol level were significantly higher at 6th month, 3rd and 6th month postoperation (all P<0.05). But there was no significant difference in other parameters between the two groups at 6th month after operation.Conclusion Either KPT or LPT can achieve excellent endocrine function, carbohydrate and lipid metabolism; and the results show that portal venous drainage does not offer major metabolic advantages within 6 months after operation.

ABSTRACT

La anomalía total del retorno venoso sistémico (ATRVS) sin malformaciones acompañantes es un hallazgo excepcional. En esta presentación se describe el caso de una niña de 24 horas de vida, derivada a nuestra institución por presentar cianosis y disnea desde el nacimiento. Por ecocardiografía Doppler color se observó una anomalía parcial del retorno venoso sistémico: drenaje de la vena cava superior en la aurícula izquierda. El cateterismo cardíaco modificó el diagnóstico al confirmar una anomalía total del retorno venoso sistémico hacia la aurícula izquierda. Se realizó la corrección quirúrgica a los dieciséis meses de vida con excelente resultado.


Total anomalous systemic venous drainage (TASVD) with no concomitant heart defects is an unusual condition. A 24- hour old female new born was referred to our center due to cyanosis and dyspnea. Color-Doppler echocardiography revealed a partial anomalous systemic venous drainage of the superior vena cava to the left atrium. Cardiac catheterization showed a total anomalous systemic venous drainage into the left atrium. The patient underwent corrective surgery at the age of sixteen months with excellent outcomes.

ference in survival and graft function between type 1 and type 2 DM recipients was noted. It is concluded that pancreas-kidney transplantation is an effective way for the treatment of type 1 DM and some type 2 DM complicated with uremia.
