ABSTRACT

Nonalcoholic fatty liver disease (NAFLD) is a multisystem disease associated with obesity, insulin resistance, and dyslipidemia and has a complex pathogenesis. Studies have shown that gut microbiota dysbiosis is closely associated with the onset of NAFLD, and traditional Chinese medicine treatment can improve the laboratory markers and clinical symptoms of NAFLD patients by regulating intestinal microbiota and its metabolites. This article elaborates on the association between NAFLD and gut microbiota, the involvement of gut microbiota dysbiosis in the pathogenesis of NAFLD, and the possible mechanism of traditional Chinese medicine treatment in improving NAFLD from the perspective of gut microbiota, in order to provide new ideas for the treatment of NAFLD.

ABSTRACT

China has a high incidence of esophageal cancer,more than 90% of which are esophageal squamous cell carcinoma (ESCC). Abnormal proliferation,migration and new microvessels of intraepithelial neoplasia cells are the important pathogenic links in the transformation from esophageal intraepithelial neoplasia (EIN) to ESCC. Studies on the progression of esophageal precancerous lesions into esophageal cancer mostly focus on environment and genetic susceptibility,such as inflammatory factors,abnormal vascular endothelial growth factor (VEGF) signaling pathway transduction,p53 gene mutation,and DNA methylation. Some pharmacology studies have confirmed that traditional Chinese medicine (TCM) can inhibit inflammatory factors,regulate abnormal signaling pathways and improve the microenvironment. A large number of patients with esophageal cancer have been found to be in advanced stage,and the 5-year survival rate is low even after active treatment. The quality of life of patients in advanced stage is worrying due to esophageal obstruction and lung infection,and therefore, early prevention is important. Early intervention in patients with esophageal precancerous lesions is in line with clinical needs and embodies the TCM theory of “treating disease before its onset.” The mechanism of action and clinical efficacy of TCM has been gradually confirmed and promoted, with certain clinical significance. To explore simple,economical and effective TCM intervention measures conforms to the clinical diagnosis and treatment standards and the modernization of TCM.

ABSTRACT

Objective:To investigate the clinical efficacy of Shugan Mingmu Decoction plus Rehmannia Decoction combined with acupuncture in the treatment of optic neuromyelitis. Methods:The data of one case of optic neuromyelitis cured by Chinese medicine combined with acupuncture and related literature were analyzed. Shugan Mingmu Decoction was used to clear the liver, nourish blood, brighten the eyes and relieve blindness. Rehmannia Decoction was used to nourish the kidney, invigorate the Yin and Yang, soften the liver, strengthen the tendons, and vibrate the withering. The combination of Shu acupoints and extraordinary acupoints was used flexibly and skillfully. Penetration of acupoints was performed to activate the meridians and Qi. Results:The patient's vision quickly recovered, and his lower limb motor function was gradually improved until complete recovery, which help improve patient's ability to live and work.Conclusion:The use of Qinggan Mingmu Decoction and Rehmannia Decoction combined with acupuncture at meridians and extraordinary acupoints for treatment of optic neuromyelitis show clinical advantages including improving curative effects, avoiding the deterioration of the disease and the risk of hormone therapy, and improving the prognosis. It has a good application prospect.

ABSTRACT

Acupuncture at Baliao points for urinary incontinence has the advantages of obvious therapeutic effect, simple operation and few complications, which is mainly used for the treatment of urinary incontinence after stroke, urinary incontinence after spinal cord injury, stress urinary incontinence and urinary incontinence of bladder overactivity, etc. Urinary incontinence after stroke and spinal cord injury is a complication of disease, most of which are combined with acupuncture at Baliao points on the basis of primary disease treatment. The bladder meridian of foot-Taiyang has the characteristics of "entering the collateral brain" and "carrying spine to the waist". Baliao points can conduct induction along the bladder meridian to the sick site along the bladder meridian, and can play a certain therapeutic role in the treatment of the primary disease. Stress urinary incontinence and urinary incontinence of bladder overactivity can be regarded as independent diseases. Since Baliao points are located in the lumbosacral region which is close to the bladder, acupuncture can play a role in the near treatment of acupoints, improve the operation of bladder qi and blood, and the curative effect has a cumulative effect.

ABSTRACT

Great achievements have been made in traditional Chinese medicine in the prevention and treatment of liver cancer in recent years. Studies have shown that traditional Chinese medicine can reduce the recurrence and metastasis of liver cancer and prolong the survival time of patients by inhibiting cell proliferation, inducing cell apoptosis, inhibiting cell migration and invasion, inhibiting angiogenesis, regulating immunity, and reversing drug resistance. This article reviews the clinical and basic studies on the effect of traditional Chinese medicine in the prevention and treatment of recurrence and metastasis of liver cancer in the recent 10 years, and it is believed that traditional Chinese medicine has the advantages of low toxicity, multiple targets, and multiple pathways in the prevention and treatment of recurrence and metastasis of liver cancer. Future research should focus on how to screen out the effective constituents for liver cancer, how to understand the complete mechanism of action of traditional Chinese medicine, whether the consistency in the efficacy of traditional Chinese medicine between in vivo and in vitro studies can be guaranteed, and whether there is a difference in the role of investigational drugs between populations from different regions in multicenter clinical studies.

ABSTRACT

Treatment regimens for primary liver cancer mainly include surgical resection, liver transplantation, local treatment, and systemic treatment, but there are still problems such as easy metastasis, easy recurrence, and poor prognosis. Most patients are already in the advanced stage at the time of diagnosis and thus lose the opportunity for surgery and require multidisciplinary combined treatment. Traditional Chinese medicine (TCM) has unique advantages in the treatment of primary liver cancer and can exert a synergistic effect in collaboration with different Western medicine treatment methods. This article establishes TCM treatment regimens based on the different clinical stages of primary liver cancer, i.e., surgical resection for early-stage liver cancer to strengthen vital Qi and prevent recurrence, minimally invasive treatment of middle-stage liver cancer to eliminate cancer toxicity and improve efficacy, and symptomatic treatment of advanced liver cancer to reduce pain and maintain quality of life.

ABSTRACT

At present, the treatment of primary liver cancer is mainly based on modern medicine. Studies have found that traditional Chinese medicine (TCM) can promote the apoptosis of hepatoma cells and regulate the immunity of human body by affecting multiple critical pathways involved in the development and progression of tumor. TCM plays an irreplaceable role in the treatment of liver cancer, and the focus on the etiology and pathogenesis of liver cancer is the key to achieving good efficacy. Although there are still no ideal anti-HBV drugs in TCM, intervention on the etiologies of dampness, heat, phlegm, toxin, stasis, and deficiency and the change in struggle between vital Qi and pathogen during each stage of disease progression may delay or even prevent disease progression, which is the treatment based on etiology and pathogenesis and is the theory that "treatment should focus on the principal cause of a disease". In clinical practice, modern medical treatment combined with TCM can improve patients' quality of life and reduce tumor recurrence, and TCM combined with sorafenib has stronger efficacy and fewer side effects. Meanwhile, further studies are needed to explore how integrated traditional Chinese and Western medicine therapy exerts a therapeutic effect on liver cancer and the detailed mechanism of action of TCM in the treatment of liver cancer.

ABSTRACT

Liver fibrosis is commonly seen in various chronic liver diseases. In history, traditional Chinese medicine (TCM) clinical practice based on syndrome differentiation and treatment formed unique experience in the diagnosis and treatment of liver fibrosis; in recent years, studies based on experimental exploration and evidence-based medicine have formed effective Chinese patent drugs and experienced prescriptions for the treatment of liver fibrosis, as well as integrated traditional Chinese and Western medicine regimens. With the improvement in the quality of TCM drugs and the development of related clinical studies, new TCM drugs with stable quality and marked clinical effect will be launched for the treatment of liver fibrosis, so as to meet the clinical needs of patients with chronic liver diseases and promote the modern and international development of TCM.

ABSTRACT

Gallbladder cancer occurs insidiously and is usually induced by basic gallbladder diseases accompanied with chronic inflammation. Surgical operation can remove cancerous focus but further treatment should be applied to relieve complications and prevent recurrence and metastasis. According to TCM theory, the basic pathogenesis of post-operation of gallbladder cancer is the combination of blood stasis and heat. Blood stasis arises from surgical operations, failure of liver-gallbladder in keeping smooth flow of qi, and dampness retention in middle-Jiao. Heat arises from qi-stagnation, phlegm-dampness, blood stasis and yin-blood deficiency. Therefore, this article proposed that the therapeutic methods should include Qingre Huoxue Method (clearing away heat and promoting blood circulation), keeping smooth qi-flow of liver-gallbladder, and removing dampness and discharging waste, with typical clinical cases for illustration.

The most common manifestations of eosinophilic gastroenteritis (EG) are abdominal pain and diarrhea. Glucocorticoid is currently used in the treatment for EG, but the adverse reactions are relatively large. Banxia Xiexin Decoction is the famous Zhongjing prescription can be used to treat many kinds of gastrointestinal diseases. This article introduced. Banxia Xiexin Decoction combined with Shaoyao Gancao Decoction in the treatment of abdominal pain predominant based EG, as well as Banxia Xiexin Decoction combined with Gegen Qinlian Decoction in treating diarrhea predominant based EG have achieved satisfactory efficacy.

This article summarized the clinical experience of Professor WANG Meng-yong in treating lupus nephritis from the symptoms and essence. Professor WANG Meng-yong believes that treatment for lupus nephritis should attach importance to clinical features of setting deficiency of liver, spleen and kidney as the essence and toxin and blood stasisas symptoms, in which the essence is deficient and the symptoms are excess and combination of external and internal pathogenesis. He flexibly uses methods of nourishing liver and kidney, tonifying spleen, and stomach, clearing heat and cooling blood, activating blood and promoting diuresis to alleviate water retention, which have obvious efficacy, effectively avoid adverse reactions and reduce the risk of recurrence, and improve patient prognosis.

ABSTRACT

One hundred patients with early diabetic nephropathy were randomly divided into control group and study group with 50 cases in each group.All patients received conventional antidiabetic therapy with valsartan tablets 80mg, q.d;patients in study group were given additional TCM Jiawen Dihuang decoction.TCM syndrome scores, relevant biochemical testing and renal function changes were evaluated before and after treatment in two groups.After 8 weeks of treatment, the results showed than the TCM scores in both groups were significantly reduced (study group: 18.9±1.6 vs.7.2±5.1, control group 18.8±1.5 vs.10.3±6.2.both P<0.05);the improvement rate of TCM syndrome in study group was significantly higher than that of control group (90% vs.68%, x2=5.3,P<0.05);and the improvement of relevant biochemical index and renal function in study group was more marked than that in control group.The study indicates that the integrated TCM and Western medicine can enhance the efficacy in treatment of early diabetic hephropathy.

ABSTRACT

Through the analysis on research status of Western medicine for treating hypertentionand the relationship between hypertension and "wind", and by combining with the characteristics of wind, this article believed that the multiple risk factors, pathogenesis and target organ damage of hypertension are related to the "wind". It pointed out that pathogenic wind is the major risk factor for hypertensionas, and linked the characteristics of carminatives to explore pathogenesis of treatment of hypertension from "wind".
