ABSTRACT

Background: The fact that about 90 % of newly discovered API’s or new molecular entity(NME) have little or no aqueous solubility, causes a significant protest to the initialization of development and their scale up of dosage form in the Pharma Industry. Aqueous solubility of API’s has critical role in drug dissolution or availability of drug at the site of action or bioavailability, when a dosage form is administered orally. Objective: The object of this study is to formulate a modified release tablet dosage form of a poorly aqueous soluble drug, which not only have higher aqueous solubility or bioavailability but also have sustained release characteristics with high mechanical strength &their commercial viability. Numerous techniques are available for the solubility enhancement but all individual techniques have its own limitations for commercialization.Method: Aqueous solubility of drugs is improved by the known Solubility enhancement techniques like Micronization &Solid dispersions. After successful solubility enhancement, sustained release or modified release tablets of poorly aqueous soluble drug can be easily formulated into a suitable shape or size by using a known Polymer Matrix Sintering Technology with commercial feasibility. Micronization of poorly water-soluble drugs can be performed by Air Jet Mill or Ball Mill. Whereas Solid dispersion technique involves, molecular dispersion of poorly soluble drug in a suitable inert carrier, to form an amorphous and highly soluble compounds. Sintering Technology is defined as the bonding of adjacent particle surfaces in a mass of powder, or in compact, by the application of heat. Conventional sintering technique involves the heating of compact at a temperature below the melting point of the solid constituents in a controlled environment under atmospheric pressure. Results: Enhanced solubility of poorly soluble API’s by these proposed techniques is due to either conversion of crystalline compound in to amorphous form or reduction of particle size to its molecular level by the application of Micronization or solid dispersion techniques. The developed modified release tablets will show a sustained release characteristic due to Sintering aspect and provides enhanced solubility of BCS class II or IV drugs. Conclusion: Novel modified release tablets have been designed through consolidation of Solubility enhancement and Polymer Matrix Sintering technologies. Simultaneous exploitation of well-known and established approaches- Micronization (optimum particle size reduction) or solid dispersion, optional surfactant and Polymer Matrix Sintering Technique in the recent concept, produces significant enhancement of solubility of poorly water soluble API’s without compromising the content uniformity of dosage form and also provide a modified or sustained release characteristics with high mechanical strength. The release profile of drug can be easily tailored by using combination of both techniques where challenges of low solubility are prominent.

ABSTRACT

Purpose: Sildenafil citrate is widely used drug for the treatment of Erectile Dysfunction (ED) and Ginseng is a natural aphrodisiac reported to benefit this condition. The objective of the present study was to develop orodispersible tablets (ODTs) containing combination of Sildenafil citrate and Ginseng extract to improve the bioavailability, reduce the dosing frequency and thereby maintaining the therapeutic efficacy of the drug. Methods: The ODTs were prepared using superdisintegrants such as Croscarmellose sodium (CCS), povidone, and sodium starch glycolate (SSG) at varying concentrations (2%, 4% and 6%) by direct compression. The bitter taste of Sildenafil citrate was masked by Doshion resin. The optimized formulation based on least disintegration time (DT) was chosen to reformulate using sublimating agents such as camphor, menthol or thymol at varying concentrations (1%, 2%, 3%) to further reduce the DT. The compatibility of drug with excipients was investigated and the prepared formulations were evaluated for pre and post-compression parameters. Results: The post-compression parameters such as weight variation, hardness, friability, DT and in-vitro drug release was found within specified limit. The formulation with camphor (2%) had DT of 12 sec and drug release >90% within 5 min hence was considered as optimized formulation. The accelerated stability study and kinetics modelling was performed for optimized formulation. Conclusion: The formulated Sildenafil citrate and Ginseng ODT’s were found to be promising formulation with quicker DT and drug release which will eventually have higher bioavailability and better efficacy along with averting the issues of swallowing and improving patient compliance.

ABSTRACT

Objective:To systematically evaluate the clinical efficacy of compound α-ketoacid tablets in the treatment of diabetic kidney disease (DKD).Methods:CNKI, Wanfang database, EMBASE, PubMed and Cochrane Library database were searched for eligible records published from the establishment of individual database to November 13 th, 2022. The quality of the included studies were assessed, data were extracted, and meta-analysis was conducted using RevMan5.3. Results:A total of 26 randomized controlled trials were included, with a total of 2 790 DKD patients (1 465 in the experimental group and 1 325 in the control group). Multiple parameters were significantly improved in the experimental group compared with the control group, including 24-hour urinary protein, blood creatinine, urea nitrogen, nutritional index, oxidative stress level, fasting blood glucose, glycated hemoglobin, homocysteine, HGF, VEGF, TGF-β1, and systolic blood pressure. Conclusions:Limited low-quality evidence showed that compound α-ketoacid tablets combined with low-protein diet may be related to the improved 24-hour urinary protein, renal function, and glucose metabolism in patients with DKD. Due to the lack of randomized controlled trials designed for respective stages of DKD, the inclusion criteria of our study were relatively general, possibly leading to the lack of pertinence of the results. Some indicators showed apparent heterogeneity among different groups, and more high-quality multi-center studies with large sample sizes are still needed to verify our findings.

ABSTRACT

OBJECTIVE To investigate the efficacy and safety of domestic Paliperidone extended-release tablets as a substitute for original Paliperidone extended-release tablets in the treatment of stable schizophrenia. METHODS A total of 65 patients with schizophrenia, who were treated with single original Paliperidone extended-release release tablets for 2 months or more in the outpatient or inpatient department of Shandong Daizhuang Hospital from June 2021 to June 2022, were collected and randomly divided into the domestic group (33 cases) and the original group (32 cases). The domestic group was treated with the same dose of domestic Paliperidone extended-release tablets instead for 2 months, and the original group continued to use the previous dose of the original drug for 2 months. Positive and negative syndrome scale (PANSS) and treatment emergent symptom scale (TESS) were used to evaluate the two groups at the time of enrollment and the end of 1 week, 1 month and 2 months after enrollment. The incidence of ADR was calculated at the end of 2 months after enrollment. The fasting blood glucose, blood lipid indicators (triglyceride, total cholesterol, low-density lipoprotein, high-density lipoprotein, very-low-density lipoprotein), serum prolactin levels, and paliperidone blood concentration were determined after the intravenous blood sample was collected. The ratio of paliperidone blood concentration to dose (C/D value) was calculated, and an electrocardiogram was performed. RESULTS There were 31 and 30 patients in the domestic group and the original group who completed the trial, respectively. There were no statistical significances in PANSS score, TESS score or C/D value at the time of enrollment and the end of 1 week, 1 month and 2 months after enrollment; there were no statistical significances in the levels of fasting blood glucose, blood lipid or serum prolactin at the time of enrollment and at the end of 2 months after enrollment (P>0.05). PANSS scores of both groups significantly decreased at the end of 1 month and 2 months after enrollment (P<0.01). The incidences of ADR were 25.81% in the domestic group and 30.00% in the original group, without significant difference (P>0.05), and there were no significant abnormalities in the electrocardiograms of the two groups. CONCLUSIONS Domestic Paliperidone extended-release tablets can directly replace the original tablets in the treatment of stable schizophrenia, and their clinical efficacy and safety are comparable.

ABSTRACT

ObjectiveTo screen the preparation technology of Baoyuan chewable tablets and to preliminarily elucidate its anti-fatigue effect and mechanism. MethodTaking encapsulation rate of volatile oil, extract rate and extraction rate of active ingredients as indexes, single factor test and orthogonal test were used to optimize the volatile oil inclusion, aqueous decoction and formulation molding processes of Baoyuan chewable tablets. ICR rats were randomly divided into the blank group, model group, Gaoshan Hongjingtian oral liquids group(6.01 mL·kg-1) and and Baoyuan chewable tablets low, medium, and high dose groups(2.1, 4.2, 8.4 g·kg-1), 8 mice in each group, and were administered by gastric gavage at the corresponding dose once a day, the blank and model groups were given equal volume of saline for 15 d. After the last administration for 30 min, the mice were loaded with 5% of the body mass of lead at the tail and swam until exhaustion to establish the fatigue model, and the weighted swimming time of the mice in each group was recorded, meanwhile, the muscle tissues of the mice were sliced, stained by hematoxylin-eosin(HE) and subjected to pathological observation, and the levels of blood urea nitrogen(BUN), lactic acid(LA), liver glycogen(LG), activities of lactate dehydrogenase(LDH) and creatine kinase(CK) in the serum were determined. ResultThe optimal inclusion process of cinnamon oil in Baoyuan chewable tablets was 10∶1 for β-cyclodextrin-volatile oil, and inclusion at 50 ℃ for 2 h with saturated aqueous solution method. The optimal water extraction process was to extract twice, adding 10 times of water to extract for 50 min for the first time, and adding 9 times of water to extract for 40 min for the second time. The ratio of the extract of Baoyuan chewable tablets with microcrystalline cellulose, maltodextrin, mannitol, citric acid, magnesium stearate was 63∶13∶8∶17∶17∶1∶1, the tablets were pressed by wet granulation, the each tablet weight was 1.2 g, and the hardness was 60-80 N. Compared with the model group, Baoyuan chewable tablets low, medium, and high dose groups could significantly prolong the exhaustion time of mice in weight bearing swimming(P<0.05, P<0.01), and improve the exercise endurance of the body, and the results of HE staining showed that all dose groups of Baoyuan chewable tablets could significantly improve the muscle tissue damage caused by exercise, significantly reduce the levels of BUN, LA and the activities of LDH and CK in serum(P<0.01), and significantly increase the content of LG(P<0.05, P<0.01). ConclusionThe optimized preparation process of Baoyuan chewable tablets is stable and feasible, and the preparation can improve exercise endurance by increasing the LG level in liver tissue, and relieve muscle soreness by accelerating the removal of LA from the body, and reduce CK and LDH activities to exert anti-fatigue effects.

The risk of bias in the included studies was assessed using RoB2.0, and Meta-analysis was performed using RevMan5.4. A total of 35 RCTs involving a total sample size of 3 261 patients were included. Meta-analysis results showed that compared with conventional western medical treatment alone, the combination of Hexue Mingmu tablets with conventional western medical treatment improved patients' macular thickness{mean difference(MD) =-39.83, 95% confidence interval(CI) [-51.60, -28.06], P<0.000 01}, improved corrected visual acuity{risk ratio(RR)dichotomous=1.09, 95% CI [1.00, 1.18], P=0.04; MDcontinuous variable=0.15, 95% CI [0.13, 0.17], P<0.000 01}, increased effective rate of fundus symptom improvement(RR=1.26, 95% CI [1.22, 1.30], P<0.000 01), improved hemorheology index{standard mean difference(SMD)=-1.53, 95% CI [-2.04, -1.01], P<0.000 01}, shortened improvement time of fundus symptoms(MD=-5.53, 95% CI [-5.96, -5.09], P<0.000 01), and there was no significant difference on adverse events between the two groups. The results show that treatment of DR and RVO with Hexue Mingmu tablets may improve the macular thickness and hemorheology index of patients, which can significantly enhance the effect of corrected visual acuity and clinical efficiency, and shorten the time to symptom improvement. However, the original literature is of low quality and the pooled results have some limitations. Subsequent studies should try to use uniform standard assessment criteria and testing methods, focus on the rigor of study design and implementation, and pay attention to the key outcomes of this disease and the clinical safety of medication, so as to provide more reliable evidence to support this kind of clinical problems.

ABSTRACT

OBJECTIVE To compare the efficacy of Danning tablet, taurosodeoxycholic acid and ursodeoxycholic acid in preventing the recurrence of choledocholithiasis after endoscopic retrograde cholangiopancreatography (ERCP). METHODS The clinical data of 153 patients who underwent ERCP choledocholithotomy from January 2017 to January 2020 in Nantong First People’s Hospital were retrospectively analyzed. According to the different drug treatment received after surgery, patients were divided into three groups, namely, Danning tablet group (group A, 49 cases), tauroursodeoxycholic acid group (group B, 44 cases) and ursodeoxycholic acid group (group C, 60 cases). The above groups of drugs are all single-use, starting from 2 weeks after surgery for a course of 180 days. The effects of bile component indicators [total bilirubin (Tbil), direct bilirubin (Dbil), alkaline phosphatase (ALP), glutamyltransferase (GGT)], lipid metabolism indicators [total cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL), high-density lipoprotein (HDL)], the occurrence of clinical symptoms (abdominal pain, bloating, nausea, and poor appetite) at 6 months after ERCP, and the recurrence of choledocholithiasis at 6, 12, 18 and 24 months after surgery were compared among 3 groups. RESULTS Compared with before surgery, the serum levels of Tbil, Dbil, ALP, GGT, TC, TG and LDL were significantly reduced (P<0.05), while serum HDL levels were significantly increased (P<0.05) in the three groups at 6 months after surgery. The proportion of patients who experienced abdominal pain, bloating, nausea, and poor appetite at 6 months after surgery was significantly reduced (P<0.05). The Tbil levels of groups A and B were significantly lower than those of group C (P<0.05), while the Dbil and ALP levels of group A were significantly lower than those of groups B and C (P<0.05); however, there was no statistically significant difference in GGT levels among the 3 groups (P>0.05). Compared with groups A and C, the levels of four lipid metabolism indicators in group B were significantly improved (P<0.05); the proportion of patients with abdominal pain, bloating, and poor appetite in group A was significantly lower than groups B and C (P<0.05), but there was no statistically significant difference in the proportion of patients with nausea among the 3 groups (P>0.05). At 6,12 and 18 months after surgery, there was no statistically significant difference in the rate of choledocholithiasis recurrence among the 3 groups (P>0.05); at 24 months after surgery, the rate of choledocholithiasis recurrence in group A (2.04%) was significantly lower than group B (15.91%) and group C (15.00%) (P<0.05). CONCLUSIONS Compared with tauroursodeoxycholic acid and ursodeoxycholic acid, the application of Danning tablet after ERCP is more beneficial to reduce the secretion of bile acid, prevent the recurrence of gallstones, and improve clinical symptoms, but tauroursodeoxycholic acid can significantly accelerate the lipid metabolism of patients compared with the other two drugs.

ABSTRACT

ObjectiveTo investigate the clinical efficacy of Tenghuang Jiangu tablets (THJGT) combined with oral non-steroidal anti-inflammatory drugs (NSAIDs) in the treatment of osteoarthritis of the knee and its applicable stage based on real-world data, and provide a basis for the rational clinical use of THJGT. MethodA total of 218 cases treated with THJGT combined with oral NSAIDs included in the "THJGT for knee osteoarthritis case registry" from September 2019 to January 2021 were selected as the observation group, and 126 cases treated with oral NSAIDs alone as the control group (CG). The data of gender, age, body mass index, Kellgren-Lawrence grading scale (K-L scale) score, visual analogue score (VAS score), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, swelling grade, joint fear of cold score, back pain and weakness score, and occurrence of adverse events/reactions of the patients in both groups were used for the evaluation of efficacy with full analysis set. The propensity score matching method was used to exclude the influence of confounding factors between groups, and the sub-data sets were established, with which the repeated measures analysis of variance (ANOVA) was carried out to evaluate the efficacy. Visit points were at registration, 4 weeks and 8 weeks after registration. The data were statistically analyzed in Excel 2019 and SPSS 23.0. ResultThe proportion of females in the observation group was 66.06% (144/218), which was higher than that (58.73%, 74/126) in the control group (χ2=1.846). The average age in the observation group was (61.12±7.01) years, which was higher than that [(59.38±5.99) years] in the control group (W=19 918.50, P<0.05). The remission rate in the observation group was 98.17% (214/218). In the observation group, the proportions of the patients at K-L grades Ⅱ and Ⅲ were 64.22% (144/218) and 25.23% (55/218), respectively. The effect analysis of the whole data set for enrollment and treatment for 8 weeks showed that the VAS score of the experimental group decreased by (3.27±1.24) points on average, which was better than that of the control group [(2.75±1.20), W=34 179.00, P<0.05]. The average WOMAC score decreased (23.43±11.46) points, which was better than that of the control group [(16.71±8.86), W=32 387.00, P<0.05]. The average swelling grade decreased (0.63±0.64), which was better than the control group [(0.33±0.59), W=33 847.50, P<0.05]. The average score of joint chills decreased (1.90±1.84), points, which was better than that of control group [(1.40±1.28), W=35 165.00, P<0.05]. The average lumbar acid fatigue score decreased by (2.02±1.64) points, which was better than that of the control group [(1.10±1.28), W=32 986.50, P<0.05]. Efficacy analysis of subdata sets for enrollment, 4 weeks of medication and 8 weeks of medication showed that VAS scores of both groups showed a downward trend after treatment, and the improvement of experimental group was more significant than that of control group at 4 weeks, with statistical significance (P<0.05). After treatment, the total WOMAC score of both groups showed a downward trend, and the improvement of experimental groups was more significant at 4 weeks and 8 weeks (P<0.05). After treatment, swelling, cold fear grade and lumbar acid fatigue score of both groups showed a decreasing trend,, and the improvement of experimental group was more significant at 8 weeks (P<0.05). The therapeutic effect analysis of patients in the attack stage and remission stage of the experimental group showed that the total WOMAC score of the two groups showed a downward trend after treatment, and the trend was basically the same, and there was no statistical difference between the two groups at enrollment, 4 weeks after treatment, and 8 weeks after treatment (t=1.675, t=2.068, t=2.364). The total WOMAC score of the patients in remission stage in the experimental group with K-L grading between grade 0 and grade Ⅲ had statistical significance at 4 weeks after treatment compared with the time of entry (P<0.05, P<0.01). Group of adverse event rate was 4.13% (9/218), lower than the control group 10.32% (13/126) (χ2= 5.109, P<0.05). ConclusionThe population receiving THJGT combined with oral NSAIDs is mostly female, old, in remission, and with K-L grades Ⅱ and Ⅲ. THJGT can enhance the anti-inflammatory and analgesic effects of oral NSAIDs and keep the drug effect in improving joint function and alleviating fear of cold, swelling, and back pain and weakness. The drug combination can be applied to patients in both attack and remission, and the clinical application should take patient's disease stage and degree of osteoarthritis into account. Furthermore, the combination has the potential to reduce the incidence of adverse events caused by NSAIDs.

ABSTRACT

OBJECTIVE To establish the ion mobility mass spectrometry method for simultaneous determination of epiberberine, berberine, coptisine, palmatine, calycosin-7-glucoside, 3,5-O-dicaffeoylquinic acid, 4,5-O-dicaffeoylquinic acid and chlorogenic acid in Jinqi jiangtang tablets. METHODS Ion mobility mass spectrometry method was used. The determination was performed on Waters ACQUITY UPLC HSS T3 (2.1 mm×50 mm, 1.8 μm) with mobile phase consisted of 0.1% formic acid solution-acetonitrile (gradient elution) at the flow rate of 0.3 mL/min. The column temperature was 40 ℃, and the injection volume was 5 μL. The contents of 8 components in Jinqi jiangtang tablets were determined by scanning detection under positive and negative ion modes with an electric spray ion source, and setting ion mobility mass parameters according to the peak response of each component. RESULTS The results showed that the linear relationship of the eight components was good within their respective ranges (r≥0.999); RSDs of precision, repeatability and stability (24 h) tests were not more than 4.0%; average recoveries were 94.6%-101.2% , RSDs were 2.6%-3.9% (n=9). The contents of the above eight components in three batches of Jinqi jiangtang tablets were 3.060-3.545, 24.50-26.74, 2.795-4.149, 1.437-2.501, 0.204-0.242, 0.950-1.281, 2.272-2.828, 7.314- 7.960 mg/g, respectively. CONCLUSIONS The established method has high sensitivity and good reproducibility, and can provide reference for the quality control of the preparation.
