ABSTRACT

Objective:To investigate the standardized construction of critical care departments in different cities and counties of Guizhou province to promote the homogenization development of critical care medicine in Guizhou Province. Methods:Using research methods such as field investigation and data collection, the public hospitals of 88 counties and urban areas in 9 prefectures and cities of Guizhou province were divided into five routes: southeast, northeast, northwest, southwest, and Guiyang. To design the survey form for the standardized construction of ICU, the e-form was sent to the director of ICU or his/her designated personnel by email or wechat 2-3 days in advance. Check the authenticity of data item by item on site, and leave the hospital after checking the receipt form. Results:From April to July 2021, the survey and research data collection was completed for 146 public hospitals (excluding provincial hospitals) with intensive care departments in 88 counties and cities of 9 dizhou cities in Guizhou Province, including 24 Grade-Ⅲ Level A hospitals. 122 Grade-Ⅱ and above hospitals (including 8 Grade-Ⅲ Level B hospitals, 11 Grade-Ⅲ comprehensive hospitals, 97 Level-Ⅱ A hospitals, 3 Level-Ⅱ B hospitals, and 3 Level-Ⅱ comprehensive hospitals). 146 public hospitals have a total of 80 983 beds and 104 017 open beds. The department of Critical Care has 2 035 beds. The ratio of actual beds in ICU to total beds in hospital was 2.51%. From 1999 to 2010, 18 (12.33%) established departments, and from 2011 to 2021, 128 (87.67%) established departments. The total area of the discipline is 113 355.48 m 2, with an average bed area of 55.70 m 2. There were 97 hospitals with 1.5-2.0 m bed spacing, accounting for 66.44%, and 49 hospitals with 2.1- > 2.5 m spacing, accounting for 33.56%. The number of negative pressure wards: 1 in each of 43 hospitals, accounting for 29.45%; 103 hospitals did not have, accounting for 70.55%. The number of single rooms: 288 in 140 hospitals, accounting for 95.89%; 6 hospitals did not have, accounting for 4.11%. Central oxygen supply: 138 hospitals have (94.52%); 8 hospitals did not have, accounting for 5.48%. Natural ventilation: in 129 hospitals with 88.36%; 17 hospitals did not have, accounting for 11.64%. Specialized ICU construction: 66 hospitals, accounting for 45.21%; none in 80 hospitals, accounting for 54.79%. There are 3 712 doctors and nurses in 146 public hospitals. The total number of doctors was 1 041, and the ratio of doctors to beds was 0.51∶1. The total number of nurses was 2 675, and the ratio of nurses to beds was 1.31∶1. Conclusions:All 88 counties and districts in 9 prefectures and cities of Guizhou province have established intensive care medicine departments. The standardization of the discipline construction has been significantly improved. Lack of talents is still an important factor restricting the rapid development of the discipline.

ABSTRACT

Objective To study the development of the first county-level key clinical specialized talent teams in Henan province. Methods Data on the first 30 county-level key clinical specialized talent teams were collected through questionnaire in 2014 and 2017 respectively, and signed rank sum test was conducted to analyze such facts as the changes of medical service volume, academic titles and scientific researches of discipline leaders before and after such construction. McNemar′s test and chi-square test were conducted to study the age structure, seniority, education and technical title makeup of these discipline leaders. Such tests were also conducted to study the age structure, seniority, education and technical title makeup of the talents. These efforts aim to discern the construction achievement of the key clinical specialties. Results The indexes of healthcare capacities of the first batch of clinical key specialties were observed as improved ( P <0. 05 ) in terms of the medical service volume. Of the discipline leaders′ age structure, the 51 -60 age groups accounted for 40. 0% and 66. 7% respectively, before and after the construction, scoring a difference of statistical significance ( P <0. 05 ); the number of senior physicians increased by 80 persons, the number of physicians having master or above titles increased by 40 persons;the number of physicians participating in continuing education held by these specialties grew from 146. 50 to 262. 50(P<0. 05). Conclusions The volume of medical service of the first batch of county-level clinical key specialties has been elevated, and a group of talents of high education and senior titles have been recruited. Whereas, the title structure needs to be improved, and more young academic leaders are expected, while more opportunities of further education and training are also expected for the physicians, in order to enhance their professional ability.

ABSTRACT

Objective To discuss the relationship between discipline ranking and personnel rationality, and explore the problems of personnel development among all the disciplines and put forward further countermeasures. Methods A 3A general hospital in Beijing was targeted, and departments in it were divided into,in descending order of hospital regulations, key disciplines,developing-key disciplines, characteristic disciplines and general disciplines. To analyze the rationality of personnel, four dimensions as educational background structure, professional title structure, age structure and academic proficiency of department leader were adopted. Results The hospital had 52 departments in total, which were divided into key disciplines (7),developing-key disciplines (10),characteristic disciplines (6) and general disciplines (29).With the decline of the discipline level, the problem of the talent team was gradually increasing. The major problem of personnel among all the disciplines was that the proportion of senior staff was too high,so did the key disciplines and developing key disciplines.Age structure irrationalized was another major problem for developing-key disciplines. As for characteristic disciplines, the problems in personnel were even diversified, which included not only the irrational age structure and senior professional proportion, but also the fact that director of one third of these departments showed low academic proficiency. In general disciplines, all above four types of problems exited. Conclusions The rationality of the talent team is closely related to the level of the discipline.Problems in personnel varied among different disciplines, and countermeasures should be provided targeted at existing problems.

ABSTRACT

The author summarizes the current situation of the development of the talent team in the acupuncture and industry, discusses the trend of talent development, analyzes the exist problems, and proposes relevant suggestions. The author considers that the overall size of the talent team, and the scale of clinical and scientific research talent in the acupuncture and industry have increased significantly, especially in the development of high-end talents among the Chinese medicine talents. The characteristics of the training scale of acupuncture and in the education of TCM colleges and universities are as follows: the growth rate at the specialist level is higher than that of undergraduates, the scale of professional masters declined, and the scale of doctoral levels remain stable. In order to meet the growing demands of the industry for talented people and the requirements for the rapid development of the industry itself, efforts should be made to further increase the construction of acupuncture and moxibustion talents, and to strengthen the training of basic clinical talents and the construction of high-end scientific research talents.
