ABSTRACT

Objective To study the effect of multi-disciplinary team (MDT) management on the outcome in neonates with omphalocele.Method A retrospective non-randomized controlled clinical study was conducted.Neonates who were diagnosed as omphalocele and admitted to the surgical neonatal intensive care unit of the Guangzhou Women and Children Medical Center from December 2010 to December 2017 were collected.Because MDT was established in December 2014,infants were assigned into non-MDT group and MDT group according to their dates of admission.The characteristics and outcomes between non-MDT group and MDT group were compared using x2,t-test or rank-sum test.Multivariate analysis was performed by Logistic regression.Result A total of 91 neonates were included in the study,50 were in non-MDT group and 41 were in MDT group.The mortality in MDT group (2.4%,1/41) was lower than that in non-MDT group (18.0%,9/50),the difference was statistically significant (P < 0.05).The median time of mechanical ventilation of giant omphalocele in non-MDT group (18.3 hours) was longer than that in MDT group (41.7 hours),the difference was also statistically significant (P < 0.05).After adjusting for the associated confounding risk factors,the risk of death in non-MDT group was 54 times higher than that in MDTgroup (OR=54.19,95%CI2.64 ~1 113.49,P<0.05).Conclusion There was significant association between the MDT management and the decreased risk of death of omphalocele.

Resumo Este artigo se propõe a aprofundar a compreensão dos elementos de coordenação informal na gestão de equipes que atuam em cenários complexos e imprevisíveis. Apresentamos os resultados de um estudo realizado numa unidade de operações especiais de polícia, o Batalhão de Operações Policiais Especiais do Rio de Janeiro (Bope/RJ), utilizando métodos quantitativos. Analisamos a relação entre os antecedentes da confiança e a dimensão distância de poder e a confiança pessoal e profissional no líder. Os resultados confirmam uma relação direta e negativa entre distância de poder e confiança profissional no líder, e uma relação direta e positiva com alguns antecedentes da confiança. Quanto maior a percepção da qualidade da comunicação interna e o compartilhamento e delegação de autoridade nos processos decisórios, maior é a confiança pessoal no líder.

Los resultados confirman una relación directa y negativa entre distancia de poder y confianza profesional en el líder, y una relación directa y positiva con algunos antecedentes de la confianza. Cuanto mayor es la percepción de la calidad de la comunicación interna y el compartir y delegación de autoridad en los procesos decisorios, mayor es la confianza personal en el líder.


Abstract This article aims to widen the understanding of informal coordination elements in the management of teams working in complex and unpredictable scenarios. It presents the results of a study carried out in a special operations police unit, the Batalhão de Operações Policiais Especiais do Rio de Janeiro (Bope/RJ), using quantitative methods. The study analyzed the relationship between the antecedents of trust and power distance and personal and professional trust in the leader. The results confirm a direct and negative relationship between power distance and professional trust in the leader, and a direct and positive relationship with some antecedents of trust. The greater the perceptions of internal communication quality, and sharing and delegation of authority in the decision process, the greater the trust in the leader.

Scientific and technological innovation plays an important role in hospital development.The process of Scientific and technological innovation in hospital is a Complex Adaptability System made up of the interactions of multi-agents in research team and also the interactions between agents and their environmental conditions.After analyzing the CAS characteristics of hospital scientific and technological innovation system,the paper put forward the management strategies for research team.For example,set up hospital scientific and technological innovation system based CAS theory,explore new research management model,construct open scientific and technological innovation platform,and optimize collaborative mechanism.

ABSTRACT

Objective:To investigate the current situation in construction and operation status of official WeChat public platform of community hospitals in Shanghai Downtown and provide the reference for effective application of WeChat public platform in community hospitals.Methods:The current situation of health service provided with WeChat public platform of community hospitals was investigated by website survey.We focused on the opening rate,menu services,information push and off-line operations of WeChat public platform from the perspective of ethics.Results:Of the investigated 98 hospitals,a total of 48 WeChat public platforms were established,accounting for 49.0%.Among which 28 public platforms provided menu bar service,accounting for 58.3%.The public platforms still needed to be improved in terms of article push quantity,reading quantity and daily management.Conclusion:The development level of WeChat public platform of community hospital in shanghai downtown is uneven and WeChat public platform of community hospital exists low service level,imperfect management,lack of publicity and other problems generally.It is recommended that hospitals strengthen the construction of WeChat platform from three aspects,including strengthening team management,keeping the "Six in One" and seeking for commercial assistance.

ABSTRACT

Craniosysostosis syndrome is caused by premature fusion of bones of skull and face during fetal development. It is related to Fibroblast growth factor receptor gene and most common craniosynostosis syndromes are Apert, Pfeiffer and Crouzon. Apert syndrome is one of the severe type of craniosynostosis syndromes which shows mutations in the Fibroblast growth factor receptor 2 (FGFR2) gene. Pfeiffer syndrome is also related with FGFR 1 or 2 gene mutation. We experienced two patients with craniosynostosis syndromes, Apert syndrome and Pfeiffer syndrome. The first baby was a in-born female baby presented with syndactly of the hands and feet and facial dysmorphism including shallow orbit with deep crease above eye brow. Apert syndrome was confirmed by the presence of a mutation in FGFR2. The second patient visited our developmental delay clinic due to developmental delay at seven month old age. He showed facial dysmorphism including cloverleaf-shaped skull, micrognathia, low set ears, low nasal bridge and high-arched palate, but there were no syndactly or limb anomalies. He was suspected of Pfeiffer syndrome, however his FGFR2 gene study was normal. These patients need multidisciplinary team management and regular follow up for visual, auditory, and cognitive development functions Pediatricians have important role on recognizing the patients with facial dysmorphism, planning to evaluate accompanying anomalies and making appropriate decisions about the timing of surgical management to minimize growth and cognitive impairments.

ABSTRACT

@#Objective To investigate the methods and effectiveness of the physical therapy team management mode.MethodsAccording to the type of cerebral palsy and a certain ratio of beds and staff, the therapists were divided into 3 groups, and relatively fixed. They treated children with differrent types of CP and different beds by turn. The implementation of treatment was 3 years.ResultsAfter the physical therapy team management mode was executed, the therapist's theoretical knowledge and technical skill were upgraded, patient satisfaction raised, and the occurrence of accidental injury reduced.ConclusionThe physical therapy team management mode is conducive to standardize therapist behavior, improve the capacity of theory and practice of therapists and promote patient recovery.
