ABSTRACT

Objective:To understand the current situation of counselors' clinical practice competence. Method: Totally 237 counselors aged 23 to 59 years were selected in Henan Province. Among all the subjects, 200 counselors were supervised occasionally, 37 counselors were often supervised. The Counselors' Clinical Practice Competence Inventory (CCPCI, including application of theory and technology, emotional perception and empathy) was used in this survey. Results: The total score of CCPCI was (41.0 ± 7.2), the scores of two factors were (26.2 ± 5.4) and (14.8 ± 2.3) respectively. The CCPCI total scores and the scores of two factors were higher in counselors over 35 years old than in counselors less than 35 years old (Ps <0.001), higher in counselors who had more than 3 years clinical practice than in freshman who had less than 3 years clinical practice (Ps <0.001), and higher in counselors who accepted once and more professional training sessions than in those who had no training session (P < 0.01 or P<0.05). The CCPCI total scores and the scores of application of theory and technology were higher in counselors who were often supervised than in those who were occasionally supervised (Ps <0.001). Conclusion: It suggests that the clinical practice competence of the counselors in Henan Province may be at middle level. Accepting supervision regularly and continuously professional training could improve the counselors' clinical practice competence.

A finales del siglo XX, la calidad se convirtió en un aspecto central del debate sobre los cuidados de salud. El ejercicio de una medicina que utiliza, consciente, explícita y racionalmente la mejor evidencia clínica disponible para tomar decisiones sobre el cuidado de los pacientes implica la integración de la maestría clínica individual con las evidencias o pruebas científicas más consistentes y confiables resultantes del avance de la investigación clínica, apropiadas, tanto para el paciente y su condición, como para el médico y el escenario clínico existente. Sin embargo, hallar estas evidencias para cada interrogante clínica no es una tarea fácil de cumplir. Se requiere entonces de nuevas herramientas para integrar adecuadamente toda la información válida y crear una base para tomar decisiones de manera racional. Se estudia con detalle qué son, dónde y cómo encontrar guías para la práctica clínica, así como la importancia del National Guideline Clearinghouse en el acceso a esta clase de documentos. Las guías para la práctica clínica constituyen poderosos aliados para un ejercicio más seguro de la medicina moderna


In the late twentieth century, quality became a central aspect of the debate on health care. The exercise of a medicine that uses conscious, explicit and rationally the best clinical evidence available to make decisions about patient care involves integrating individual clinical expertise with evidence or evidence of more consistent and reliable result of the advancement of clinical research, appropriate for both the patient and his condition and the doctor and the existing clinical setting. However, finding this evidence for each clinical question is not an easy task to accomplish. It then requires new tools to properly integrate valid information and create a basis for making rational decisions. We study in detail what, where and how to find clinical practice guidelines and the importance of the National Guideline Clearinghouse access to such documents. The clinical practice guidelines are powerful allies for a more secure modern medicine
