ABSTRACT

@#Acute suppurative thyroiditis is a rare and potentially fatal condition. We present a case of an 18-year-old Malay female who presented with one-week history of painful right sided neck swelling, fever and odynophagia. Neck CT confirms ruptured multiloculated abscess with posterosuperior extension into prevertebral space. Pus and tissue cultured Streptococcus anginosus and Eikenella corrodens with positive TB PCR. She responded well to ampicillin/sulbactam and anti-tuberculosis treatment with no evidence of residual collection from ultrasound.

ABSTRACT

The thyroid is resistant to infection due to its anatomical and physiological characteristics. We present a rare case of invasive liver abscess with metastatic thyroid abscess and endogenous endophthalmitis in a previously healthy 55-year-old female patient without diabetes or other medical illness. This report raises an important question of the virulence of Klebsiella pneumoniae as an increasingly common causative agent of liver abscess.

ABSTRACT

Acute suppurative thyroiditis (AST) is a rare non-specific infectious disease of thyroid gland.The signs and symptoms of AST may mimic those of a variety of noninfectious inflammatory conditions or other anterior neck infectious diseases.AST is a progressive disease and is also potentially life threatening if devoid of prompt diagnosis and proper treatment.Herein,we report a case in order to call attention to the diagnosis and treatment of AST.

ABSTRACT

Tuberculosis (TB) of the thyroid gland is rare, with acute abscess formation being the least common form of presentation. As such, TB of the thyroid may be overlooked as an initial clinical impression. A careful approach by history, physical examination, laboratory testing and radiologic imaging may aid in defining the etiology of the thyroid abscess, but these remain nonspecific. An accurate diagnosis may only be made by histologic examination. We present a case of a tuberculous abscess of the thyroid gland in a 37-year-old female. TB of the thyroid should also be considered when evaluating patients presenting with a painful thyroid nodule.

ABSTRACT

ObjectiveTo discuss the management of chronic thyroid abscess in children. MethodsThe diagnosis and management of 16 children with chronic thyroid abscess admitted from Jul. 2007 to Jun. 2010 in Department of Thyroid Surgery of Sun Yat-ssn Memorial Hospital of Sun Yat-sen University were retrospectively analyzed. All the patients were checked by Doppler ultrasound. ResultsOf the patients, 6 were males and 10 were females. The time of onset was from 3.2 years to 8.5 years, with 4. 6 years as the median. Hypoechoic or mixed echoic lesions in thyroid were seen on Doppler ultrasound scan in all patients. Abscess was found on the left side of thyroid in 11 cases (68.8%), and on the right side in 5 cases (31.2%). Abscess in 12 cases (75%) occupied the whole thyroid and began encroaching the adjacent tissues. Hypodermic fistula was found in 7 cases (43.75%). 12 cases (75%) underwent part resection of thyroid gland, and 4 cases (25%) underwent total resection of thyroid gland and debridement. All patients recovered and no complication like vocal hoarseness occurred. No recurrence happened within the follow-up of 3 months to 5 years. ConclusionsThe effective diagnosis of chronic thyroid abscess in children is to perform Doppler ultrasound scan of thyroid gland before operation. Abscess and fistula resection, partial or total resection of the affected side of thyroid gland are needed. Wound drainage and postoperative antibiotc are also helpful.

We report a case of thyroid abscess caused by Scedosporium apiospermum in a patient with cirrhosis of liver and autoimmune haemolytic anaemia. To date, there are no reports of isolation of this fungus from thyroid abscess.

ABSTRACT

It is well known that the thyroid gland is resistant to infection due to its anatomic and physiological characteristics. Thyroid abscess in an adult is extremely rare. It is more commonly found in children than in adults. The treatment goal of this disease is to eliminate the source of infection by incision and drainage, or by a thyroidectomy and administration of antibiotics, depending on the clinical findings.We report a case of thyroid abscess found in a 29-year-old woman. The patient presented with a painful mass in the left thyroid for 6 days duration. The patient had a history of subacute thyroiditis that was treated with steroidsand thyroid hormone. Computed tomography showed a large, fluid contained, cystic predominant mass in the left thyroid. Aspiration of the cystic fluid confirmed the presence of the thyroid abscess. Under local anesthesia, an incision and drainage was performed. The patient improved dramatically after surgery and the patient was discharged 8 days later.

ABSTRACT

Infectious disease took the leading cause of death in patients with systemic lupus erythematosus (SLE). Especially Salmonella infection occurs more common in SLE patients. We report a case of thyroid abscess caused by Salmonella Group D in a patient with SLE. A patient, 29-year-old female, with SLE was admitted with a painful swelling of the left neck and diarrhea. The painful swelling of neck was revealed as an abscess of the thyroid and Salmonella Group D was isolated from the aspirated material of the thyroid abscess. The patient recovered after surgical drainage and administration of intravenous 3rd cephalosporin.

ABSTRACT

Subacute thyroiditis is a frequent benign thyroid disease associated with previous viral upper respiratory tract infection. Known complications of this disease are long-standing subclinical hypothyroidism, persistent anterior neck pain and rarely Graves disease. In general, thyroid abscess is an uncommon disease because of anatomic isolation of the gland and its rich system of drainage for blood and lymph. Especially, development of thyroid abscess in subacute thyroiditis is extremely rare phenomenan, but significant bad outcomes can be resulted. Its clinical BACKGROUND containes immune-suppressed state, anatomic defect, presence of underlying other thyroid disease and of non-thyroidal infectious foci. We experienced a case of subacute thyroiditis complicated with streptococcal thyroid abscess during glucocorticoid therapy. The patient was a 19-year-old female who was admitted due to anterior neck pain for 1 month. Typical subacute thyroiditis was suggested from initial laboratory findings including CBC, erythrocyte sedimentation rate, serum T3, T4, TSH levels, thyroid scan & thyroid uptake. But during oral prednisolone therapy, unexpected bacterial thyroid abscess was developed. We report this unusual case with review of literatures.

ABSTRACT

Acute thyroid abscess is an uneommon type of neck infection. We experieneed a case of staphylococcal thyroid abscess in 29 year old man, diagnosed by needle aspira~tion and culture of the aspirate. The patient had complained fever and slowly growing anterior neck swelling. Needle aspiration of the cold nodule prior to broad spectrum antibiotics was performed. After Surgical excision combined with antibiotics, he was discharged with elinical improvement. The laboratory diagnosis and clinical course was summarized and is reported with relevent references.
