ABSTRACT
Objective To analysis and identify the components of Tianqi Jiangtang Tables. Methods The components of Tianqi Jiangtang Capsules were separated on Waters ACQUITY UPLC® BEH C18 column (100 mm × 2.1 mm, 1.7 μm). The mobile phase consisted of ammonium formate (A) and methol (B) using a gradient elution (buffer pH ≈ 3.5). The mass spectrometer was operated in both positive and negative mode, and the main chromatographic peaks were detected by UPLC-LTQ/Orbitrap/MS/MS. Compounds were identified on the basis of their accurate molecular weight and MSn fragment data, or by comparing the relative retention time and the mass spectrometric data of the standard substance and consulting the reference literature. Results A total of 17 compounds were identified, including 9 alkaloids, 3 glycosides, 3 organic acids, and 2 flavonoids. Conclusion The results of components identification can provide the evidence for pharmacological studies, quality control, and clinical application of Tianqi Jiangtang Capsules.
ABSTRACT
OBJECTIVE:To observe clinical efficacy and safety of Tianqi jiangtang capsules combined with metformin in the treatment of elderly patients with type 2 diabetes mellitus complicated with cerebral microvascular lesions. METHODS:A total of 96 elderly patients with type 2 diabetes mellitus complicated with cerebral microvascular lesions were randomly divided into control group(48 cases)and observation group(48 cases). Control group was given Metformin hydrochloride tablets 0.5 g orally,3 times a day. Observation group was additionally given Tianqi jiangtang capsules 1.6 g orally,3 times a day,on the basis of control group. Both groups received treatment for 4 weeks. Clinical efficacies of 2 groups were observed as well as total score of TCM symptoms,the levels of blood glucose(FPG,2 hPG,HbA1c),blood lipid(TC,TG,HDL-C,LDL-C),hs-CRP,ET-1,blood rheology and oxidative stress indexes(MDA,SOD),the occurrence of ADR. RESULTS:The total response rate of observation group was significantly higher than that of control group(89.58% vs. 70.83%),with statistical significance(P<0.05). After treat-ment,total score of TCM symptoms,the levels of blood glucose,hs-CRP,ET-1,TC,TG,LDL-C,blood rheology indexes and MDA in 2 groups were significantly lower than before;total score of TCM symptoms,the levels of FPG,2 hPG,hs-CRP,ET-1, TC,TG,LDL-C,blood rheology indexes and MDA in observation group were significantly lower than control group;the level of SOD in 2 groups were significantly higher than before,and the observation group was significantly higher than control group,and the level of HDL-C in observation group was significantly higher than before treatment and control group,with statistical signifi-cance(P<0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Tianqi jiangtang capsules combined with metformin show significant therapeutic efficacy in the treatment of elderly patients with type 2 diabetes mellitus,and can re-duce the levels of blood glucose and lipid,improve blood rheology and oxidative stress reaction,with goud safety.

Objective To investigate the effect of Tianqi Jiangtang capsules combined with metformin on blood viscosity, cognitive disorder in elderly patients with type 2 diabetes mellitus complicated with cerebral microvascular lesions.Methods 76 patients with type 2 diabetes mellitus complicated with cerebral microvascular lesions were divided into the observation group and the control group according to odd and even numbers, 38 cases in each group.The observation group were given Tianqi Jiangtang capsules combined with metformin while the control group were received metformin alone.The clinical curative effect, adverse reactions were compared between the two groups.The total score of TCM symptoms, blood glucose, blood lipid, high-sensitivity C reactive protein(hs-CRP), hemorheology and Mini Mental State Examination(MMSE)scores were determined before and after treatment.Results The total effective rate in the observation group(89.47%)was significantly higher than that in the control group(71.05%)(P< 0.05);After treatment, the total score of TCM symptoms, FPG, 2 h PG, hs-CRP, TC, TG, LDL-C, whole blood viscosity at high shear, whole blood viscosity at low shear, plasma viscosity and hematocrit were significantly lower than those before treatment, while MMSE scores were significantly higher(P< 0.05), and the above indexes in the observation group after treatment were significantly better than those in the control group(P< 0.05).There were no significant adverse reactions in the 2 groups.Conclusion Tianqi Jiangtang Capsules combined with metformin is safe and effective in the treatment of elderly patients with type 2 diabetes mellitus complicated with cerebral microvascular lesions.It can significantly improve lipid metabolism and hemorheology, reduce blood viscosity, and alleviate cognitive disorder.
