ABSTRACT

With high morbidity, branch retinal vein occlusion (BRVO) is a common retinal vascular disease in the clinic. Although the classic characteristics of BRVO have been recognized for a long time, the traditional understanding of BRVO has been challenged along with development and application of new imaging technologies, including the reasonable classification and staging of the disease, and the vascular characteristics at the occlusive site via multimodal imaging, etc. Thus, re-summarizing and refining these features as well as further improving and optimizing traditional imaging evaluation, can not only deepen the correct acknowledge of the entity, but also find biomarkers of prognosis of visual function, which is helpful to establish better diagnosis and treatment strategy. In the meanwhile, it is necessary that clinical characteristics of BRVO on imaging and the reliability of these imaging techniques are worth correct understanding and objective assessment.

ABSTRACT

ABSTRACT Optical coherence tomography is often used for detection of glaucoma as well as to monitor progression. This paper reviews the most common types of artifacts on the optical coherence tomography report that may be confused with glaucomatous damage. We mainly focus on anatomy-related artifacts in which the retinal layer segmentation and thickness measurements are correct. In such cases, the probability maps (also known as deviation maps) show abnormal (red and yellow) regions, which may mislead the clinician to assume disease is present. This is due to the anatomic variability of the individual, and the normative database must be taken into account.


RESUMO A tomografia de coerência óptica é frequentemente usada para detectar glaucoma, bem como para monitorar a progressão. Este artigo analisa os tipos mais comuns de artefatos no relatório de tomografia de coerência óptica que podem ser confundidos com danos glaucomatosos. Nós nos concentramos principalmente nos artefatos relacionados à anatomia em que a segmentação da camada da retina e as medidas de espessura estão corretas. Nesses casos, os mapas de probabilidade (também conhecidos como mapas de desvio) mostram regiões anormais (vermelho e amarelo), o que pode induzir o clínico em erro ao supor que a doença está presente. Isto se deve à variabilidade anatômica do indivíduo, e o banco de dados normativo deve ser levado em conta.

ABSTRACT

RESUMO Objetivos: Determinar a freqüência e as características das alterações em exame oftalmológico, em exame de Tomografia de coerência óptica (OCT) do nervo e mácula e Campimetria em pacientes com Escrelose Multipla(EM). Métodos: Foram examinados 60 olhos sendo 30 de pacientes com o diagnóstico de EM e 30 de pacientes controles, atendidos no Hospital de Base do Distrito Federal. Os pacientes foram avaliados quanto aos parâmetros: características e alterações do exame oftalmológico, do OCT do nervo e da macula e Campimetria. Resultados: Os pacientes com EM apresentaram piores resultados em todos os parâmetros avaliados. No exame de campo visual Foram encontradas perdas localizadas em 50%. Em relação ao OCT de nervo óptico foi observado redução da camada de fibras nervosas em quadrantes temporal (p=0,0251) e inferior (p=0,0041), o OCT de mácula revelou diminuição da CFN principalmente nos quadrantes nasal interno (p=0,0002) e externo (p=0,0016),inferior interno (p=0,0007) e superior externo (p=0,0108) e interno (p=0,0046). Os pacientes com menores valores de espessura macular também tiveram piores resultados no campo visual (p=0,0001). Conclusão: Este estudo demonstrou que a EM é uma doença capaz de ocasionar alterações nos exames de OCT e Campo visual mesmo na ausência de sintomas visuais relatados pelos pacientes. A realização de exames como campo visual e de OCT de macula e nervo podem ser uma ferramenta útil para estimar o comprometimento pela doença e auxiliar no seguimento desses pacientes.


ABSTRACT Objectives: To determine the frequency and characteristics of alterations in ophthalmologic examinations of optic nerve and macula coherence tomography (OCT), and Campimetry in multiple sclerosis (MS) patients. Methods: Sixty eyes were examined, 30 of which were diagnosed with MS and 30 of the control patients, all attended at the General Hospital of the Federal District of Brazil. The patients were evaluated regarding the parameters: characteristics and alterations of the ophthalmological examination, OCT of the nerve and macula and Campimetry. Results: Patients with MS presented worse results in all parameters evaluated. On visual field examination localized losses were found in 50% of the cases . In relation to OCT of the optic nerve it was observed a reduction of the nerve fiber layer in temporal quadrants (p = 0.0251) and lower (p = 0.0041). The macular OCT revealed a decrease in the CFN, mainly in the internal nasal quadrants (p = 0.0002) and external (p = 0.0016), internal inferior (p = 0.0007) and external superior (p = 0.0108) plus internal (p = 0.0046). Patients with lower values of macular thickness also had worse results in the visual field (p = 0.0001). Conclusion: This study demonstrated that MS is a disease capable of causing changes in OCT and visual field tests even in the absence of visual symptoms reported by patients. Examinations such as visual field and OCT of macula and nerve can be a useful tool to estimate the damage by the disease and to assist in the follow-up of these patients

ABSTRACT

Background Intravitreal injection of ranibizumab (IVR) is one of the most effective therapies for neovascular age-related macular degeneration (nAMD).Understanding the influence of IVR on retinal pigment epithelium (RPE) and choroidal thickness is helpful for us to choose the operative times and timing based on pharmacologic effects and tissue response.However,limited studies are available about quantitative analysis of RPE atrophic area and subfoveal choroidal thickness after IVR for nAMD.Objective This study was to report the changes of RPE atrophic area and subfoveal choroidal thickness after IVR for nAMD.Methods A prospective series cases-observational study was designed.Forty-one eyes of 41 consecutive patients with nAMD were enrolled in Renmin Hospital of Wuhan University from January 2015 to June 2015,and written informed consent was obtained from each patient prior to entering the cohort.The affected eyes received intravitreal injection of 0.05 ml ranibizumab (10 mg/ml) and then followed up monthly for 12 months.The RPE atrophy area around macula and subfoveal choroidal thickness were measured by a newly developed RPE analysis software spectral-domain optical coherence tomography (OCT) and enhanced depth imaging of SD-OCT (EDI-OCT),respectively,and the RPE atrophy area and choroidal thickness changes were compared before IVR and 3,6 and 12 months after IVR.The correlation between RPE atrophy area and choroidal thickness before and after IVR was analyzed.Results All the patients finished the treating procedure and follow up.The visual acuity (logMAR) after IVR was considerably improved in comparison with before IVR (F=7.631,P<0.001).The mean subfoveal choroidal thickness value was (264.55 ± 100.95) μm before IVR,and that of 3,6,12 months after IVR was (247.42±105.46),(246.81± 99.85) and (253.97±101.15)μm,respectively,showing a significant difference among different time points (F =2.030,P < 0.05),and the mean subfoveal choroidal thickness values 3,6,12 months after IVR were evidently thinned in comparison with before IVR (all at P<0.05).No significant difference was found in RPE atrophic area among different time points (F=0.116,P =0.951).Weak linear correlations were seen between RPE atrophy area and choroidal thickness (r =-0.185),the RPE atrophy area change values and choroidal thickness change values between IVR > 6 times and ≤ 6 times (r =0.297,-0.327),but these results were not statistically significant (P =0.248,0.282,0.103).At the end of the follow up,weak linear correlations were seen in RPE atrophy area change values and choroidal thickness change values with IVR times (r,=-0.266,0.342),but these results were not statistically significant (P =0.148,0.060).Conclusions IVR for nAMD can lead to subfoveal choroid atrophy instead of RPE atrophy.IVR does not accelerate the atrophy progression of both RPE and choroid.

ABSTRACT

We report a case of a 66-year-old man with a history of high myopia and who was referred for acute decreased visual acuity of the right eye. Fundus examination and optical coherence tomography (OCT) showed a mild epiretinal membrane (ERM) and splitting of retinal layers. Pars plana vitrectomy was performed with intravitreous triamcinolone injection, posterior hyaloid and ERM peeling, and 12% perfluoropropane (C3F8) gas tamponade. After remaining asymptomatic for 17 months, the patient reported a new episode of sudden decreased visual acuity in his right eye, and OCT showed recurrent myopic foveoschisis (MF). He underwent vitrectomy and internal limiting membrane (ILM) peeling. Six months later, the patient’s best corrected visual acuity had improved to 20/25. Optical coherence tomography showed a remarkably improved macular anatomy, with residual traction along the inferotemporal arcade, which was attributed to the vessel stiffness itself. We conclude that removing the internal limiting membrane is a challenging maneuver in myopic foveoschisis, even with staining approaches. Although myopic foveoschisis may be resolved without peeling the internal limiting membrane, its removal should be considered if the condition recurs.


Relatamos caso de um homem de 66 anos, com antecedente de alta miopia, que referiu baixa de acuidade visual aguda no olho direito. O mapeamento de retina e o exame de tomografia de coerência óptica (OCT) mostraram discreta membrana epirretiniana (ERM) e isquise retiniana. Foi realizada vitrectomia via pars plana com injeção intravítrea de triancinolona, retirada da hialóide posterior, peeling da membrana epirretiniana e tamponamento com gás perfluorpropano (C3F8) a 12%. O paciente permaneceu assintomático por 17 meses, quando queixou-se de novo episódio de baixa de acuidade visual súbita no olho direito e o tomografia de coerência óptica mostrou recorrência da isquise miópica. Ele foi submetido a nova vitrectomia com peeling da membrana limitante interna (ILM). Após 6 meses, a acuidade visual corrigida era de 20/25. A tomografia de coerência óptica mostrou melhora importante da anatomia macular, com área de tração residual observada na arcada inferotemporal, que foi atribuída à rigidez do próprio vaso. A retirada da membrana limitante interna é uma manobra desafiadora em olhos alto míopes, mesmo estando corada. A resolução da isquise miópica pode ser atingida sem o peeling da membrana limitante interna, mas sua remoção deve ser considerada em casos de recorrência.

O diagnóstico do edema macular diabético deve ser feito precocemente, a fim de evitar perda visual significativa. Para tal, contamos com exames complementares cada vez mais desenvolvidos e específicos. Os autores fazem uma revisão dos principais métodos existentes para diagnosticar a retinopatia diabética.


Diabetic macular edema must be assessed as early as possible, to avoid significant vision loss. To achieve this goal, there are many specific diagnostic tests that can be performed. The authors review the most common methods in diagnosing diabetic retinopathy.
