ABSTRACT

Objective:As the problem of global aging intensifies,postmenopausal osteoporosis (PMOP) has become a global health problem among females. At present,the commonly used biological agents have been proved not suitable for long-term use due to multiple adverse reactions. Several Meta-analyses have confirmed the good safety and effectiveness of kidney-tonifying method against PMOP,but its therapeutic mechanism remains unclear. The purpose of this Meta-analysis was to evaluate the effect of kidney-tonifying method on osteoclastogenesis inhibitory factor(OPG)/receptor activator of nuclear transcription factor (NF)-<italic>κ</italic>B (RANK)/receptor activator of NF-<italic>κ</italic>B ligand (RANKL) signaling pathway in PMOP animal model,so as to provide an experimental basis for the treatment of PMOP with kidney-tonifying method. Method:The related articles were retrieved from PubMed,Ovid Medline,Embase,China National Knowledge Infrastructure (CNKI),Chongqing Weipu Database for Chinese Technical Periodicals (VIP),and Wanfang Data Knowledge Service Platform with the retrieval time set from their inception to January 2020. The quality of each included article was evaluated using the SYRCLE's risk of bias tool. Then RevMan 5.3 was utilized for Meta-analysis according to the Cochrane systematic review methodology. Result:Thirty-two studies involving 619 rats were included. The quality score of these studies ranged from 3 to 5 points. The results of the Meta-analysis indicated obvious advantages of kidney-tonifying method in increasing bone mineral density (BMD)[standardized mean difference (SMD)=2.01,95% confidence interval(CI)=1.50-2.52,<italic>P</italic><0.000 01]),serum OPG level (SMD=3.33,95% CI=2.59-4.07,<italic>P</italic><0.000 01),and OPG mRNA expression (SMD=11.81,95% CI=7.49-16.13,<italic>P</italic><0.000 01),promoting OPG protein production (SMD=4.95,95% CI=3.09-6.81,<italic>P</italic><0.000 01),reducing serum RANKL(SMD=-4.88,95% CI=-6.01--3.75,<italic>P</italic><0.000 01) and RANK levels (SMD=-7.30,95% CI=-9.53--5.07,<italic>P</italic><0.000 01),and down-regulating RANKL (SMD=-6.22,95%CI=-8.95--3.49,<italic>P</italic><0.000 01) and RANK mRNA (SMD=-3.18,95% CI=-6.19--0.18,<italic>P</italic><0.05) expression and RANKL protein expression in bone tissue (SMD=-3.99,95% CI=-5.47--2.50,<italic>P</italic><0.000 01). Conclusion:The kidney-tonifying method has been proved to possess potential advantages in regulating the balance of OPG/RANK/RANKL signaling pathway in PMOP animal model. Nevertheless,more large-sample sized,properly designed,and high-quality animal experiments are still needed for further verification.

ABSTRACT

This study was aimed to evaluate impacts on immune regulation with spleen-strengthening and kidney-tonifying method among advanced malignant tumor patients. The enrolled advanced malignant tumor patients were randomly divided into the control group and the treatment group. The control group was treated with conventional treatment plan. The treatment group was treated with spleen-strengthening and kidney-tonifying Chinese herbs on the basis of conventional treatment plan for three months. Clinical effect, integral of clinical symptom (ICS), T-lym-phocyte subsets, and drug safety index were observed and detected before and after treatment. The results showed that the effective rate and response rate in the control group were 31.1% and 71.1%, respectively. And the effective rate and response rate in the treatment group were 54.2% and 85.4%, respectively. The treatment group was better than the control group (P< 0.05). Compared to the control group, the treatment group had obvious regulation on T-lymphocyte subsets. The improvement on ICS in the treatment group was better than the control group (P < 0.05). There was no obvious adverse reaction with the application of Chinese herbs. It was concluded that the spleen-strengthening and kidney-tonifying method can improve clinical effect, life of quality, and reduce adverse reaction a-mong advanced malignant tumor patients. Its mechanism may be through the regulation of immune function.

ABSTRACT

This study was aimed to evaluate the clinical effect and safety of Chinese medicine treatment of lumbar disc herniation with qi-tonifying, stasis-resolving and kidney-tonifying method. The randomized con-trolled trial (RCT) was applied in the study to evaluate the clinical effect of qi-tonifying, stasis-resolving and kidney-tonifying method in the treatment of lumbar disc herniation . A total of 122 lumbar disc herniation pa-tients were randomly divided into the treatment group ( n = 61 ) and the control group ( n = 61 ) . Chinese medicine treatment with the qi-tonifying, stasis-resolving and kidney-tonifying method was applied in the treatment group . And Celecoxib and Methycobal were orally administered in the control group . Then , the VAS scores, JOA scores, Oswestry disability index (ODI) were recorded and analyzed pre-treatment, four weeks af-ter treatment and the twelfth week of follow-up in order to evaluate the clinical effect . Adverse reactions were also observed and recorded at the same time to give a comprehensive evaluation on its safety . The results showed that there were no significant differences between the treatment group and control group in the baseline data before treatment . Hence , data from two groups were comparable . Compared with pre-treatment , the VAS scores and ODI scores were obviously reduced in both groups after four-week treatment . The JOA scores were increased obviously ( P both groups . There were no statistical differences between two groups . There were no statistical differences on the total effective rate between two groups . In the treatment group , four patients received surgery , four cases lost to follow-up , and four cases with mild adverse event . In the control group , six patients received surgery , three cases lost to follow-up , and two cases with mild adverse event . It was concluded that the RCT of Chi-nese medicine treatment of lumbar disc herniation with q i-tonifying , stasis-resolving and kidney-tonifying method received same clinical effect as the combination of Celecoxib and Methycobal . The Chinese medicine treatment can effectively relieve pain degree of lumbar disc herniation , improve function of the lumbar vertebrae and improve the daily life and social activity ability of patients. The short-term follow-up effects were con-firmed . However , the long-term efficacy still requires further study .

ABSTRACT

Tonifying method has been used to treat various types of deficiency syndrome in traditional Korean medicine. Tonifying medicinal including ginseng and deer horns is one of the various methods of the treatments, but they are recognized as a representative of traditional Korean remedies nowadays in Korea. That is concerned with the new trends of medicine manifested at Naeuiwon(Royal Infirmary) in the late Chosun period. The period that manifested the tonifying method obviously was the reign of King Youngjo(r.1724-1776). King Youngjo who lived longest among Chosun kings considered tonifying yang very important in keeping him healthy. He had taken a large quantity of ginseng, he and others considered the reason for his longevity as taking ginseng. From that time, the method of tonifying yang became one of the principles in health care and treatment as well. In the 19th century, the theory of tonifying method had been changed, in that tonifying yin was considered more important among Naeuiwon physicians. Tonifying yang alone was thought to be harmful because of its warm and dry nature. The main cause of prevalence of tonifying method in Naeuiwon was the fact that it was safe and had little side effects. The method of health care and treatments of the kings was considered as an ideal model by the ordinary people at that time. The new trends of medicine manifested at Naeuiwon in the late Chosun period had a strong influence on traditional Korean medicine, which emphasized the importance of tonifying method.

ABSTRACT

Chronic pelvic pain is a common symptom of gynecological diseases,traditional Chinese medicine treatment is a important method in treating chronic pelvic pain without operation indication.According to his clinical experience of more than sixty years,famous gynecological professor XU Run-san suggested that deficient syndrome of chronic pelvic pain possesses a definite proportion in clinic;most of chronic repeated pelvic pain of long course has the pathogenesis of qi,yang deficiency,stagnation of blood circulation and deficiency complicated with blood stasis.The etiology of chronic pelvic pain is complicated,according to the TCM theory of "treating different diseases with the same method",tonifying method such as nourishing qi and raising yang,warming spleen and stomach,nourishing qi and activating blood,warming meridian and resolving stasis can be used for syndrome of qi,yang deficiency.Treatment with tonifying decoctions such as buzhongyiqi decoction,huangqijianzhong decoction based on syndrome differentiation is the key to improve the curative effect.
