ABSTRACT

Objective To observe the short-term clinical effect of compound phellodendron gargle combined with the total glucosides of paeony (TGP) in the treatment of erosive oral lichen planus (OLP). Methods 62 patients were divided into observation group and control group through a designed parallel randomized controlled study. All the patients used the total glucosides of paeony capsule , the patients in the observation group also used the compound phellodendron gargle. The pain condition, the healing condition of face, and the treatment effective in both group were evaluated through physical signs and VAS scores evaluations by SPSS 22.0, which could further evaluate the short-term clinical efficacy of compound phellodendron. Results After 30 days, the scores of VAS and physical signs of each group are much better than before. And the scores of VAS and treatment effective of observation group were significantly better than those of the other control groups (P<0.05). Conclusion The application of phellodendron gargle with total glucosides of paeony capsule could improve the treatment effect of OLP patients and reduce the oral pain , which could be used widely in clinical practice.

ABSTRACT

OBJECTIVE To investigate the effects of total glucosides of paeony (TGP) on enhancing efficacy and reducing toxicity of Tripterygium wilfordii polyglycoside (TWP) in the treatment of eczema. METHODS Totally 50 SD male rats were collected to establish eczema model by sensitizing with 2,4-dinitrofluorobenzene-acetone olive oil solution (volume ratio was 4∶1) on the abdominal area and provoking on the back and ear. Model rats were randomly divided into model group, loratadine group (0.9 mg/kg), TWP group (9.45 mg/kg), TGP group (162 mg/kg) and compatibility group (TWP 9.45 mg/kg+TGP 162 mg/kg), with 10 rats in each group. Other 10 rats were collected to set as normal group. Three days after the first sensitization, administration groups were given relevant medicine intragastrically, and normal group and model group were given constant volume of 0.1% CMC-Na solution intragastrically, once a day, for consecutive 21 d. Twenty-four hours later after the final administration, the general condition of rats in each group was observed, and the eczema area and severity index (EASI) were scored; ear swelling degree of rats was measured, and the skinhistomorphology observation and pathological score were performed; protein expressions of p38 mitogen-activated 13938427612@126.com protein kinase (p38 MAPK), phosphorylated p38 MAPK (p- MAPK) and phosphorylation level of p38 MAPK in rat skin tissue were detected; the levels of inflammatory indexes (interleukin-4, interferon- γ), liver and kidney function indexes [glutamic-pyruvic transaminase (GPT), glutamic-oxaloacetic transaminase (GOT), serum creatinine (SCr) and blood urea nitrogen (BUN)] and oxidant stress indexes [total superoxide dismutase (T-SOD) and malondialdehyde (MDA)] were measured. RESULTS Compared with normal group, EASI score, ear swelling degree, pathological score, protein expressions of p38 MAPK and p-p38 MAPK, phosphorylation level of p38 MAPK, the levels of inflammatory indexes and BUN were all increased significantly in model group (P<0.05). Compared with model group, EASI scores, ear swelling degree, pathological scores, protein expressions of p38 MAPK and p-p38 MAPK, phosphorylation levels of p38 MAPK and levels of inflammatory indexes were all improved significantly in administration groups (P<0.05). The levels of GPT, GOT, SCr and BUN were increased significantly in TWP group, while the serum levels of GOT and SCr in TGP group and serum level of SCr in loratadine group were all decreased significantly (P<0.05). The levels of T-SOD in liver and kidney tissue were all decreased significantly in TWP group and compatibility group, while the levels of MDA were increased significantly (P<0.05). The compatibility group showed more obvious effect in improving the ear swelling degree, pathological score, p38 MAPK expression and its phosphorylation level and levels of inflammatory indexes, and could reverse the abnormality of liver and kidney indexes caused by TWP (P<0.05). CONCLUSIONS The combination of TGP and TWP has the effects of anti-inflammatory, synergistic and hepatorenal detoxification in eczema model rats. Its mechanism may be associated with down-regulating the expression of serum proinflammatory indexes and inhibiting the activation of p38 MAPK pathway.

ABSTRACT

OBJECTIVE@#To investigate the therapeutic effects of total glucosides of paeony (TGP) on psoriasis based on the immunomodulatory effect of dermal mesenchymal stem cells (DMSCs).@*METHODS@#A total of 30 male BALB/c mice were divided into 6 groups (n=5 in each) by a random number table method, including control, psoriasis model (model, 5% imiquimod cream 42 mg/d), low-, medium- and high-dose TGP (50, 100, and 200 mg/kg, L, M-, and H-TGP, respectively), and positive control group (2.5 mg/kg acitretin). After 14 days of continuous administration, the skin's histopathological changes, apoptosis, secretion of inflammatory cytokines, and proportion of regulatory T cells (Treg) and T helper cell 17 (Th17) were evaluated using hematoxylin-eosin (HE) staining, TdT-mediated dUTP nick end labeling staining, enzyme-linked immunosorbent assay, and flow cytometry, respectively. DMSCs were further isolated from the skin tissues of normal and psoriatic mice, and the cell morphology, phenotype, and cycle were observed. Furthermore, TGP was used to treat psoriatic DMSCs to analyze the effects on the DMSCs immune regulation.@*RESULTS@#TGP alleviated skin pathological injury, reduced epidermis layer thickness, inhibited apoptosis, and regulated the secretion of inflammatory cytokines and the proportion of Treg and Th17 in the skin tissues of psoriatic mice (P<0.05 or P<0.01). There was no significant difference in cell morphology and phenotype between control and psoriatic DMSCs (P>0.05), however, more psoriatic DMSCs remained in G0/G1 phase compared with the normal DMSCs (P<0.01). TGP treatment of psoriatic DMSCs significantly increased cell viability, decreased apoptosis, relieved inflammatory response, and inhibited the expression of toll-like receptor 4 and P65 (P<0.05 or P<0.01).@*CONCLUSION@#TGP may exert a good therapeutic effect on psoriasis by regulating the immune imbalance of DMSCs.

ABSTRACT

ObjectiveTo investigate the intervention effect of total glucosides of paeony (TGP) on the renal injury of MRL/lpr mice based on the Toll-like receptor 9 (TLR9)/myeloid differentiation factor 88 (MyD88)/nuclear transcription factor-κB (NF-κB) signaling pathway and explore the immunological mechanism of TGP in preventing and treating systemic lupus erythematosus (SLE). MethodMRL/lpr female mice of SPF grade were randomly divided into a model group, a dexamethasone group (0.15 g·kg-1), and high- (0.078 g·kg-1) and low-dose (0.039 g·kg-1) TGP groups, and female C57BL/6J mice were assigned to a blank group, with 7 mice in each group. Mice in each group were treated with corresponding drugs or normal saline by gavage at the same time every day. After 4 weeks, samples were collected. The kidney and spleen were weighed, and the organ index was calculated. Serum creatinine (SCr) and blood urea nitrogen (BUN) levels in each group were detected by biochemical assay. Hematoxylin-eosin (HE) staining was used to observe the histopathological changes in the kidney. The degree of renal fibrosis was evaluated by Masson staining. The serum levels of interleukin (IL)-2, interferon (IFN)-α, IL-4, and anti-nuclear antibody (ANA) were detected by enzyme-linked immunosorbent assay (ELISA). The mRNA expression of TLR9, MyD88, and NF-κB p65 in renal tissues was detected by real-time quantitative polymerase chain reaction (Real-time PCR). The protein expression of TLR9 and NF-κB p65 in renal tissues was detected by immunofluorescence. The protein expression of TLR9, MyD88, and NF-κB p65 in renal and spleen tissues was tested by Western blot. ResultCompared with the blank group, the model group showed increased SCr, BUN, spleen index, and kidney index (P<0.05), deteriorated pathological injury and fibrosis in renal tissues, elevated serum levels of IFN-α, IL-4, and ANA, decreased level of IL-2 (P<0.05), and up-regulated TLR9, MyD88, and NF-κB p65 mRNA and protein levels in the kidney and spleen (P<0.05). Compared with the model group, the TGP groups displayed reduced SCr, BUN, spleen index, and kidney index (P<0.05), relieved pathological damage and fibrosis in renal tissues, decreased serum levels of IFN-α, IL-4, and ANA (P<0.05), increased level of IL-2, and declining mRNA and protein expression levels of TLR9, MyD88, and NF-κB p65 in the kidney and spleen (P<0.05). ConclusionTGP may inhibit the expression of downstream inflammatory factors to regulate immunity and resist SLE-induced renal injury by regulating the TLR9/MyD88/NF-κB signaling pathway.

ABSTRACT

Objective:To investigate the effects of total glucosides of paeony (TGPs) on intestinal motility, barrier function, and gut microbiota in non-obese diabetic (NOD) mice with Sjogren's syndrome (SS). Method:Thirty NOD mice were randomly assigned into the model group (deionized water), prebiotic fructo-oligosaccharide (FOS) group (700 mg∙kg<sup>-1</sup>), and the low- (160 mg∙kg<sup>-1</sup>), medium- (320 mg∙kg<sup>-1</sup>), and high-dose (640 mg∙kg<sup>-1</sup>) TGP groups, with six mice in each group. Moreover, the BALB/c mice were employed as the normal control and administered with deionized water. The food and water intakes, number of discharged fecal particles, and fecal moisture content were observed to evaluate the effect of TGPs on intestinal motility in SS mice. The levels of <italic>D</italic>-lactate (<italic>D</italic>-Lac) content, diamine oxidase (DAO), and junction-associated protein zonula occludens-1 (ZO-1) in mouse serum were detected by enzyme linked immunosorbent assay (ELISA). The fecal samples collected at different time points were determined by spread plate method and gas chromatography for uncovering the intestinal microbial communities and the content of short-chain fatty acids. Result:Compared with the normal group, the model group exhibited decreased food and water intakes (<italic>P</italic><0.01), weakened intestinal propulsion (<italic>P</italic><0.01), elevated <italic>D</italic>-Lac and DAO (<italic>P</italic><0.05,<italic>P</italic><0.01), lowered ZO-1 and SCFAs (<italic>P</italic><0.05,<italic>P</italic><0.01), and reduced number of intestinal bacteria (<italic>P</italic><0.01). The comparison with the model group revealed that TGPs significantly increased the number of discharged fecal particles and fecal moisture content (<italic>P</italic><0.05,<italic>P</italic><0.01), enhanced intestinal propulsion (<italic>P</italic><0.05, <italic>P</italic><0.01), decreased serum <italic>D</italic>-Lac and DAO levels (<italic>P</italic><0.05,<italic>P</italic><0.01), and up-regulated ZO-1 expression (<italic>P</italic><0.01). Apart from increasing the proportions of <italic>Bifidobacterium</italic> and <italic>Lactobacillus</italic> and decreasing the proportion of<italic> Enterobacter </italic>in intestinal flora (<italic>P</italic><0.05,<italic>P</italic><0.01), TGPs also accelerated the production of acetic acid and butyric acid (<italic>P</italic><0.05,<italic>P</italic><0.01). Conclusion:TGPs attenuate SS-mediated constipation and restore the impaired intestinal barrier function in mice by increasing fecal moisture content, boosting intestinal motility, regulating intestinal microbial communities, elevating acetic acid and butyric acid levels, and up-regulating tight junction protein expression.

ABSTRACT

Objective:To investigate the clinical efficacy of Shashen Maidongtang plus total glucosides of paeony capsule on primary Sjogren's syndrome (pSS) based on the theory of fluid metabolism. Method:In this study, 84 patients of Qi-Yin deficiency type pSS admitted in Zhengzhou Chinese Medicine Hospital from January 2018 to January 2019 were divided into observation group (42 cases) and control group (42 cases) on the basis of random number table. The control group was orally given total glucosides of paeony capsule and iguratimod tablet, while the observation group was orally given Shashen Maidongtang combined with glucosides of paeony capsule. After 3 months of continuous treatment to all subjects, the clinical efficacy was evaluated, and side effects were recorded. Before and after the treatment, the saliva flow rate and basal tear secretion Schirmer I test (SIt) value were measured, European League Against Rheumatism Sjogren's syndrome patient reported index and Sjogren's syndrome disease activity index (ESSPRI and ESSDAI) were scored, the erythrocyte sedimentation rate (ESR) was determined by Westergren, and the levels of serum rheumatoid factor (RF) and immunoglobulin (Ig) G were tested by immunoturbidimetry and rate scattering turbidimetry, respectively. Result:The overall effective rate of the observation group was 90.48% (38/42), which was much higher than 69.05% (29/42) of the control group (χ2=5.974,P<0.05). After treatment, the saliva flow rates and SIt values of both groups got significantly increased compared with those before the treatment (P<0.05), but the saliva flow and SIt of the observation group were significantly better than those of the control group over the same period after treatment (P<0.05). After treatment, both groups had a great decrease in ESSPRI and ESSDAI scores compared with those before the treatment (P<0.05), and the above scores of the observation group were dramatically lower than those of the control group over the same period (P<0.05). After treatment, ESR, serum RF, and IgG levels of both groups were significantly lower than those before the treatment (P<0.05), and the observation group showed higher levels of ESR, serum RF and IgG than the control group over the same period after treatment (P<0.05). Side effects were few and mild in both groups. Conclusion:In treating patients of Qi-Yin deficiency type pSS, Shashen Maidongtang plus total glucosides of paeony capsule was proven to be effective generally. It could significantly inhibit excessive inflammation and hyperhumoral immunity in patients, and control their disease activity. This may be related to the effect of Shashen Maidong decoction and its decomposed recipes in correcting body fluid infusion and metabolic disorder in patients of Qi-Yin deficiency type pSS.

ABSTRACT

OBJECTIVE@#To summarize the evidence from systematic reviews (SRs) on the benefits and safety of Tripterygium glycosides (TG) and total glucosides of paeony (TGP), commonly used to treat rheumatoid arthritis (RA) in China, for patients with RA.@*METHODS@#SRs of randomized controlled trials (RCTs) on TG or TGP in treating RA were included, by searching 8 databases from their inception until December 2017. Two authors extracted data independently. We assessed the quality of SRs using AMSTAR and graded the quality of evidence according to the GRADE approach.@*RESULTS@#Eleven SRs containing an average of 7.6 RCTs, involving a total of 7,012 participants were included in this overview. On the basis of included SRs, TG and TGP could improve the following indexes for RA patients: American College of Rheumatology (ACR) 20 response rate, ACR50 response rate and ACR70 response rate, swollen joint count, tender joint count, erythrocyte sedimentation rate and C-reactive protein. Moreover, TGP could reduce incidence of hepatotoxicity. The most common adverse effects of TG were gastrointestinal discomfort and gonad toxicity, while for TGP was mild to moderate diarrhea. The overall quality of evidence for these findings ranged from "low" to "moderate".@*CONCLUSIONS@#TG and TGP might be 2 potentially effective complementary and alternative drugs for patients with RA. Nevertheless, due to gonad toxicity, TG should only be considered in elderly patients or patients without reproductive needs. More evidence from high quality RCTs and SRs is warranted to support the use of TG and TGP for RA patients.

ABSTRACT

Objective @# To investigate the short-term clinical effect of semiconductor laser exposure combined with total glucosides of paeony (TGP) capsules for the treatment of erosive oral lichen planus (OLP).@*Methods@#Sixty-four patients with erosive oral lichen planus were randomly divided into two groups: the experimental group and the control group. Patients in the control group were treated with TGP capsules, while patients in the experimental group were treated with TGP capsules and semiconductor laser irradiation. The clinical effects were evaluated 3 months after treatment. The data were analyzed using the SPSS 17.0 software package.@*Results @#Three months after treatment, the effective rate in the experimental group was 90.6%, which was significantly higher than that in the control group (59.4%, χ 2=5.62, P < 0.05). The physical condition and visual analogue scale (VAS) scores in the experimental group were 2.17 ± 1.49 and 1.25 ± 1.29, respectively. The physical condition and VAS scores in the control group were 3.55 ± 1.41 and 2.09 ± 1.24, respectively. The physical condition and VAS scores in both groups were significantly higher after treatment than before (P < 0.05). Three months after treatment, the physical condition score (t=3.805) and VAS score (t=2.655) in the experimental group were significant higher than those in the control group (P < 0.05).@*Conclusion@#Semiconductor laser irradiation combined with TGP capsules can improve the short-term clinical efficacy in the treatment of erosive OLP.

ABSTRACT

Objective Objective To evaluate the effects and safety of total glucosides of paeony (TGP) in the treatment of patients with Sjogren syndrome.Methods The Cochrane Library,PubMed,EMBASE,CBM,VIP,CNKI,Wanfang databases were searched from their establishments up to September 30,2015.We used the method recommended by the Cochrane collaboration to perform a meta-analysis of randomized controlled trails (RCTs) of total glucosides of peony in the treatment of patients with Sjogren syndrome.Two reviewers analyzed these data independently.The Cochrane Collaboration's software RevMan 5.3 was used for meta-analysis.Results A total of 573 patients in 10 studies were finally included,and were divided into different subgroups.The results of subgroup-analysis showed that:①Schimer test:TGP group had a higher effective rate than the blank control group [MD=2.41,95%CI (0.08,4.74)],lower effective rate than Chinese herbal medicine [MD=-2.55,95%CI (-3.88,-1.22);②Salivary flow:TGP group had a lower effective rate than the control group [SMD=-0.87,95%CI (-1.20,-0.54)].③Rheumatic factors (RF):TGP group had a higher effective rate than Chinese herbal medicine [SMD=0.44,95%CI (0.06,0.82)] and Chinese patent drug [SMD=0.74,95%CI (0.36,1.12)],lower effective rate than the blank control group [SMD=2.23,95%CI (-2.79,-1.67);④ C-reactive protein (CRP):TGP group had a higher effective rate than the control group [MD=4.51,95%CI (1.75,7.26)];⑤IgG:TGP group had a higher effective rate than Chinese patent drug group [MD=2.73,95%CI (1.63,3.84)],lower effective rate than the blank control group [MD=-3.90,95%CI (-5.67,-2.13),but no statistical difference was noted when compared with Chinese herbal medicine and Western medicine groups;⑥ESR:TGP group had a higher effective rate than Chinese herbal medicine group [MD=12.73,95%CI (3.62,21.84)] and Chinese patent drug group [MD=7.82,95%CI (5.39,10.24)],lower effective rate than the blank control group [MD=-7.13,95%CI (-12.70,-1.56) and Western medicine group [MD=-12.19,95%CI (-24.19,-0.19)];⑦Safety:8 studies reported adverse effects in 41 patients.TGP group had a higher adverse reaction rate than the control group [OR=3.23,95%CI (1.60,6.50)].Conclusion Current evidence demonstrates that TGP can effectively improve CRP,but its effects on Salivary flow,Schimer test,IgG,ESR,RF were not significant.However,the heterogeneity and high risk of bias in the reports involved in this study limits the reliability of this conclusion.

ABSTRACT

Objective Psoriasis vulgaris (PV) is easy to prone to recur and hard to cure and little research has been done on combined treatment on PV.The article was to study the clinical effects of total glucosides of paeony capsules (TGP) combined with acitretin and compound flumethasone on PV as well as the peripheral blood cytokine levels.Methods 126 patients with PV who visited our hospital from October 2015 to January 2017 were randomly divided into combined treatment group (63 cases) and control group (63 cases).Both groups were treated with oral acitretin and topical compound flumethasone, what's more, the compound flumethasone group received oral TGP treatment, 8 weeks for a course.The clinical therapeutic effects were evaluated by the levels of peripheral blood IL-17, IL-18, IL-23, TNF-α level, PASI score and percentage of total skin lesions before and after the treatment.Results After the treatment, the concentration of IL-17, IL-18, IL-23 significantly decreased(P<0.05), which was significantly less in combined treatment group compared with control group (IL-17 [61.18±8.91] vs [78.64±7.85], IL-18 [68.56±17.95] vs [79.49±18.64], IL-23 [70.13±12.16] vs [91.18±16.89] pg/ML)(P<0.05).Moreover, the TNF-α level, the PASI score and the percentage of total skin lesions significantly decreased in both groups after treatment(P<0.05), which was significantly less in combined treatment group compared with control group (TNF-α level [14.47±7.53] vs [23.49±8.12]ng/L, PASI score [4.09±1.29] vs [7.29±5.13], the percentage of total skin lesions [6.17±4.59]% vs [8.09±5.18]%) (P<0.05).Conclusion TGP combined with acitretin and compound flumethasone can significantly enhance the clinical therapeutic effects and effectively regulate the levels of the IL-17, IL-18, IL-23 and TNF-α level, which results in treating psoriasis vulgaris.

ABSTRACT

OBJECTIVE: To prepare the total alkaloids of Strychni Semen(TASS)-total glucosides of Paeony(TGP) gel and study its in vitro transdermal absorption of two alkaloids(strychnine and brucine) after the combination of TASS and TGP. METHODS: The excised abdominal skin of mice was used as the permeation model. Utilizing the modified Franz diffusion cell, the suitable receiving solution was elected to test the content of two alkaloids by HPLC, and thus the percutaneous rates and permeability coefficients were obtained. RESULTS: 20% Ethanol-normal saline was taken as receiving solution. With combination use of TASS and TGP, the penetration quantities of strychnine and brucine in different (1:1, 1:3, 1:6) gels were felled by 22.7%, 48.4%, 69.1% and 5.93%, 23.8%, 80.7% after 24 h. And with the increase of compatibility proportion, infiltration rate and skin retention rate also gradually reduced. CONCLUSION: The compatibility of TASS and TGP drug delivery can reduce the toxic ingredients through capacity, there is a"attenuated"effect, the best ratio is 1:6.

ABSTRACT

Objective To investigate the protective effects and mechanism of total glucosides of paeony (TGP) on the injury of neonatal rat cadiocytes induced by H2O2. Methods Cadiocytes of 3 days neonate rat was cultivated for 72 hours and divided into the model group, the TGP low (30μg/ml), medium (60 μg/ml), high (120 μg/ml) dose groups, Shuxuening injection (100 μg/ml) group and normal control group (n=8);and except the normal control group, the cells in other groups were induced by H2O2 (100μg/ml). After 6 hours, the morphology changes were observed and the survival rates were detected;the content of AST, CPK, LDH in culture medium were detected; the activity of SOD, CAT, GSH-Px and the content of MDA in cardiomyocytes were determinted;the apoptosis rates were detected. Results Compared with the model group, the survival rates of cardiomyocytes in TGP medium and high dose groups significantly improved (64.1%± 7.3%, 81.3%± 7.8%vs. 42.3%± 6.2%;P<0.05 or P<0.01);the content of AST (26.72 ± 6.03 U/ml, 23.86 ± 5.38 U/ml vs. 34.75 ± 6.91 U/ml), CPK (1.96 ± 0.35 U/ml, 1.59 ± 0.32 U/ml vs. 2.54 ± 0.40 U/ml), LDH (811.55 ± 162.32 U/L, 683.48 ± 134.14 U/L vs. 936.07 ± 140.94 U/L) in culture medium were significantly decreased (P<0.05 or P<0.01);the activity of SOD (85.34 ± 16.87 U/mg , 94.62 ± 17.75 U/mg vs. 56.45 ± 13.76 U/mg), CAT (22.76 ± 3.38 U/mg, 26.49 ± 3.72 U/mg vs. 16.13 ± 3.18 U/mg) in cardiomyocytes were significantly increased and the content of MDA (8.74 ± 2.05 nmol/mg, 6.91 ± 1.80 nmol/mg vs. 11.56 ± 2.19 nmol/mg) were significantly decreased (P<0.05 or P<0.01), the apoptosis rate (24.2% ± 5.5%, 13.4% ± 3.9% vs. 51.2% ± 9.1%) were significantly decreased (P<0.05 or P<0.01);and the activity of GSH-Px (3.54 ± 0.83 U/mg vs. 2.50 ± 0.67 U/mg) in cardiomyocytes of TGP high dose treatment group was significantly increased (P<0.05 or P<0.01). Conclusion TGP had dose-dependent protective effects on the injury of neonatal rat cadiocytes induced by H2O2, which perhaps related to its effects of improving the activity of antioxidase, inhibiting the oxidative stress, and decreasing the apoptosis rates.

ABSTRACT

Objective To investigate the effect of total glucosides of paeony ( TGP ) on the expression of Toll-like receptor 9 (TLR9) in peripheral blood B lymphocytes of the patients with systemic lupus erythematosus (SLE). Methods Sixty SLE patients and thirty healthy volunteers were enrolled, peripheral blood mononuclear cells ( PBMC) were isolated from blood samples and divided into 4 groups, which were incubated with CpG-ODN(final concentration was 1 μmol·L-1), CpG-ODN+TGP ( TPG final concentration was 1í10-4 mol·L-1 ) ,TGP and RPMI medium ( as the blank control group) for 48 hours, respectively. FLA ( Flow cytometry analysis) was used to detect the expression of TLR9 on peripheral blood B lymphocytes after incubated. Results ①In the health people, TLR9 expression in the group of CpG-ODN was(9. 10±2. 12) %, which was higher than the blank control group(4. 96±2. 11) % (P<0. 01).②In the SLE patients, the TLR9 expression in the group of CpG-ODN was(14. 86±3. 42)% , which was significantly higher than the blank control group(9. 20±3. 43) %(P<0. 01). The TLR9 expression in the group of CpG-ODN+TGP was (11. 95±3. 63)%, which was lower than that in the group of CpG-ODN (P<0. 05). The TLR9 expression in the group of CpG-ODN with lightly active degree SLE patients was (10. 74±3. 17)%, which was higher than the blank control group(5. 19±2. 05) % (P<0. 01). For SLE from the moderately to vigorously active degree, the expression of TLR9 in the group of CpG-ODN(16. 51±1. 72) % was higher than the blank control group(10. 80±2. 37) %(P<0. 01), but that in the group of CpG-ODN+TGP (13. 59±2. 58) % was lower than the group of CpG-ODN (P<0. 01). Conclusion Our data indicate that TGP antagonize upregulation effect of CpG-ODN on the expression of TLR9 in peripheral blood B lymphocytes.

ObjectiveA method was proposed for the determination of the concent of total glucoside of peaony(TGP) fromPaeonia lactifloriaPall. by UV spectrophotometry.MethodsBased on the extract with 80% acidities alcohol, extracted by aether and n-butyl alcohol, TGP was obtained fromPaeonialactifloriaPall.. The determination was established with paeoniflorin which was the main component of TGP as a contrast by UV spectrophotometry at 230 nm.ResultsA good liner relationship between concentrationof paeoniflorin and its absorbance was obtained,and the liner range was between 0.040 2~0.201 1 mg/ml. The averagerecovery was 101.81%.ConclusionThis method is simple rapid and reliable. It could be used for the semi-quantitative determination of TGP of paeony in paeonialactifloria pall..

ABSTRACT

Objective To study effect and its possible mechanism of total glucosides of paeony ( TGP ) on adriamycin-induced nephropathy rats.Methods Thirty male SD rats were randomly divided into normal group ( CON) , adriamycin-induced nephropathy group ( ADRN) and TGP-treated ADRN group (TGP).The rat nephropathy model was established by adriamycin injection.At the end of the 8th week after treatment, ELISA was used to detect the level of Cr and BUN.The values of 24 urine protein was determined by urinary protein kit.Masson staining was used to observe the fibrosis.RT-PCR was used to detect the mRNA levels of TLR4/NF-κB/TGF-β1 .Immunohistochemical method was used to detect the content of TLR4/NF-κB/TGF-β1.ResuIts Compared with the CON group, the level of Cr, BUN, 24 urine protein of ADRN group rised(P<0.01), the degree of fibrosis of ADRN group were aggravated(P<0.01), and the expressions of TLR4/NF-κB/TGF-β1 of ADRN group increased significantly.However, compared with ADRN group, the(P<0.01) level of Cr, BUN, 24 urine protein of TGP-treated rats reduced(P<0.01), the degree of fibrosis of TGP-treated rats eased(P<0.05), and the expressions of TLR4/NF-κB/TGF-β1 of TGP-treated rats decreased significantly.ConcIusion TGP retards the process of fibrosis and reduces the pathological damage in adriamycin-induced rats by down-regulating the expression of TLR4/NF-κB/TGF-β1 signaling.

ABSTRACT

Objective To investigate the therapeutic efficacy and safety of using total glucosides of paeony (TGP)for treatment of patients with systemic lupus erythematosus(SLE)accompanied by low white blood cell(WBC) count. Methods According to the revised SLE Classification Standard of American College of Rheumatology in 1982,62 cases of SLE accompanied by low WBC consistent with the standard were randomly divided into experimental group(31 cases)and control group(31 cases). The conventional therapy plus prednisone of 0.5-1.0 mg?kg-1?d-1 was applied for treatment in both groups,and in the experimental group,additionally TGP(0.6 g orally each time,three times a day)was given. The therapeutic course in both groups was 12 weeks. The changes in following parameters of two groups were noted at baseline and after treatment:WBC,complement C3,erythrocyte sedimentation rate (ESR),C-reactive protein(CRP),liver and kidney function,etc;the SLE disease activity index(SLEDAI)score, the therapeutic effects and incidence of adverse reactions were evaluated in two groups. Results After 12 weeks of treatment,WBC and C3 were significantly increased,and ESR,CRP,SLEDAI were significantly decreased in the two groups,the improvement being more significant in the experimental group〔WBC(×109/L):5.38±1.32 vs. 4.16±1.39,complement C3(g/L):1.28±0.32 vs. 0.86±0.26,ESR(mm/1 h):22.36±5.26 vs. 28.85±8.56, CRP(g/L):23.25±8.26 vs. 28.32±8.52,SLEDAI(score):2.58±1.86 vs. 4.18±2.31,P0.05). Conclusion TGP is a safe and effective treatment for patients with SLE accompanied by low WBC,and it also can significantly reduce the use of glucocorticoid dosage and the incidence of infection in such patients.

ABSTRACT

Objective To analyze causative factors for and therapeutic effect of total glucosides of paeony (TGP) on palmoplantar pustulosis (PPP).Methods Ninety patients with PPP were recruited in this study.A questionnaire survey was carried out to collect clinical data from these patients,and a patch test to identify contact allergens for these patients.All the patients were randomly divided into two groups:observation group treated with TGP capsule 0.6 g thrice a day,and control group treated with tripterygium glycosides tablets 20 mg thrice a day.Both groups topically applied mometasone furoate cream once a day.The efficacy and side effects were evaluated 4 and 8 weeks after initiation of treatment.Results Of the 90 patients,the ratio of male to female was 1 ∶ 1.64,smokers amounted to 42.22%,and 23 (26.67%) reported a history of infection before the onset of PPP.As the patch test showed,potassium dichromate and nickel sulfate were the most common contact allergens in these patients.The response rate was significantly higher in the observation group than in the control group after 4 weeks of treatment (64.44% vs.40.00%,x2 =5.388,P < 0.05),but similar between the two groups after 8 weeks of treatment (88.89% vs.80.00%,x2 =1.353,P > 0.05).No significant difference was observed in the occurrence of adverse reactions between the two groups (17.78% vs.11.11%,x2 =0.809,P > 0.05).Conclusions Smoking may play a certain role in the development of PPP,and infection seems to be a primary inducing factor.TGP combined with mometasone furoate cream is effective for the treatment of PPP.

ABSTRACT

s:Aim To study the effects of total gluco-sides of paeony ( TGP) on the differentiation of co-cul-tured osteoclasts and the mechanisms of how TGP influ-ences the osteoclasts. Methods The synovial fibro-blasts and monocytes of peripheral blood in adjuvant-induced arthritic rats were separated and co-cultured to induce osteoclasts. The cells were treated with different TGP dosages (5, 50, 500, 5 000 mg·L-1 , and 50 g ·L-1 ) for 48 h. The proliferation, the TRAP activi-ty, and the bone resorption of osteoclasts were ob-served. The levels of IL-1,TNF-α,M-CSF and RANKL in the supernatants of osteoclasts were detected using ELISA. Meanwhile, the expression of ERK, JNK and p38 was detected by real time PCR. Results The ex-periments revealed that 50, 500, 5 000 mg·L-1 TGP inhibited the osteoclast growth, the TRAP activity, and the resorption pit area in a dose-dependent manner. TGP also inhibited the levels of IL-1 , TNF-α, M-CSF and RANKL in the supernatants and the expression of ERK, JNK and p38 in osteoclasts. The appropriate concentrations were 50 mg·L-1 to 5 000 mg·L-1 and had dose-dependent effects within this range. Conclu-sions TGP regulates the differentiation and activity of co-cultured osteoclasts. The effects of TGP are related to its inhibiting the cytokines secretion of synovial fi-broblasts and the activity of MAPK pathways.

Total glucosides of paeony (TGP)is used to treat rheumatoid arthritis(RA)as the first immunoregulatory drug in clinic.The efficacy of TGP in treating RA patients is definite, which is accompanied with mild adverse reaction and slow effects simultaneously.Paeoniflorin (Pae ),the main constituents of TGP,has a bioavailability of 3% ~4%,which may be served as the main reason for its slow effects.Therefore,in this paper,the pharmacokinetics of Pae as well as its influencing factors were re-viewed on basis of pulished papers.In addition,some problems about Pae were also discussed.

ABSTRACT

Objective To investigate the effect of total glucosides of paeony(TGP) on hepatic dysfunction caused by Methotrexate (MTX) and Leflunomide (LEF) in patients with moderate or severe rheumatoid arthritis (RA).Methods From July 2010 to February 2011,204 cases with definite-diagnosed rheumatoid arthritis were included in three hospitals in Anhui province.All these patients suffered from moderate or severe rheumatoid arthritis and they were divided randomly into two groups,the therapeutic groups were treated with TGP combined with MTX and LEF and the control group were treated without TGP.The incidence of hepatic dysfunction was observed.Statistical anylysis was carried out by using t test and x2 test.Results One hundred and ninty-four patients had completed observation for at least 12 weeks.In the therapeutic group,only 10 of 105 patients had hepatic dysfunction compared to 31 of 89 patients in the control group.The incidence of hepatic dysfunction of the therapeutic group was significantly lower than the control group.After 12 weeks treatment with TGP,the effective respohse rates of the therapeutic group was 81.9%,which was higher than 74.6% in the control group.The good response rate in the therapeutic group was 21.6%,which was better than 11.7% in the control group.Although the difference between the two groups was not statistically significant,we could observed the tendency of increasing in effectiveness and the good response rate in the therapeutic group when compared with the control group.Conclusion TGP can decrease the incidence of hepatic dysfunction during the treatment with MTX and LEF,and it may improve the therapeutic effecacy of patients with RA.
