ABSTRACT

This article aimed to discuss the protection of trans - nerolidol on vascular endothelial cells (ECs) injured by lipopolysac charides. ECs were divided into four groups: normal, model, low and high dose trans - nerolidol treatment groups. The cell survival rate and the contents of NO in the cell culture supernatant were determined. The protein expression and transcript level of pe roxisome proliferator - activated receptor - γ (PPARγ), endothelial nitric oxide synthase (eNOS), and inducible nitric oxide synthase (iNOS) were determined by western blotting and RT - PCR respectively. Compared with the normal group, cell livability, protein e xpression and mRNA transcript level of PPARγ and eNOS decreased, NO contents, protein expression and mRNA transcript tlevel of iNOS increased in model group significantly. Compared with model group, all the changes recovered in different degree in treatmen t groups. Hence, it was concluded that trans - nerolidol can alleviate the ECs injuryby the regulation of iNOS/eNOS through activating PPARγ in a dose - dependent manner


Este artículo tiene como objetivo discutir la protección del trans - nerolidol en las células endoteliales vasculares (CE) dañadas por lipopolisacáridos. Las CE se di vidieron en cuatro grupos: normal, modelo, grupos de tratamiento con trans - nerolidol de baja y alta dosis. Se determinó la tasa de supervivencia de las células y los contenidos de óxido nítrico (NO) en el sobrenadante del cultivo celular. La expresión de p roteínas y el nivel de transcripción del receptor activado por proliferadores de peroxisomas - γ (PPARγ), el óxido nítrico sint et asa endotelial (eNOS) y el óxido nítrico sint et asa inducible (iNOS) se determinaron mediante western blot y RT - PCR, respectivamen te. En comparación con el grupo normal, la viabilidad celular, la expresión de proteínas y el nivel de transcripción de PPARγ y eNOS disminuyeron, los contenidos de NO, la expresión de proteínas y el nivel de transcripción de iNOS aumentaron significativam ente en el grupo modelo. En comparación con el grupo modelo, todos los cambios se recuperaron en diferentes grados en los grupos de tratamiento. Por lo tanto, se concluyó que el trans - nerolidol puede aliviar el daño en las CE regulando iNOS/eNOS a través d e la activación de PPARγ de manera dependiente de la dosis.

ABSTRACT

Aim To investigate the role of metabolites of eicosapentaenoic acid (EPA) in promoting the transdifferentiation of pancreatic α cells to β cells. Methods Male C57BL/6J mice were injected intraperitoneally with 60 mg/kg streptozocin (STZ) for five consecutive days to establish a type 1 diabetes (T1DM) mouse model. Resumo Este artigo de opinião consiste em uma entrevista realizada com Leonardo Peçanha, um homem negro trans, ativista no campo dos direitos humanos e dos direitos das pessoas trans e pesquisador do campo da Saúde Coletiva. Na conversa que se segue Leonardo Peçanha, de forma interseccional, reflete sobre o lugar da beleza, modificações corporais e atividade física no processo de transição de gênero de sujeitos homens trans e transmasculinos, como também toca no tema das (im)possibilidades das pessoas trans no campo dos esportes de alto rendimento e nos conta sobre o concurso de beleza Mister Trans Brasil.


Abstract This opinion article consists of an interview with Leonardo Peçanha, a Black trans man, a human rights and trans people rights activist and a researcher in Collective Health. In this interview, he reflects intersectionally on the place of beauty, body changes, and physical activity in the gender transition process of trans and transmasculine men. He discusses the (im)possibilities of trans people in high-performance sports and tells us about the Mister Trans Brazil beauty pageant contest.

Resumo As pessoas trans estão expostas a diferentes situações de vulnerabilidade que se entrecruzam, causando impactos negativos no processo saúde-doença-cuidado. Para melhor compreender essa questão, buscamos conhecer as experiências de sete estudantes trans de uma universidade pública de Minas Gerais, a partir de entrevistas norteadas por roteiro semiestruturado. As falas foram analisadas pela perspectiva da análise de conteúdo, e as categorias finais foram tensionadas com estudos das ciências sociais e da saúde coletiva. De forma geral, as narrativas se diferenciaram daquelas que compõem a maioria das pesquisas sobre o tema no Brasil, sobretudo em relação à manutenção de vínculos familiares e ao acesso à universidade pública. Nesse sentido, formulamos a hipótese de que a interação desses elementos com outros marcadores de diferenças pode contribuir positivamente para o estabelecimento de alianças e estratégias, a fim de lidar com o regime de inteligibilidade das identidades de gênero e com as dificuldades de acesso à educação, ao trabalho e à saúde. Compreender essa dinâmica e suas limitações, em uma perspectiva em que o individual se enlaça ao coletivo, possibilitou evidenciar o funcionamento da (hétero)normatividade nos processos de segregação de pessoas trans, assim como refletir sobre ações políticas que possam transformar essa realidade social efetivamente.


Abstract Transgender people are exposed to different situations of vulnerability that intersect, having negative impacts on the health-illness-care process. To better understand this issue, we sought to know the experiences of seven transgender undergraduates at a public university in Minas Gerais, from semi-structured interviews. The narratives were analyzed on the perspective of content analysis, and the final categories juxtaposed with studies of social sciences and public health. In general, the narratives are different from those which account for most of the research on the theme in Brazil, especially regarding maintaining family bonds and the access to public universities. Hence, we formulated the hypothesis that the interaction between these elements and other markers of differences can positively contribute to establishing alliances and strategies to deal with the regime of gender identity intelligibility and the constraints of access to education, work, and health. Understanding these dynamics and their limitations, under the perspective in which the individual and the collective are intertwined, enabled us to highlight the role of (hetero)normativity in the segregation processes of transgender people, as well as reflecting on the political actions that might effectively transform this social reality.

Este estudo objetivou identificar, analisar e integrar resultados de estudos que examinaram o impacto da pandemia de COVID-19 na saúde mental de pessoas transexuais e travestis. Foi realizada uma revisão integrativa da literatura, em conformidade com as diretrizes PRISMA. Conduziu-se uma busca sistemática dos estudos primários publicados em três bases de dados: PubMed/MEDLINE, LILACS e PsycINFO. Foram incluídos oito estudos que atenderam aos critérios de inclusão. A análise temática do material gerou seis categorias temáticas. Os estudos analisados apontaram que a situação de vulnerabilidade social ao qual as pessoas trans encontram-se submetidas no cotidiano foi ampliada durante a pandemia. A necessidade do distanciamento físico resultou no aumento de sintomas de ansiedade e depressão, agravamento das condições econômicas, dificuldades para acessar procedimentos e medicamentos que compõem o processo de afirmação de gênero, além de exacerbar conflitos familiares e manifestações de transfobia. Há necessidade de implementar políticas públicas e uma rede de proteção social para diminuir a vulnerabilidade e sofrimento psicossocial de pessoas trans. (AU)


This study aimed to identify, analyze, and integrate findings from studies that examined the impact of the COVID-19 pandemic on the mental health of transgender and transvestite individuals. An integrative literature review was conducted according to PRISMA guidelines. A systematic search of primary studies was conducted across three databases: PubMed/MEDLINE, LILACS, and PsycINFO. Eight studies that met the inclusion criteria were included. Thematic analysis of the material generated six thematic categories. The studies analyzed pointed out that the situation of social vulnerability faced by trans people in their daily lives intensified during the pandemic. The need for physical distancing led to increased symptoms of anxiety and depression, worsening economic conditions, and difficulties in accessing procedures and medications that make up the gender affirmation process, in addition to exacerbating family conflicts and manifestations of transphobia. Therefore, there is a need to implement public policies and a social protection network to reduce the vulnerability and psychosocial suffering of the transgender and transvestite community. (AU)


El objetivo de este estudio es identificar, analizar e integrar los resultados de los estudios que examinaban el impacto de la pandemia de COVID-19 en la salud mental de las personas transexuales y travestis. ABSTRACT

Abstract Existing meta-analyses on the effect of online psychological interventions (OPIs) have found small to medium effect sizes for the treatment of anxiety and depression. On the other hand, third-generation trans-diagnostic OPIs are very rare and, due to the large variability among disorders, symptoms or target populations, it is difficult to assess their overall effect. Other systematic reviews and meta-analyses have overly broad inclusion criteria that make the understanding of the findings more difficult. The current study aims to analyze the empirical evidence for third-wave trans-diagnostic OPIs designed to decrease symptoms and promote psychological flexibility, including studies that compare a OPI to some control condition (e. g., waiting list, treatment as usual or other that should not have any effect) and include a general symptomatology scale as dependent variable. A search without filters or timeframe was performed on Scopus and 1 408 articles were found, among which 21 were reviewed in depth and 6 were included for meta-analysis. Risk of bias was assessed by a quality and heterogeneity assessment. Separate meta-analyses were performed for general distress and psychological flexibility at post-treatment and last follow-up. Risk of bias analysis suggest low risk of threats to validity and attribute heterogeneity to between-study attrition rates. Additionally, meta-regression models for duration, attrition rate, and mean age are proposed for each time point. The results show significantly large effect sizes for both variables at both time points. According to the meta-regression models attrition rates are a mediating variable for the effect on general distress both at completion and at the last follow-up. On the other hand, duration, age and attrition rate are all mediating variables for the effect on psychological flexibility at the end of treatment. The findings suggest that the high attrition rates observed on tele-psychology need to be mitigated; if this is not possible, intention-to-treat approaches should be adopted for data analysis.


Resumen Los metaanálisis existentes sobre el efecto de las intervenciones psicológicas en línea (OPIs, por sus siglas en inglés) han encontrado tamaños de efecto pequeños o medianos para el tratamiento de la ansiedad y la depresión. Por otra parte, las OPIs transdiagnósticas de tercera generación son muy escasas y, debido a la gran variabilidad entre los trastornos, los síntomas o las poblaciones objetivo, es difícil evaluar su efecto global. Otras revisiones sistemáticas y metaanálisis tienen criterios de inclusión demasiado amplios que dificultan la comprensión de los hallazgos. El presente estudio tiene como objetivo analizar la evidencia empírica actual para las OPIs transdiagnósticas de tercera generación diseñadas para disminuir los síntomas y promover la flexibilidad psicológica, incluyendo estudios que comparen una OPI con alguna condición de control (por ejemplo lista de espera, tratamiento habitual u otro que no debería tener efecto) y que incluyan una escala de sintomatología general como variable dependiente. Se excluyeron estudios basados en otras terapias y/o diseñados para prevenir o tratar una población, un trastorno o un conjunto de síntomas específicos. También se excluyeron los protocolos de estudio, los diseños pretest-postest y otros en los que era imposible calcular el tamaño del efecto. Se realizó una búsqueda sin filtros ni marco temporal en Scopus y se encontraron 1 408 artículos entre los cuales 21 fueron revisados en profundidad y 6 fueron incluidos en el presente estudio. El riesgo de sesgo se evaluó mediante una evaluación de calidad y heterogeneidad; no fue posible realizar análisis de sesgo de publicación. Se realizaron metaanálisis separados para el malestar general y la flexibilidad psicológica en postratamiento y último seguimiento. El análisis del riesgo de sesgo sugiere un bajo riesgo de amenazas a la validez y atribuye la heterogeneidad principalmente a las tasas de deserción entre los estudios. Además, se proponen modelos de metarregresión para la duración, la tasa de deserción y la edad promedio en cada punto temporal. Los resultados muestran tamaños de efecto significativamente grandes para ambas variables en ambos puntos temporales y se evalúa su heterogeneidad, que se atribuye en gran medida a las tasas de deserción de los estudios incluidos. Según los modelos de metarregresión, las tasas de deserción son una variable mediadora del efecto sobre el malestar general tanto en el momento de la finalización como en el último seguimiento. Por otra parte, la duración, la edad y la tasa de abandono son variables mediadoras del efecto sobre la flexibilidad psicológica al final del tratamiento. Los resultados sugieren que es necesario mitigar las altas tasas de deserción observadas en la telepsicología y, cuando no sea posible, adoptar enfoques de intención de tratar para el análisis de los datos.

ABSTRACT Introduction: Acute promyelocytic leukemia currently presents an excellent chance of cure with protocols based on all-trans-retinoic acid (ATRA) and anthracycline or only differentiation agents. However, high early mortality rates continue to be reported Methods: Between 2000 and 2018, patients were enrolled and retrospectively analyzed by medical records. A modified AIDA protocol, with a 1-year shortening of the treatment duration, reduction in the number of drugs and a strategy to reduce early mortality by the postponement of the initiation of anthracyclines were employed. Overall and event-free survival rates and toxicity were analyzed Results: Thirty-two patients were enrolled, of whom 56% were female, with a median age of 12 years and 34% belonged to the high-risk group. Two patients had the hypogranular variant and three had another cytogenetic alteration, in addition to the t(15;17). The median start of the first anthracycline dose was 7 days. There were two early deaths (6%) due to central nervous system (CNS) bleeding. All patients achieved molecular remission after the consolidation phase. Two children relapsed and were rescued by arsenic trioxide and hematopoietic stem cell transplantation. The presence of disseminated intravascular coagulation (DIC) at diagnosis (p = 0.03) was the only factor with survival impact. The five-year event-free survival (EFS) was 84% and 5-year overall survival (OS) was 90% Conclusion: The survival results were comparable to those found in the AIDA protocol, with a low rate of early mortality in relation to the Brazilian reality.

Abstract Background: Metabolic disorders such as obesity and type 2 diabetes (T2D) coincide with an increased expression of pro-inflammatory factors. The NLRP3 inflammasome is a complex that activates the pro-inflammatory cytokine IL-1β. (NOD-like receptor protein 3). Some nutrients, such as fatty acids, influence inflammatory processes. For example, in clinical studies, higher trans-palmitoyl acid (TP) concentrations coincide with lower adiposity and lower risk of developing T2D. This study aims to evaluate the effect of TP on NLRP3 expression in a rodent model of diet-induced obesity (DIO). Methods: C57BL/6J mice were fed ad libitum with a control or a high-fat diet (HFD), added with or without TP (3 g/kg diet), for 11 weeks. IL-1β was quantified in serum, and NLRP3-related gene expression was explored in epididymal adipose tissue. Results: Despite increased weight gain in both high-fat groups, the high-fat TP group gained less weight than the high-fat group. In addition, NLRP3 and caspase-1 expression was higher in the HFD groups, but no differences were observed between the HFD and the HFD TP groups. Serum IL-1β levels were not different among groups. Conclusions: Diet supplementation with TP prevents weight gain and has a neutral influence over NLRP3 expression and IL-1β concentration in a DIO mice model.


Resumen Introducción: Las alteraciones metabólicas como la obesidad y diabetes tipo 2 (DT2) coinciden con la expresión aumentada de factores proinflamatorios. Un complejo que induce la activación de la citocina proinflamatoria IL-1β es el inflamasoma NLRP3 (NOD-like receptor protein 3). Algunos nutrimentos, como los ácidos grasos, influencian los procesos inflamatorios. Por ejemplo, en estudios clínicos, mayores concentraciones del ácido trans-palmitoléico (TP) coinciden con una menor adiposidad y un menor riesgo de desarrollar DT2. El objetivo de este estudio fue evaluar el efecto del TP sobre la expresión del inflamasoma NLRP3 en un modelo de obesidad inducida por dieta (OID) en roedores. Métodos: Se alimentaron ratones C57BL/6J ad libitum con una dieta control o alta en lípidos (AL), adicionada o no con TP (3 g/kg dieta), durante 11 semanas. Se cuantificó la concentración de IL-1β en elsuero de los animales, y en el tejido adiposo epididimal se midió la expresión de los componentes del inflamasoma. Resultados: A pesar del aumento de peso en ambos grupos de dieta con alto contenido en lípidos, el grupo alto en lípidos TP ganó menos peso que el grupo AL. Por otro lado, la expresión de genes del inflamasoma resultó mayor en los grupos AL, pero no se encontraron diferencias entre los grupos AL y AL TP. Además, no se observaron diferencias en la concentración de IL-1β en suero entre grupos. Conclusiones: La dieta suplementada con TP previno el aumento del peso corporal, pero no modificó la expresión de los componentes del inflamasoma ni la concentración de IL-1β en suero.

Abstract Objective This study aimed to investigate the molecular mechanism of miR-150-5p regulating the malignant biological behavior of Human Epidermoid cancer cell (HEp-2) by targeting peptidyl-prolyl cis/trans isomerase NIMA-Interacting-1 (PIN1). Methods Firstly, qRT-PCR and Western blot were adopted to detect the expression levels of miR-150-5p and PIN1 in cancer tissue and paracancerous tissues of patients with LSCC, and those in human bronchial epithelial cells (16 HBE) and HEp-2. Next, the targeted relationship between miR-150-5p and PIN1 was assessed by bioinformatics website and dual-luciferase reporter assay, followed by their correlation analysis. Besides, after interfering with miR-150-5p or PIN1 expression in HEp-2 cells, CCK-8, cell colony formation assay, and transwell assay were utilized to detect the proliferation, viability, and invasion of cells, respectively. Subsequently, the protein levels of MMP-2, MMP-9, and EMT-related proteins in HEp-2 cells were checked by Western blot. Results Expression of miR-150-5p was down-regulated in LSCC tissues and HEp-2 cells. Moreover, miR-150-5p suppressed proliferation and invasion of HEp-2 cells, affected protein expression related to MMP and EMT, thereby inhibiting development of cancer. The expression of PIN1 was significantly increased in cancer tissues and HEp-2 cells, and there was a targeted relationship and negative correlation between miR-150-5p and PIN1 in cancer tissue. However, overexpression of PIN1 could reverse the effect of miR-150-5p on the proliferation and invasion of HEp-2 cells. Conclusion In a nutshell, there is a targeted relationship between PIN1 and miR-150-5p. Besides, miR-150-5p can inhibit the proliferation and invasion of HEp-2 cells by regulating the expression of PIN1. Level of evidence 3.

ABSTRACT

Introducción: la hernia inguinal es uno de los principales motivos de consulta quirúrgica y su reparación es uno de los procedimientos más comunes en cirugía. Objetivo: determinar la experiencia en el abordaje laparoscópico de las hernias inguinales por técnica transabdominal preperitoneal en el Servicio de Cirugía General del Centro Médico Nacional-Hospital Nacional. Metodología: estudio observacional descriptivo retrospectivo de corte temporal transversal. En pacientes de 16 a 90 años de edad con diagnóstico de hernia inguinal internados en el Servicio de Cirugía General del Centro Médico Nacional-Hospital Nacional para hernioplastia electiva. Resultados: se llevaron a cabo 30 hernioplastias por técnica técnica trans-abdominal pre-peritoneal de los cuales el 73 % fue realizado en hombres y 27 % en mujeres; se identificó una media de edad de 48,4 años, el grupo etario con mayor frecuencia fue de 38 a 48 años. En el examen físico pre quirúrgico se encontraron hernias inguinales unilaterales en el 76.6 % y bilaterales en el 23.3 %; en la mayor parte de los pacientes las hernias fueron primarias en el 86.6 %y recidivada en el 13.3 %. El tiempo quirúrgico en promedio fue de 93.1 minutos; con un tiempo máximo de 120 minutos y mínimo de 60 minutos. El tiempo de hospitalización en el 100 % de los pacientes fue de 48 h. De las complicaciones post operatorias se establece que el 76.6 % no presento ningún tipo de complicación; el 20 % presentó seroma como complicación principal y 3.3 % infección del sitio quirúrgico. Conclusión: debido a su alta frecuencia y a su impacto en la incapacidad laboral y social, las hernias inguinales representan una de las patologías quirúrgicas más importantes con bajas tasas de complicaciones post operatorias y corta estancia hospitalaria.


Introduction: inguinal hernia is one of the main reasons TAPP, e-TEP (Totally extraperitoneal with extended vision) for surgical consultation and its repair is one of the most common surgical procedures. Objective: to determine the experience in the laparoscopic approach of inguinal hernias by preperitoneal transabdominal technique in the Servicio de Cirugía General of the Centro Médico Nacional-Hospital Nacional. Methodology: retrospective descriptive observational study of cross-sectional time. In patients from 16 to 90 years of age with a diagnosis of inguinal hernia admitted to the Servicio de Cirugía General of the Centro Médico Nacional-Hospital Nacional for elective hernioplasty. Results: 30 hernioplasties were carried out by the TAPP technique, of which 73 % were performed in men and 27 % in women; a mean age of 48.4 years was identified, the age group most frequently being 38 to 48 years. In the pre-surgical physical examination, unilateral inguinal hernias were found in 76.6 % and bilateral in 23.3 %; in most of the patients the hernias were primary in 86.6 % and recurred in 13.3 %. Average surgical time was 93.1 minutes; with a maximum time of 120 minutes and a minimum of 60 minutes. The hospitalization time in 100 % of the patients was 48 hours. Of the post-operative complications, it is established that 76.6% did not present any type of complication; 20 % presented seroma as the main complication and 3.3 % surgical site infection. Conclusion: due to its high frequency and its impact on work and social disability, inguinal hernias represent one of the most important surgical pathologies with low rates of postoperative complications and short hospital stay.

RESUMEN INTRODUCCIÓN El acceso a la salud para las personas trans ha sido históricamente obstaculizado por el estigma y la discriminación institucional. A pesar de los avances legales y sociales en materia de reconocimiento de derechos de los últimos años, todavía se identifican barreras para su efectivo cumplimiento. Este trabajo tuvo como objetivo describir las principales barreras en la accesibilidad a los consultorios inclusivos (CI) y analizar las necesidades específicas de la población trans que se atiende en la provincia de Buenos Aires (PBA). MÉTODOS Se presenta un subanálisis de un estudio cualitativo más amplio realizado en 2017, cuyo objetivo fue relevar información sobre demandas sanitarias, accesibilidad y calidad de atención de los servicios de salud específicos para población de lesbianas, gays, bisexuales y trans (LGBT) en ocho efectores de la PBA. La muestra fue intencional, conformada por 29 entrevistas semiestructuradas a referentes y profesionales y 2 grupos focales con masculinidades y feminidades trans, respectivamente. Se realizó un análisis temático. RESULTADOS Se identificaron barreras organizacionales y simbólicas, sobre todo en los CI que funcionan en hospitales. Entre las simbólicas, se evidencia la persistencia de prácticas patologizantes en salud mental. DISCUSIÓN Los CI brindan una respuesta satisfactoria, pero transitoria. Es necesario transversalizar la perspectiva de género a todas las intervenciones en salud.


ABSTRACT INTRODUCTION Access to health for trans people has historically been hampered by institutional stigma and discrimination. Despite the legal and social advances achieved in recent years in terms of recognition of rights, barriers to effective enforcement are still identified. The objective of this work was to describe the main barriers to access inclusive healthcare services and analyze the specific needs of trans people receiving healthcare in the province of Buenos Aires (PBA). METHODS A subanalysis of a wider qualitative study conducted in 2017 is presented, its objective was to collect information on healthcare needs, accessibility and quality of care of health services specific for lesbian, gay, bisexual and transgender (LGBT) population in eight healthcare providers of PBA. The sample was intentional, including 29 semi-structured interviews to social organization leaders and professionals, and 2 focus groups with trans masculinities and femininities, respectively. Data were analyzed using thematic analysis. RESULTS Organizational and symbolic barriers were identified, especially in inclusive healthcare services functioning in hospitals. One of the symbolic ones was the persistence of pathologizing practices in mental healthcare. DISCUSSION Inclusive healthcare services provide a satisfactory, but temporary response. It is necessary to adopt a cross-cutting gender perspective for all healthcare interventions.

durante la adolescencia se adquieren conocimientos y se desarrollan actitudes hacia el sexo, la anticoncepción y la reproducción, aspectos que influirán en el comportamiento sexual que repercute directamente en la calidad de vida. Por tanto, el objetivo de este estudio fue determinar los factores sociales y emocionales asociados al inicio de la actividad sexual, así como sus consecuencias como el embarazo y las enfermedades de transmisión sexual en adolescentes y adultos jóvenes. Materiales y métodos: estudio descriptivo, transversal y correlacional de una muestra de 1191 estudiantes de secundaria que recibieron cuestionarios sobre factores socioemocionales e iniciación sexual. Los datos se procesaron mediante estadística descriptiva y la prueba chi-cuadrado de Pearson para determinar la asociación entre los factores evaluados. Resultados: la tasa de inicio de la actividad sexual entre los adolescentes fue del 45 %, de los cuales el 14 % utilizaba protección. El 88,8 % de los adolescentes dice recibir apoyo de los padres, el 39 % acepta tratos violentos en el hogar y el 47 % tuvo una madre adolescente embarazada, entre otros aspectos. Conclusiones: existe una relación significativa (con p < 0.05) entre los factores socioemocionales de género, proyectos de vida, actividades extracurriculares, apoyo de los padres, violencia intrafamiliar y maltrato, con respecto a las madres con antecedente de embarazo precoz e inicio de la actividad sexual en la adolescencia.


During adolescence, knowledge is acquired and attitudes toward sex, contraception, and reproduction are developed, aspects that will influence sexual behavior, which has a direct impact on quality of life. Therefore, this study was designed to determine the social and emotional factors associated with the onset of sexual activity and its consequences, such as pregnancy and sexually transmitted dis-eases, among adolescents and young adults. Materials and Methods: A descriptive, cross-sectional, and cor-relational study involving 1,191 high school students was conducted. A questionnaire containing questions on socio-emotional factors and sexual initiation was administered. Data were processed using descriptive statistics and Pearson's chi-square test to determine the association between the evaluated factors. Results:The initiation rate of sexual activity among adolescents was 45%, of whom 14% used protection. However, 88.8% of the adolescents received parental support, 39% accepted violent treatment at home, and 47% had a pregnant adolescent mother, among other aspects. Conclusions: There is a significant relationship between the socio-emotional factors of sex, life projects, extracurricular activities, parental support, intra-family violence, and mistreatment. Having a mother with a history of early pregnancy is associated with the onset of sexual activity among adolescents.

ABSTRACT

Numerous studies have highlighted the harmful effect of trans fatty acid (TFAs) on human health. Based on a review of health hazards of TFAs, intake levels among residents in China and other countries, and current regulatory policies, this paper discusses the proposals on strengthening TFAs restriction policies in China. According to the literature review, dietary intake level of TFAs among residents in China was relatively low across the world, which remained a certain distance from the World Health Organization (WHO) recommended limit of 1% of total energy intake. However, consumption of edible oils, baked food and dairy products by Chinese residents is likely to continue to rise in the future, which might lead to an increase in TFAs intake. Therefore, it is suggested that China should strengthen the investigation and assessment of TFAs intake, adopt the WHO best policy of restricting TFAs (a national limit of 2 grams of industrially produced trans fat per 100 grams of total fat in all foods, or a national ban on the production and use of partially hydrogenated oils), and reduce the use of edible oil.

ABSTRACT

Objective:To explore the causal associations of two blood pressure phenotype and four lipid fractions with type 2 diabetes mellitus(T2DM) in European and East Asian populations using Two-Sample Mendelian randomization analysis.Methods:Blood pressure phenotype, lipid fractions and T2DM genetic loci from two ethnics were matched and combined according to single nucleotide polymorphisms(SNPs) numbering. With SNPs closely related to the exposure phenotype as instrumental variables, the inverse variance weighting method was used to analyze the causal effects of blood pressure phenotype and lipid fractions on T2DM in different ethnic groups. The sensitivity analysis was conducted using MR-Egger regression model, Weighted Median method, MR-PRESSO, MR-robust Adjusted Profile Score, and leave-one-out method.Results:Among European populations, systolic blood pressure( OR=1.40, 95% CI 1.23-1.59, P<0.001) and diastolic blood pressure( OR =1.24, 95% CI 1.08-1.42, P=0.002)were associated with increased risk of T2DM while high density lipoprotein-cholesterol( OR=0.68, 95% CI 0.62-0.76, P<0.001) reduced the risk of T2DM. In East Asian ethnicity, elevated diastolic blood pressure( OR=0.75, 95% CI 0.59-0.95, P=0.007) reduced the risk of T2DM. Sensitivity analysis confirmed the results. Conclusion:There are differences in the effects of blood pressure phenotype and lipid composition on T2DM in different ethnic groups, which may be related to population heterogeneity and exposure sensitivity. It should be taken into consideration in extrapolation.

ABSTRACT

Objective:To observe the effect of the antioxidant N-acetylcysteine (NAC) and selective endoplasmic reticulum stress response inhibitor salubrinal on the apoptosis of retinal pigment epithelial cells induced by all-trans-retinoic acid (ATRA).Methods:Human ARPE-19 cell line was used as the experimental cell line, and was divided into normal control group cultured with complete medium, model control group cultured with complete medium containing 10 μmol/L ATRA, NAC treatment group cultured with complete medium containing 10 μmol/L ATRA+ 5 mmol/L NAC, salubrinal group cultured with complete medium containing 10 μmol/L ATRA+ 40 μmol/L salubrinal, NAC+ salubrinal group cultured with complete medium containing 10 μmol/L ATRA+ 5 mmol/L NAC+ 40 μmol/L salubrinal.After 24-hour culture, apoptosis rate, multicaspase level and reactive oxygen species (ROS) level of ARPE-19 cells were detected by flow cytometry.The expressions of vascular endothelial growth factor A (VEGF-A), C/EBP-homologous protein (CHOP), cleaved-caspase 3 in cells were detected by Western blot.Results:There were significant differences in the apoptosis rate, multicaspase and ROS levels among the five groups ( F=113.23, 602.41, 160.39; all at P<0.001). The apoptosis rate, multicaspase and ROS levels of normal control group, NAC treatment group, salubrinal group and NAC+ salubrinal group were significantly lower than those of model control group (all at P<0.05). There were significant differences in the expression levels of VEGF-A, CHOP and cleaved-caspase 3 among the five groups ( F=24.62, 36.35, 60.25; all at P<0.001). The protein expression levels of VEGF-A, CHOP and cleaved-caspase 3 of normal control group, NAC treatment group, salubrinal group and NAC+ salubrinal group were significantly lower than those of model control group (all at P<0.05). Conclusions:ATRA can induce RPE cells to produce oxidative stress and endoplasmic reticulum stress injury, which leads to apoptosis.NAC and salubrinal can effectively reduce the RPE cell apoptosis by inhibiting stress response.

ABSTRACT

Objective:To investigate the influencing factors of anastomotic leakage after laparoscopic intersphincter resection (ISR) for extremely low rectal cancer and construction of nomogram prediction model.Methods:The retrospective case-control study was conducted. The clinicopathological data of 812 patients who underwent laparoscopic ISR for extremely low rectal cancer in the Second Affiliated Hospital of Naval Medical University (Shanghai Changzheng Hospital) from February 2012 to February 2022 were collected. There were 459 males and 353 females, aged (51±11)years. Observation indicators: (1) surgical situations; (2) follow-up; (3) influencing factors of postoperative anastomotic leakage; (4) construction and evaluation of nomogram prediction model for postoperative anastomotic leakage. Measurement data with normal distribution were represented as Mean± SD, and measurement data with skewed distribution were represented as M(range). Count data were described as absolute numbers. The COX proportional hazard model was used for univariate and multivariate analyses. The R software(3.5.1 version) was used to construct nomogram prediction model. The receiver operating characteristic (ROC) curve was drawn and the area under curve (AUC) was used to evaluate the efficacy of the nomogram prediction model. The Bootstrap method was used for internal verification and to calculate the average consistency index (C-index). Results:(1) Surgical situations. All 812 patients underwent laparoscopic ISR for extremely low rectal cancer, including 388 cases undergoing partial ISR, 218 cases undergoing subtotal ISR and 206 cases undergoing complete ISR. All 812 patients underwent ileal protective ostomy, and there were 306 cases with double anastomosis and 203 cases with left colic artery preserved, respectively. The operation time and volume of intraoperative blood loss of 812 patients was (179±33)minutes and (33±13)mL, respectively. (2) Follow-up. All 812 patients were followed up for (13.5±0.9)months. Of the 812 patients, there were 62 cases with postoperative anastomotic leakage and the healing time of these cases was (33±6)days. (3) Influencing factors of postoperative anastomotic leakage. Results of multivariate analysis showed that male, neoadjuvant chemoradiotherapy, failure of reser-ving left colic artery were independent risk factors of anastomotic leakage after laparoscopic ISR for extremely low rectal cancer ( hazard ratio=5.98, 4.00, 16.26, 95% confidence interval as 1.66-24.12, 1.30-12.42, 3.00-90.89, P<0.05). (4) Construction and evaluation of nomogram prediction model for postoperative anastomotic leakage. According to the results of multivariate analysis, male, neoadju-vant chemoradiotherapy and failure of reserving left colic artery were used to construct the nomogram prediction model for anastomotic leakage after laparoscopic ISR for extremely low rectal cancer, and the score of these indexes in the nomogram prediction model was 50, 49, 93, respectively. The total score of these index corresponded to the incidence rate of anastomotic leakage. Results of ROC curve showed that the AUC of nomogram prediction model of anastomotic leakage after laparoscopic ISR for extremely low rectal cancer was 0.87 (95% confidence interval as 0.80-0.93, P<0.05), with sensi-tivity and specificity 0.96 and 0.60, respectively. Results of internal verification showed that the C-index of nomogram prediction model was 0.87. Conclusion:Male, neoadjuvant chemoradiotherapy, failure of reserving left colic artery are independent risk factors of anastomotic leakage after laparo-scopic ISR for extremely low rectal cancer, and the nomogram prediction model based on these indexes can predict the incidence rate of postoperative anastomotic leakage.

ABSTRACT

Objective:To construct an intervention program of sports and medical integration for patients with acute myocardial infarction (AMI) in phase Ⅱ cardiac rehabilitation, so as to provide reference for the effective implementation of physical activity behavior change in AMI patients during phase Ⅱ cardiac rehabilitation.Methods:Before the research, we browsed relevant literature and guidelines published from July 2016 to June 2021, made a retrospective study on the influencing factors of cardiac rehabilitation behavior for AMI patients, and carried out a qualitative interview on cognitive and compliance motivations for AMI patients. Furthermore based on trans-theoretical model, the first draft of the intervention program was developed and and the expert consultation questionnaire was formed. From February to April 2022, Delphi method was used in 16 experts from 10 hospitals and 1 nursing college in 4 provinces. After 2 rounds of expert consultations on the importance and operability of items, the intervention program was finally determined.Results:The effective recovery rates of the 2 rounds of expert consultations questionnaires were both 16/16. The authority coefficient of expert consultation was 0.90, the judgment basis coefficient was 0.96, and the familiarity degree was 0.84. After the second round of expert consultation, the coefficient of variation of the importance of each item was (0.0-13.4)%, and the coefficient of variation of operability was (0.0-18.1)%. The final intervention program of sports and medical integration for patients with AMI in phase Ⅱ cardiac rehabilitation had 37 items, containing precontemplation stage (8 items), contemplation stage (7 items), preparation stage (5 items), action stage (9 items) and maintenance stage (8 items).Conclusions:The construction process of the intervention program of sports and medical integration for patients with AMI in phase Ⅱ cardiac rehabilitation is scientific and feasible. The content is focused on the patient-centred conception and the whole-process management for the exercise rehabilitation of AMI patients in phase Ⅱ cardiac rehabilitation. This intervention program may improve the safety, feasibility, participation and compliance in phase Ⅱ cardiac rehabilitation in patients with AMI. So it is recommended to be popularized and used in phase Ⅱ cardiac rehabilitation.

Objective To evaluate the immunoprotective effect of active immunization with recombinant peptidyl-prolyl cis-trans isomerase from Babesia microti against B. microti infection in mice. Methods Female BALB/c mice at 6 weeks of age, each weighing approximately 20 g, were divided into the recombinant protein immunization group, the infection control group and the normal control group, of 25, 18, 15 mice in each group, respectively. Mice in the recombinant protein immunization group were given active immunization with recombinant BmPPIase protein, and 18 mice with the highest antibody titers were intraperitoneally injected with 100 μL of B. microti-infected whole blood 2 weeks after the last immunization. Mice in the infection control group were intraperitoneally injected with 100 μL of B. microti-infected whole blood, while 15 mice in the normal control group received no treatment. Blood samples were collected from mice in the recombinant protein immunization group and the infection control group on days 0 to 30 post-immunization for detection of B. microti infection, and blood samples were collected on days 0, 7, 14, 21, and 28 post-immunization for routine blood tests with a blood cell analyzer and for detection of serum cytokines using cytometric bead array. Results Anti-BmPPIase antibodies were detected in 25 mice in the recombinant protein immunization group 2 weeks after the last immunization, with titers of 5 × 103 to 8 × 104. B. microti infection rate peaked in mice in both the recombinant protein immunization and the infection control group on day 7 post-immunization, with positive infection rates of 13.3% and 50.0%, and there were significant differences between the two groups in terms of B. microti infection rate on days 3 (χ2= 113.18, P < 0.01), 5 (χ2 = 475.22, P < 0.01), 7 (χ2 = 465.98, P < 0.01) and 9 post-infection (χ2= 18.71, P < 0.01), while the B. microti infection rate tended to be 0 in both groups on day 11 post-immunization. Routine blood tests showed higher red blood cell counts [(5.30 ± 0.50) × 1012 to (9.87 ± 0.24) × 1012 counts/L)] and hemoglobin levels [(89.67 ± 22.80) to (148.60 ± 3.05) g/L)] in the recombinant protein immunization group than in the infection control group on days 0 to 28 post-immunization. Cytometric bead array detected higher serum interferon-γ [(748.59 ± 17.56) to (3 858.28 ± 1 049.10) fg/mL], tumor necrosis factor-α [(6 687.34 ± 1 016.64) to (12 708.13 ± 1 629.79) fg/mL], interleukin (IL)-6 [(611.05 ± 75.60) to (6 852.68 ± 1 554.00) fg/mL] and IL-17a [(167.68 ± 185.00) to (10 849.27 ± 355.40) fg/mL] and lower IL-10 levels [(247.65 ± 138.00) to (18 787.20 ± 2 830.22) fg/mL] in the recombinant protein immunization group than in the infection control group during the study period. Conclusions Recombinant BmPPIase protein induces up-regulation of interferon-γ, tumor necrosis factor-α and presents a high immunoprotective activity against B. microti infection in mice, which is a potential vaccine candidate protein.

ABSTRACT

The extremely low bioavailability of oral paclitaxel (PTX) mainly due to the complicated gastrointestinal environment, the obstruction of intestinal mucus layer and epithelium barrier. Thus, it is of great significance to construct a coordinative delivery system which can overcome multiple intestinal physicochemical obstacles simultaneously. In this work, a high-density PEGylation-based glycocholic acid-decorated micelles (PTX@GNPs) was constructed by a novel polymer, 9-Fluorenylmethoxycarbonyl-polyethylene glycocholic acid (Fmoc-PEG-GCA). The Fmoc motif in this polymer could encapsulate PTX via π‒π stacking to form the core of micelles, and the low molecular weight and non-long hydrophobic chain of Fmoc ensures the high-density of PEG. Based on this versatile and flexible carriers, PTX@GNPs possess mucus trapping escape ability due to the flexible PEG, and excellent intestine epithelium targeting attributed to the high affinity of GCA with apical sodium-dependent bile acid transporter. The in vitro and in vivo results showed that this oral micelle could enhance oral bioavailability of PTX, and exhibited similar antitumor efficacy to Taxol injection via intravenous route. In addition, oral PTX@GNPs administered with lower dosage within shorter interval could increase in vivo retention time of PTX, which supposed to remodel immune microenvironment and enhance oral chemotherapy efficacy by synergistic effect.
