ABSTRACT

Objective: To summarize the research progress on the application of tranexamic acid (TXA) in traumatic orthopedic surgery in recent years. Methods: The domestic and foreign literature in recent years was reviewed, and the efficacy and safety of TXA in traumatic orthopedic surgeries with different regimen, dose and route of administration were comprehensively summarized and compared. Results: The application of TXA in traumatic orthopedic surgeries increased gradually in recent years. Intravenous or topical administration of TXA efficaciously reduced blood loss and transfusion requirements during hip fracture surgery without significantly increasing the risk of thromboembolic events. However, the efficacy was not clear in other traumatic orthopedic surgeries such as pelvic and acetabular fractures. Conclusion: More studies are needed to confirm the efficacy and safety of TXA in traumatic orthopedic surgeries.

ABSTRACT

Objective:To explore the application value of minimally invasive surgery techniques in orthopedic trauma.Methods: From January 2012 and January 2015, 100 cases of orthopedic trauma patients were selected according to surgical methods, and were divided into traditional surgery group with 49 cases and minimally invasive surgery group with 51 cases, comparing two groups with incision, operation time, intraoperative blood loss, hospital stay, postoperative1 week incision pain score, the incidence of postoperative complications and treatment satisfaction.Results: The minimally invasive surgical incision length, operative time, intraoperative blood loss, pain score, incision healing time and hospital stay in the minimally invasive surgery group were better than that in the traditional surgery group (t=5.452,t=11.358,t=28.560, t=9.210,t=6.820;P<0.05). The incidence of postoperative concurrent with minimally invasive surgery group was 9.8%, traditional surgical complication rate was 18.4%, and the incidence of postoperative concurrent with minimally invasive surgery group was significantly lower than traditional surgical group (x2=12.325,P<0.05). Minimally invasive surgical patients’ satisfaction was 96.08%, traditional surgical operation patients’ satisfaction was 83.67%, the minimally invasive surgery group’s satisfaction was higher than traditional surgery group (H=16.465,P<0.05).Conclusion: Clinical curative effect of minimally invasive technique is better, and easy to be accepted by the patients. It is more popular in clinical application.

ABSTRACT

A preliminary exploration of the application and effects of digital orthopaedic technique combined with the clinical practice in our clinical teaching of orthopaedics and trauma for 8-year program medical students has been made. Construction of 3D-model of bone and joint with the digital reconstruction technology, combination of 3D-display with detailed clinical cases, individual studying of knowledge, and fracture models performed by three dimensional printing, all have enhanced the students' understanding of the basics knowledge of orthopaedic trauma and accelerated the process of its joining into clinical practice. Related software has been used to realize the virtual design of fracture reduction and fixation and based on it, students have been guided to personally involve in the development and realization of surgical simulation program on a personal computer, thus improving students' capabilities of analysis, diagnosis and treatment of illness. Besides, students have been actively guided to use digital technology to conduct the finite ele-ment analysis of posterior malleolar fractures and calcaneus fractures , and to produce APP software for mobile devices such as ankle fracture classification and function scoring , which has aroused students' in-terest of clinical research and innovation.
