ABSTRACT

Objective@#To understand the occurrence and predictive factors of depressive symptoms among multi ethnic middle school students in Yunnan Province, so as to provide a referential framework for schools to carry out targeted mental health education.@*Methods@#From October to December 2022, 8 500 first grade students from 23 middle schools were selected from 11 minority areas in Yunnan Province by cluster random sampling method. Demographic information and data relating to the students lifestyles were collected by questionnaire, and the Children s Depression Inventory (CDI) was used to evaluate depressive symptoms. Chi square test was performed to compare differences in the detection rate of depressive symptoms among first grade middle school students for univariate analysis. A decision tree model of depressive symptoms in middle school students was established by using the Chi squared automatic interaction detector (CHAID).@*Results@#The detection rate of depressive symptoms among first grade students from multi ethnic middle schools in Yunnan Province was 28.26%. The decision tree model of depressive symptoms was academic stress ( χ 2=469.08) at the first level, breakfast behaviors (low/moderate academic stress: χ 2=155.49; severe academic stress: χ 2=105.24) at the second level, and the number of close friends (low/moderate academic stress and consuming breakfast 0- 2 days weekly: χ 2=23.15; low/moderate academic stress and consuming breakfast 3-4 days weekly: χ 2=14.99; severe academic stress and consuming breakfast 0-2 days weekly: χ 2=29.26; severe academic stress and consuming breakfast 3-4 days weekly: χ 2=20.15), ethnicity ( χ 2=78.22) and drinking ( χ 2=50.36) at the third level ( P <0.01).@*Conclusions@#The study identifies academic stress, breakfast behaviors, number of close friends, drinking and ethnicity as predictive factors of depressive symptoms among multi ethnic middle school students in Yunnan Province. Schools should develop targeted strategies for preventing and managing depressive symptoms in middle school students, so as to reduce their occurrence.

Abstract Introduction Cardiovascular disease (CVD) is the lead-ing cause of death globally, with a high proportion of hospitalizations and costs. In view of this, it is essential to understand the main CVDs in patients admitted to hospital emergency services and the role of physiotherapists, in order to plan and direct health services, and to denote participation and encourage specific physiotherapy training in the context of tertiary care. Objective To outline the profile of cardiovascular emergencies and to evaluate physiotherapy in adult patients in the emergency department of a hospital in the interior of the state of São Paulo. Methods This was an observational study which analyzed 1,256 on-call records over a period of eight months. The data collected included age, gender, cardiovascular diagnostic hypothesis and physiotherapy treatment carried out. Results A total of 75 patients with cardiovascular emergencies were included, the most prevalent of which were: heart failure (n = 21), acute coronary syndrome (n = 14), acute myocardial infarction (n = 13), bradyarrhythmia (n = 6) and hypertensive crisis (n = 5). Regarding physiotherapeutic actions and their applications, the most frequent were invasive mechanical ventilation management (n = 34), lung re-expansion maneuvers (n = 17), orotracheal intubation assistance (n = 17), non-invasive mechanical ventilation (n = 14), bronchial hygiene maneuvers (n = 12), kinesiotherapy (n = 10) and sedation (n = 10). Conclusion Heart failure and acute coronary syndrome were the cardiovascular diseases that caused the most admissions to the hospital emergency department and that the procedures with an emphasis on the respiratory system were the most applied.


Resumo Introdução As doenças cardiovasculares (DCV) repre-sentam a principal causa de morte global, destacando-se em internações e gastos. Diante disso, é essencial compreender as principais DCV em pacientes admitidos em serviços de emergência hospitalar e a atuação do fisioterapeuta para planejamento e direcionamento dos serviços de saúde e para denotar a participação e incentivar formações fisioterapêuticas específicas no contexto da atenção terciária. Objetivo Traçar o perfil de emergências cardiovasculares e avaliar a atuação fisioterapêutica em pacientes adultos de serviço de emergência de um hospital no interior do estado de São Paulo. Métodos Trata-se de um estudo observacional, em que foram analisadas 1.256 fichas de passagem de plantão, no período de oito meses. Os dados coletados foram idade, sexo, hipótese diagnóstica cardiovascular e tratamento fisioterapêutico realizado. Resultados Foram incluídos 75 pacientes que apresentavam o perfil de emergências cardiovasculares, sendo as mais prevalentes: insuficiência cardíaca (n = 21), síndrome corona-riana aguda (n = 14), infarto agudo do miocárdio (n = 13), bradarritmia (n = 6) e crise hipertensiva (n = 5). Em relação à atuação fisioterapêutica e suas aplicações, as mais frequentes foram manejo da ventilação mecânica invasiva (n = 34), manobras de reexpansão pulmonar (n = 17), auxílio a intubação orotraqueal (n = 17), ventila-ção mecânica não invasiva (n = 14), manobras de higiene brônquica (n = 12), cinesioterapia (n = 10) e sedestação (n = 10). Conclusão A insuficiência cardíaca e a síndrome coronária aguda foram as doenças cardiovasculares que mais ocasionaram internação no serviço de emergência hospitalar e as condutas com ênfase no aparelho respiratório foram as mais aplicadas.

Introducción: La gran diversidad de especies maderables tropicales demanda el desarrollo de nuevas tecnologías de identificación con base en sus patrones o características anatómicas. La aplicación de redes neuronales convolucionales (CNN) para el reconocimiento de especies maderables tropicales se ha incrementado en los últimos años por sus resultados prometedores. Objetivo: Evaluamos la calidad de las imágenes macroscópicas con tres herramientas de corte para mejorar la visualización y distinción de las características anatómicas en el entrenamiento del modelo CNN. Métodos: Recolectamos las muestras entre el 2020 y 2021 en áreas de explotación forestal y aserraderos de Selva Central, Perú. Luego, las dimensionamos y, previo a la identificación botánica y anatómica, las cortamos en secciones transversales. Generamos una base de datos de imágenes macroscópicas de la sección transversal de la madera, a través del corte, con tres herramientas para ver su rendimiento en el laboratorio, campo y puesto de control. Resultados: Usamos tres herramientas de corte para obtener una alta calidad de imágenes transversales de la madera; obtuvimos 3 750 imágenes macroscópicas con un microscopio portátil que corresponden a 25 especies maderables. El cuchillo ''Tramontina'' es duradero, pero pierde el filo con facilidad y se necesita una herramienta para afilar, el cúter retráctil ''Pretul'' es adecuado para madera suave y dura en muestras pequeñas de laboratorio; el cuchillo ''Ubermann'' es apropiado para el campo, laboratorio y puesto de control, porque tiene una envoltura duradera y láminas intercambiables en caso de pérdida de filo. Conclusiones: La calidad de las imágenes es decisiva en la clasificación de especies maderables, porque permite una mejor visualización y distinción de las características anatómicas en el entrenamiento con los modelos de red neuronal convolucional EfficientNet B0 y Custom Vision, lo cual se evidenció en las métricas de precisión.


Introduction: The great diversity of tropical timber species demands the development of new technologies capable of identifying them based on their patterns or anatomical characteristics. The application of convolutional neural networks (CNN) for the recognition of tropical timber species has increased in recent years due to the promising results of CNNs. Objective: To evaluate the quality of macroscopic images with three cutting tools to improve the visualization and distinction of anatomical features in the CNN model training. Methods: Samples were collected from 2020 to 2021 in areas of logging and sawmills in the Central Jungle, Peru. They were later sized and, after botanical and anatomical identification, cut in cross sections. A database of macroscopic images of the cross-section of wood was generated through cutting with three different tools and observing its performance in the laboratory, field, and checkpoint. Results: Using three cutting tools, we obtained high quality images of the cross section of wood; 3 750 macroscopic images were obtained with a portable microscope and correspond to 25 timber species. We found the ''Tramontina'' knife to be durable, however, it loses its edge easily and requires a sharpening tool, the ''Pretul'' retractable cutter is suitable for cutting soft and hard wood in small laboratory samples and finally the ''Ubermann'' knife is suitable for use in the field, laboratory, and checkpoint, because it has a durable sheath and interchangeable blades in case of dullness. Conclusion: The quality of the images is decisive in the classification of timber species, because it allows a better visualization and distinction of the anatomical characteristics in training with the EfficientNet B0 and Custom Vision convolutional neural network models, which was evidenced in the precision metrics.

INTRODUCTION: Gastric cancer (GC) is the fifth most diagnosed neoplasia and the third leading cause of cancer-related deaths. A substantial number of patients exhibit an advanced GC stage once diagnosed. Therefore, the search for biomarkers contributes to the improvement and development of therapies. OBJECTIVE: This study aimed to identify potential GC biomarkers making use of in silico tools. METHODS: Gastric tissue microarray data available in Gene Expression Omnibus and The Cancer Genome Atlas Program was extracted. We applied statistical tests in the search for differentially expressed genes between tumoral and non-tumoral adjacent tissue samples. The selected genes were submitted to an in-house tool for analyses of functional enrichment, survival rate, histological and molecular classifications, and clinical follow-up data. A decision tree analysis was performed to evaluate the predictive power of the potential biomarkers. RESULTS: In total, 39 differentially expressed genes were found, mostly involved in extracellular structure organization, extracellular matrix organization, and angiogenesis. The genes SLC7A8, LY6E, and SIDT2 showed potential as diagnostic biomarkers considering the differential expression results coupled with the high predictive power of the decision tree models. Moreover, GC samples showed lower SLC7A8 and SIDT2 expression, whereas LY6E was higher. SIDT2 demonstrated a potential prognostic role for the diffuse type of GC, given the higher patient survival rate for lower gene expression. CONCLUSION: Our study outlines novel biomarkers for GC that may have a key role in tumor progression. Nevertheless, complementary in vitro analyses are still needed to further support their potential.

ABSTRACT

Objective:To construct a classification and regression tree which can be used to guide the tracheostomy for traumatic cervical spinal cord injury (TCSCI) based on the identification of the risk factors for TCSCI.Methods:The 498 patients with TCSCI were retrospectively analyzed who had been treated at Department of Orthopedics, The Second Hospital Affiliated to Army Medical University from January 2009 to December 2018. There were 403 males and 86 females, with an age of (50.2±13.6) years. Of the patients, 69 received tracheostomy and 420 did not. The gender, age, smoking history, injury cause, neurological level of injury (NLI), American Spinal Cord Injury Association (ASIA) grade, injury severity score (ISS), thoracic injuries, prior pulmonary diseases, prior basic diseases, and operative approaches of the patients were statistically analyzed by single factor analysis. After the independent risk factors for tracheostomy were analyzed by binary logistic regression, the classification and regression tree was developed which could be used to guide the tracheostomy.Results:The logistic regression analysis showed age>50 years ( OR=4.744, 95% CI: 1.802 to 12.493, P=0.002), NLI at C 4 and above ( OR=23.662, 95% CI: 8.449 to 66.268, P<0.001), ASIA grade A ( OR=40.007, 95% CI: 12.992 to 123.193, P<0.001), and ISS score>16 ( OR=10.502, 95% CI: 3.909 to 28.211, P<0.001) were the independent risk factors for the tracheotomy. The classification and regression tree revealed that ASIA grade A and NLI at C 4 and above were the first and second decision nodes, which had a strong predictive effect on tracheostomy. 86.84% of the patients with ASIA grade A and NLI at C 4 and above underwent tracheostomy. Conclusion:Our classification and regression tree shows that NLI at C 4 and above and ASIA grade A have a strong guiding effect on tracheotomy for TCSCI.

ABSTRACT

Objective:To establish a decision tree model of pediatric complicated appendicitis (CA) based on Pediatric Appendicitis Score (PAS) combined with inflammatory indicators, and to evaluate its clinical application efficacy in pediatrics.Methods:The clinical data of 544 children diagnosed with appendicitis in Children′s Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2018 to December 2021 was retrospectively analyzed. According to postoperative pathology, the children were divided into uncomplicated appendicitis group and CA group. The independent risk factors of CA were screened by univariate and multivariate logistic regression analysis, and these parameters were included to establish the decision tree model. The accuracy of the decision tree model was verified by receiver operating characteristic (ROC) curve.Results:Binary logistic regression analysis indicated that the PAS, C-reactive protein (CRP) and neutrophil to lymphocyte ratio (NLR) were identified as independent risk factors for complicated appendicitis in children (all P<0.05). PAS, CRP and NLR were included as covariables to construct the decision tree model and binary logistic regression model for predicting CA. The decision tree demonstrated an overall accuracy of 79.2% with a sensitivity of 86.7% and specificity of 71.9%, and achieved an area under curve (AUC) of 0.821(95% CI: 0.786-0.857). The binary logistic regression model had a sensitivity of 79.6% and specificity of 69.1%, with an overall accuracy of 75.1% and achieved an AUC of 0.808(95% CI: 0.770-0.845). Conclusions:The decision tree model based on PAS score combined with CRP, NLR is a simple, intuitive and effective tool , which can provide pediatric emergency physicians a reliable basis for diagnosis of pediatric CA.

Objective:To analyze the influencing factors of delayed nausea and vomiting in patients with primary liver cancer after transarterial chemoembolization based on Logistic regression model and decision tree model.Methods:This was a cross-sectional study. A total of 236 patients with primary liver cancer after transarterial chemoembolization in The Second Affiliated Hospital of Air Force Military Medical University from March 2021 to June 2022 were conveniently selected as the research subjects. The factors related to delayed nausea and vomiting were collected, and Logistic regression and decision tree models were established, respectively, and the differences between the two models were compared.Results:The incidence of delayed nausea and vomiting of patients with primary liver cancer after transarterial chemoembolization was 45.34% (107/236). Logistic regression model showed that age, anxiety, sleep disorder, emetic risk level of chemotherapeutic drugs, embolic agent type, and pain 24 hours after surgery were the influencing factors of delayed nausea and vomiting in patients with primary liver cancer after transarterial chemoembolization(all P<0.05). Decision tree model showed that age, sleep disorder, emetic risk level of chemotherapeutic drugs, embolic agent type, and pain 24 hours after surgery were the influencing factors of delayed nausea and vomiting in patients with primary liver cancer after transarterial chemoembolization (all P<0.05). The classification accuracy rates of Logistic regression, decision tree model and combined diagnosis of two models were 72.9%, 71.2% and 72.0% respectively; the areas under the ROC curve were 0.778, 0.781 and 0.806 respectively, with no significant difference (all P>0.05). Conclusions:The analysis results of Logistic regression and decision tree model on the influencing factors of delayed nausea and vomiting in patients with primary liver cancer after transarterial chemoembolization are highly consistent, which can be combined to provide a more comprehensive reference for the evaluation and intervention of medical staff.

ABSTRACT

Colletotrichum, a genus of Ascomycota fungi with a diversity of species grouped into several species complexes, or clades, is associated with diseases known as "Anthracnose". It affects significantly different tropical and subtropical fruit species. Infections occur in the field and postharvest; quiescent stages are mainly responsible for postharvest losses. An analysis of this pre-infective stage is made based on consulted papers. The infective process is modulated by the length of the quiescent period of the fungus during the vegetative or pre-productive stages of trees. Quiescent stage is determined by the host's biochemical responses and the pathogen's activity. Once the pathogen is activated, it develops an infectious necrotrophic process. Colletotrichum quiescence has been studied mainly in subtropical fruits and horticultural species, and the studies highlight the role of volatile compounds, metabolites and enzymes involved in the length and breaking of quiescence, as well as the differential responses according to the phenological stage and the genotype of the plant. Transcriptomic and proteomic analysis of the host-fungus interaction have revealed the role of genes in the occurrence and breaking of quiescence. Knowledge about the quiescence of Colletotrichum in tropical fruits is necessary to improve management efficiency. Detection and induction of quiescent infections has been studied and genomics has allowed to understand the occurrence of quiescence in the infective process; however, in crops in the tropics, such as mango, an important crop in Colombia, questions about associated species and biology of their quiescence are still unanswered.


Colletotrichum es un hongo ascomicete, con diversidad de especies agrupadas en complejos o clados y se asocia a enfermedades conocidas, como "Antracnosis" y afecta significativamente especies de frutas tropicales y subtropicales. Las infecciones ocurren en el campo y postcosecha; los estados quiescentes son los principales responsables de pérdidas en postcosecha. Se realizó un análisis de esta etapa pre-infectiva, a partir de publicaciones consultadas. La infección está modulada por la duración del período de quiescencia del hongo durante las etapas vegetativa o pre-productivas de los árboles y está determinado por las respuestas bioquímicas del hospedante y la actividad del patógeno. Una vez se activa el patógeno, se desarrolla un proceso infeccioso necrotrófico. La quiescencia de Colletotrichum se ha estudiado, principalmente, en frutas subtropicales y especies hortícolas y destacan el papel de compuestos volátiles, metabolitos y enzimas en la duración y en la pérdida de la quiescencia, así como las respuestas diferenciales, según la fenología y el genotipo. Análisis transcriptómicos y proteómicos de la interacción hospedante-hongo han revelado el papel de genes en la ocurrencia y pérdida de quiescencia. Conocer la quiescencia de Colletotrichum en frutas tropicales es necesario para hacer más eficiente el manejo de la enfermedad. Se ha estudiado la detección e inducción de infecciones quiescentes y estudios genómicos han permitido entender su ocurrencia durante la infección; sin embargo, en cultivos en el trópico, como el mango, un cultivo importante en Colombia, las preguntas sobre las especies asociadas y la biología de la quiescencia de estas, aún están sin respuesta.

Intercropped systems with Conilon coffee might provide a better environment for coffee production. The aim of this study was to assess the microclimate and development of Conilon coffee intercropped with papaya trees. Papaya was planted with spacing of 3.20 x 2.40 m. The coffee trees were planted after eight months, with spacing of 3.20 x 1.60 m, in-between papaya trees (in the same row). The measurements were taken 0, 40 and 80 cm away from the coffee plants, both in the north and south direction. Concomitantly, an adjoining full sunlight coffee system (not intercropped) was also assessed. The measurements included atmospheric parameters (temperature, irradiance, and relative humidity) and vegetative parameters for the coffee plants (leaf area, relative chlorophyll index, length of plagiotropic branches, length of orthotopic branches and number of nodes) in three periods of the year. The intercropped system of Conilon coffee and papaya trees led to a decrease in both irradiance and temperature, and higher means of relative humidity during daytime in all the periods assessed, which contributes to a better environment for coffee cultivation. The shadow provided by papaya trees in the coffee plants contributed to a higher leaf area but did not affect neither the growth of both plagiotropic and orthotopic branches, nor the number of nodes and the etiolation. The intercropped system of Conilon coffee and papaya trees may be potentially used as a farming system to mitigate climate change.

Root deformation (RD) caused by errors in the pricking out process are irreversible and very difficult to detect in container-grown seedlings at the time of planting in the field. The objective of this study was to evaluate the effects of RD on leaf gas exchange, growth, biomass allocation and mineral nutrition of G. americana seedlings during the recovery phase after soil flooding. Four-months-old seedlings, with and without RD, were flooded for 42 days and their recovery was evaluated 28 days after soil drainage. There were no significant interactions between RD and soil flooding for all leaf gas exchange, growth and mineral nutrition after soil drainage, with the exception of leaf P concentrations. In plants with no RD, the P concentration in leaves of non-flooded plants was significantly higher than that of plants with RD. Soil flooding and RD did not influence leaf or root N concentrations or whole-plant N content. RD increased the K concentration in the roots, but not in the leaves. Changes in the nutrient concentrations in leaves and roots indicate that RD may affect physiological performance of seedlings after planting in the field.

ABSTRACT

Root deformation (RD) caused by errors in the pricking out process are irreversible and very difficult to detect in container-grown seedlings at the time of planting in the field. The objective of this study was to evaluate the effects of RD on leaf gas exchange, growth, biomass allocation and mineral nutrition of G. americana seedlings during the recovery phase after soil flooding. Four-months-old seedlings, with and without RD, were flooded for 42 days and their recovery was evaluated 28 days after soil drainage. There were no significant interactions between RD and soil flooding for all leaf gas exchange, growth and mineral nutrition after soil drainage, with the exception of leaf P concentrations. In plants with no RD, the P concentration in leaves of non-flooded plants was significantly higher than that of plants with RD. Soil flooding and RD did not influence leaf or root N concentrations or whole-plant N content. RD increased the K concentration in the roots, but not in the leaves. Changes in the nutrient concentrations in leaves and roots indicate that RD may affect physiological performance of seedlings after planting in the field.


A deformação da raiz (RD) causada por erros no processo de repicagem é irreversível e difícil de detectar em mudas produzidas em embalagens no momento do plantio no campo. O objetivo deste estudo foi avaliar os efeitos do RD nas trocas gasosas foliares, crescimento, alocação de biomassa e nutrição mineral de mudas de G. americana na fase de recuperação após o alagamento do solo. Mudas com quatro meses de idade, com e sem RD, foram alagadas por 42 dias e a sua recuperação foi avaliada 28 dias após a drenagem do solo. Não houve interação significativa entre RD e alagamento do solo nas trocas gasosas foliares, crescimento e nutrição mineral após a drenagem, com exceção das concentrações de P foliar. Em plantas sem RD, a concentração de P nas folhas de plantas não alagadas foi significativamente maior que a das plantas com RD. O alagamento do solo e a RD não influenciaram as concentrações de N nas folhas e raízes, e no conteúdo de N na planta inteira. A RD aumentou a concentração de K nas raízes, mas não nas folhas. Alterações nas concentrações de nutrientes nas folhas e raízes indicam que a RD pode afetar o desempenho fisiológico das mudas após o plantio no campo.

ABSTRACT

Esta tese objetivou identificar estratégias de triagem para infecção latente por Mycobacterium tuberculosis (Mtb) ­ ILTB em profissionais de saúde que viabilizem o uso mais eficiente dos recursos disponíveis. No Brasil, recomenda-se que os profissionais de saúde, um dos grupos de risco para a ILTB, realizem triagem periódica para detecção da infecção e aqueles que apresentarem conversão aos testes de diagnóstico, indica-se o tratamento preventivo da tuberculose (TB) ­ TPT uma vez que pessoas com conversão recente apresentam elevada chance de progressão para a doença. Desenvolvemos, no primeiro artigo, um modelo preditivo para identificar profissionais de saúde com maior probabilidade de resultado negativo para dois testes de diagnóstico da ILTB a partir de uma análise secundária de dados publicados anteriormente de 708 profissionais de saúde da atenção primária, de cinco capitais brasileiras, submetidos à prova tuberculínica (PT) e ao Quantiferon®-TB Gold in-tube (QFT-IT®). Construímos um modelo preditivo utilizando árvore de classificação e regressão (CART, classification and regression tree). A avaliação do desempenho foi realizada por meio da análise receiver operating characteristics (ROC) e area under the curve (AUC). Utilizou-se o mesmo banco de dados para validação cruzada do modelo. Entre os 708 profissionais de saúde, 247 (34,9%) apresentaram resultado negativo para os testes. A CART identificou que os médicos e agentes comunitários de saúde apresentaram chances duas vezes maior de testes negativos (probabilidade = 0,60) do que os enfermeiros e técnicos/auxiliares de enfermagem (probabilidade = 0,28) naqueles com menos de 5,5 anos de atuação na atenção primária. Na validação cruzada, a acurácia do modelo preditivo foi de 68% [intervalo de confiança de 95% (IC95%) 65 ­ 71) ], AUC de 62% (IC95% 58 ­ 66), especificidade de 78% (IC95% 74 ­ 81) e sensibilidade de 44% (IC95% 38 ­ 50). Apesar do baixo poder preditivo do modelo, a CART permitiu identificar subgrupos com maior probabilidade de terem ambos os testes negativos. No segundo artigo, analisou-se a razão de custo-efetividade de dois testes de sensibilidade cutânea baseados em antígenos específicos do Mtb -Diaskintest® e C-TST® - e a do QFT-Plus® para o diagnóstico da ILTB comparadas com a estratégia diagnóstica atual (PT) entre profissionais de saúde. Desenvolveu-se um modelo analítico de decisão, representado por coorte hipotética de 100.000 profissionais de saúde, de ambos os sexos, com resultado negativo para a PT no ano anterior, horizonte temporal de cinco anos, na perspectiva do Sistema Único de Saúde. Avaliaram-se três regimes de tratamento para a ILTB: três meses de doses semanais de rifapentina (900 mg) e isoniazida (900 mg) (3HP), seis e nove meses de doses diárias de isoniazida (300 mg) (6H e 9H, respectivamente). Aplicou-se taxa de desconto de 5% na efetividade, medida em casos de TB ativa evitados, e nos custos das estratégias de triagem e de tratamento avaliados, estimados em dólares americanos (US$) com taxa média anual de 2021 de acordo com o Banco Central (US$ 1 = 5,39 reais). Foram realizadas análises de sensibilidade determinística univariada e probabilística. Os testes Diaskintest®, C-TST® e QFT-Plus® apresentam maior especificidade (0,98, 0,98 e 0,97, respectivamente). Os custos com QFT-Plus® foram maiores devido aos equipamentos, mão de obra e ao custo do kit. O Diaskintest® foi o teste mais econômico (US$ 7.042, US$ 5.781 e US$ 18.892 por caso de TB ativa evitado para os regimes de tratamento com 3HP, 6H e 9H, respectivamente), inclusive nas análises de sensibilidade. No cenário nacional, o Diaskintest® foi o teste de melhor custo-efetividade para avaliação anual dos profissionais de saúde.


This thesis aimed to identify screening strategies for tuberculosis infection (TBI) in healthcare workers (HCW) that enable the most efficient use of available resources. Investigation of TBI in HCWs is recommended in Brazil as part of the worker's pre-employment and periodic (annual) health visits. HCWs with a first tuberculin skin test (TST) induration < 10 mm are invited to repeat the test in one to three weeks to assess the booster effect (induration size increment of 10 mm). Those with a persistent TST < 10 mm will undergo a one-step TST every 12 months. TPT is recommended when conversion (10 mm increment over latest induration size) occurs. We developed, in the first manuscript, a predictive model to identify HCWs best targeted for TBI screening. We carried out a secondary analysis of previously published results of 708 HCWs working in primary care services in five Brazilian State capitals who underwent two TBI tests: tuberculin skin test and Quantiferon®-TB Gold in-tube. We used a classification and regression tree (CART) model to predict HCWs with negative results for both tests. The performance of the model was evaluated using the receiver operating characteristics (ROC) curve and the area under the curve (AUC), cross-validated using the same dataset. Among the 708 HCWs, 247 (34.9%) had negative results for both tests. CART allowed us to identify that physicians or a community health agents were twice more likely to be uninfected (probability = 0.60) than registered or aid nurse (probability = 0.28) when working less than 5.5 years in the primary care setting. In cross validation, the predictive accuracy was 68% [95% confidence interval (95%CI): 65 ­ 71], AUC was 62% (95%CI 58 ­ 66), specificity was 78% (95%CI 74 ­ 81), and sensitivity was 44% (95%CI 38 ­ 50). Despite the low predictive power of this model, CART allowed to identify subgroups with higher probability of having both tests negative. In the second manuscript, we analyzed the cost-effectiveness of two TB antigen-based skin tests (TBST) using the recombinant ESAT-6 and CFP-10 immunogens (Diaskintest® and C-TST®) and of QFT-Plus® for TBI diagnosis compared with the current standard of care, TST, among HCWs in Brazil. A state-transition Markov model was created, simulating a cohort of 100,000 HCWs (five annual cycles) for TBI treatment scenarios with 3 months of weekly doses of rifapentine (900 mg) and isoniazid (900 mg) (3HP). We adopted the Brazilian public health system perspective. Effects [tuberculosis disease (TBD) averted) and costs for screening and treating TBI were discounted at 5%. Incremental cost-effectiveness per TBD averted was calculated. Hypothetical cohort of 100,000 HCWs of both sexes with a negative result of TST in the previous year. Diaskintest®, C-TST® and QFT-Plus® tests have higher specificity (0.98, 0.98 and 0.97, respectively). Costs with QFT-Plus® were higher due to equipment, human labor and cost of the kit by test. Diaskintest® was the most cost-effective test (US$ 7,042, US$ 5,781, and US$18,892 per case of TBD averted for the 3HP, 6H, and 9H treatment regimens, respectively), including sensitivity analyses. In the Brazilian scenario, Diaskintest® is the most cost-effective test for sequential testing of HCWs.

SUMMARY: Analysis and systematization of the longitudinal dimensions of the phalanges of the index and ring fingers for the classification of morphological types of the hand using classification and regression trees (CART). X-rays of the hands of 50 men and 50 women (mean age 47.16 (10.1) years, range 23-65 years) were studied. Each hand, depending on the ratio of the length of the index and ring fingers, was classified into three types: radial (R, 2d>4d), indefinite (N, 2d=4d), ulnar (U, 2d>4d). Morphometry of radiographs included measurements of the lengths of the proximal (PP), middle (MP), and distal (DP) phalanges. The sex differences of the analyzed indicators are statistically significant. There were no significant bilateral differences between the phalanges of the II and IV fingers in length, regardless of sex (p>0.05). A set of rules for classifying the morphological types of the hand depending on the lengths of the phalanges of the index and ring fingers was created by constructing a binary decision tree. The CART method demonstrates the usefulness of this statistical procedure for developing a scientifically based prediction of the morphological type of the hand. The results of this study can be the basis of an algorithm for determining the morphological type of the hand depending on the length of the phalanges of the fingers.


RESUMEN: En este estudio se realizó un análisis y sistematización de las dimensiones longitudinales de las falanges de los dedos índice y anular para la clasificación de tipos morfológicos de la mano, mediante árboles de clasificación y regresión (CART). Se estudiaron radiografías de las manos de 50 hombres y 50 mujeres (edad media 47,16 (10,1) años, rango 23-65 años). Cada mano, según la proporción de la longitud de los dedos índice y anular, se clasificó en tres tipos: radial (R, 2d> 4d), indefinida (N, 2d = 4d), ulnar (U, 2d> 4d). La morfometría de las radiografías incluyó mediciones de las longitudes de las falanges proximal (FP), media (FM) y distal (FD). Las diferencias de sexo de los indicadores analizados fueron estadísticamente significativas. No hubo diferencias bilaterales significativas entre las falanges de los dedos II y IV en longitud, independientemente del sexo (p> 0,05). Se creó un conjunto de reglas para clasificar los tipos morfológicos de la mano en función de las longitudes de las falanges de los dedos índice y anular mediante la construcción de un árbol de decisión binario. El método CART demuestra la utilidad de este procedimiento estadístico para desarrollar una predicción con base científica del tipo morfológico de la mano. Los resultados de este estudio pueden ser la base de un algoritmo para determinar el tipo morfológico de la mano en función de la longitud de las falanges de los dedos.

Introducción: las características de transmisión del virus SARS-CoV-2 incrementaron la necesidad de implementar medidas de prevención a fin de evitar su propagación; por lo tanto, hay un interés en la percepción de riesgo y una intención de la población para adoptar las medidas de protección frente a la COVID-19. Objetivo: evidenciar los factores causales que contribuyen a una percepción alta/baja del riesgo de la pandemia de COVID-19. Material y métodos: se hizo una investigación exploratoria cuantitativa, analítica y explicativa con diseño transversal. Se diseñó una encuesta de autoacceso con el procedimiento de Evaluación Dimensional del Riesgo Percibido, basado en el paradigma psicométrico. Para el análisis de los datos se utilizó la técnica árboles de clasificación. Resultados: el análisis descriptivo indicó que los encuestados tienen una preocupación por la pandemia de 3.8 en una escala de 1 a 5, que evidencia una percepción media-alta del riesgo de la pandemia de COVID-19; sin embargo, subestiman su riesgo personal (vulnerabilidad) en relación con el de los demás. Del análisis con árboles de clasificación, se obtuvo que las variables independientes que contribuyen directamente a la percepción global de riesgo son la gravedad de la pandemia, la vulnerabilidad y el poder catastrófico. Conclusiones: se encontró un efecto positivo: cuanto mayor es la percepción de severidad del virus y la susceptibilidad de contraerlo, hay una mayor toma de medidas preventivas.


Background: The characteristics of the transmission of SARS-CoV-2 virus increased the need to implement prevention measures, in order to avoid its spread; therefore, there is an interest in the risk perception and an intention in people to adopt protective measures against COVID-19. Objective: To show the causal factors that contribute to a high/low perception of risk during the COVID-19 pandemic. Material and methods: A quantitative, analytical and explanatory exploratory study was carried out with a cross-sectional design. To collect data, a self-access survey was designed, using the Dimensional Assessment of Perceived Risk procedure, based on the psychometric paradigm. For data analysis, the classification tree method was utilized. Results: The descriptive analysis indicated that the surveyed population had a concern about the pandemic of 3.8 on a scale ranging from 1 to 5, which shows a medium-high perception of COVID-19 risk; however, respondents underestimated their personal risk (vulnerability) in relation to that of others. The classification tree analysis showed that the independent variables that contribute directly to the global perception of risk are the severity of the pandemic, vulnerability and catastrophic power. Conclusions: It was found a positive effect: The greater perception of severity, and susceptibility to acquiring the virus, the more taking of preventive measures.

ABSTRACT Introduction: With the continuous development of society and the continuous improvement of the economic level, the willingness of Chinese people to participate in sports is also showing an upward trend. However, how to reduce sports damage as much as possible during exercise should be a hot issue of particular concern to athletes in the sports world. Objective: It aimed to discuss the simulation of the relationship between joint motion amplitude (JMA) and motion damage (MD) via a rough set decision-making algorithm to avoid MD. Based on the rough set decision algorithm, JMA and MD models were constructed, and a motion data decision table was established. Methods: Joint change parameters and constraint conditions were set, and joint change parameters were analyzed. Moreover, the changing parameters, feature strength, and algorithm partition accuracy of the simulation model were analyzed. Results: The feature strength and the division accuracy of the rough set decision algorithm all showed good accuracy. The model constructed by such a method can well describe the relationship between JMA and MD. Conclusion: The proposed rough set decision algorithm can describe the relationship between JMA and MD scientifically and effectively, which provided reference value for sports. RESUMO Introdução: Com o desenvolvimento contínuo da sociedade e a melhoria contínua do nível econômico, a disposição do povo chinês para a prática de esportes também apresenta uma tendência ascendente. No entanto, como reduzir os danos ao esporte tanto quanto possível durante o exercício deve ser uma questão importante de particular preocupação para os atletas do mundo dos esportes. Objetivo: o objetivo foi discutir a simulação da relação entre amplitude de movimento articular (JMA) e dano de movimento (MD) por meio de um algoritmo de tomada de decisão de conjunto aproximado, para evitar MD. Com base no algoritmo de decisão de conjunto aproximado, os modelos JMA e MD foram construídos e uma tabela de decisão de dados de movimento foi estabelecida. Métodos: os parâmetros de mudança da junta e as condições de restrição foram definidos, e os parâmetros de mudança da junta foram analisados. Além disso, foram analisados os parâmetros de alteração, a força do recurso e a precisão da partição do algoritmo do modelo de simulação. Resultados: A força do recurso e a precisão da divisão do algoritmo de decisão do conjunto aproximado mostraram boa precisão. O modelo construído por esse método pode descrever bem a relação entre JMA e MD. Conclusão: O algoritmo de decisão de conjunto aproximado proposto pode descrever a relação entre JMA e MD de forma científica e eficaz, o que forneceu valor de referência para a área de esportes. RESUMEN Introducción: Con el desarrollo continuo de la sociedad y la mejora continua del nivel económico, la disposición del pueblo chino a participar en deportes también está mostrando una tendencia al alza. Sin embargo, cómo reducir el daño deportivo tanto como sea posible durante el ejercicio debería ser un tema candente de especial preocupación para los atletas en el mundo del deporte. Objetivo: Su objetivo era discutir la simulación de la relación entre la amplitud del movimiento articular (JMA) y el daño por movimiento (MD) a través de un algoritmo de toma de decisiones de conjunto aproximado, para evitar MD. Con base en el algoritmo de decisión de conjunto aproximado, se construyeron modelos JMA y MD, y se estableció una tabla de decisión de datos de movimiento. Métodos: Se establecieron los parámetros de cambio de la articulación y las condiciones de restricción, y se analizaron los parámetros de cambio de la articulación. Además, se analizaron los parámetros cambiantes, la fuerza de la característica y la precisión de la partición del algoritmo del modelo de simulación. Resultados: La fuerza de la característica y la precisión de la división del algoritmo de decisión de conjunto aproximado mostraron una buena precisión. El modelo construido por tal método puede describir bien la relación entre JMA y MD. Conclusión: El algoritmo de decisión de conjunto aproximado propuesto puede describir la relación entre JMA y MD de manera científica y efectiva, lo que proporcionó un valor de referencia para el campo de los deportes. Abstract The present study was conducted to explore morphometric variations of Pteropus medius (the Indian flying fox) and the roosting trees in Lower Dir, Pakistan. The bats were captured from Morus alba, Morus nigra, Brousonetia papyrifera, Pinus raxburghii, Hevea brasiliensis, Platanus orientalis, Populous nigra, Melia azedarach, Eucalyptus camaldulensis and Grevillea robusta through sling shot and mess net methods. A total of 12 bats were studied for the differential morphological features based on age and sex. Male bats were recorded higher in weight than females. The variations were found in body mass (821.1±34.65gm), circumference of body with wings (25.43±0.39cm), wingspan (112.58±1.90cm), Body length (20.73±0.68cm), Snout length (3.42±0.04cm), Eye length (1.45±0.033cm), Length of ear (3.56±0.05cm), Width of ear (2.46±0.04cm), Length b/w ear (5.51±0.11cm), Circumference of neck (12.23±0.24cm), Circumference of body without wings (18.68±0.31cm), Arm wing length (23.2±1.03cm), Length of thumb (5.43±0.1cm), Length of nail (1.89±0.05cm), Hand wing length (29.1±0.51cm), Maximum width of wing (21.03±0.68cm), Length b/w tip of wing to 5th digit (29.39±0.30cm), Length b/w 5th digit to foot (22.97±1.09cm), Length b/w feet (18.31±0.74cm) and Length of foot claw (4.23±0.05cm). This study was designed for analysis of external morphological variations for P. medius (the Indian flying fox) that may help in identification of these bats and their roosting sites.


Resumo O presente estudo foi conduzido com o intuito de explorar variações morfométricas de Pteropus medius (raposa-voadora indiana) e árvores de repouso em Lower Dir, Paquistão. Os morcegos foram capturados de Morus alba, Morus nigra, Broussonetia papyrifera, Pinus roxburghii, Hevea brasiliensis, Platanus orientalis, Populus nigra, Melia azedarach, Eucalyptus camaldulensis e Grevillea robusta por meio de estilingues e redes. Doze morcegos foram estudados quanto às características morfológicas diferenciais com base na idade e no sexo. Os morcegos machos apresentaram maior peso do que as fêmeas. As variações foram encontradas em massa corporal (821,1 ± 34,65 g), circunferência do corpo com asas (25,43 ± 0,39 cm), envergadura (112,58 ± 1,90 cm), comprimento do corpo (20,73 ± 0,68 cm), comprimento do focinho (3,42 ± 0,04 cm), comprimento do olho (1,45 ± 0,033 cm), comprimento da orelha (3,56 ± 0,05 cm), largura da orelha (2,46 ± 0,04 cm), comprimento entre as orelhas (5,51 ± 0,11 cm), circunferência do pescoço (12,23 ± 0,24 cm), circunferência do corpo sem asas (18,68 ± 0,31 cm), comprimento da asa da pata dianteira (23,2 ± 1,03 cm), comprimento do polegar (5,43 ± 0,1 cm), comprimento da unha (1,89 ± 0,05 cm), comprimento da asa até a ponta do dedo (29,1 ± 0,51 cm), largura máxima da asa (21,03 ± 0,68 cm), comprimento entre a ponta da asa e o quinto dedo (29,39 ± 0,30 cm), comprimento entre o quinto dedo e a pata (22,97 ± 1,09 cm), comprimento entre as patas (18,31 ± 0,74 cm) e comprimento da garra da pata (4,23 ± 0,05 cm). Este estudo foi desenvolvido para análise de variações morfológicas externas de P. medius (raposa-voadora indiana) e pode auxiliar na identificação desses morcegos e seus locais de nidificação.

ABSTRACT

The identification of species primordium has been one of the hot issues in the identification of traditional Chinese medicine. Sea snake is one of the most valuable Chinese medicinal materials in China. In order to understand the origin and varieties of sea snake in the market, we studied the molecular identification of 46 sea snakes by cytochrome B(Cytb). After comparison and manual correction, the sequence length was 582 bp, and the content of A+T(58.9%) was higher than that of G+C(41.1%). There exist 197 variable sites and 179 parsimony-informative sites of the sequence. There are 44 kinds of sequence alignment with consistency equal to 100%, and 2 kinds equal to 96%. A total of 408 Cytb effective sequences were downloaded from GenBank database, with a total of 68 species. Phylogenetic tree of a total of 454 sea snake sequences with the samples in this study were constructed by neighbor-joining trees and Bayesian inference method, respectively, which can identify 42 samples of medicinal materials, while 4 samples can not be identified because of their low node support. The results showed that the species of the sea snake medicine were at least from 2 genera and 5 species, namely, Aipysurus eydouxii, Hydrophis curtus, H. caerulescen, H. curtus, H. ornatus and H. spiralis. This study suggested that the original species of commercial sea snake are very complex and can provide insight into the identification of sea snakes.

Objective To establish a lung cancer risk prediction model using data mining technology and compare the performance of decision tree C5.0 and artificial neural networks in the application of risk prediction model, and to explore the value of data mining techniques in lung cancer risk prediction. Methods We collected the data of 180 patients with lung cancer and 240 patients with benign lung lesion which contained 17 variables of risk factors and clinical symptoms. Decision tree C5.0 and artificial neural networks models were established to compare the prediction performance. Results There were 420 valid samples collected in total and proportioned with the ratio of 7:3 for the training set and testing set. The accuracy, sensitivity, specificity, Youden index, positive predictive value, negative predictive value and AUC of artificial neural networks model were 65.3%, 61.7%, 73.3%, 0.350, 54.9%, 73.1% and 0.675 (95%CI: 0.628-0.720) in testing set; those of decision tree C5.0 model were 61.0%, 47.8%, 80.4%, 0.282, 35.3%, 80.6% and 0.641 (95%CI: 0.593-0.687) in testing set. Conclusion The artificial neural networks model is superior to the decision tree C5.0 model at overall performance and it has potential application value in the risk prediction of lung cancer.

ABSTRACT

Introduction: Senna spectabilis is a multipurpose pantropical tree, used in agroforestry systems. Objective: To determine pod production (Pp) and their relationship with dasometric variables in S. spectabilis in the tropical dry forest. Methods: From August 2016 to February 2017, thirty trees in production stage were randomly selected. The random selection was formed of the more isolated trees from the total dispersion. The trees were monitored at the beginning and end of the study period, to determine dasometric measurements such as total height (Th), height to the first branch (Hb), crown height (Ch), Stem diameter (at 0.2 m height from the ground) (Db), crown diameter (Cd), and crown volume (Cv) measured. Pods were harvested by the researcher with cutting and height cutting tongs when their color began to change. Pearson correlations and univariate and multivariate regression analyses were performed between the dasometric variables and pod production. The potential number of trees/ha (NPa) was calculated by determining the occlusion percentage (Op) and the shadow area/tree (Ca); to estimate the production potential of fruits/ha, the production of fruits/tree was multiplied by (NPa). Results: Th was 6.16 ± 1.23 m, Hb 2.75 ± 0.52 m, Ch 3.41 ± 0.98 m, Db 20.43 ± 4.80 cm, Cd 7.46 ± 1.20 m and Cv 108.43 ± 61.38 m3/tree. There was a significant positive correlation between Hb, Cd, Db, with Pp of 0.592**, 0.592**, and 0.446* respectively. Pp was 32.73 ± 16.13 kg/tree and the dry matter production (MSP) was 17.84 ± 8.80 kg/tree. The result of the multivariate regression indicated that the second-order polynomial model presented best goodness of fit. Op was 73.4 7.92 %, the cup area was 49.3 m2/tree, Ca was 36.2 m2/tree, and NPa was 83 trees. Conclusions: The production of fresh pods/ tree in the S. spectabilis presents a potential in its availability as feed for ruminant or seed production. The potential production of pods in silvopastoral with S. spectabilis could be 2.72 t/ ha, and 1.64 t/ ha of dry pods, this shows the importance of trees and of pods production and nutritional contribution obtained for dry ecosystems.


Introducción: Senna spectabilis es un árbol pantropical multipropósito, utilizado en sistemas agroforestales. Objetivo: Determinar la producción de vainas (Pv) y la relación con las variables dasométricas en S. spectabilis en el bosque seco tropical. El número potencial de árboles/ha (NPa) fue calculado determinando el porcentaje de oclusión (Po) y el área de sombra/árbol (As); para calcular la producción potencial de frutos/ha, la producción de frutos/árbol fue multiplicada por (NPa). Métodos: Desde agosto del 2016 hasta febrero de 2017, treinta árboles en etapa de producción fueron seleccionados al azar, los más aislados del total de árboles dispersos fueron seleccionados, y fueron monitoreados al inicio y al final del período de estudio, para determinar las mediciones dasométricas como la altura total (At), altura a la primera rama (Apr), altura de la copa (Ac), diámetro del tallo (a 0.2 m altura desde el suelo) (Dt), diámetro de la copa (Dc) y volumen de copa (Vc). Las vainas se cosecharon cuando su color comenzó a cambiar. Se realizaron correlaciones de Pearson y análisis de regresión univariada y multivariada entre las variables dasométricas y la producción de vainas. El número potencial de árboles/ha (NPa) se calculó determinando el porcentaje de oclusión (Po) y el área de sombra/árbol (Asa); para estimar el potencial de producción de las vainas/ha, la producción de vainas/árbol se multiplicó por NPa. Resultados: La At fue de 6.16 ± 1.23 m, Apr 2.75 ± 0.52 m, Ac 3.41 ± 0.98 m, Db 20.43 ± 4.80 cm, Dc 7.46 ± 1.20 m y Vc 108.43 ± 61.38 m3/árbol. Existió una correlación positiva significativa entre Apr, Dc, Db, Pv de 0.592**, 0.592 ** y 0.446 * respectivamente. La Pv fue de 32.73 ± 16.13 kg y la producción de materia seca (PMS) fue de 17.84 ± 8.80 kg/árbol. El resultado de la regresión multivariada indicó que el modelo polinomial de segundo orden presentó la mejor bondad de ajuste. El Po de los árboles fue de 73.4 % ± 7.92 %, el área de copa fue de 49.3 m2/árbol, el Asa fue de 36.2 m2/árbol, el NPa fue de 83 árboles. Conclusiones: La producción de vainas frescas/árbol en el S. spectabilis presenta un potencial en la disponibilidad de alimento para los rumiantes o la producción de semillas. El potencial de producción de vainas en u arreglos silvopastoriles podría ser de 2.72 t/ha, y 1.64 t/ha de vainas secas, esto muestra la importancia del árbol de producción de vainas y la contribución nutricional para los ecosistemas secos.
